For most men who can safely take them, prescription phosphodiesterase type 5 (PDE5) inhibitors are the usual first treatment for erectile dysfunction (ED). They are not automatically “better” than a vacuum erection device, injections, or counseling. The best option depends on the cause of ED, other medicines and health conditions, how often you want sex, your tolerance for devices or needles, and whether anxiety or relationship stress is involved.
Vacuum devices offer a drug-free approach, injections can work when pills fail, and counseling or cognitive behavioral therapy (CBT) treats psychological and relationship contributors. A clinician should evaluate ED before you choose among them, because ED can accompany diabetes, obesity, medication effects, smoking, alcohol use, anxiety, stress, and cardiovascular disease risk.

The American Urological Association (AUA) recommends a medical, sexual, and psychosocial history, a physical examination, selective laboratory testing, and measurement of morning total testosterone. The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) describes history and examination, blood tests, ultrasound or other tests when indicated, and an office injection test in selected cases.

This evaluation helps distinguish medication effects, hormonal issues, vascular disease, neurologic problems, penile conditions, and psychological contributors. It also identifies contraindications and interactions that can make a treatment unsafe. Do not self-treat persistent ED without reviewing your health and medicines with a qualified professional.
Why ED deserves a health review
ED may occur with diabetes, obesity, smoking, alcohol use, cardiovascular disease risk, medication effects, anxiety, or stress. Treating the erection symptom without addressing a contributing condition can leave the underlying problem untreated. A clinician can also clarify whether testosterone testing, additional cardiovascular assessment, or a referral is appropriate.
Prescription ED pills (PDE5 inhibitors)
The AUA says men with ED should be informed about an FDA-approved oral PDE5 inhibitor unless contraindicated. U.S. examples include sildenafil, tadalafil, vardenafil, and avanafil. The 2024 international consensus also describes PDE5 inhibitors as first-line treatment for most men.

Why pills are often the starting point
PDE5 inhibitors are non-invasive and usually easier to plan than a device or injection. They can support a more natural-feeling sexual encounter, but they do not create an automatic erection: sexual stimulation is required. A clinician must review your medical history and medicines before prescribing because contraindications and interactions can make an oral drug unsafe.

How to use a pill safely
- Complete the medical and sexual evaluation, including medication review and any testing your clinician recommends.
- Take only the prescribed medicine and dose. Do not combine products or increase the dose on your own.
- Allow time for the medicine to work as directed and include sexual stimulation; a pill without stimulation may appear ineffective.
- Record what happened across several properly timed attempts, including dose, timing, stimulation, erection quality, and side effects.
- Discuss the result with the prescriber. Dose titration, a different PDE5 inhibitor, or another treatment may be appropriate.
Why a pill may seem to fail
Apparent failure can result from incorrect timing, insufficient sexual stimulation, an unsuitable dose, unrealistic expectations, or an untreated medical or psychological contributor. The answer is not automatically to take more. A clinician can check technique, adjust the dose, reassess the diagnosis, and decide whether a VED, ICI, counseling, or a combination is safer and more useful.
Trade-offs
The principal burdens are screening for contraindications and interactions, possible side effects, and variable response. Pills may be less attractive when you want a drug-free method, cannot use oral medication, or prefer a treatment that acts locally. They also do not directly address performance anxiety, relationship conflict, depression, or other emotional factors; those may require counseling in addition to medical treatment.
Vacuum erection devices (vacuum pumps)
A VED places the penis in a cylinder and uses negative pressure to draw blood into the erectile tissue. A constriction ring helps maintain the erection for intercourse. It is non-invasive and drug-free, and devices may be available over the counter or by prescription.

These figures come from different evidence summaries and should not be read as a guarantee for an individual user. Satisfaction depends on comfort, dexterity, expectations, partner preference, and the cause of ED.

Using a VED
- Choose a device that includes a vacuum limiter. The AUA recommends using only VEDs with this safety feature.
- Follow the manufacturer’s instructions for lubrication, cylinder placement, and pressure. Increase suction gradually rather than forcing a seal.
- When an erection is adequate, place the constriction ring at the base as instructed.
- Use the device for the time specified in its instructions. Remove the ring within that limit; do not leave it in place longer than directed.
- Stop if you develop significant pain, marked swelling, severe discoloration, or other concerning symptoms, and contact a clinician.
Common problems and who needs medical advice first
Transient petechiae or bruising, discomfort or pain, numbness, difficulty ejaculating, and difficulty operating the device are reported problems. Men taking anticoagulants, those with bleeding disorders, or anyone with a history of priapism should obtain clinician advice before use. A VED may be a good alternative when pills are contraindicated, but “drug-free” does not mean risk-free.
When a VED is a practical choice
It can suit someone who wants to avoid systemic medicine, has a contraindication to oral treatment, or values a reusable option that works on demand. The mechanical setup and reduced spontaneity are the main trade-offs. A partner’s comfort and willingness to incorporate the device can strongly affect satisfaction.
Intracavernosal injections (ICI)
ICI places medicine directly into the erectile tissue. Alprostadil alone is commonly used; compounded combinations are also used in practice. ICI may be considered when oral drugs are contraindicated, ineffective, or unwanted.

How ICI treatment is started
- Have an in-office injection test so a clinician can observe your response and identify a starting dose.
- Receive hands-on instruction about sterile technique, injection location, dose measurement, storage, and disposal.
- Use only the prescribed dose. Do not repeat an injection because the first response seems slow unless your clinician has given explicit instructions.
- Keep a record of erection duration, quality, pain, and any skin or tissue changes so the prescriber can titrate treatment safely.
- Ask what to do if the erection lasts too long. An erection lasting four hours or longer is an emergency because of priapism risk.
Risks and regulatory considerations
Possible complications include penile pain, fibrosis or plaque, and priapism. Correct dosing is essential. Non-alprostadil injectable combinations may not be formally FDA-approved for ED in the United States, so ask exactly what medicine is being prescribed, whether it is approved for this use, and how it is prepared.
Who may prefer ICI
ICI can be appropriate for a well-informed man who needs a dependable response after pills have failed or cannot be used, and who accepts needle use and training. It is less attractive if you cannot manage the technique, are unwilling to inject, or cannot reliably follow dosing instructions.

ED is not “imaginary” when emotional or relationship factors are involved. Anxiety, stress, depression, performance worries, communication problems, and relationship strain can interfere with arousal and erection even when blood flow and nerves are otherwise adequate. Physical and psychological causes frequently overlap.
When counseling is appropriate
NIDDK says health professionals may suggest counseling when mental-health or emotional issues affect ED. Counseling can be the main treatment when psychological causes predominate, or it can accompany pills, a VED, or ICI when causes are mixed.

What CBT-oriented treatment may address
- Fear of losing the erection and the cycle of monitoring performance.
- Anxiety, depression, chronic stress, or shame that reduces sexual interest or arousal.
- Communication, desire differences, conflict, and rebuilding intimacy with a partner.
- Unhelpful beliefs about masculinity, penetration, or what counts as successful sex.
- Practical strategies for gradual, lower-pressure sexual activity while medical treatment is optimized.
The EAU guideline states that integrating CBT with pharmacological treatment has empirical efficacy and is regarded as best practice. Improvement depends on the accuracy of the assessment, the provider’s qualifications, and your engagement over time; counseling is not an instant substitute for treating a vascular, hormonal, neurologic, or medication-related cause.
Choosing one treatment or combining them
Use the following decision sequence with your clinician:
- Check safety first. Review health conditions, medicines, bleeding risk, prior priapism, and cardiovascular status.
- Identify the dominant contributors. Physical findings and laboratory results may point toward medical treatment; anxiety, stress, or relationship strain may point toward counseling; many men have both.
- Start with the least burdensome effective option you can safely use. For most eligible men, that is a PDE5 inhibitor. A VED is a non-drug alternative, and ICI is a clinician-trained option when oral treatment is unsuitable or ineffective.
- Reassess technique before declaring failure. Confirm correct pill timing and stimulation, proper VED setup and limiter use, or correct ICI dose and injection technique.
- Add treatment for a second cause when needed. Counseling can complement a pill, VED, or ICI when anxiety or relationship factors remain; medical treatment is still needed when a physical contributor is present.
- Choose the method you and, when relevant, your partner can use consistently. A theoretically effective treatment is of little value if its setup, timing, cost, needle use, or side effects make you avoid it.
Safety rules that should not be compromised
- Do not start or change a PDE5 inhibitor without clinician review of contraindications and medication interactions.
- Use only a VED with a vacuum limiter, and seek medical advice first if you take anticoagulants, have a bleeding disorder, or have had priapism.
- ICI requires an in-office test, dose instruction, and titration. Never improvise the dose or combine injections unless specifically directed.
- An erection lasting four hours or longer requires emergency care because of priapism risk.
- Seek help for severe pain, substantial bruising, new penile curvature or plaque, marked swelling, or persistent numbness.
- Do not buy prescription pills or injectable medicines from unverified sellers; use a licensed prescriber and a legitimate pharmacy.
Bottom line for the common questions
Are ED pills better than a vacuum pump?
Not for everyone. Pills are usually the first-line option for men who can safely take them and want a simple, relatively spontaneous treatment. A VED is a strong alternative when you prefer to avoid drugs or cannot use oral medicine, provided you use a device with a vacuum limiter and accept the mechanical setup.

Do injections work when Viagra or Cialis fails?
They can. ICI studies summarized by the AUA reported successful-intercourse percentages from 53.7% to 100%, and the EAU reports alprostadil efficacy above 70% in general ED populations. Injections require office training, careful dose titration, and readiness to respond to priapism or other complications.
Can counseling help erectile dysfunction?
Yes, when anxiety, stress, depression, relationship strain, or another psychological factor contributes. Counseling or CBT may be used alone for predominantly psychological ED or combined with medical treatment for mixed causes. It treats a genuine contributor and is not a suggestion that the symptoms are fabricated.
