Short answer: Studies consistently find an association between osteoporosis or lower bone mineral density (BMD) and cognitive impairment or dementia. Some analyses also find the reverse association: cognitive impairment is linked with higher osteoporosis risk. This is not proof that one condition causes the other, that osteoporosis predicts an individual’s dementia, or that osteoporosis treatment prevents cognitive decline.
The strongest evidence comes from systematic reviews of observational studies and a meta-analysis of three longitudinal cohorts. Results vary substantially between studies, so the useful response is coordinated bone and cognitive care—not assuming that a bone diagnosis explains memory symptoms or that bone medication is a dementia treatment.
What the current studies actually found
The literature contains two different questions. One asks whether osteoporosis or low BMD is followed by cognitive problems. The other asks whether people who already have cognitive impairment later develop osteoporosis. A third line of work uses baseline BMD to estimate the chance of a later dementia diagnosis. These exposures and outcomes are related but not interchangeable.

| Direction and design | Evidence base | Main result | How to interpret it |
|---|---|---|---|
| Osteoporosis followed by cognitive impairment | 2023 systematic review and meta-analysis by Zhao and colleagues; 8 studies, 136,222 participants; searches through January 2022 | Odds ratio (OR) 2.01, 95% confidence interval (CI) 1.63–2.48 | The pooled association was higher odds of cognitive impairment among people with osteoporosis. Study design, participant age and outcome definitions varied, producing significant heterogeneity. |
| Cognitive impairment followed by osteoporosis | 2023 systematic review and meta-analysis; 10 studies, 9,872 participants; searches through August 2023 | Relative risk (RR) 1.56, 95% CI 1.30–1.87 | People with cognitive impairment had a higher pooled risk of osteoporosis. Heterogeneity for osteoporosis outcomes was substantial (I² 88%), so the average does not describe every population. |
| Bidirectional association | Nasiri and colleagues, 2025 systematic review and meta-analysis in Bone; 15 observational studies; searches through August 15, 2024 | The abstract reports significant associations in both directions | The review strengthens the case that the conditions often occur together, while explicitly calling for larger longitudinal studies. It is not a causal study or a treatment guideline. |
| Baseline BMD and later dementia | Meta-analysis of the Framingham, Rotterdam and Rush Memory and Aging Project cohorts; 4,431 participants, 606 incident dementia diagnoses, including 498 Alzheimer’s disease diagnoses | Hazard ratio (HR) 0.47 for each 1 g/cm² higher baseline BMD, 95% CI 0.23–0.96 | Higher measured BMD was associated with lower subsequent dementia incidence in the pooled longitudinal analysis. The estimate concerns a BMD measurement, not the effect of raising BMD with treatment. |
Because these studies use different designs, the figures should not be combined into one personal risk estimate. An odds ratio from one review, a relative risk from another and a hazard ratio from a cohort analysis answer different statistical questions.

How to read the reported numbers
Odds ratio of 2.01
An OR of 2.01 means that the odds of cognitive impairment were about twice as high in the osteoporosis group as in the comparison group in the pooled studies. Odds are not the same as probability. If the underlying probability is low, doubling the odds may still represent a modest absolute difference; if it is high, the absolute difference can be larger. The 95% CI of 1.63–2.48 expresses uncertainty around the pooled estimate, not a range that captures every individual’s outcome.
Relative risk of 1.56
An RR of 1.56 means the measured risk of osteoporosis was 56% higher for participants with cognitive impairment than for the comparison group in the included studies. The result does not say that 56% of people with cognitive impairment will develop osteoporosis. Baseline risk, age, sex, health status, follow-up time and how each study defined both conditions all affect absolute risk.
Hazard ratio of 0.47 per g/cm²
The longitudinal BMD analysis found a lower rate of incident dementia associated with each 1 g/cm² increase in baseline BMD. The unit is an absolute BMD difference, not a 1% increase, and the association is statistical rather than a treatment effect. The confidence interval, 0.23–0.96, is wide enough to show meaningful uncertainty even though it does not cross 1.0.

Why association is not proof of cause
All of the bidirectional review evidence is observational. Researchers recorded health measures and outcomes as they occurred rather than assigning people to an osteoporosis exposure or a cognitive intervention. That creates several ways an apparent link can arise:

- Confounding: another characteristic may influence both bone and cognitive outcomes. Statistical adjustment can reduce confounding but cannot guarantee that every relevant factor was measured.
- Reverse causation: early, undiagnosed cognitive changes could affect nutrition, activity, medication use or healthcare attendance before a formal diagnosis, thereby influencing bone measurements.
- Detection differences: people with one chronic condition may receive more examinations, making the second condition more likely to be detected.
- Different definitions: studies may use a BMD threshold, a fracture history, a screening score, a clinical diagnosis of mild cognitive impairment or a dementia record. These are not identical endpoints.
- Selective follow-up: loss to follow-up and differences in survival can change which participants remain available for assessment.
These limitations do not make the associations meaningless. They explain why a pooled association should be treated as evidence of a relationship that needs better testing, not as proof that osteoporosis initiates dementia or that cognitive impairment directly weakens bone.
What the osteoporosis-to-cognition evidence means
The 2023 review of eight studies and 136,222 participants found higher odds of cognitive impairment among people classified as having osteoporosis. The review also reported significant heterogeneity. Variation by study type, participant age and outcome indicator means that the pooled OR 2.01 is an average across unlike settings.
That review searched the literature through January 2022. It does not establish whether the association is strongest for a particular skeletal site, osteoporosis definition, cognitive test or age group unless a reader examines the individual studies. Nor does it show that a person with osteoporosis will progress from normal cognition to dementia.

What the cognitive-impairment-to-osteoporosis evidence means
The separate 2023 review examined ten studies with 9,872 participants and found an RR of 1.56 for osteoporosis among people with cognitive impairment. Its search extended through August 2023. The full-text record reports I² of 88% for osteoporosis outcomes, indicating substantial statistical heterogeneity.
High heterogeneity means that the studies’ estimates differed more than would be expected from sampling variation alone. Differences in age, setting, follow-up, cognitive assessment, bone measurement and adjustment for other health factors can all contribute. The pooled direction is informative, but it is not a single uniform risk that clinicians can apply without considering the individual patient.
The reverse association also has a practical implication: a person with cognitive impairment may need deliberate attention to bone health, not because dementia has been proven to cause osteoporosis, but because the two conditions frequently coexist and can complicate assessment, mobility and adherence to care.
What the 2025 bidirectional review adds
Nasiri and colleagues’ 2025 review in Bone included 15 observational studies identified in searches of PubMed, Scopus and Cochrane through August 15, 2024. Its abstract reports significant associations in both directions. This is a broader and more recent synthesis than either 2023 review, but it remains observational.

The authors call for larger longitudinal studies. That recommendation matters because temporality is essential for causal inference: researchers need repeated bone and cognitive measurements, clear timing of exposure and outcome, careful control of confounding and enough follow-up to distinguish early disease from long-term progression. A review that combines cross-sectional, case-control and cohort evidence cannot supply all of that by itself.
What the longitudinal BMD analysis adds—and does not add
The analysis of the Framingham, Rotterdam and Rush Memory and Aging Project cohorts contributes time-order information. It included 4,431 people, recorded 606 incident dementia diagnoses and identified Alzheimer’s disease in 498 of those cases. Higher baseline BMD was associated with lower subsequent dementia risk, with an HR of 0.47 per g/cm² increase.
Temporal ordering makes the result more informative than a measurement taken after dementia is established, but it still does not prove that BMD controls dementia biology. BMD may mark other characteristics associated with later health outcomes, and one baseline measurement cannot show whether changing BMD would change risk.

An association with dementia appeared in the Framingham cohort, but bone loss was not statistically significant in the pooled analysis. Therefore, the findings support a distinction between a single baseline BMD level and the rate of change over time. They should not be summarized as proof that accelerated bone loss causes dementia.

Do osteoporosis medicines, calcium or vitamin D prevent cognitive decline?
The evidence summarized here does not demonstrate that treating osteoporosis prevents or delays dementia. No clinical trial evidence establishing that outcome was identified in the reviewed material. That is an evidence limitation, not proof that no relevant study exists anywhere.
Osteoporosis medication, calcium and vitamin D should therefore be discussed for established bone-health indications, dietary needs or documented deficiency under individualized clinical advice—not promised as cognitive protection. A treatment that reduces fracture risk can be valuable even when it has no demonstrated effect on memory, and a lack of cognitive benefit would not make appropriate osteoporosis treatment unnecessary.
What to do if you have osteoporosis and notice memory changes
Neither an osteoporosis diagnosis nor a BMD result can diagnose dementia. Memory complaints deserve their own assessment, while bone care should continue on its own clinical basis.
- Record the concern precisely. Note when symptoms began, examples of changes, whether they fluctuate and how they affect medication use, finances, driving, work or daily tasks.
- Arrange a clinical evaluation. A primary-care clinician or appropriate specialist can review history, medicines and other potentially reversible contributors, then decide whether cognitive screening or further testing is warranted.
- Bring the bone record. Provide BMD reports, fracture history, current osteoporosis medicines and any calcium or vitamin D products. This helps the clinician coordinate rather than infer a cognitive diagnosis from bone status.
- Review safety and adherence. Cognitive symptoms can make complex dosing schedules harder to follow. Ask for a regimen and follow-up plan that match the person’s abilities and support system.
- Seek urgent help for sudden change. A rapid onset of confusion is not the pattern addressed by these long-term association studies and needs prompt medical assessment.
Questions that improve a clinician discussion
- Is my bone diagnosis based on BMD, a fracture, or both, and what does the actual report show?
- Does my cognitive symptom pattern require screening, formal testing or evaluation for another cause?
- Could any current medicine, sleep problem, mood symptom or acute illness be affecting memory or balance?
- What osteoporosis treatment is recommended for my fracture risk, and what benefits and monitoring does it have?
- How should dosing, appointments and follow-up be organized if remembering instructions is difficult?
- Which changes should prompt an urgent call rather than waiting for a routine visit?
How to evaluate a new headline about bone density and dementia
Before accepting a claim, check five details:
- Exposure: Was the study about diagnosed osteoporosis, low BMD, fracture, or bone loss over time?
- Outcome: Did it measure a screening score, mild cognitive impairment, all-cause dementia or Alzheimer’s disease?
- Design: Was it cross-sectional, case-control, prospective cohort or a randomized treatment trial?
- Effect measure: Is the number an OR, RR or HR, and what is the confidence interval?
- Population and timing: How many people were studied, how long were they followed, and were the participants similar to the person asking?
A headline that omits these details can make a population association sound like an individual prediction or a treatment recommendation. The distinction is especially important when a study reports a relative measure without the underlying absolute risks.
What research is still needed
- Large, diverse longitudinal cohorts with repeated BMD and standardized cognitive assessments.
- Clear separation of osteoporosis, low BMD, fracture history and measured bone-loss rate.
- Consistent definitions for mild cognitive impairment, incident dementia and Alzheimer’s disease.
- Analyses that address medication use, health status, mobility, nutrition and other plausible confounders without overadjusting for factors that lie on the causal pathway.
- Randomized or otherwise rigorous treatment studies that test cognitive outcomes as prespecified endpoints, not merely as incidental observations.
- Results reported with absolute risks and clinically meaningful follow-up periods, so readers can understand magnitude as well as statistical significance.
The bottom line
Osteoporosis, lower BMD and cognitive impairment often appear together in observational research, and the association may run in both directions. A 2023 meta-analysis linked osteoporosis with higher odds of cognitive impairment, another linked cognitive impairment with higher osteoporosis risk, and a longitudinal cohort meta-analysis associated higher baseline BMD with lower later dementia incidence. None of these findings proves causation or shows that osteoporosis treatment prevents dementia. Treat bone disease and cognitive symptoms as related but separate clinical issues, and use individualized evaluation rather than a BMD result to guide diagnosis or expectations.
