A polygenic risk score (PRS) combines the tiny effects of many inherited genetic variants into one estimate of susceptibility to a disease or trait. It can rank risk relative to a reference population, but it cannot diagnose disease or tell you what will happen to you. Its practical value depends on the disease, the score’s validation, your ancestry and baseline risk, and whether a proven follow-up action is available.
What is a polygenic risk score?
A polygenic risk score, also called a polygenic score, is a numerical summary of inherited genetic variation. Researchers identify variants associated with a disease or trait, estimate the effect of each variant, and combine those effects into a single score. A score may include thousands or millions of variants, each usually contributing a very small amount.
The result is probabilistic. Most reports express it as a percentile or as a comparison with a reference group. For example, a person might be placed above 90% of the reference population for a particular disease-related score. That ranking means the person has a higher estimated genetic susceptibility than the people in that comparison group. It does not mean a 90% chance of developing the disease.
PRSs describe inherited susceptibility, not current health. They do not measure symptoms, an existing tumor, blood pressure, cholesterol, infections, environmental exposures or lifestyle. Those factors can materially change a person’s actual risk.

How a PRS is calculated
Variant discovery
Most scores begin with genome-wide association studies (GWAS), which compare genetic variants in people with and without a defined disease or trait. The quality of the score depends partly on how the disease endpoint was defined, how large and representative the discovery cohort was, and whether the associations were reproduced.
Weighted combination
Each selected variant receives a weight based on its estimated association with the outcome. The scoring method combines a person’s genotype at those variants with the corresponding weights. Variants with larger estimated effects contribute more than variants with smaller effects, but the final number still represents a statistical estimate rather than a biological certainty.
Reference distribution and percentile
A raw score becomes easier to communicate when compared with a reference distribution. The same raw value can produce a different percentile if the reference population changes. A percentile therefore needs its reference population and score version attached to it.

What PRSs can tell you
Relative genetic ranking
A validated score can place people into relative-risk groups, such as lower, intermediate or higher genetic susceptibility. This ranking can help researchers or clinicians decide who might benefit from closer attention when the score has been tested for the intended disease, population and decision.
Additional information beyond conventional factors
In some settings, a PRS adds predictive information to established factors such as age, sex where relevant, family history and clinical measurements. The gain is not automatic. It must be demonstrated in an independent validation population and judged against the risk model or care pathway already in use.
Screening and prevention planning
A score can support research, screening prioritization or prevention planning when a clear, evidence-based action follows. For example, a health system might study whether a validated risk group should receive a different screening schedule. The score itself is not the intervention; its usefulness depends on what happens after the result.

Access matters. NIH reporting on diverse genomic data emphasizes that even optimized scores cannot address health disparities alone. People also need access to recommended screening, prevention and follow-up care. A result that cannot lead to appropriate care has limited practical value.

What a PRS cannot predict
It cannot guarantee disease or health
A high score does not mean that a person will develop cancer, heart disease or another condition. A low score does not eliminate risk. Both groups include people who do and do not develop the disease because genetic susceptibility is only one component of the outcome.
It cannot diagnose disease
A PRS generally cannot establish that disease is present. Diagnosis requires the relevant clinical evaluation, which may include symptoms, examination, imaging, laboratory tests, pathology or other disease-specific evidence.
It cannot replace clinical information
Family history, environmental exposures, lifestyle, symptoms, examination findings, laboratory data and established clinical risk calculators remain relevant. A score should be interpreted alongside those factors, not used to overrule them without evidence.
It cannot turn a percentile into a personal probability
A percentile is a relative position in a reference distribution. It is not an absolute probability. The same percentile can correspond to different event rates in populations with different baseline risks, age structures, disease prevalence or clinical profiles.
How accurate are polygenic risk scores?
There is no single accuracy number for PRSs. Performance depends on the disease, the score’s variant and GWAS sources, the validation cohort, ancestry, endpoint definition and the threshold used to label someone “high risk.” A responsible report should identify discrimination, calibration, sensitivity, specificity, predictive values and absolute-risk estimates for the intended population.

Illustrative performance figures
| Example | Reported result | Qualification |
|---|---|---|
| Typical PRS performance summarized by BMJ | 10–15% sensitivity at 95% specificity | BMJ commentary published in 2023, summarizing a 2022 preprint; this is a broad example, not a universal PRS standard. |
| One breast-cancer example | 39% sensitivity and 78% specificity | Targeted women above a 17% lifetime-risk threshold; reported in the same BMJ commentary summarizing a 2022 preprint. |
Sensitivity is the share of future cases identified by a chosen threshold. Specificity is the share of people who will not develop the disease who are correctly left below that threshold. A threshold with high specificity can still miss many future cases when sensitivity is modest. Conversely, a broader threshold can identify more cases while producing more false positives.
Predictive values also depend on baseline risk. Even a technically well-performing score can have a low positive predictive value when the disease is uncommon in the population being tested. Calibration addresses whether predicted risks match observed event rates in the target population. A score can rank people reasonably well yet provide poorly calibrated absolute probabilities.
Relative risk versus absolute risk
Relative ranking answers, “Where does this person fall compared with the reference group?” Absolute risk answers, “What is the estimated chance of an event over a stated time period?” Clinical decisions generally require the second question.

Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →Converting a PRS into absolute risk requires a suitable baseline-risk model. Depending on the disease, that model may include age, sex, family history, clinical measurements and other established predictors. The conversion must be validated for the population in which it will be used. Applying a percentile from one ancestry or age distribution to a different baseline-risk setting can produce a misleading probability.
Reports should state the time horizon and endpoint, such as a defined lifetime or 10-year risk, rather than presenting an unqualified percentage. They should also identify whether the estimate describes first diagnosis, a particular disease subtype or another endpoint.
Ancestry, transferability and equity
Many PRSs were developed using cohorts with predominantly European genetic ancestry. A score built in one ancestry group may perform less well in people with different or complex ancestries because linkage patterns, allele frequencies, environmental contexts and baseline risks can differ.

Lower transferability can reduce discrimination or calibration and may widen existing health disparities if a score is used to allocate screening or prevention unevenly. Ancestry is not a simple biological label for an individual, and people may have admixed or underrepresented backgrounds. The relevant question is whether the score was evaluated in a population sufficiently similar to the person and the intended clinical setting.

Clinical validity is not clinical utility
Clinical validity
Clinical validity means that a score is associated with, and can predict, a defined outcome under specified conditions. It is demonstrated through appropriate development and validation studies, with transparent endpoints, populations and performance measures.
Clinical utility
Clinical utility asks a harder question: does using the score improve decisions or health outcomes compared with existing care? A score may be statistically associated with disease yet fail to change management, improve outcomes or justify its costs and potential harms.
Evidence needed for adoption
Routine use generally calls for independent validation, prospective studies, transparent methods, cost-effectiveness analysis and a defined action pathway. The action might be a guideline-supported screening or prevention option, but it must be shown that acting on the score helps more than it harms. NIH’s reporting framework contains 22 items intended to improve transparency and reproducibility. As NIH quoted co-senior author Michael Inouye, Ph.D., “Without that transparency and reproducibility, efforts to use polygenic risk scores may be undermined.”

How to interpret a PRS report
- Identify the exact outcome. Check whether the score concerns a specific disease, subtype, age range, severity measure or broader trait. Similar-sounding endpoints are not interchangeable.
- Record the score version. Note the release or version identifier, variant set, weighting method and date. A later version may not be comparable with an older report.
- Check development and validation populations. Ask which ancestries, cohort sizes and clinical settings were represented. Look for independent replication rather than performance only in the discovery dataset.
- Examine calibration and discrimination. A report should show how well the score separates people with different outcomes and whether its predicted probabilities match observed rates in the intended population.
- Convert ranking to absolute risk. Ask which baseline-risk model was used, the time horizon, and whether age, sex where relevant, family history and clinical factors were included.
- Read the threshold definition. Find out why a percentile or risk cut-off was chosen, how many cases it captures, and how many people below or above it may be misclassified.
- Define the action before acting on the result. A result should connect to an evidence-based screening or prevention decision. If no validated action exists, the score may be informative but not clinically actionable.
- Discuss uncertainty with a qualified professional. A genetic counselor or clinician can place the score alongside family history, symptoms and established risk tools without treating it as a diagnosis.
What to do with a high, intermediate or low score
If the score is high
Do not assume disease is inevitable. Confirm what the high category means, including the reference population and absolute-risk estimate. Ask whether guidelines support earlier, more frequent or otherwise different screening, and whether the recommendation applies to your age, family history and clinical profile.

If the score is intermediate
An intermediate result often cannot determine management by itself. The decision may depend more on established risk factors, symptoms, family history and clinical measurements. Ask whether the score meaningfully changes the estimate compared with the existing risk model.
If the score is low
A low score does not cancel symptoms, family history or routine prevention. Continue care recommended for the general population and seek evaluation for concerning symptoms. Genetic susceptibility is only one part of disease risk.
How to compare two PRSs
Two scores for the same disease are not automatically interchangeable. Compare them on the following points:

Common interpretation mistakes
- Calling a percentile a probability: a 95th-percentile result is not a 95% chance of disease.
- Using a universal accuracy claim: performance figures apply to a specified disease, cohort and threshold.
- Ignoring ancestry and baseline risk: a score may not transfer reliably to another population.
- Treating association as actionability: statistical validity does not prove that changing care improves outcomes.
- Replacing established risk assessment: a PRS should complement, not automatically displace, clinical information.
- Assuming a low score is protective: people with low genetic susceptibility can still develop disease.
- Ordering a test without a follow-up plan: interpretation has value only when an appropriate next step is available.
Bottom line for readers
A PRS is best understood as one probabilistic input into risk assessment. It can improve stratification for selected diseases when independently validated, appropriately calibrated and linked to an evidence-based action. It cannot diagnose disease, guarantee an outcome or erase the effects of ancestry, family history, environment, lifestyle and access to care. Before acting on a result, verify the score’s population and version, ask for an absolute-risk estimate, and discuss the implications with a clinician or genetic counselor.
