IntakeQ is cloud-based client intake and practice-management software for clinicians, allied health practitioners, and healthcare practices. Its core workflow covers customizable online forms, conditional logic, electronic signatures, file attachments, secure document sharing, client communication, and portal access. Forms can be sent by email or SMS and completed on any device. Practices that need more than intake can add appointment scheduling, reminders, payments, invoicing, telehealth, and booking tools through Practice Management plans.

Pricing is organized around Forms Only and Practice Management plans. Forms Only low volume costs $29.90 per month and includes unlimited form templates, branded custom forms, attachments, client and provider e-signatures, secure document sharing, and a HIPAA BAA. Forms Only costs $49.90 per month and adds treatment notes. Practice Management low volume costs $54.90 per month and adds appointment management, a booking widget, automated intake, a secure client portal, payments and invoices, and telehealth. The published plans include a 14-day free trial, while the full Practice Management plan adds complete appointment management alongside the same listed capabilities.

The product’s main strength is breadth: conditional forms and adaptive workflows sit alongside signatures, document handling, messaging, scheduling, reminders, payments, invoicing, and telehealth. Its REST API and webhooks support custom connections, while integrations include Dropbox, Google Drive, Zapier, RevolutionEHR, Elation Health, and SRFax. That scope suits established practices that want intake and operational workflows in one web-based system. A smaller practice seeking only simple forms may find the broader Practice Management feature set unnecessary, while organizations requiring a free plan should look elsewhere.
