A PSMA PET/CT scan is a diagnostic test, not PSMA-directed radioligand treatment. It exposes you to radiation from two sources: the injected tracer and the CT scan. Published dose figures are population reference values, not a prediction of your personal dose or cancer risk. Ask your imaging team which tracer, administered activity, and CT protocol are planned.
What a PSMA PET scan is—and what it is not
PSMA PET imaging uses a small amount of a radioactive molecule that attaches to prostate-specific membrane antigen (PSMA). A PET scanner detects where the tracer travels, while CT supplies anatomical detail so areas of uptake can be located. Doctors commonly use the examination to stage men who may have metastatic disease or to investigate suspected persistent or recurrent prostate cancer after treatment, particularly when PSA rises.
This is diagnostic imaging. It should not be confused with PSMA-directed radioligand therapy, in which a PSMA-targeting molecule carries a therapeutic radionuclide intended to damage cancer cells. A diagnostic scan generally uses much less radioactivity than treatment, and its dose must not be used as a proxy for treatment exposure. Your nuclear medicine physician can explain the separate risk and monitoring requirements if therapy is being considered.
How much radiation does a PSMA PET/CT involve?
The examination has two radiation contributions:
- Internal exposure from the radiotracer: the tracer circulates in your body and emits radiation while it is present. The amount depends on the specific ligand and the activity injected.
- External X-ray exposure from CT: the CT portion adds radiation according to its purpose, scanner hardware, and technical settings.
The 2023 joint EANM/SNMMI procedure guideline reports radiopharmaceutical effective-dose coefficients from 0.0116 to 0.022 mSv per MBq for the PSMA ligands listed below. These coefficients are not a personal dose estimate: the administered activity, patient characteristics, imaging timing, and local protocol all matter.

Each number in the last column is an example based on the named tracer and activity; it is not what every center administers. The same guideline estimates 1–20 mSv for the CT component, depending on whether the CT is a low-dose acquisition for attenuation correction and anatomical correlation or a higher-dose diagnostic CT. The guideline recommends calculating exposure for the actual protocol used at a particular service.
Why the CT protocol changes the total
A low-dose CT may be sufficient to correct PET data and show basic anatomy. A diagnostic CT can use more X-rays and may be performed to answer a separate clinical question, sometimes with iodinated contrast. Scanner model, scan length, tube settings, body size, and whether contrast-enhanced diagnostic images are required all influence dose. Do not assume that every “PSMA PET/CT” has the same CT exposure.
Before the appointment, ask:
- Which PSMA tracer will be injected?
- What activity will be administered, and is it adjusted for the protocol?
- Is the CT for low-dose attenuation correction, diagnostic evaluation, or both?
- Will iodinated contrast be used, and are there kidney-function or allergy considerations?
- What is the estimated total dose for this examination?
These questions help you obtain a protocol-specific estimate. A population average cannot be converted into an individual prediction without those details.

What side effects can occur?
Tracer-specific reactions
Side effects depend on the agent. In the U.S. Food and Drug Administration’s 2021 safety summary for piflufolastat F 18 (Pylarify), the most common reported adverse reactions were headache, altered taste, and fatigue. The same summary warns that hypersensitivity reactions can occur, with particular concern for people who have previously reacted to medicines or foods. Tell the imaging team about all serious allergies before injection and remain available for observation according to the center’s policy.
Those adverse-reaction findings are specific to that product’s clinical data. They should not be copied to every PSMA tracer. For gallium-68 or other fluorine-18 agents, ask which product is being used and review its current local labeling. A trial supporting the Pylarify safety summary included 593 men who each received one injection across two prospective studies; that is a study sample size, not a percentage of patients who experienced a side effect.
What you may notice during the visit
The injection itself involves placement of an intravenous line. Most people spend about an hour resting while the tracer circulates, followed by approximately 30 minutes on the scanner table, according to SNMMI patient information. Timing varies by facility and tracer. The scanner is open at both ends, but you must lie still; tell staff if pain, claustrophobia, or difficulty remaining still could be a problem.

After the scan, the tracer naturally loses radioactivity and is cleared from the body. Your service will give any specific hydration, urination, breastfeeding, or contact instructions that apply to the tracer and your circumstances. Follow those instructions rather than relying on generic advice.

PSMA uptake is not synonymous with prostate cancer. Normal uptake can be seen in the kidneys, bladder, salivary glands, small bowel, and spleen. PSMA binding can also occur in some other cancers and in certain non-malignant conditions. Consequently, a bright focus may be benign or unrelated to prostate cancer, and a negative scan does not prove that no cancer is present.
False positives
A false-positive result is uptake that looks suspicious but is caused by normal physiology, inflammation, another disease, or another tumor type. Radiologists and nuclear medicine physicians compare the location and appearance of uptake with CT findings, PSA behavior, prior treatment, symptoms, and other tests. Additional imaging, follow-up, or tissue sampling may be needed when the finding would change treatment.

False negatives and small lesions
SNMMI notes that tumors smaller than about 0.5 cm may not be visible. Some tumors also express little PSMA, and technical factors can obscure disease. A negative scan therefore has to be interpreted in the context of your PSA, pathology, examination, and previous images.
Why interpretation context matters
Ask who will read the study and how the result will be combined with your clinical information. The report may classify findings by a structured system, describe indeterminate uptake, or recommend correlation with MRI, CT, bone scintigraphy, another PET tracer, or biopsy. The most useful question is not simply “Is it positive?” but “What does this finding mean for my specific clinical question, and what would confirm or change the interpretation?”
Who is usually considered for PSMA PET?
Common indications include initial staging when the risk of metastatic disease is significant and evaluation of suspected persistent or recurrent disease after definitive treatment when PSA rises. It is not automatically the best test for every person with prostate cancer. SNMMI notes that approved tracers generally perform similarly for detecting prostate cancer, while also noting that detection can be lower in some settings and that another imaging method may answer a particular question better.

Your team may compare PSMA PET with MRI, conventional CT, bone scintigraphy, or PET using another tracer. The decision should reflect:
- the clinical question—initial staging, biochemical recurrence, treatment planning, or another purpose;
- the tracer available and its approved indication;
- the planned CT type and whether contrast is needed;
- your kidney function, allergy history, ability to lie still, and prior imaging;
- how a positive, negative, or indeterminate result would change management.
What happens during the scan?
- Registration and screening: staff confirm your identity, indication, medications, pregnancy or breastfeeding status where relevant, kidney history if contrast is planned, and prior allergies.
- IV placement: a small intravenous catheter is inserted for the tracer.
- Tracer injection: the prescribed activity of the named PSMA agent is administered.
- Uptake period: you rest for about an hour while the tracer distributes. The exact interval depends on the tracer and local protocol.
- Scanning: you lie on the PET/CT table for roughly 30 minutes. PET maps tracer distribution; CT supplies attenuation correction and anatomical localization, and may include a separate diagnostic acquisition.
- Afterward: staff explain when you may leave and any agent-specific instructions. The interpreting physician reviews the images with your PSA, pathology, treatment history, and other relevant information.
Tell staff immediately if you develop itching, hives, swelling, breathing difficulty, severe dizziness, or another sudden symptom after injection. Such symptoms can indicate a hypersensitivity reaction and require prompt assessment.
Long-term radiation risk in perspective
The FDA describes cumulative exposure to ionizing radiation as associated with an increased cancer risk. That is a population-level relationship. The sources used for this article do not provide a validated absolute percentage for the added lifetime risk from one complete PSMA PET/CT examination for an individual patient, so no honest article can assign you a personal number from the reference doses above.

The relevant decision is whether the information gained is likely to affect diagnosis, staging, or treatment enough to justify the exposure. A clinically indicated scan should not be declined solely because the CT portion uses radiation; instead, ask whether the lowest protocol that still answers the medical question is being used. Your nuclear medicine physician or medical physicist can calculate or estimate dose for the actual tracer and CT settings.
Questions to take to your imaging appointment
- What exact tracer and administered activity are planned?
- Is the CT low-dose, diagnostic, or both?
- Will contrast be injected, and do I need kidney-function testing?
- What side effects are associated with this specific product?
- What should I do if I have a medication or food allergy?
- How long will the uptake and scanning stages take?
- What findings might be false positive or false negative in my case?
- Which other test would be considered if the result is indeterminate?
- How will this scan change the next treatment decision?
In short, PSMA PET/CT is generally a carefully controlled diagnostic examination with two distinct radiation sources and tracer-specific adverse effects. The safest interpretation of any dose figure is as a reference for a named agent and protocol—not as your personal radiation dose, cancer probability, or a substitute for a discussion with your care team.
