When Parkinson’s medicine no longer controls symptoms reliably, the next step is a prompt review with the prescribing neurologist or a movement-disorders specialist—not an unsupervised dose change. Symptoms that return before the next dose, unpredictable “off” periods, troublesome dyskinesia, or medication side effects can indicate that timing, combinations, delivery method, or the overall care plan needs to change.

Options range from adjusting oral medicines to non-oral infusions, implanted deep brain stimulation (DBS), or a selected lesion procedure. The appropriate choice depends on which symptoms are limiting you, how strongly they still respond to levodopa, cognitive and psychiatric health, balance and other medical issues, treatment burden, and what is authorized and covered where you live.

Start with a specialist review, not a self-directed medication change

A change in control does not prove that every medicine has “failed,” and it does not automatically mean that surgery is needed. Parkinson’s disease evolves, and the benefit from a dose may become shorter or less predictable. A clinician may first examine the relationship between symptoms and dose timing, look for side effects, and decide whether the combination or schedule can be optimized.

Before the appointment, record when each dose is taken and when symptoms appear. Note delayed or missed doses, wearing-off before the next dose, involuntary movements, falls, swallowing or speech problems, sleep, mood, and the activities you can no longer complete. Bring the complete list of prescription medicines, over-the-counter products, and supplements. Do not stop or substantially alter Parkinson’s medicines without instructions from the care team.

What “not controlled” can mean

  • Wearing-off or “off” time: benefit fades before the next scheduled dose, so stiffness, slowness, tremor, or difficulty walking returns.
  • Unpredictable on/off changes: the response varies from dose to dose or switches suddenly.
  • Dyskinesia or hyperkinesia: involuntary movements occur when medication is working or at other points in the cycle.
  • Symptoms that respond poorly to levodopa: balance, freezing, speech, cognition, or autonomic problems may not improve simply by increasing dopaminergic treatment.
  • Adverse effects: a dose may help movement but cause effects that make the regimen unacceptable or unsafe.

These patterns require different strategies. A treatment that reduces off time may not eliminate dyskinesia, and a therapy that helps tremor may have little effect on balance or thinking.

Medication optimization remains the first decision point

Guidance from the UK National Institute for Health and Care Excellence (NICE) recommends best medical therapy for advanced disease. “Best medical therapy” is individualized: a specialist may alter dose intervals, formulations, or combinations, while balancing benefit against adverse effects. The exact adjustment depends on your examination and symptom diary; there is no safe universal schedule to copy.

Questions the clinician will assess

  • How long does each dose take to work, and how long does benefit last?
  • Are meals, constipation, or other medicines affecting the response?
  • Is a movement actually dyskinesia, tremor, dystonia, freezing, or a non-motor symptom?
  • Would treating one problem worsen hallucinations, sleepiness, blood-pressure symptoms, or involuntary movements?
  • Do cognitive, psychiatric, balance, or other health conditions limit escalation or make an advanced therapy unsuitable?

NICE specifically identifies intermittent apomorphine injection and/or continuous subcutaneous apomorphine infusion among options that may be considered when oral treatment is insufficient. These are specialist treatments, not rescue products to start independently.

Non-oral drug delivery options

Continuous or intermittent delivery can reduce the fluctuations caused by relying only on tablets. Eligibility, training, device support, and availability differ by health system.

Option How it is delivered When clinicians may consider it Practical issues to discuss
Intermittent apomorphine An as-needed injection under the skin Episodes of immobility or off time that need rapid, clinician-directed rescue Training, timing, injection technique, side effects, and whether it fits your daily routine
Continuous subcutaneous apomorphine A small pump infuses apomorphine under the skin Persistent or frequent fluctuations not adequately managed with tablets Wearing and maintaining the pump, skin-site care, dose titration, and follow-up
Levodopa–carbidopa intestinal gel An external pump delivers gel through a tube into the small intestine Advanced levodopa-responsive disease with significant fluctuations or dyskinesia Tube and pump care, training, equipment support, and procedure-related requirements
Foslevodopa–foscarbidopa Continuous subcutaneous infusion A defined population with advanced, levodopa-responsive disease and severe motor fluctuations with hyperkinesia or dyskinesia Eligibility rules, infusion management, monitoring, local authorization, and coverage

Apomorphine injection or infusion

Apomorphine is a dopamine agonist. An intermittent injection can be used when an individual off episode occurs, while a pump provides continuous subcutaneous infusion. A specialist team must determine whether your symptoms, response, other medicines, and medical history make either approach appropriate. Ask how the dose will be established, what training is provided, how injection sites are managed, and who to contact if the pump or infusion is interrupted.

Intestinal levodopa–carbidopa gel

This treatment sends levodopa–carbidopa continuously into the small intestine through a tube connected to an external pump. The goal is steadier levodopa delivery for people with clinically important fluctuations. It involves a procedure to establish the tube, daily equipment management, and ongoing follow-up. Your team should explain the tube pathway, troubleshooting plan, skin and site care, and what happens if the pump cannot be used temporarily.

Foslevodopa–foscarbidopa under NICE TA934

NICE technology appraisal TA934, published 29 November 2023, recommends foslevodopa–foscarbidopa as an option for adults with advanced levodopa-responsive Parkinson’s disease who have severe motor fluctuations and hyperkinesia or dyskinesia when medicines are not working well enough, and who cannot have apomorphine or DBS or whose symptoms are no longer controlled by those treatments. The recommendation applies under those stated conditions; it is not a blanket recommendation for every person with Parkinson’s or every country. NICE describes administration by continuous subcutaneous infusion. Confirm whether the treatment is authorized, commissioned, and funded in your health system.

Deep brain stimulation (DBS)

DBS uses electrodes implanted in specific brain targets and connected to a stimulation device. Programming can be adjusted after implantation, allowing the clinical team to refine settings as symptoms and needs change. DBS is considered for selected movement symptoms, particularly when advanced Parkinson’s is no longer adequately controlled by best medical therapy.

NICE recommendation 1.8.3 states: “Consider deep brain stimulation for people with advanced Parkinson’s disease whose symptoms are not adequately controlled by best medical therapy.” The same guidance says not to offer DBS when symptoms are adequately controlled by best medical therapy. This is a recommendation to consider the procedure after assessment, not a promise that every candidate will benefit.

How candidacy is evaluated

A specialist multidisciplinary assessment typically examines levodopa response, the pattern of motor fluctuations, dyskinesia, tremor, gait and balance, cognition, mood and psychiatric history, general medical fitness, and the ability to attend programming and follow-up. The team may compare your function during medication “on” and “off” periods. Symptoms that do not respond to levodopa, or substantial cognitive or balance problems, can reduce the likely benefit even when tremor or wearing-off is severe.

What DBS can and cannot do

  • It is an implanted stimulation treatment, not a cure for Parkinson’s disease.
  • It primarily targets selected movement symptoms; it does not reliably correct every non-motor symptom.
  • It requires surgery, device management, programming visits, and long-term clinical follow-up.
  • Potential benefits and procedure or device risks must be weighed against pump-based and medication options for your specific circumstances.

Focused ultrasound and other lesion procedures

Focused ultrasound concentrates energy on a small brain target to create a lasting lesion. Other lesion procedures also intentionally destroy a selected target. Unlike DBS, a lesion is not adjusted by reprogramming after it is made, and these procedures are not a cure. They are used for carefully selected symptoms and patients, and they are less common than DBS or continuous drug-delivery approaches.

The Parkinson’s Foundation’s US-focused information reports FDA milestones for focused ultrasound: approval in 2018 for tremor-dominant Parkinson’s tremor, in 2021 for other Parkinson’s movement symptoms, and in 2025 for treatment on both sides when procedures are separated by at least six months. These statements are specific to the United States and the cited indications. Regulatory status, approved targets, eligibility, and access can differ elsewhere and should be checked with the relevant regulator and treating center.

Questions before a lesion procedure

  • Which exact symptom and brain target are being treated?
  • Is the procedure unilateral or bilateral, and what spacing is required?
  • What evidence and regulatory authorization apply to this indication in your location?
  • What effects are permanent, and what options remain if the result is incomplete or causes a problem?
  • How does the center assess speech, balance, cognition, and other risks before treatment?

Rehabilitation and supportive care continue alongside advanced treatment

Medication or a procedure does not replace rehabilitation. Parkinson’s Foundation and Parkinson’s UK identify exercise, physiotherapy, occupational therapy, nutrition support, and mental-health care as parts of ongoing management. The plan can be matched to the problem that limits you most:

  • Physiotherapy and exercise: gait, balance, strength, mobility, freezing strategies, and fall prevention.
  • Occupational therapy: safer dressing, bathing, cooking, work tasks, home layout, and assistive equipment.
  • Speech and swallowing therapy: communication, voice, eating, and aspiration-risk assessment when indicated.
  • Nutrition support: maintaining weight, hydration, bowel regularity, and practical meal planning around the medication schedule.
  • Mental-health care: assessment and treatment for depression, anxiety, sleep problems, caregiver strain, or other psychological needs.

These services can improve safety and daily function even when a treatment does not fully control motor symptoms. Ask which professionals are available through your clinic and how referrals are arranged.

A practical framework for comparing options

There is no universal ranking of pumps, DBS, lesion procedures, or medication changes. Use the same questions for each option so the decision reflects your priorities and the evidence relevant to your case.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Define the target: identify whether the main problem is off time, tremor, dyskinesia, freezing, falls, or a non-motor symptom.
  2. Check levodopa responsiveness: clarify which symptoms improve during your best medication “on” state and which do not.
  3. Match the delivery method: compare tablets, injections, continuous subcutaneous infusion, intestinal pump delivery, implanted stimulation, and lesion treatment.
  4. Assess safety and burden: review medical, cognitive, psychiatric, and balance status; surgery or tube requirements; pump or device care; programming; and follow-up.
  5. Verify access: ask whether the option is authorized, offered by a qualified center, and covered in the health system where you receive care.
  6. Set measurable goals: agree how you will judge success, such as fewer hours off, less disabling tremor, easier transfers, or safer walking.

Questions to take to your appointment

  • Are my symptoms returning between doses, and could timing or the treatment combination be adjusted?
  • Which of my symptoms is each option most likely to improve, and which is unlikely to respond?
  • Am I a candidate for intermittent or continuous apomorphine, intestinal levodopa gel, foslevodopa–foscarbidopa, DBS, or a lesion procedure?
  • What examinations, medication testing, imaging, or multidisciplinary assessments are required?
  • What are the likely adverse effects, daily practical demands, and follow-up schedule?
  • If a pump, implanted device, or lesion does not achieve the desired result, what alternatives remain?
  • Which treatments are available and covered where I receive care?

When to contact the care team urgently

Contact your Parkinson’s clinician promptly if you cannot take or absorb your prescribed medicine, a pump or infusion is interrupted, falls or confusion increase suddenly, or symptoms change markedly after a medication or device adjustment. The team can provide an individualized contingency plan; do not improvise by doubling doses or stopping treatment.

Bottom line

Loss of reliable symptom control calls for a structured specialist reassessment. Depending on the symptom pattern and your health, the next step may be medication optimization, apomorphine, continuous levodopa-based infusion, a defined foslevodopa–foscarbidopa indication, DBS, or a selected lesion procedure, with rehabilitation and supportive care throughout. Decisions should be made with a movement-disorders team that can evaluate likely benefit, risks, practical workload, local authorization, and coverage.
