Pneumonic plague is a medical emergency. It is a rapidly progressive lung infection caused by Yersinia pestis. Fever followed by cough, chest pain, shortness of breath, or bloody or watery sputum—particularly after plausible contact with infected animals, fleas, respiratory secretions, or travel to an affected area—requires immediate medical evaluation. Do not wait for symptoms to “see whether they pass,” and do not attempt to treat suspected plague with leftover antibiotics.
What pneumonic plague is
Pneumonic plague is the form of plague in which Yersinia pestis infects the lungs. It can develop when plague spreads to the lungs from untreated bubonic or septicemic disease, or when a person inhales infectious respiratory particles from a person or animal with pneumonic plague. Unlike bubonic plague, pneumonic plague can spread directly from person to person through respiratory particles.
The illness can worsen very quickly. The World Health Organization (WHO) reports that untreated pneumonic plague may lead to death in as little as 18–24 hours in severe cases. That figure describes a potential untreated outcome, not a prediction for any individual. Prompt professional treatment substantially changes the situation, which is why suspected cases are handled urgently.
Symptoms and how quickly they appear
Symptoms can begin soon after exposure. After inhalation, the incubation period may be as short as 24 hours; the Centers for Disease Control and Prevention (CDC) describes a usual incubation of about one to three days. A person can therefore become seriously ill only a short time after an exposure that initially seemed minor.

Early and characteristic symptoms
- Fever, often with chills
- Headache and marked weakness
- A rapidly developing pneumonia
- Cough, which may produce watery or bloody mucus
- Chest pain
- Shortness of breath
WHO also highlights sudden fever, painful swollen lymph nodes, and breathing symptoms with cough or bloody expectoration as warning signs. Swollen nodes are more typical of bubonic plague, but they can occur when infection has more than one form or is progressing.
Why the combination matters
These symptoms overlap with many other serious infections, so symptoms alone cannot confirm plague. The combination of abrupt fever, rapidly worsening respiratory illness, and a plausible exposure is what makes immediate assessment essential. Tell the clinician exactly when symptoms started, how quickly they changed, and whether anyone nearby is ill.
When to seek emergency care
Seek medical help immediately if you have a sudden febrile illness with cough, chest pain, shortness of breath, or bloody sputum and any realistic possibility of plague exposure. The CDC’s public guidance states: “If you are experiencing symptoms like those listed here, seek immediate medical attention.” Do not wait for a routine appointment or for laboratory confirmation.

Call emergency services rather than traveling alone when
- Breathing is severely difficult or rapidly deteriorating.
- You are confused, faint, unusually drowsy, or unable to stay awake.
- Your lips or face appear blue or gray.
- You are coughing blood and feel weak, dizzy, or unstable.
- You cannot reach urgent medical care safely without assistance.
When contacting a clinic or emergency service, say that pneumonic plague is a concern before arrival. Mention recent travel or residence in a plague-endemic area, contact with a person who has a severe respiratory illness, flea bites, handling of sick or dead animals, or contact with animal tissue or fluids. Advance notice allows the facility to arrange appropriate infection-control precautions.

What causes pneumonic plague and how it spreads
The bacterium and its animal cycle
Yersinia pestis circulates naturally among small mammals and their fleas. People may be infected after a bite from an infected flea, by handling infected tissue or body fluids, or by inhaling infectious particles. Plague remains a naturally occurring disease in some regions; risk is not limited to one country even though reported human cases from 2019 through 2025 were concentrated in the Democratic Republic of the Congo and Madagascar, according to WHO’s 2026 fact sheet.
Person-to-person respiratory spread
Pneumonic plague is the form capable of direct human transmission. A person with infection in the lungs can release infectious respiratory particles while coughing or during close contact. Transmission risk is therefore greatest around an untreated or newly treated patient with respiratory symptoms. Not every person with plague is contagious in the same way: bubonic disease is usually linked to flea exposure and swollen lymph nodes, while pneumonic disease is the respiratory-spread form.
Exposure situations that deserve disclosure
- Recent travel to or residence in an area where plague occurs.
- Close contact with someone who has suspected or confirmed pneumonic plague.
- Handling a dead or visibly ill wild or domestic animal.
- Contact with animal carcasses, tissues, blood, or other body fluids.
- Flea exposure in an endemic setting.
Exposure does not prove infection, but it changes how rapidly symptoms should be evaluated. Give public-health or medical staff a complete, accurate exposure history rather than trying to decide yourself whether an event “counts.”

These categories describe the dominant clinical presentation, not a guaranteed sequence. A case does not have to progress from bubonic to pneumonic disease, and a person can have signs of more than one form. Only a clinician and laboratory evaluation can determine what is occurring.

How clinicians diagnose it
Clinical assessment comes first
Clinicians assess the speed of illness, breathing status, physical findings, travel and exposure history, and possible contact with infected people or animals. Because pneumonic plague can deteriorate before a result is available, a strong clinical suspicion is itself a reason to begin urgent management.
Laboratory confirmation
Confirmation requires laboratory testing. Depending on the presentation, clinicians may collect blood, sputum, or material from a swollen lymph node (bubo) for testing. The exact tests and specimen handling are determined by the treating team and public-health laboratory; patients should not attempt to collect or transport samples themselves.
Treatment should not wait for the final report
CDC clinical guidance says treatment should not be delayed while laboratory results are pending when plague is strongly suspected. A negative early test or an uncertain initial result does not justify stopping care without a clinician’s direction, because the timing of collection and the test used affect interpretation.

Clinician-directed antimicrobial therapy
Plague is treated with prescription antibiotics selected by the medical team. The drug, route (such as oral or intravenous), duration, and monitoring plan depend on illness severity, age, pregnancy status, medical history, kidney and other underlying conditions, allergies, and local clinical guidance. Current CDC treatment tables are written for clinicians and may require individual adjustment. Do not self-prescribe, share medication, or use leftover antibiotics.

Supportive treatment
Patients with pneumonic disease may need oxygen, fluids, monitoring of breathing and circulation, and other hospital support. The level of support depends on how the lungs and other organs are functioning. A person who appears stable can still worsen quickly, so observation and repeat assessment are important.
Infection-control precautions
Suspected and confirmed pneumonic-plague patients are isolated and managed with respiratory droplet precautions. CDC emergency guidance says these precautions continue during the first 48 hours of antimicrobial treatment and until clinical improvement occurs or plague has been ruled out. The hospital and public-health authorities decide the appropriate room, protective equipment, testing, and duration. Patients and visitors should follow those instructions rather than choosing their own mask or isolation plan.
What to do while seeking care
- Call ahead. Tell the healthcare service that you have respiratory symptoms and possible plague exposure.
- Limit close contact. Stay away from household members and avoid crowded transport while arranging care.
- Follow instructions about masking. WHO notes that masks may reduce spread when properly used by sick people and may protect healthcare workers, but suitability depends on exposure and correct use.
- Cover coughs and dispose of tissues safely. Do not share cups, utensils, towels, or cigarettes.
- Do not handle animals or carcasses. Leave dead or sick animals alone and report them through local authorities where appropriate.
- Bring an accurate medication and allergy list. Include the time symptoms began and any recent antibiotics, but do not delay departure to gather paperwork.
These steps do not replace medical evaluation. They reduce avoidable exposure while professional help is being arranged.
Prevention in endemic or outbreak settings
- Avoid close contact with a person who is coughing during a suspected outbreak, and follow public-health instructions.
- Use measures recommended locally to prevent flea bites, especially around small mammals.
- Do not touch dead animals, carcasses, or animal tissues without appropriate professional guidance.
- Keep pets away from sick or dead wildlife and ask a veterinarian or public-health service about flea control.
- Report relevant travel and animal exposures promptly if illness develops.
There is no consumer test or home treatment that can safely rule out pneumonic plague. Prevention advice must be adapted to the local outbreak and the specific exposure, so public-health directions take precedence over internet checklists.

How dangerous is untreated plague?
WHO reports a 30%–60% case-fatality ratio for untreated bubonic plague and a 30%–100% range for plague overall when left untreated. Those figures are context for the seriousness of delayed care and are not a pneumonic-plague survival estimate for one person. WHO’s warning that untreated pneumonic plague can become fatal within 18–24 hours illustrates why respiratory symptoms require immediate action. Early, clinician-directed antibiotics and supportive care are the factors patients can act on; an individual prognosis depends on the form, severity, timing of treatment, age, and underlying health.
Questions people commonly ask
Can pneumonic plague spread between people?
Yes. Respiratory particles from a person with pneumonic plague can transmit infection, particularly during close contact. Healthcare teams use droplet precautions and isolation to reduce this risk.
Does every plague infection become pneumonic?
No. Bubonic, pneumonic, and septicemic plague have different dominant sites and transmission patterns. Pneumonic disease can result from inhalation or from spread of another form, but progression is not inevitable or uniform.

No. Diagnosis uses laboratory testing, but CDC guidance supports starting treatment when plague is strongly suspected rather than waiting for final results.
What should I do if I suspect I have plague?
Immediately contact a medical provider, explain the symptoms and exposures, and disclose recent travel from an affected area. If breathing is severely impaired or you are collapsing, call emergency services.
