Formgic review
A WordPress plugin for branching, media-rich forms with annual and lifetime plans.

Formgic is a self-hosted WordPress plugin for presenting form questions one at a time in a chat-style interface. It is aimed at website owners, freelancers, small businesses, agencies, marketers, and online stores that want conversational flows embedded on WordPress pages. A visual drag-and-drop builder combines input fields with conditional routing, making it more suited to guided, branching forms than to a basic static contact form.
The builder covers multi-branch logic with AND/OR conditions, more than 20 input fields, and rich-media chat bubbles for images, video, audio, and GIFs. A visual formula builder can show live calculation results, while submission tracking, partial-entry capture, and CSV export support response management. Shortcode and WordPress editor integrations provide embedding options. Form responses are stored in the site's WordPress database, so the self-hosted model fits organizations that want their forms within their WordPress environment; it is not a fit for teams seeking a hosted form service independent of WordPress.
According to the vendor's pricing page, annual plans start with Starter at $59 per year, Growth at $149 per year, and Agency at $259 per year. Each includes unlimited form flows and chat responses, all features and integrations, with Premium, Priority, and VIP support respectively. Lifetime options cost $99, $249, and $499 one time for those same tiers and support levels. There is no free plan. Integrations include Mailchimp, Slack, Google Sheets, Zapier, HubSpot, WooCommerce, and webhooks, among others. Formgic makes sense for WordPress users who need logic-rich flows and are comfortable managing a self-hosted plugin. Those who need a free tier or a non-WordPress deployment should look elsewhere.
