The key decision is not “operate or never operate.” It is whether treating the endometrioma now is more likely to help your immediate goal—pain control, natural conception, access to follicles for assisted reproductive technology (ART), or future fertility planning—than to reduce ovarian reserve. The 2022 European Society of Human Reproduction and Embryology (ESHRE) guideline advises against routine endometrioma surgery before ART solely to improve live-birth rates: available evidence shows no benefit, and surgery can harm ovarian reserve.
Surgery can still be reasonable for significant endometriosis-associated pain, an endometrioma that blocks follicle access during egg retrieval, or a carefully selected plan to try conceiving without ART. Decisions should account for age and pregnancy timing, ovarian-reserve results, previous ovarian operations, whether disease affects both ovaries, other infertility factors, and your preferences. A gynecologist or endometriosis specialist should coordinate with a reproductive endocrinologist when fertility is a priority.
What an endometrioma means for fertility planning
An endometrioma is ovarian endometriosis that forms a cyst. Management is individualized because the cyst itself, the underlying endometriosis, prior operations and age-related fertility decline can all affect the path to pregnancy. Treatment that improves pain does not automatically improve the chance of a live birth, and an operation that removes a cyst can also remove healthy ovarian tissue.

- Pain relief: treatment may be considered when symptoms are a major problem, regardless of an immediate pregnancy plan.
- Natural conception: operative laparoscopy may be discussed for endometrioma-associated infertility, but comparative studies proving a fertility benefit are absent.
- ART access: surgery may help when the cyst prevents the retrieval needle from reaching follicles, but it is not routinely done just to raise IVF live-birth rates.
- Future fertility: fertility-preservation counseling can be appropriate in extensive or higher-risk ovarian disease, although the true benefit for endometriosis remains uncertain.
Do you need surgery before IVF or another ART cycle?
Usually, no—not for the sole purpose of improving live-birth rates. ESHRE’s 2022 recommendation is that clinicians should not routinely operate on an ovarian endometrioma before ART because current evidence shows no live-birth benefit and surgery is likely to have a negative effect on ovarian reserve.

The guideline summarizes a 2015 meta-analysis of five studies involving 655 women. Surgery before ART did not improve live birth compared with conservative management; clinical pregnancy, average oocytes retrieved and cycle cancellation also did not differ. Findings on antral follicle count and stimulation-dose requirements suggested possible reserve harm. A second meta-analysis, published in 2018 and summarized by ESHRE, found no significant live-birth advantage (odds ratio 1.08, 95% confidence interval 0.80–1.45, seven studies).
These are group-level results reviewed in the guideline, not a prediction for an individual patient. Your team may still recommend surgery before retrieval when:
- pain from the endometriosis is substantial and other approaches are inadequate;
- the cyst’s location or size prevents safe access to follicles; or
- another clinical concern makes treatment necessary after individualized assessment.
An older practice of using a universal 4 cm threshold is not current ESHRE advice. The 2022 guideline favors a case-by-case decision rather than a single size cutoff.

Can you become pregnant naturally with an endometrioma?
Natural conception is possible for some people, but an endometrioma can coexist with other causes of infertility and does not provide a reliable forecast of your individual chance. ESHRE says operative laparoscopy may be considered for endometrioma-associated infertility because it may increase spontaneous-pregnancy chances; importantly, comparative study data establishing that benefit are lacking. Surgery should therefore be presented as an option with limited evidence, not as a guaranteed fertility treatment.
If trying naturally is your priority, the consultation should include your age and desired timeline, ovarian-reserve assessment, semen testing where relevant, tubal evaluation when indicated, the severity and location of endometriosis, and any previous ovarian surgery. Those factors determine whether time spent trying without treatment is reasonable or whether ART would better protect your reproductive options.
How surgery can affect ovarian reserve
Ovarian reserve describes the remaining pool of recruitable follicles; it is not the same as egg quality and does not guarantee a pregnancy. Endometrioma surgery can reduce reserve when healthy ovarian tissue is removed or blood supply is affected. The risk is particularly important when an ovary has already been operated on, when both ovaries are involved, or when surgery is extensive.

Before deciding, clinicians commonly integrate the antral follicle count, anti-Müllerian hormone (AMH), ultrasound appearance, prior operative history and the expected benefit of treatment. A low or declining reserve may shift the balance toward proceeding directly to ART or discussing egg freezing before an indicated operation. A reassuring reserve result does not eliminate surgical risk, and a low result does not by itself dictate one pathway.

Technique selection requires specialist counseling because pain control, recurrence, ovarian tissue preservation and the reason for surgery can point in different directions.

The operation’s indication matters. A procedure performed to relieve disabling pain may be worthwhile even when it is not expected to improve ART live-birth rates. Conversely, operating on an asymptomatic cyst that does not interfere with retrieval may expose reserve to risk without a demonstrated reproductive benefit.

Should you freeze eggs before endometrioma surgery?
There is no single answer. ESHRE recommends discussing the pros and cons of fertility preservation in extensive ovarian endometriosis and gives examples of higher-risk situations for discussion, such as bilateral endometriomas or recurrence in the opposite ovary after previous unilateral surgery. The guideline also states that the true benefit of fertility preservation in endometriosis remains unknown.
Egg freezing may be considered before an operation when age, reserve, bilateral disease or a history of ovarian surgery suggests that treatment could substantially narrow future options. It is not a guarantee against infertility or a promise of a future birth. The decision involves:
- your age and how long you can reasonably defer pregnancy;
- AMH, antral follicle count and expected response to stimulation;
- whether surgery is urgent for pain or needed for follicle access;
- the number of stimulation cycles that might be feasible before surgery;
- cost, treatment burden and local access to fertility-preservation services; and
- your willingness to accept the possibility that retrieval yields few eggs.
When surgery is clearly needed, delaying it for preservation may not be appropriate. When surgery is elective, a reproductive endocrinology consultation before scheduling can clarify whether preservation adds meaningful value in your circumstances.

A practical decision framework
Use these questions to structure a joint appointment with an endometriosis specialist and fertility clinician:
- What is the immediate goal? Name pain relief, natural conception, ART access, or preserving future options. A single operation may not serve all four goals equally well.
- What is your time horizon? Age and the desired timing of pregnancy determine how much time is reasonable for observation, surgery or attempts at natural conception.
- What is your current reserve? Review AMH, antral follicle count and prior stimulation or retrieval results, if any.
- What has happened before? Document every ovarian operation, the side treated and whether the cyst has returned.
- Is disease bilateral or extensive? Bilateral involvement and contralateral recurrence after prior surgery are settings in which fertility-preservation counseling deserves particular attention.
- Will the cyst interfere with retrieval? Ask the fertility team to explain whether the planned needle path is accessible without surgery.
- What does the evidence actually support? Surgery before ART has no demonstrated live-birth advantage; surgery for natural conception has possible benefit but no comparative evidence proving it.
- What matters most to you? Preferences about pain, avoiding surgery, minimizing recurrence, maximizing egg numbers and starting treatment quickly are legitimate parts of the decision.

- What specific problem would surgery solve in my case?
- Could the planned ART retrieval reach every target follicle without operating first?
- How might this operation affect each ovary’s reserve, given my prior procedures?
- Which technique do you recommend—cystectomy, laser vaporization, excision or another approach—and why?
- What is the plan if the cyst recurs?
- Should I meet a fertility-preservation team before setting a surgical date?
- How long would observation, surgery, recovery or egg freezing add to my pregnancy timeline?
What the evidence can and cannot tell you
The ESHRE guideline was issued on 2 February 2022 and replaced its earlier guideline. Its conclusions synthesize available studies rather than measuring your personal probability of pregnancy. The absence of a proven live-birth benefit before ART does not mean surgery is never appropriate; it means the operation should have a clear indication beyond a routine attempt to improve IVF outcomes. NICE and Royal College of Obstetricians and Gynaecologists patient guidance similarly emphasize balancing fertility wishes, ovarian reserve and individualized treatment choices.
Because recommendations can vary by jurisdiction and clinical context, ask for a coordinated plan from clinicians experienced in both endometriosis and reproductive medicine. Do not start supplements, fertility products or surgical interventions as substitutes for this assessment.
Bottom line
For people preserving fertility, an ovarian endometrioma is a balancing decision. Avoid routine surgery before ART when the only aim is a higher live-birth rate; consider surgery for pain or blocked follicle access, and discuss it selectively when natural conception is the goal. If an operation is indicated, review cystectomy versus other techniques with explicit attention to ovarian reserve, recurrence, prior surgery, bilateral disease, age and your preferred timeline. Fertility preservation is worth discussing in extensive or higher-risk ovarian endometriosis, but its endometriosis-specific benefit is still uncertain.
