Choose managed PostgreSQL if the exact extensions and versions your application needs are supported by the service, and its permissions, configuration, upgrade process, and operational limits fit your workload. Choose self-managed PostgreSQL when you need server-side extension files, administrative control, or configuration the service does not expose—and are prepared to own that control. The deciding factor is not whether PostgreSQL supports an extension in general; it is whether your chosen deployment supports the extension version and the way your application uses it.
Start with the extensions your application actually needs
Before comparing deployment models, make an inventory of extensions that the application requires. For each one, record the extension name, required version, PostgreSQL major version, installation privileges, configuration requirements, and any dependencies on replication, failover, backup, or upgrade behavior. This turns “Does this database support extensions?” into the more useful question: “Can this service run these extensions, in these versions, with the permissions and behavior this workload requires?”
PostgreSQL’s CREATE EXTENSION reference describes how an extension is installed into the current database. PostgreSQL runs the extension’s installation script and records the objects it creates. The supporting files must already be installed on the database server. As a result, having permission to run the SQL command is not sufficient when the server does not have the extension’s files.

What “managed PostgreSQL” changes—and what it does not
In a managed service, the provider operates the database service, but the customer does not necessarily receive PostgreSQL superuser access or unrestricted control of the host. Instead, the provider documents supported extensions, database roles, approved configuration options, and service-specific procedures. The exact limits vary among providers and can also vary with the PostgreSQL major version.
Self-managed PostgreSQL gives the operator control over the server packages, extension files, configuration, and administrative access. That control is useful when an extension’s requirements are outside a managed service’s supported set. It also makes the operator responsible for installing compatible files and handling both the extension’s server-side binaries and its database-level version. How much control—and how much work—self-management entails depends on how the server is operated; the deployment label alone does not establish staffing, cost, performance, or uptime outcomes.
| Decision area | Managed PostgreSQL | Self-managed PostgreSQL |
|---|---|---|
| Extension availability | Limited to the provider’s supported extensions and PostgreSQL-version matrix. Confirm the exact extension version. | The operator can install compatible supporting files for the selected PostgreSQL build and operating system. |
| Privileges and configuration | Provider roles, allow-lists, approved libraries, and configuration options apply. | The operator controls administrative access, server packages, and configuration, subject to how the environment is operated. |
| Extension updates | The provider may make newer binaries available while requiring a separate database-level extension update. | The operator owns the binary installation and database catalog update process. |
| Operational work | The provider operates the database service; the team still needs to check extension-specific setup and service limitations. | The team or infrastructure provider owns the database host and extension lifecycle. |
| Portability | Dependencies on provider-specific behavior can complicate a move. Verify the destination’s support and behavior. | Greater installation control can help with unusual dependencies, but the operator takes on more responsibility. |
The operational-work and portability distinctions are practical implications of the installation and service models, not published comparative measurements. No universal cost, performance, uptime, or staffing advantage follows from this comparison alone.

PostgreSQL itself: files, database privileges, and trusted extensions
PostgreSQL requires an extension’s supporting files to be installed on the server before CREATE EXTENSION can install it in a database. Many extensions require superuser privileges. An extension marked trusted can instead be installed by a user with CREATE privilege on that database, as described in the PostgreSQL reference. These are PostgreSQL’s general rules; they do not guarantee that a particular service exposes the files or grants the required provider role.

Amazon RDS for PostgreSQL
AWS publishes supported extensions and versions by PostgreSQL version. Check the RDS supported extension versions for the major version you plan to run; do not treat a match on one RDS engine version as proof of availability on another.
AWS documents that PostgreSQL 13 and later allow users with database CREATE permission to install trusted extensions. Other extensions generally require the rds_superuser role. AWS also documents rds.allowed_extensions as a control for limiting extension installation. Read the service’s RDS extension instructions alongside the version list: the list answers which versions are supported, while the instructions explain relevant installation controls.
Azure Database for PostgreSQL Flexible Server
Microsoft says Flexible Server supports a subset of existing PostgreSQL extensions and does not allow customers to bring their own extensions. Its documented process includes adding an extension to the allow-list; some extensions also require shared_load_libraries configuration. Untrusted extensions require azure_pg_admin; trusted extensions listed in azure.extensions can be created by a user with database CREATE permission. Consult Microsoft’s Flexible Server extension instructions for the service’s current allow-listing and setup procedure.

Google Cloud SQL for PostgreSQL
Google says Cloud SQL permits only listed extensions and that users cannot create their own extensions. Its documentation says extensions can be created by members of cloudsqlsuperuser. Extension creation takes place on the primary instance, and the extension replicates to read replicas. Those details make it important to validate not just whether an extension installs, but also how your topology uses it. See Google’s Cloud SQL extension documentation for the supported set and service instructions.

Google’s documentation also distinguishes server binaries from the extension’s database catalog version: maintenance installs current extension binaries, but does not automatically update the database catalog. Customers should check installed versions and use ALTER EXTENSION when an update is appropriate. A provider maintenance event therefore does not, by itself, establish that the extension is at the desired database-level version.
Why major-version checks matter
Support can change by PostgreSQL major version. For example, Google’s cited support table lists PostGIS 3.5.2 for PostgreSQL 13–17 and PostGIS 3.6.0 for PostgreSQL 18. This is a version-specific example from the Cloud SQL support table, not a timeless guarantee that all Cloud SQL instances have those versions. Check the live matrix for the intended engine version when making a deployment decision.
Use this verification process before choosing a deployment
- Inventory extension dependencies. Record every extension the application needs, including required versions and whether it is essential in production or only used by development, administration, or a migration. Confirm the PostgreSQL major version the workload will run.
- Check the provider’s current support matrix. Search the exact managed service’s extension list for each required extension and version. Confirm the entry applies to the intended PostgreSQL major version and service configuration. If the documentation lists an extension but not the version you need, treat that version as unconfirmed rather than assuming compatibility.
- Verify how the extension can be installed. Check whether it is trusted, which database privilege or provider role is required, and whether an allow-list must be changed. Confirm the required role can be used in the environment where deployment will run; a command that works for a database administrator may not work for the application’s migration role.
- Identify extra server configuration. Look for requirements such as approved libraries, flags, or
shared_load_libraries. Establish whether the provider permits the setting and whether it requires a service change or restart according to that provider’s instructions. Do not assume that the ability to create the extension proves its configuration is complete. - Test the full lifecycle in a representative environment. Install it in a non-production database using the same service, PostgreSQL major version, role model, and configuration you intend to use. Exercise the extension’s application paths, and validate the backup, restore, replication, failover, and upgrade behavior relevant to your architecture. Cloud SQL specifically documents service-operation incompatibilities for some extensions, so installation success alone is not a complete compatibility test.
- Write down the upgrade procedure. Establish who installs or receives updated extension binaries, who checks the database catalog version, and when
ALTER EXTENSIONshould be run. Verify the supported sequence for the service and extension rather than assuming that a server maintenance update also updates database objects. - Recheck at change points. Revisit the matrix when changing PostgreSQL major version, moving providers, enabling a new topology, or adopting an extension upgrade. Support claims are tied to a service and engine version, not merely to the product name “PostgreSQL.”
When managed PostgreSQL is the better fit
A managed service is a strong candidate when every required extension and version appears in the service’s current matrix and the documented installation and operation model fits the application. That means more than seeing the extension name on a list: the team should be able to obtain the necessary role, set required flags, update the extension when appropriate, and operate the application within any documented service limitations.
- The required set is fully supported. There is no dependency on extension files or versions the provider does not make available.
- The permissions match the deployment workflow. The role that runs database setup can install or update the extension under the service’s rules.
- The configuration is exposed. Any allow-list, flag, or library requirement can be met through the provider’s supported process.
- The service behavior fits the topology. Replication, failover, backups, and upgrades work with the extension in the way the workload requires.
- The team accepts the provider’s lifecycle. The team has a clear process for checking versions and applying database-level extension updates when needed.
In this case, choosing self-managed PostgreSQL merely to obtain theoretical control does not solve an identified extension constraint. The useful decision is whether the supported service meets the workload’s verified requirements, not whether one model sounds more flexible in the abstract.

When self-managed PostgreSQL is the better fit
Self-management is worth considering when a required extension cannot be installed or operated within the chosen service’s documented limits. The clearest cases are a missing extension, an unavailable required version, inaccessible supporting files, a need for server configuration the service does not expose, or an administrative requirement that the provider role model cannot satisfy.
- A required extension is absent from the provider’s list. PostgreSQL’s ability to support an extension does not make it installable on a managed service that lacks its files.
- The available version does not meet the requirement. A different PostgreSQL major version may have a different support matrix, but changing engine versions is not a solution unless the resulting combination is supported and acceptable to the workload.
- The extension requires control the provider withholds. This may include server-side files, privileges, or configuration that the service does not allow customers to supply.
- Provider-specific limits conflict with required behavior. A service may permit installation yet still impose restrictions affecting replication or other operations the application depends on.
- The team can own the additional lifecycle. Someone must maintain compatible server files, database extension versions, and the surrounding database operations.
Self-managed PostgreSQL is not a way to eliminate extension compatibility work. It changes who controls and performs that work. The operator must ensure the extension files match the PostgreSQL build and operating system, manage configuration, and coordinate binary and catalog updates.
Plan upgrades and portability as separate checks
Keep binary availability separate from catalog updates
An extension has server-side supporting files and database objects installed through PostgreSQL. Those are related but distinct parts of its lifecycle. The Cloud SQL guidance is a concrete warning against treating provider maintenance as the complete update process: Google says maintenance installs newer binaries but does not automatically update the database catalog. Check the installed version and follow the applicable instructions for ALTER EXTENSION when an update is appropriate. For RDS, Azure, and other services, consult that service’s procedures rather than assuming the Cloud SQL behavior applies identically.
Before an upgrade, determine which extension versions are available for the target PostgreSQL major version, whether the extension has a supported upgrade path, what permissions are needed, and whether the change affects dependent application objects. The cited provider documentation establishes support and service procedures, but it does not provide a universal upgrade sequence for every extension. Use the relevant extension and provider instructions for the particular combination.

Test the destination before a provider move
Portability depends on more than PostgreSQL’s SQL compatibility. A destination must provide the extension and version, support the necessary setup and privileges, and behave suitably for the workload’s operations. Provider-specific configuration or role assumptions can become migration dependencies. Before moving, test extension installation and application behavior on the destination, then validate the operational procedures that the source environment relies on. Broader control on self-managed infrastructure can reduce reliance on one provider’s allow-list, but it does not remove the need for a compatible build and a tested migration plan.
A practical decision rule
Make the choice in this order: first verify the exact extension-and-version set; then confirm roles, configuration, and lifecycle procedures; then test operational compatibility. If the managed service passes those checks, it is a suitable choice on the evidence of the workload’s requirements. If a required condition fails because the provider does not expose the needed files, permissions, configuration, or behavior, self-managed PostgreSQL may be necessary—provided the team is willing to take responsibility for the extension lifecycle.
Do not choose a deployment model based on a generic claim that managed databases are always easier or that self-hosting is always more flexible. The decision depends on the workload’s actual extension dependencies and the service’s current, version-specific rules.
