Ozempic and Wegovy contain the same active ingredient, semaglutide, but they are not interchangeable prescriptions. Ozempic is labeled mainly for adults with type 2 diabetes, including specific cardiovascular and chronic-kidney-disease risk-reduction uses. Wegovy is labeled for chronic weight management in eligible people with obesity or overweight and for cardiovascular-risk reduction in certain adults with established cardiovascular disease. Both are once-weekly injections that must be increased gradually. Eye risks depend more on the indication, speed of glucose improvement, and a person’s existing diabetic retinopathy than on the brand name alone.
What is the difference between Ozempic and Wegovy?
The medicines share semaglutide as their active ingredient, but each has its own FDA-labeled indications, prescribing information, dose limits, and clinical context. A prescription for one should not be treated as a prescription for the other.

These distinctions matter when a clinician chooses a product. The decision can depend on diabetes control, cardiovascular or kidney disease, body-mass category, previous treatment, tolerability, kidney status, and the reason treatment is being prescribed.
What is Ozempic labeled to treat?
Type 2 diabetes and glucose control
Ozempic’s FDA labeling covers improving glycemic control in adults with type 2 diabetes. The dose is started low and increased only after the patient has spent enough time at the preceding dose. The purpose of this gradual schedule is to follow the product’s prescribing instructions and allow the clinician to assess tolerability and glucose response.

Cardiovascular-risk reduction
For adults with type 2 diabetes and established cardiovascular disease, the Ozempic label includes reduction of major cardiovascular events. This is a specific labeled population; it does not mean every person taking semaglutide has the same cardiovascular indication.
Chronic-kidney-disease risk reduction
Ozempic is also labeled for adults with type 2 diabetes and chronic kidney disease to reduce the risk of sustained decline in estimated glomerular filtration rate (eGFR), end-stage kidney disease, and cardiovascular death. A kidney indication does not remove the need to monitor diabetes, kidney function, and other medicines.
What is Wegovy labeled to treat?
Long-term weight management
Wegovy is labeled for use with a reduced-calorie diet and increased physical activity for chronic weight reduction in adults with obesity, adults with overweight plus at least one weight-related comorbidity, and adolescents aged 12 years and older with obesity. The label describes Wegovy as part of a continuing management plan rather than a short course intended to replace nutrition or activity changes.

Wegovy also carries a cardiovascular-risk-reduction indication for adults with established cardiovascular disease who have overweight or obesity. This indication is different from Ozempic’s type 2 diabetes cardiovascular indication, even though both products contain semaglutide.
Why the labels cannot be swapped
The same active ingredient does not make two branded products automatically interchangeable. Their approved purposes, dose-escalation instructions, patient populations, and supporting clinical evidence are product-specific. Do not substitute one for the other, combine them, or change the injection amount without the prescriber’s direction.

How do the doses compare?
Ozempic’s labeled escalation
Ozempic starts at 0.25 mg once weekly. After four weeks, the labeled dose rises to 0.5 mg once weekly. If additional glycemic control is needed, the dose can be increased to 1 mg after at least four weeks at 0.5 mg, and to 2 mg after at least four weeks at 1 mg. Those increases are not automatic; the prescriber decides whether the next step is appropriate.
Wegovy’s escalation
Wegovy is also increased stepwise according to the current Wegovy prescribing information. The supplied labeling summary establishes the gradual weekly escalation but does not provide a complete numeric schedule here. Patients should use the dose printed on their prescription and follow the current instructions from the prescriber and dispensing pharmacy rather than infer a Wegovy dose from an Ozempic dose.

A milligram-for-milligram comparison is not a reliable way to decide which product is stronger. Even though the ingredient is semaglutide, each brand has a different labeled purpose and titration framework. A higher number on one product’s pen does not by itself prove greater benefit, safer use, or suitability for a particular patient. The clinically relevant question is whether the prescribed product and dose match the person’s indication, glucose or weight goals, adverse effects, kidney and diabetes status, and response over time.
Why gradual escalation matters
Both products are designed for gradual dose increases. Taking a higher dose sooner than directed can worsen adverse effects and may change how quickly glucose improves. Rapid improvement in glucose control is relevant to diabetic-retinopathy risk, so dose changes should be supervised rather than accelerated independently.
What do the eye warnings mean?
Two different eye concerns appear in the safety information: worsening diabetic-retinopathy complications and the very rare report of non-arteritic anterior ischemic optic neuropathy (NAION). They are not the same condition and require different clinical thinking.

Diabetic-retinopathy complications with Ozempic
In a two-year trial of adults with type 2 diabetes and high cardiovascular risk, the Ozempic label reported diabetic-retinopathy complications in 3.0% of Ozempic-treated patients versus 1.8% with placebo. The result is a trial statistic for that studied population, not a prediction that every Ozempic user will experience an eye complication.
Diabetic-retinopathy complications with Wegovy
The Wegovy label reports a different pattern when participants are separated by their retinopathy status at baseline. Among patients with known diabetic retinopathy at the start, complications occurred in 8.2% of semaglutide-treated patients versus 5.2% with placebo. Among those without known baseline retinopathy, the corresponding figures were 0.7% versus 0.4%.
The larger absolute difference in people who already had retinopathy is why a history of diabetic eye disease changes monitoring needs. The numbers come from the Wegovy prescribing information and should not be used as a head-to-head comparison with the Ozempic trial percentage; the populations and study contexts are not identical.
Why glucose improvement can temporarily worsen retinopathy
The Wegovy labeling explains that rapid improvement in glucose control can temporarily worsen diabetic retinopathy. Ozempic’s FDA labeling likewise states: Patients with a history of diabetic retinopathy should be monitored.
A patient with diabetes may therefore need an eye-care plan when semaglutide is started or increased, especially if retinopathy is already present or glucose levels are improving quickly.
Does Wegovy have a safer eye profile than Ozempic?
The available warnings do not support a universal answer that one brand is safer for the eyes. The reported retinopathy rates differ by trial, baseline retinopathy status, indication, and study population. The important risk questions are whether the patient has diabetic retinopathy, how quickly glucose is changing, what dose-escalation plan is being used, and whether new visual symptoms appear.

What is NAION, and how urgent is it?
Non-arteritic anterior ischemic optic neuropathy, or NAION, affects the optic nerve. The UK medicines regulator describes it as a condition that can cause sudden deterioration in vision, usually in one eye at a time, and says it has been very rarely reported in association with semaglutide used for diabetes, weight management, and cardiovascular-risk reduction.
In 2025, the World Health Organization reported that its safety committee supported adding NAION as a potential risk to semaglutide risk-management plans. The European Medicines Agency’s Pharmacovigilance Risk Assessment Committee concluded that NAION is a very rare side effect of semaglutide medicines. “Very rare” does not mean impossible, and it does not identify which individual patient will be affected.
Symptoms requiring urgent assessment
- Sudden loss of vision in one eye or both eyes
- Marked, abrupt deterioration in visual sharpness
- A new area of missing or dark vision
- Sudden visual change accompanied by severe eye pain, neurological symptoms, or other emergency symptoms
Do not wait for the next routine diabetes or weight-management appointment if vision suddenly worsens. Seek urgent medical assessment and tell the clinician that you use semaglutide, including the product name, dose, and date of the last injection. A sudden visual change can have causes unrelated to semaglutide, so it needs an examination rather than a self-diagnosis.
Who needs extra diabetic-eye monitoring?
People with known retinopathy
A history of diabetic retinopathy is the clearest reason to plan monitoring. Before starting or escalating treatment, make sure the prescribing clinician knows about previous retinal bleeding, laser treatment, injections, abnormal retinal photographs, or an eye specialist’s diagnosis.

People whose glucose is improving rapidly
Rapid improvement in glucose control can temporarily worsen retinopathy. This does not mean glucose should remain high; it means the diabetes and eye-care teams should coordinate monitoring while control changes.

People without known retinopathy
Lower reported rates in people without known baseline retinopathy do not eliminate the need for routine diabetes eye care. New visual symptoms still warrant prompt evaluation, regardless of which semaglutide product is prescribed.

- Confirm the indication. Ask whether the prescription is for type 2 diabetes, chronic weight management, cardiovascular-risk reduction, kidney-risk reduction, or more than one labeled purpose.
- Review eye history. Report diabetic retinopathy, prior retinal procedures, recent eye examinations, and any current visual symptoms.
- Confirm the exact product. Check that the pen or package says Ozempic or Wegovy as prescribed. Do not assume that a dose from one product can be transferred to the other.
- Write down the escalation plan. Record the starting dose, the date of each planned increase, and which symptoms should prompt a call before increasing.
- Coordinate monitoring. Ask the diabetes prescriber and eye-care professional whether an examination is due and how quickly to report changes in vision.
- Review contraindications and allergies. Both products carry a boxed warning about thyroid C-cell tumors based on rodent findings and are contraindicated for people with a personal or family history of medullary thyroid carcinoma, multiple endocrine neoplasia syndrome type 2 (MEN 2), or a previous serious hypersensitivity reaction to semaglutide or formulation ingredients.
What to do if blurry vision develops
Sudden or marked change
Treat sudden blurry vision, a curtain-like loss, a new missing area, or abrupt loss of sight as urgent. Arrange immediate medical assessment rather than taking another dose to “test” the symptom or waiting for it to resolve. Bring the medication package or a written record of the product and dose.
Gradual or intermittent change
Contact the prescribing clinician and an eye-care professional promptly for new or worsening blur that is not sudden. Provide the timing, whether one or both eyes are affected, glucose readings if available, recent dose changes, and any history of retinopathy. Do not independently stop, restart, or increase the medicine without clinical advice unless an emergency clinician tells you to do so.

What clinicians need to know
Useful details include the exact product, current weekly dose, date of the last injection, recent glucose improvement, other diabetes medicines, known retinopathy, and whether the change is in one eye or both. These facts help distinguish possible retinopathy progression, NAION, glucose-related visual effects, and unrelated eye or neurological emergencies.

Common mistakes to avoid
- Comparing milligrams as if the pens were identical. The products have separate labels and escalation instructions.
- Using a higher dose sooner than prescribed. Escalation is stepwise and should reflect tolerability and clinical need.
- Assuming no retinopathy history means no eye risk. Routine diabetes eye care remains important, and new symptoms still require evaluation.
- Waiting on sudden vision loss. Very rare NAION reports and other possible causes make abrupt visual deterioration an urgent problem.
- Treating trial percentages as a head-to-head ranking. The Ozempic and Wegovy figures come from different labeled studies and populations.
- Ignoring the thyroid-tumor contraindication. A personal or family history of medullary thyroid carcinoma or MEN 2 requires the prescriber to choose an alternative approach.
Bottom line for choosing between Ozempic and Wegovy
Ozempic is the semaglutide product labeled for adults with type 2 diabetes, with additional cardiovascular and chronic-kidney-disease risk-reduction indications in defined populations. Wegovy is the semaglutide product labeled for chronic weight management in eligible people with obesity or overweight and for cardiovascular-risk reduction in certain adults with established cardiovascular disease. Both require gradual weekly dose escalation.
Neither product can be called universally safer for the eyes. Diabetic-retinopathy complications are more consequential in people who already have retinopathy and during rapid improvement in glucose control. NAION is classified by regulators as a very rare potential or confirmed side effect and can cause sudden visual deterioration, usually in one eye. Follow the exact product label, arrange appropriate diabetic-eye monitoring, and seek urgent care for sudden or marked vision loss.
