Yes: the investigational personalized mRNA therapy intismeran, given with pembrolizumab, met both recurrence-free survival and distant metastasis-free survival endpoints in a Phase 3 melanoma trial. But the August 19, 2026, announcement did not disclose the numerical effect size, so it does not establish that Phase 3 cut recurrence by half. The often-quoted 49% reduction comes from a separate Phase 2b study.
Did the personalized mRNA melanoma vaccine work in Phase 3?
Merck and Moderna announced positive topline results on August 19, 2026, from INTerpath-001, a global, randomized, double-blind Phase 3 trial. The investigational therapy intismeran autogene—also called intismeran, V940, or mRNA-4157—was tested with pembrolizumab (KEYTRUDA) against pembrolizumab alone after surgery for cutaneous melanoma. The combination met the trial’s primary recurrence-free survival (RFS) endpoint and key secondary distant metastasis-free survival (DMFS) endpoint at a prespecified interim analysis. The sponsors described the improvements as statistically significant and clinically meaningful, but did not provide hazard ratios, confidence intervals, event counts, absolute event rates, or survival curves in the topline announcement. Merck and Moderna’s announcement also says the trial continues to assess additional endpoints, including overall survival (OS).
RFS measured time from randomization until investigator-assessed local, locoregional, regional, or distant recurrence, or death from any cause. DMFS assesses the time until distant metastasis or death. Meeting these endpoints is a positive trial result, but without the Phase 3 numerical data, readers cannot judge the size of the benefit or translate it into an individual patient’s absolute chance of recurrence.
Does the Phase 3 result mean recurrence fell by half?
No such numerical estimate was disclosed for INTerpath-001. The “49%” figure belongs to the five-year follow-up of the earlier Phase 2b KEYNOTE-942 trial, not this Phase 3 readout. In that Phase 2b study, the combination was reported to reduce the risk of recurrence or death by 49% versus pembrolizumab alone (hazard ratio 0.51; 95% confidence interval 0.294–0.887). The reported reduction in distant metastasis or death was 59% (hazard ratio 0.411; 95% confidence interval 0.200–0.843). These are relative risk reductions from Phase 2b follow-up; they should not be presented as Phase 3 findings or as absolute reductions in the number of patients who recur. The companies’ five-year Phase 2b update reports those figures.

Who was eligible for INTerpath-001?
The study enrolled 1,137 patients after complete surgical resection of stage IIB, IIC, III, or IV cutaneous melanoma. Participants had not received prior systemic therapy for their melanoma. They were randomized in a 2:1 ratio to receive intismeran plus pembrolizumab or pembrolizumab alone. Because both groups received pembrolizumab, the trial tested whether adding the individualized therapy improved outcomes beyond pembrolizumab alone; it was not a comparison with no adjuvant treatment.
The announced schedule was intismeran 1 mg every three weeks for up to nine doses, alongside pembrolizumab 400 mg every six weeks for up to nine cycles—approximately one year. Treatment could end earlier for recurrence, unacceptable toxicity, or completion of the planned course. Trial participation criteria are not a substitute for a clinician’s assessment of whether a treatment is appropriate for a particular person.

How is intismeran personalized?
Intismeran is an individualized neoantigen therapy, not a preventive vaccine or a consumer product. A tumor sample is analyzed to identify mutations, and a synthetic mRNA treatment is designed to encode up to 34 neoantigens associated with that patient’s tumor. The intended aim is to train and activate an immune response directed at the tumor. In INTerpath-001, it was combined with pembrolizumab, an anti-PD-1 medicine. Moderna describes its personalized cancer vaccine approach.
What is known about safety and approval?
The sponsors said the combination’s safety profile was consistent with earlier studies and that no new safety signals were observed. The topline announcement did not include detailed adverse-event counts or a complete safety table, so it is not enough to compare specific side effects or rates between the trial arms.

Intismeran remains investigational based on the cited announcement; the Phase 3 topline result does not mean it is approved or available as standard treatment. Merck and Moderna said they planned to present the data at an international medical meeting, share them with regulators, and engage regulators on potential filing submissions. Regulatory review and any subsequent decision are separate from reporting positive trial endpoints. The announcement contains the companies’ stated next steps.

The full Phase 3 results will matter because the topline release leaves key questions unanswered: the magnitude of the RFS and DMFS differences, the absolute number of events in each arm, detailed safety outcomes, treatment burden, and the ongoing OS results. Those data will show how the benefit compares with the Phase 2b signal and help clinicians assess the balance of benefit and risk. Until then, the most accurate conclusion is that the combination met two important Phase 3 endpoints, while its numerical effect and full clinical profile have not yet been reported in the announcement.

Professor Georgina Long, INTerpath-001 principal investigator and medical director of Melanoma Institute Australia, called it the first Phase 3 study to show that the tumor-fingerprint-based treatment plus pembrolizumab reduced recurrence or death compared with KEYTRUDA alone. That is her characterization in the sponsor announcement, not an independent analysis of published full trial data. Merck Research Laboratories president Dean Y. Li likewise described the goal of adjuvant therapy as increasing the possibility of cure by treating after surgery. These comments explain the rationale; they do not substitute for the undisclosed Phase 3 effect estimates.
