Paper persists because a digital tool does not automatically make the surrounding task easier or complete. A sheet can be quick to view, mark up, pass to someone else, or use when a system is awkward or unavailable. That can make paper a rational choice for a particular step—not proof that paper is always faster, safer, more accurate, or legally required.
Why do we still use paper when everything is digital?
Often, not everything in the workflow is digital. An organization may have an electronic records system but still rely on paper for a task the system handles poorly, or for a familiar process that was never redesigned. Paper can also act as a practical coordination layer when information is difficult to find, interpret, or share in the official system.
A qualitative study of U.S. healthcare organizations using electronic medical records and computerized order entry found paper remained widespread at the sites it examined. The researchers grouped the reasons into continued value in established uses, new uses for paper, and regulatory factors. Their fieldwork included observation, informal interviews, focus groups, and oral histories; it was not a controlled comparison of paper and digital outcomes. The 2009 AMIA study offers evidence about how paper persisted in those settings, not a current estimate for healthcare as a whole or for other industries.

Paper can suit the immediate task
In the healthcare settings studied, reported reasons included familiarity, the ability to hold and view a document, difficulty interpreting some information on screen, and clinicians’ concern that looking at or typing on a computer during a patient encounter could distract from the interaction. Those are context-specific observations, not evidence that everyone thinks better on paper or that paper improves clinical outcomes.

The study also reported that one 300-bed hospital printed or copied 1.6 million pieces of paper per month, with 40% reportedly printed and destroyed. That striking figure is an observation from one institution, not an industry average.
Why do online forms still need to be printed and signed?
Because putting a form online is not the same as making the whole process digital. A person may fill out a web form, print it to sign in ink, and mail it. The organization then has to capture the information into its records, perhaps through optical character recognition (OCR) or manual data entry. Each handoff adds work and creates an opportunity for delay, omission, or mismatch.

The U.S. National Archives and Records Administration (NARA) describes these kinds of hybrid processes in its guidance on federal forms. It recommends capturing information digitally from the start and, where appropriate, using electronic or digital signatures instead of requiring wet ink. Whether a particular signature or paper original is legally necessary depends on the applicable rules and circumstances.
Why digitization does not automatically eliminate paper
Replacing a records system does not necessarily redesign every task connected to it. Staff may keep using paper because it fits a local need, because a new system does not support a step well, or because the official process still requires printing, signing, mailing, or re-entry.

NARA’s federal guidance says agencies were directed to manage records electronically to the fullest extent possible by June 30, 2024. Its discussion of digital records and paper workflows notes that agencies may still print digital documents for signatures or receive paper submissions. Converting a digital record to paper and then back to digital can be inefficient and risks losing or altering information. The guidance concerns U.S. federal agencies; it is not a universal rule for businesses, other governments, or every signature.
Older federal findings illustrate another reason paper can endure: uncertainty about which copy counts as the record. NARA’s report, based on data collected in 2001, found that employees sometimes treated paper as the official record while electronic copies were deleted or left on desktops or shared drives. The report on federal recordkeeping practices is historical evidence of hybrid habits, not a measure of current practice.

How to compare paper and digital fairly
The useful comparison is between complete workflows, not between paper and screens in the abstract. A digital record can be searchable and available to many people at once; paper can be easy to inspect or annotate without navigating a device. Either can fail if access, custody, or procedures are poor.

- Access and availability: Can the record be reached when needed, including during device, network, or system outages? Does the task also require search or simultaneous access by several people?
- Handoffs and duplication: Does information have to be printed, signed, scanned, interpreted by OCR, or entered again? Where might omissions, mismatches, or changes occur?
- Trust and accountability: Is there a designated authoritative copy, a retention schedule, an audit trail, and a controlled process for changes and disposal?
- Long-term preservation: Can the record be retrieved and understood throughout its required retention period? Digital files need managed systems and attention to accessibility and migration; paper needs physical custody and preservation.
NARA’s imaging study, published in 1994 and focused on older optical-storage technology, makes a still-useful general point: records integrity depends on procedures and controls, not simply on the storage medium. It discusses written procedures and periodic audits as ways to demonstrate that a system produces accurate results. Its technology-specific advice is dated, so current operational or legal decisions should rely on applicable current requirements.

Paper is not automatically authentic, admissible, secure, or durable, and a scan is not automatically a trustworthy record. The U.S. Government Accountability Office’s 2002 report on electronic records described problems at that time with identifying and preserving historically valuable federal records. It is a reminder that digital recordkeeping also requires active management, not evidence of present-day agency performance.
When paper is rational—and when it is a warning sign
Paper is locally rational when it makes a real task easier or fills a gap that the available digital process has not solved. But recurring printing, scanning, or re-entry may signal that a workflow is only partly digitized. It is especially important to clarify which copy is authoritative when both paper and electronic versions circulate.
The practical question is not whether an organization can eliminate every sheet. It is whether the chosen process captures the information accurately, makes it available to the people who need it, preserves it for the required period, and clearly establishes responsibility for the record.
