Short answer: For chronic primary low back pain—pain lasting more than three months without a specific underlying disease—structured exercise and other non-drug care are usually the starting point. Physical therapy can make exercise safer and more specific to your limitations; the exercises themselves are the part you can continue independently. Medication may be a time-limited add-on when non-drug care is insufficient, unavailable, or unsuitable, but average pain relief is modest and side effects matter.

The best-supported approach is not an either-or contest. The World Health Organization recommends a person-centred package that can combine education, exercise, selected physical therapies, psychological support, and carefully chosen medicines. Your symptoms, function, medical history, access, and goals should determine the mix.

What “chronic back pain” means

Chronic primary low back pain is pain in the lower back that lasts longer than three months and is not explained by an identified disease, structural emergency, or another condition that requires a different treatment. The World Health Organization (WHO) estimates that this category makes up at least 90% of chronic low-back-pain presentations in primary care.

That definition matters because treatment evidence for nonspecific chronic pain cannot automatically be applied to pain caused by fracture, infection, cancer, inflammatory disease, severe nerve compression, or another diagnosed disorder. New or worsening neurologic problems and red-flag symptoms require medical assessment rather than a self-directed exercise or medication experiment.

“To achieve universal health coverage, the issue of low back pain cannot be ignored, as it is the leading cause of disability globally.”

— Bruce Aylward, WHO Assistant Director-General, 2023

Low back pain accounted for 8.1% of all-cause years lived with disability worldwide in 2020, according to WHO. That burden explains why a treatment that preserves walking, sleep, work, and daily activity can be more valuable than a small change on a pain scale.

At a glance: how the options compare

The table describes average findings from guidelines and trials. An average group effect does not predict exactly how you will respond; some people improve substantially, some notice little change, and symptoms can fluctuate naturally.

Option Expected pain and function change Time to benefit Durability Main adverse effects or limits Adherence, cost, and access When individual assessment is especially important
Physical therapy Can improve function and may reduce pain through an individualized program combining exercise, education, and selected hands-on or directed therapies. No single protocol is proven best. Often develops over several visits and weeks as movement, strength, and confidence improve; immediate relief is not guaranteed. More likely to persist when you continue the prescribed activity and self-management skills; benefits can fade when activity stops. Temporary soreness or a flare can occur. An unsuitable load, poor technique, or treatment that ignores contributing factors can limit progress. Requires a trained clinician, appointments, and sometimes travel or copayments. Telehealth or community options may improve access where available. Marked movement restriction, complex medical history, uncertainty about diagnosis, or neurologic symptoms.
Independent exercise Exercise training can improve pain and function. Recommended categories include trunk strengthening/endurance, multimodal, aerobic, aquatic, and general exercise; evidence does not identify one clearly superior type. Progress is usually gradual as tolerance and capacity build rather than after one session. Potentially durable because the activity can continue without appointments, but maintenance is essential. Overloading too quickly, using painful technique, or choosing an activity that does not fit your abilities can aggravate symptoms. Usually the least expensive option once you have a safe plan. Space, equipment, transportation, fatigue, and time can still be barriers. Uncertainty about safe movements, rapidly changing symptoms, major weakness, or conditions that affect balance, bone, heart, or lungs.
Medication NSAIDs may provide a small average pain benefit. Evidence for investigated drugs does not show a large or medium pain effect with high or moderate certainty. Duloxetine is an option for some patients. Some medicines act within days; the useful question is whether pain and function improve enough to justify continued exposure. Benefits may not persist after stopping. Long-term use can accumulate harm, and evidence for prolonged treatment is limited or unfavorable for several drug classes. NSAIDs can cause gastrointestinal, kidney, blood-pressure, cardiovascular, or bleeding problems in susceptible people. Duloxetine can cause adverse effects and interactions. Opioids increase adverse-event risk and are not routine care. Often easier to obtain than therapy, but recurring prescriptions and monitoring add cost and burden. Availability and coverage differ by country and plan. Kidney, stomach, heart, liver, bleeding, pregnancy, mental-health, or medication-interaction considerations; a history of substance-use problems; or neurologic symptoms.

Why non-drug care is usually the starting point

The 2024 American Academy of Family Physicians (AAFP) guidance describes nonpharmacologic care as first-line treatment for chronic low back pain. WHO similarly recommends a holistic package rather than reliance on one intervention. This is not a claim that medication never helps. It reflects the small average drug effects, the possibility of preventable adverse effects, and the fact that exercise and education can address strength, endurance, movement confidence, sleep, and activity limitations at the same time.

A useful goal is not necessarily zero pain. It may be walking farther, returning to work tasks, sleeping more consistently, lifting with less fear, or reducing flare frequency. A plan should define one or two measurable functional targets so that you can judge whether a treatment is worthwhile.

Physical therapy: what you are actually getting

Assessment and planning

Physical therapy is a clinician-delivered process, not a single machine or massage technique. The therapist reviews your history, examines movement and strength, screens for signs that need medical referral, and asks what activities matter to you. The resulting plan may include exercise prescription, education about pacing and flare management, and selected manual or other directed therapies.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The assessment is valuable when you are unsure which movements are safe, when pain has changed, or when previous self-directed attempts failed. It can also identify a starting dose that is challenging enough to build capacity without provoking a prolonged flare.

What sessions may include

  • Trunk, hip, or whole-body strengthening and endurance work.
  • Aerobic conditioning such as walking, cycling, or another tolerable activity.
  • Mobility work when restricted movement is limiting function.
  • Education about posture variability, pacing, sleep, lifting, and responding to symptom flares.
  • Selected hands-on or other directed therapies as an adjunct to active rehabilitation, not a replacement for it.

Because protocols vary widely, trial results do not establish that one branded method, set of exercises, or manual technique is universally superior. The practical test is whether the plan improves your chosen functions without unacceptable worsening.

When physical therapy is worth prioritizing

  • You need help translating pain into a safe, progressive activity plan.
  • You have weakness, movement restrictions, balance concerns, or fear of movement.
  • You want feedback on technique and load progression.
  • You have tried general exercise but cannot identify what to change.
  • You need coordinated screening for symptoms that may require a physician or other clinician.

Limitations and how to improve the odds

Appointments cannot substitute for activity between visits. Ask for a written home plan, the purpose of each exercise, what level of discomfort is acceptable, and the rule for progressing or backing off. Tell the therapist if symptoms persistently worsen, spread with numbness or weakness, or interfere with sleep and daily safety. If access is limited, ask whether a brief evaluation followed by remote check-ins, group sessions, or community-based exercise is appropriate.

Exercise: the active skill you can keep

Which type is best?

The Academy of Orthopaedic Physical Therapy (APTA) recommends exercise training for chronic low back pain, including trunk strengthening and endurance, multimodal programs, aerobic exercise, aquatic exercise, and general exercise. Its 2021 guidance does not identify one clearly superior exercise type. That means the best choice is usually the one you can perform consistently, progress gradually, and adapt to your symptoms, preferences, equipment, and other health conditions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How to start safely

  1. Choose a functional target. Examples include a ten-minute walk, standing to cook, or completing a work task with fewer breaks.
  2. Start below your maximum. Use a duration, resistance, or range that you can repeat without a prolonged flare. A clinician can help set the initial dose.
  3. Use more than one capacity when practical. A plan might combine aerobic activity with trunk or general strengthening and mobility, rather than treating one muscle in isolation.
  4. Progress one variable at a time. Increase time, repetitions, resistance, speed, or range gradually. If symptoms remain worse into the next day or function declines, reduce the latest change and reassess.
  5. Keep a simple log. Record the activity, approximate dose, pain trend, and the function you are trying to improve. This separates a temporary response from a consistently unhelpful plan.

Soreness does not automatically mean damage, but sharp, escalating, or neurologic symptoms are reasons to stop and seek advice. Exercise should be adapted to tolerance and function, not forced through a fixed pain target.

Making exercise sustainable

  • Schedule short sessions at a predictable time instead of waiting for motivation.
  • Use walking, stairs, a chair, water, or household movements if formal equipment is unavailable.
  • Break sedentary periods with brief movement rather than relying on one demanding workout.
  • Plan a lower-dose version for flare days so that activity does not become all-or-nothing.
  • Revisit the plan when work, sleep, mood, or another illness changes your capacity.

Medication: what the evidence supports and what it does not

NSAIDs

AAFP identifies nonsteroidal anti-inflammatory drugs (NSAIDs) as the initial medication choice when medication is appropriate. Cochrane’s 2023 review found that NSAIDs may provide a small benefit for chronic low back pain, but found no high- or moderate-certainty evidence that the investigated drugs produce a large or medium pain effect.

“Small benefit” should be treated as a trial question, not a promise. A clinician should review kidney function, stomach or ulcer history, bleeding risk, blood pressure, cardiovascular disease, pregnancy, allergies, and other medicines before recommending an NSAID. Do not combine multiple NSAIDs or extend use beyond the agreed plan without medical advice. Seek urgent help for signs of serious bleeding, an allergic reaction, chest pain, or markedly reduced urination.

Duloxetine

Duloxetine is a possible option for some patients when an NSAID is unsuitable, ineffective, or insufficient. It is a prescription medicine with its own contraindications, interactions, and adverse effects, so the decision should include your other medicines, liver and kidney considerations, mental-health history, and how benefits will be measured. Report troublesome or severe symptoms promptly rather than stopping or changing the dose on your own.

Opioids and other drug classes

WHO advises against routine opioid painkillers for most people with chronic primary low back pain because potential harms can outweigh benefits. Cochrane found that opioids may increase adverse events, and AAFP describes long-term opioids and several other drug classes as having uncertain or unfavorable benefit-risk profiles. A 2023 BMJ network meta-analysis found increased adverse events with tramadol-containing regimens and baclofen versus placebo in acute nonspecific low back pain; that evidence is acute and should not be presented as direct proof of chronic-pain effectiveness.

Do not treat a short-lived reduction in pain as evidence that a medicine is safe for indefinite use. If an opioid is ever considered for an exceptional situation, the prescriber should document goals, duration, monitoring, interactions, and a stopping plan. WHO also advises against routine lumbar braces or supports and traction for most people; these should not be treated as default substitutes for active rehabilitation.

Choosing a sequence for your situation

Start with assessment when the picture is unclear

Arrange medical evaluation before self-management if you have progressive leg weakness, new loss of bladder or bowel control, numbness in the saddle area, fever or unexplained illness, major trauma, a history that raises concern for cancer or infection, unexplained weight loss, or pain that is severe and unremitting at rest. These features do not diagnose a serious condition by themselves, but they change the appropriate next step.

If symptoms are stable and nonspecific

  1. Set a functional goal and obtain an exercise plan from a physical therapist or another qualified clinician if you need individualized instruction.
  2. Begin a tolerable, progressive exercise routine and track function, not pain alone.
  3. Use education, pacing, sleep support, and psychological strategies when fear, stress, low mood, or poor sleep amplify disability.
  4. If progress is inadequate, access is impossible, or exercise is temporarily limited, discuss a time-limited medication trial with a clinician.
  5. Reassess whether the treatment improves meaningful activity enough to justify its burden and risk. Stop ineffective or harmful approaches with professional guidance.

If medication is the only immediately available option

Access barriers are real. If therapy is unavailable, an NSAID may be considered after a safety review, while you pursue an exercise or education plan that fits your circumstances. Medication availability does not make it the preferred long-term strategy; it simply may serve as a bridge while non-drug care is arranged.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Why combining treatments can make sense

Different components solve different problems. A therapist can individualize movement and identify barriers; exercise builds capacity outside appointments; education improves pacing and confidence; psychological therapy can address fear, distress, or coping; and medication may reduce symptoms enough to permit activity. WHO’s recommendation for a coordinated package reflects this complementarity.

  • 【Reduce Safety Risks】Cable tray under desk keeps all plugs away from your kids, no more worry about tripping. Using 2 holes wire tray saves extra space, helps you easily cabling and wire protection. The mesh design is not easy to accumulate dust, but avoids safety accidents caused by messy wires.

Combination care should still be deliberate. Adding several interventions at once makes it difficult to know what helps and can increase cost or adverse effects. Introduce changes in a way that lets you monitor function and safety, unless a clinician recommends a coordinated start.

Monitoring: decide with function, not hope

Before starting, write down a baseline such as walking time, sitting tolerance, sleep interruptions, work duties, or a daily activity score. Recheck it at an agreed interval. Useful questions include:

  • Can I do more of the activity that matters to me?
  • Is the improvement large enough to justify appointments, effort, or medication risk?
  • Are flares shorter or easier to manage?
  • Are new adverse effects appearing?
  • Do I need a different diagnosis or a referral because symptoms changed?

Short-term group differences in trials do not guarantee durable individual benefit. Continue what is helping, modify what is tolerable but insufficient, and discontinue what is ineffective or unsafe with appropriate clinical advice.

Common mistakes to avoid

  • Waiting for complete pain relief before moving. Capacity often improves through gradual activity while some pain remains.
  • Choosing the most passive treatment. Hands-on care, braces, or traction may feel appealing, but WHO does not recommend routine braces, traction, or opioids for most people; active rehabilitation should remain central.
  • Assuming a stronger medicine is a better medicine. Cochrane found no high- or moderate-certainty evidence of large or medium pain effects from investigated drugs, and opioids can increase adverse events.
  • Copying someone else’s exercise plan. A movement that suits one person may be inappropriate for another’s strength, balance, diagnosis, or goals.
  • Ignoring access and adherence. The theoretically best plan fails if travel, cost, caregiving, work, or fatigue makes it impossible to follow.
  • Continuing a treatment without a review date. Every medication trial and exercise progression should have a way to judge benefit, harm, and next steps.

Bottom line for the decision

For chronic primary low back pain, prioritize a person-centred non-drug plan: education plus a progressive exercise program, with physical therapy when individualized assessment or coaching is useful. No exercise format has been shown to be clearly superior, so consistency and fit matter more than a fashionable method. Consider an NSAID or, for selected patients, duloxetine only after discussing contraindications, interactions, expected small average benefit, and a review plan. Routine opioids, lumbar braces, and traction are not recommended by WHO for most people. Escalate to medical evaluation whenever red flags or progressive neurologic symptoms appear.
