A 2008 analysis of clinical trials found substantially higher mortality and heart-attack rates among patients given hemoglobin-based oxygen carriers (HBOCs) than among controls. Those results are an important historical safety signal—not a current pooled estimate or proof that every blood-substitute candidate has the same risks. FDA research summaries also describe the field’s limits: the products carry oxygen, but they do not replace every function of whole blood, and research activity is not the same as regulatory approval.

What “blood substitute” means in this report

“Blood substitute” is a broad label for products intended to take on some role of blood. This article focuses on hemoglobin-based oxygen carriers, or HBOCs: products that use hemoglobin molecules outside red blood cells to carry oxygen. They are not interchangeable with whole blood and should not be understood as replacing all of its functions.

That distinction matters when interpreting risk reports. Findings about HBOCs do not automatically apply to every product described as a blood substitute, such as a volume expander, or to every HBOC formulation.

What the 2008 safety report found

In 2008, Chemistry World summarized a meta-analysis by Charles Natanson and colleagues covering 16 trials of five HBOC products and more than 3,500 patients. The report said mortality was over 30% higher and heart-attack risk was three times higher in HBOC recipients than in control groups.

Elebase USB to USB C Adapter for iPhone 18 Pro Max,USBC Car Charger Adapter
  • Read Before You Buy — No Video Output: These adapters support charging and USB 2.0 data transfer, but cannot transmit video signals. Except for standard USB webcams (which use USB data only), they are not compatible with HDMI/DisplayPort cables, video-capable USB-C hubs, or docking stations with video output.
  • Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
  • Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
  • Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.

These are findings as reported in a 2008 news account of the analysis. They should not be presented as the latest pooled estimate, or expanded into an exact risk for a particular product or patient: the figures here do not specify confidence intervals or provide a product-by-product breakdown. The report establishes a serious historical safety signal across the trials it summarized, not that every candidate had identical outcomes.

Why carrying oxygen can still create risk

HBOCs use hemoglobin outside the red blood cells that normally contain it. FDA researchers have described how free hemoglobin can accumulate and undergo oxidation, producing reactive molecules associated with high blood pressure, heart attack, and stroke.

This provides a biological concern consistent with the adverse outcomes reported in the trials, but it does not by itself show that every product causes each outcome, or explain an individual patient’s risk. FDA researchers also emphasize that HBOC products differ in their chemical and biochemical characteristics, so the class should not be treated as one uniform formulation.

Products and evidence are not interchangeable

The 2008 report named PolyHeme and Hemopure among the products discussed. The available summary does not establish that those products had the same formulation, trial evidence, intended use, adverse outcomes, or regulatory status. FDA researchers’ description of differences among HBOCs reinforces why class-wide findings should not be turned into identical product-by-product conclusions.

Historical context also includes Fluosol: Chemistry World reported in 2008 that it had received U.S. FDA approval in 1989 and was withdrawn five years later because of side effects and storage problems. That account concerns one product and its history; it is not a current assessment of all blood substitutes.

What can be said about FDA status

An FDA research summary from 2018 stated that none of the HBOCs it described had been approved for human use. The same summary discussed continued preclinical research into possible future roles. Preclinical work is research, not evidence that a product is approved or available for treatment.

The available information does not establish current, product-by-product status for PolyHeme or Hemopure. The 2018 statement should therefore be attributed to that FDA summary rather than treated as a verified status update for every candidate today.

How to read the company’s counterclaim

In the 2008 report, Northfield Laboratories chairman and CEO Steven Gould defended PolyHeme’s trial-specific results, saying: “We believe our studies demonstrate that PolyHeme offers a survival benefit in cases of life-threatening red blood cell haemoglobin levels and has an acceptable safety profile and favourable benefit-to-risk ratio when red blood cells are not available.” This is the company’s position as reported at the time, not an independent conclusion or a substitute for the broader safety findings.

What the evidence does—and does not—answer

  • It does establish: a 2008 meta-analysis, as summarized by Chemistry World, reported elevated mortality and heart-attack risk across the HBOC trials it covered.
  • It helps explain: why FDA researchers have examined biological risks associated with free, oxidizing hemoglobin.
  • It does not establish: a current pooled risk estimate, identical effects across all HBOCs, or current approval status for each named candidate.
  • It is not: a consumer treatment guide or evidence that an investigational HBOC is available to readers.
