Fever with a swollen lymph node does not, by itself, mean plague. The same combination can occur with tularemia, cat-scratch disease, ordinary bacterial lymphadenitis, mycobacterial infection and some sexually transmitted infections. What changes the level of concern is the combination of exposure, geography, timing, the node’s location and tenderness, skin or eye findings, and other symptoms.

Bubonic plague is uncommon but time-sensitive. It typically causes a rapidly developing, very painful regional lymph node (a bubo) after an infected-flea exposure, often in someone who lives in or recently visited a plague-endemic area. No symptom pattern can confirm or exclude it at home. If that history fits, seek medical care immediately rather than waiting for the lump or fever to resolve.

Why fever and a swollen node have many possible causes

Lymph nodes enlarge when immune cells respond to infection or inflammation in the area they drain. A neck node may react to an infection in the throat, mouth, eyes or scalp; a groin node may respond to a problem in the leg or genital area; an armpit node may reflect an infection in the arm or chest wall. Tenderness often indicates an active inflammatory response, but neither tenderness nor size identifies one organism.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The useful questions are:

  • Where and when did the fever begin?
  • Did the node enlarge quickly, and is it painful?
  • Was there a flea, tick or deer-fly bite, animal contact, cat scratch, wound or possible sexual exposure?
  • Have you lived in or traveled to a region where plague occurs?
  • Are there an ulcer, papule, pustule, eye inflammation, sore throat, cough or breathing problems?

These clues help a clinician choose tests and treatment. They are not a do-it-yourself diagnostic system.

Bubonic plague: the pattern clinicians must not miss

Typical symptoms and timing

According to the CDC, bubonic plague causes fever, headache, chills, weakness and one or more swollen, painful lymph nodes. After an infected flea bite, the usual incubation period is 2–8 days. The affected node is generally near where the bacteria entered the body. Common bubo locations are the groin, armpit or neck, although location varies with the entry site. See the CDC description of plague signs and symptoms.

A rapidly enlarging, intensely painful node accompanied by acute fever is more concerning than a small, mildly tender node that has been present during a routine cold. That distinction still cannot diagnose plague: other infections can produce an acute, painful node, and plague does not always present with a classic bubo.

Geography and exposure change the prior probability

Plague is plausible when symptoms occur in someone who lives in or recently traveled to the western United States or another plague-endemic area, particularly after a known flea bite or contact with a potentially infected animal. Tell the clinician about outdoor activity, rodent exposure, sick or dead animals and pets that may carry fleas. Absence of a remembered bite does not rule out exposure.

CDC’s public-health instruction is direct: “If you live or have recently traveled to the western U.S. or any other plague endemic area and have symptoms suggestive of plague, seek health care immediately.” The statement appears on the CDC diagnosing-plague page.

Plague can appear without the classic swollen node

Plague also has septicemic and pneumonic forms. Septicemic disease involves the bloodstream and may not produce a recognizable bubo. Pneumonic plague causes a severe respiratory illness; rapidly developing cough, chest pain or breathing difficulty with fever requires emergency assessment. Pneumonic plague is the form that can spread from person to person. Do not use the absence of a swollen node to dismiss a serious plague exposure.

Tularemia: an exposure-driven look-alike

Ulceroglandular and glandular disease

Tularemia is caused by Francisella tularensis. In the common ulceroglandular form, a tick or deer-fly bite or handling an infected animal introduces the organism through the skin. Fever and swollen regional nodes develop, and an ulcer at the entry site may appear. The ulcer is not inevitable. Glandular tularemia resembles ulceroglandular disease but lacks an ulcer, so a clean-looking skin surface does not exclude it.

Ask about hunting, skinning or handling rabbits and other wildlife, tick or deer-fly bites, and contact with sick or dead animals. Tularemia is rare and can be mistaken for more common infections, which is why a detailed exposure history matters. The CDC tularemia page also notes that ticks carrying tularemia are not the same ticks that transmit Lyme disease; do not treat every tick exposure as interchangeable.

Eye, throat and inhalational forms

The route of exposure changes the presentation. Inoculation into the eye can cause eye inflammation with nearby lymph-node swelling. Contaminated food or water can cause a sore throat or tonsillitis with neck nodes. Inhalation can produce a respiratory illness. All clinical forms are accompanied by fever. These variations explain why looking only for a skin ulcer can miss tularemia.

Cat-scratch disease: slower onset after cat exposure

Cat-scratch disease, associated with Bartonella henselae, commonly causes low-grade fever and tender enlarged lymph nodes. CDC describes node enlargement developing 1–3 weeks after cat exposure. A small papule or pustule at the scratch site can provide a useful clue, but it may be unnoticed or absent.

The delayed onset after a cat scratch and generally low-grade fever can differ from the abrupt, highly painful bubo pattern described for bubonic plague. That is a comparison of typical descriptions, not a diagnostic rule. A cat history should guide the clinical evaluation, not end it.

Other infections that can enlarge regional nodes

CDC’s plague-response training lists streptococcal or staphylococcal adenitis, tularemia, cat-scratch disease, mycobacterial infection and some sexually transmitted infections among the differential diagnoses for a bubonic-plague-like presentation. The list is illustrative rather than complete.

Routine bacterial adenitis

Streptococcal or staphylococcal infections in the skin, throat or a wound can cause fever and a tender node in the corresponding drainage area. Look for an obvious source such as a spreading red skin infection, pus, dental infection or sore throat, but do not assume that finding one automatically excludes another illness.

Mycobacterial infection

Mycobacterial disease can cause persistent lymph-node enlargement and systemic symptoms. The pace of illness, node texture and location, immune status and laboratory findings determine the work-up. A node that remains enlarged beyond the expected course of a minor infection deserves medical review even when fever has settled.

Sexually transmitted infections

Some sexually transmitted infections cause groin nodes or other regional lymphadenopathy. A confidential, nonjudgmental sexual history allows appropriate testing. Do not omit this possibility simply because there is no obvious genital lesion.

How location, timing and route fit together

The same-size node can mean different things in different contexts. A groin node after a leg wound, a genital symptom or a flea exposure is interpreted alongside those findings; an armpit node after an arm lesion or animal scratch raises a different set of questions; and a neck node with tonsillitis or eye inflammation points clinicians toward a nearby source. More than one enlarged-node region, a rapidly changing illness or severe systemic symptoms can alter the urgency and testing plan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Timing is equally useful but never decisive. Bubonic plague after an infected flea bite usually has a 2–8-day incubation period. Cat-scratch disease commonly produces tender nodes 1–3 weeks after cat exposure. Tularemia may follow a bite, animal handling, eye exposure, contaminated food or water, or inhalation, so the relevant interval and symptoms depend on the form. A person may not remember the exact day of a bite or scratch, which is why clinicians use the whole history rather than a single date.

How clinicians distinguish these illnesses

1. Reconstruct the timeline

Write down the first day of fever, when the node was noticed, how quickly it grew, and whether symptoms followed a bite, scratch, wound, animal contact or travel. A 2–8-day interval after a possible flea exposure raises a different set of questions than node enlargement 1–3 weeks after a cat scratch.

2. Map the node to the likely entry site

Clinicians examine the node and the skin, eye, mouth, throat and genital areas that drain to it. They assess tenderness, mobility, overlying redness, fluctuance and whether more than one node region is involved. A bubo is a swollen, painful node near the entry site, but similar regional inflammation occurs with other infections.

3. Check for focal signs and exposure-specific clues

  • An ulcer after a tick or animal exposure supports consideration of ulceroglandular tularemia, but its absence does not exclude glandular disease.
  • A scratch-site papule or pustule with delayed tender nodes supports consideration of cat-scratch disease.
  • Eye inflammation, tonsillitis, a wound, cough or breathing difficulty redirects the differential toward a particular route or form.
  • Flea, rodent, wildlife, tick, deer-fly, cat, food, water and sexual exposures each carry different implications.

4. Use laboratory testing rather than appearance alone

For suspected plague, CDC describes testing patient samples, especially blood or material from a swollen lymph gland. The public diagnosis guidance and the clinical testing guidance dated October 7, 2026 emphasize collecting pretreatment specimens when possible, while not delaying treatment when clinical suspicion is strong.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Testing choices for other infections depend on the suspected source and form. A clinician may sample blood, a node, an ulcer or another affected site, or order targeted tests based on the history. There is no reliable home symptom checklist or consumer test that safely rules plague in or out.

When to seek care—and how urgently

Seek emergency care now

  • Fever with rapidly worsening breathing difficulty, chest pain, confusion, collapse or severe weakness.
  • A rapidly enlarging, very painful node plus a plausible plague exposure or travel history.
  • Severe illness after contact with a potentially infected animal, flea exposure or an endemic-area visit.

Call ahead and mention the exposure and concern about plague so the facility can use appropriate infection-control procedures. Do not wait for a bubo to “declare itself.”

Arrange prompt same-day medical assessment

  • Fever and a new tender regional node after a tick or deer-fly bite, animal handling, cat scratch, contaminated food or water, or a significant wound.
  • Fever with an unexplained ulcer, eye inflammation, tonsillitis or spreading skin redness.
  • A node that is worsening, unusually painful or accompanied by systemic symptoms even without a known exposure.

What to tell the clinician

  1. State the fever’s highest measured temperature and the date symptoms began.
  2. Identify the node’s location, growth rate and tenderness.
  3. List travel, outdoor activity, flea or tick bites, deer-fly bites, animal contact, cat scratches, food or water exposures and sexual exposures.
  4. Report cough, chest pain, shortness of breath, eye symptoms, sore throat, skin ulcers or wounds.
  5. Bring a medication list and mention immune-system conditions or medicines.

If plague is strongly suspected, clinicians should obtain specimens when feasible and start appropriate treatment without waiting for final laboratory confirmation. Public-health authorities should be notified promptly, as described in the CDC clinical guidance.

What not to infer from a swollen node

  • “It is in the groin, so it must be plague.” Groin nodes drain many skin and genital conditions; location alone is nonspecific.
  • “There is no ulcer, so it cannot be tularemia.” Glandular tularemia can lack an ulcer.
  • “The cat scratch was weeks ago, so it cannot matter.” Cat-scratch disease nodes commonly appear 1–3 weeks after exposure.
  • “I do not remember a flea bite, so plague is impossible.” People may not notice bites, and exposure history is only one part of the assessment.
  • “The fever is mild, so urgent evaluation is unnecessary.” Severity, trajectory, exposure and respiratory symptoms matter more than one temperature reading.
  • “A normal-looking skin surface rules out an exposure infection.” Glandular tularemia may have no ulcer, and a cat-scratch papule can be tiny or already healed.
  • “One negative-looking symptom comparison settles it.” Plague diagnosis relies on clinical assessment and laboratory testing; no home checklist can safely exclude it.

Questions readers commonly ask

Can a swollen lymph node alone identify plague? No. A node is a nonspecific immune response. Plague becomes more concerning when a rapidly painful bubo occurs with acute fever and a compatible flea, animal or geographic history, but confirmation requires clinical evaluation and laboratory testing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Does every case of plague have a bubo? No. Septicemic plague may not produce a recognizable swollen node, and pneumonic plague is primarily a severe respiratory illness. Fever with rapidly worsening cough, chest pain or breathing difficulty is an emergency regardless of whether a bubo is present.

Does no remembered flea bite rule out plague? No. A person may not notice a bite. Clinicians consider residence or travel in an endemic area, contact with rodents or sick animals, pet-flea exposure and the overall symptom pattern.

How is tularemia different from plague? Both can cause fever and regional nodes after an animal or arthropod exposure. Tularemia may produce an entry-site ulcer, but glandular disease lacks one; eye, throat and inhalational forms have different local symptoms. Exposure route is therefore especially important.

When do cat-scratch disease nodes appear? CDC describes tender enlarged nodes developing 1–3 weeks after cat exposure, often with low-grade fever. A papule or pustule at the scratch site may help, but timing and a cat history do not prove the diagnosis.

What samples might be collected if plague is suspected? CDC describes blood or material from a swollen lymph gland. Clinicians should collect pretreatment specimens when feasible, but treatment should not be delayed while awaiting final results if suspicion is strong.

Should I test or treat myself at home? No consumer symptom test can safely rule plague in or out, and choosing antibiotics without an examination can miss the correct diagnosis. Seek urgent professional care when the exposure and symptoms are concerning.

Why does the clinician ask about sex, food and water? Some sexually transmitted infections cause regional lymphadenopathy, while contaminated food or water can produce tularemia with sore throat or tonsillitis and neck nodes. These questions help select appropriate testing and protect confidentiality.

Bottom line for readers

Fever plus swollen lymph nodes is a symptom combination, not a diagnosis. Tularemia, cat-scratch disease and ordinary regional lymphadenitis are important alternatives to plague, and their distinguishing clues come mainly from exposure, timing and associated local findings. A rapidly painful bubo after possible plague exposure—or any fever with rapidly worsening breathing symptoms—requires immediate medical attention. Laboratory testing and clinical judgment, not home comparison, determine the diagnosis.
