Short answer: Pig kidney xenotransplants are working well enough in a small number of carefully selected people to prove that the approach is feasible, but they remain experimental. Some recipients have lived for months without dialysis; others have experienced graft failure or had the pig kidney removed. No pig kidney is approved for routine treatment, and patients cannot obtain one outside an authorized clinical study or expanded-access pathway.

Gene-edited pigs are intended to reduce immediate immune attack, cross-species clotting problems and infection hazards. They do not eliminate those risks. A recipient still needs powerful immunosuppressive drugs, intensive infection surveillance and long-term follow-up. The central question is no longer whether a pig kidney can function in a human at all, but how long it can function, how safely it can be used and which patients benefit most.

What a pig kidney xenotransplant is

A xenotransplant is a transplant between different species. In this case, surgeons place a kidney from a genetically modified pig into a person with kidney failure. The organ must filter blood, regulate fluid and electrolytes, and produce urine inside a human body despite major biological differences between pigs and people.

The donor pigs are bred in controlled facilities and tested for pathogens. Their kidneys are altered before transplantation using gene-editing techniques. The edits are designed to make the organ less visible to the human immune system, improve compatibility with human blood vessels and reduce the chance that pig viruses could replicate in the recipient.

This is different from receiving a temporary laboratory device or a kidney grown from a patient’s own cells. The organ is living pig tissue, and the recipient’s immune, clotting and infectious-disease systems all interact with it.

Current status as of October 2026

The U.S. Food and Drug Administration (FDA) considers xenotransplantation an investigational field. The agency says the shortage of human organs is a major reason for developing it, while warning that recipients could acquire recognized or previously unknown infectious agents and potentially transmit them to close contacts or the wider population.

Clinical development has now moved beyond laboratory and nonhuman-primate studies. Early U.S. human cases have generally avoided the immediate, catastrophic “hyperacute” rejection that historically made cross-species transplantation impossible. However, the number of recipients is still very small, follow-up is short and outcomes are uneven.

eGenesis expanded access and RESTORE

eGenesis reported on September 3, 2026, that five patients had received its EGEN-2784 kidneys through expanded access since March 2024. Three had maintained kidney function for more than eight months without dialysis. These are encouraging results, but they are not the same as a completed randomized trial or proof that the organs will last for years.

The FDA has cleared a Phase 1/2/3 RESTORE trial. The planned study includes 33 adults aged 50 to 70 with kidney failure who are on a waiting list for a human donor kidney. eGenesis said it expects the trial to begin in the first quarter of 2027. Trial entry will depend on the final protocol, a participant’s medical condition and review by the treating center and regulators.

United Therapeutics EXPAND

United Therapeutics reported that the first transplant in its EXPAND clinical trial occurred in the fourth quarter of 2025. Its 2025 annual filing describes an initial cohort of six people with end-stage kidney disease, with possible expansion to 50, and says the study is ongoing. The company’s UKidney is described as coming from a pig with 10 gene edits.

The two programs therefore do not use identical organs. EGEN-2784 has a 69-edit design, while UKidney is described as having 10 edits. The edit count alone does not show which kidney is safer or more durable; the location and purpose of each edit, the accompanying immunosuppression and the clinical results matter more.

How the gene edits are intended to work

Removing major pig antigens

Human antibodies recognize sugar molecules and other proteins on pig cells. That recognition can activate complement and destroy blood vessels in the graft within minutes or hours. EGEN-2784 includes three glycan-antigen deletions intended to remove prominent antibody targets. This lowers, but does not eliminate, antibody and cellular rejection.

Disabling porcine endogenous retroviruses

Pig genomes contain porcine endogenous retroviruses (PERVs). EGEN-2784 uses edits to inactivate these viral sequences. PERV inactivation is an important biosafety measure, not a guarantee that every infectious threat has been removed. Other pig viruses, newly recognized agents or infections acquired during and after transplantation remain possible.

Adding human compatibility genes

EGEN-2784 also carries seven inserted human transgenes. Such additions can make pig blood vessels more compatible with human complement, inflammation and coagulation pathways. The aim is to reduce injury after blood flow is restored. UKidney’s 10-edit strategy uses a different balance of deletions and insertions, so results from one product cannot automatically be applied to the other.

Are pig kidney transplants actually working?

Yes, in the limited sense that several transplanted kidneys have produced useful kidney function for months. The International Xenotransplantation Association reported that early U.S. cases showed no hyperacute rejection and adequate pig-kidney function for periods of up to several months. eGenesis reported three expanded-access recipients with more than eight months of function without dialysis.

That evidence demonstrates feasibility, not established standard-of-care effectiveness. The first widely reported living-recipient case in the New England Journal of Medicine involved a 62-year-old man with end-stage kidney disease who received an eGenesis gene-edited kidney. A Nature Medicine review noted that the first living patient, transplanted on March 16, 2024, died about two months later from causes considered apparently unrelated to the transplant. A death unrelated to the graft does not prove the organ failed, but it also cannot establish long-term safety.

Researchers still do not know whether a pig kidney can routinely last several years like a successful human-donor graft, or whether its best role will be a bridge that keeps a patient alive and off dialysis until a human organ becomes available. The most informative future outcomes will include graft survival, dialysis independence, rejection episodes, infections, immunosuppression complications, quality of life and the ability to receive a later human kidney.

Potential benefits and realistic limits

A larger organ supply

Pigs can be bred in a controlled, repeatable way, unlike human donors whose deaths and medical suitability are unpredictable. A reliable supply could reduce the number of people who die while waiting and could make transplantation possible for patients who are unlikely to receive a human kidney.

Less time on dialysis

A functioning graft could reduce or eliminate dialysis sessions, vascular-access procedures, dietary restrictions and the fatigue associated with advanced kidney failure. In current cases, dialysis independence has lasted months, but it is not guaranteed and should not be represented as permanent.

A bridge to a human kidney

A pig kidney might stabilize someone enough to remain eligible for a later human transplant. Reports of patients who subsequently received human kidneys are encouraging bridge evidence. They do not establish that every recipient will qualify for, or safely reach, a later human transplant.

What it cannot yet promise

  • It cannot promise graft survival measured in years.
  • It cannot establish superiority to a kidney from a human donor.
  • It cannot remove the need for immunosuppression or medical monitoring.
  • It cannot guarantee freedom from dialysis if the graft underperforms or fails.

Major risks

Immune rejection

Even with multiple edits, human antibodies, complement proteins and immune cells can injure the pig kidney. Rejection may be immediate, develop over days or weeks, or appear later as chronic damage. Treatment may require additional immunosuppression, removal of the graft or a return to dialysis.

Infection and zoonotic transmission

The FDA distinguishes between known and unknown infectious agents and warns that an infection could spread beyond the recipient. PERV inactivation addresses one category of concern; it does not prove that all pig pathogens have been excluded. Recipients may need lifelong or long-term testing, restrictions on donating blood or tissue, detailed medical records and instructions for household contacts. The public-health consequences are one reason xenotransplant programs require formal oversight.

Clotting and vascular injury

Pig and human coagulation systems are not naturally compatible. Platelets, clotting factors and the lining of blood vessels can interact in ways that cause thrombosis, bleeding or progressive graft injury. Gene edits and drug protocols are intended to reduce this problem, but cross-species coagulation remains an active translational challenge.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Immunosuppression toxicity

Recipients need potent drugs to prevent rejection. These medicines can cause serious infections, kidney and liver toxicity, high blood pressure, diabetes, malignancy and difficult drug interactions. A kidney that works but requires unsafe immunosuppression would not be a successful treatment, so drug-related adverse events must be weighed alongside graft function.

Unknown long-term effects

Human follow-up is measured in months, not decades, and the cohorts are small. Researchers do not yet know the long-term durability of the organs, cumulative infection risk, effects on fertility, consequences for future children or population-level effects if an animal-derived infection adapts to humans. These uncertainties cannot be resolved by a few successful cases.

Pig kidneys compared with human-donor kidneys and dialysis

Consideration Gene-edited pig kidney Human-donor kidney Dialysis
Availability and waiting time Potentially repeatable supply, but currently limited to authorized programs Scarce; waiting time varies by country, blood type, sensitization and medical factors Usually available when a center and access can be arranged, but does not replace all kidney functions
Durability evidence Months of function in a small number of recipients; years-long durability not established Decades of clinical experience with measurable long-term graft survival Can sustain life long term, but outcomes depend on modality, adherence and comorbidities
Rejection and immunosuppression High rejection risk; intensive immunosuppression and monitoring required Rejection remains possible; immunosuppression is standard No transplanted organ to reject, but dialysis has its own complications
Infection concerns Routine infections plus possible animal-to-human transmission and contact monitoring Donor-derived infections are screened for; no cross-species pathogen concern Access, bloodstream and treatment-related infections are possible
Eligibility and ethics Only under a clinical trial or authorized expanded access, with additional consent and public-health duties Transplant-center evaluation and allocation rules apply Eligibility is broader, but treatment burden and medical suitability still matter
Quality of life Could reduce dialysis if the graft works; actual benefit is still being measured Often improves freedom from dialysis, subject to graft function and drug effects Requires recurring treatments or home procedures and ongoing dietary and fluid management

Who can receive a pig kidney now?

There is no general public eligibility list because each program sets its own protocol. Current candidates are people with severe or end-stage kidney disease for whom investigators believe the potential benefit justifies the unknown risks. A study may require a particular age range, dialysis status, waiting-list status, ability to take immunosuppressants, reliable follow-up and willingness to comply with infection surveillance.

Being on a transplant waiting list does not automatically qualify someone. Centers also assess cardiovascular health, cancer history, active infections, other organ failure, medication adherence, psychosocial support and whether a conventional human kidney is a realistic near-term option. The protocol may exclude people whose condition makes surgery or intensive immunosuppression too dangerous.

People interested in participation should ask their nephrologist or transplant center about FDA-authorized studies and expanded-access requests. They should not stop dialysis, change medication or travel for an unapproved procedure based on an internet advertisement. The National Kidney Foundation provides patient-facing information that can help frame questions for a transplant team.

What participation involves

  1. Referral and records review: A transplant center confirms kidney-failure status, prior treatments, comorbidities and human-donor options.
  2. Protocol screening: Investigators check age, wait-list status, laboratory values, infection history, cardiovascular risk and ability to attend frequent visits.
  3. Informed consent: The team explains uncertain durability, rejection, infection, drug toxicity, possible graft removal, dialysis and public-health monitoring obligations.
  4. Pre-transplant testing: Candidates undergo imaging, blood tests, antibody testing and assessments needed to plan surgery and immunosuppression.
  5. Transplant and intensive monitoring: The recipient receives the gene-edited kidney and a protocol-specific drug regimen. Kidney function, biopsies, clotting markers and infectious tests are checked closely.
  6. Long-term follow-up: Participants may need regular sampling, documentation of infections and contact tracing, even if the kidney later fails or is removed.

Questions patients should ask a study team

  • What is the exact gene-edit design and which human data support it?
  • How many recipients have received this product, and what were the graft-survival and dialysis-free results?
  • What happens if the kidney rejects or develops thrombosis?
  • How long will immunosuppression continue, and which toxicities are most common?
  • What infectious-disease tests and restrictions apply to the recipient and household contacts?
  • Could I still receive a human kidney later, and how would this transplant affect my wait-list position?
  • Who pays for surgery, monitoring, complications and treatment if the graft fails?

Bottom line for people considering xenotransplantation

Gene-edited pig kidneys have reached genuine human clinical testing and have provided dialysis-free kidney function for months in some recipients. That is a major scientific milestone, not proof of a routine replacement for human transplantation or dialysis. The field still faces rejection, clotting, infection, immunosuppression toxicity and unresolved long-term public-health questions. In October 2026, access remains limited to regulated trials and exceptional expanded-access decisions; there is no approved consumer treatment.
