For most adults with osteoarthritis (OA), treatment starts with tailored exercise, education and—when relevant—weight management. Medicines can make activity more tolerable, injections may provide brief relief, and joint replacement is considered when pain and loss of function continue to substantially impair quality of life despite suitable non-surgical care. The right combination depends on the affected joint, symptoms, daily goals, other health conditions and medicine risks.

This guide uses the UK National Institute for Health and Care Excellence (NICE) guideline for adults with OA across joints and identifies where recommendations from the American Academy of Orthopaedic Surgeons (AAOS) apply only to knee OA. Guidelines support shared decisions; they are not personal prescriptions.

How the main treatment options fit together

OA care is usually a progression rather than a choice between “trying everything” and having surgery. Exercise and self-management address function over time. A topical or oral medicine may reduce symptoms enough to let you exercise. An injection can be a short-term bridge during a flare or while building an exercise routine. Replacement surgery is a separate decision for persistent, substantial disability after non-surgical options have been ineffective or unsuitable.

NICE NG226 covers adults with OA in any joint. The AAOS non-arthroplasty and surgical guidelines cited here address knee OA, so their findings should not be transferred automatically to hip, hand, shoulder or other joints.

NICE’s full recommendations, published 19 October 2022, are available at NG226 recommendations. The AAOS non-surgical knee guideline was adopted 31 August 2021; its evidence process reviewed 15,103 abstracts, recalled 2,426 articles for full review and assessed 617 abstracts after full-text review and quality analysis, as shown in its 2021 evidence infographic.

Physical therapy and therapeutic exercise: the foundation

NICE says: “For all people with osteoarthritis, offer therapeutic exercise tailored to their needs (for example, local muscle strengthening, general aerobic fitness).” Exercise is not merely a last step before surgery. It is the core intervention used alongside education and symptom control.

What a tailored programme can include

  • Local strengthening: exercises that build the muscles supporting the affected joint.
  • Aerobic conditioning: walking, cycling, swimming or another tolerable activity to improve general fitness.
  • Movement practice: work on range of motion and the activities that matter to you, such as stairs or getting out of a chair.
  • Pacing and progression: begin at a manageable level and increase frequency, resistance or duration gradually.

Some pain when you begin or increase activity is possible. That does not automatically mean the joint is being damaged. A clinician or physiotherapist can adjust the dose, technique and progression if symptoms remain difficult. The benefit depends on regular, long-term participation rather than a few isolated sessions.

Supervised sessions and manual therapy

NICE allows supervised exercise sessions when they are appropriate to a person’s needs. AAOS recommends supervised, unsupervised or aquatic exercise over no exercise for knee OA. Supervision can help with technique, confidence, progression and adapting around other conditions, but a home programme may be more practical for some people.

NICE considers manual therapy only alongside therapeutic exercise for hip or knee OA; evidence does not support using it alone as the main treatment. Acupuncture, dry needling and electrotherapies listed by NICE, including transcutaneous electrical nerve stimulation (TENS), are not recommended because sufficient benefit has not been established.

Weight management and mobility aids

Weight loss when overweight or obese

For people with OA who have overweight or obesity, NICE states that any weight loss is likely to help, and that losing 10% of body weight is likely to be better than losing 5%. AAOS gives a moderate-strength recommendation for sustained weight loss in overweight or obese people with knee OA. Weight management should be supportive and individualized, not framed as blame or as a condition for receiving care.

NICE specifically says age, sex or gender, smoking, comorbidities and overweight or obesity alone should not exclude someone from referral for joint replacement. A person can work on weight and still receive assessment when symptoms warrant it.

Walking aids and supports

A walking aid is an optional tool for lower-limb OA. NICE says to consider walking aids, and AAOS reports moderate evidence that canes can reduce knee-arthritis pain and improve function. Fit, height, hand strength, balance and the side on which the aid is used matter; a physiotherapist or other clinician can check these points.

NICE does not recommend routinely offering insoles, braces, tape, splints or other supports. They may be considered when instability or abnormal loading exists, exercise is ineffective or unsuitable without the device, and the aid is likely to improve movement and function. AAOS finds that bracing could help selected people with knee OA, illustrating why the joint and guideline source should be identified before choosing a device.

Medication options and their safety trade-offs

Topical non-steroidal anti-inflammatory drugs (NSAIDs)

For knee OA, NICE says to offer a topical NSAID, and AAOS strongly recommends topical NSAIDs when they are not contraindicated. A topical product can provide local symptom relief while supporting participation in exercise. NICE says to consider topical treatment for other affected joints as well.

“Topical” does not mean risk-free. Follow the specific product label, check allergies and skin problems, and ask a pharmacist or clinician about other medicines and medical conditions. Different products are not automatically interchangeable.

Oral NSAIDs

NICE says to consider an oral NSAID when a topical NSAID is ineffective or unsuitable, after accounting for gastrointestinal, kidney, liver and cardiovascular toxicity and the person’s individual risk factors. Gastroprotection should be offered while taking an NSAID. Use the lowest effective dose for the shortest possible time.

Review is especially important for people with a history of ulcers or bleeding, kidney or liver disease, cardiovascular disease, pregnancy, older age, or medicines that can interact with NSAIDs. A pharmacist or prescriber can determine whether an oral NSAID is appropriate and whether monitoring or an alternative is needed.

Paracetamol, opioids and supplements

NICE does not recommend routine paracetamol or weak opioids for OA. It allows infrequent, short-term use only when other pharmacological options cannot be used or have not worked. Strong opioids are advised against. NICE also advises against glucosamine. These recommendations mean that a medicine should not become a default long-term substitute for exercise and activity.

Medication is intended to support non-drug care. Reassess whether it is improving walking, sleep, daily tasks or exercise participation, rather than judging success only by taking a tablet or applying a gel.

Joint injections: what they can and cannot do

Corticosteroid injections

NICE says an intra-articular corticosteroid injection may provide short-term relief, estimated at 2 to 10 weeks. It may be considered when other medicines are ineffective or unsuitable, or when temporary symptom reduction would help someone begin or maintain therapeutic exercise. AAOS likewise says steroid injections could provide short-term relief for knee OA.

An injection is therefore a time-limited symptom-management option, not a cure or a way to restore cartilage. The clinician should discuss whether the affected joint, current medicines and health conditions make an injection appropriate.

Hyaluronan injections

NICE advises against intra-articular hyaluronan injections, and AAOS says hyaluronic-acid injections are not for routine knee OA use. Marketing claims that an injection rebuilds cartilage or permanently reverses OA go beyond these recommendations.

When surgery becomes a reasonable discussion

Referral for joint replacement

NICE says to consider referral for hip, knee or shoulder replacement when joint symptoms—such as pain, stiffness, reduced function or progressive deformity—substantially affect quality of life and non-surgical management, including exercise, weight loss and pain relief, is ineffective or unsuitable.

Referral is not triggered by an X-ray alone. The decision should reflect symptoms, functional loss, goals, health conditions, medicine risks and what non-surgical care has been tried. The cited sources do not establish one age limit, imaging threshold or numeric severity score that applies to everyone. Surgery has an invasive recovery burden and individual risks, so an orthopaedic assessment is needed before deciding.

Arthroscopy is not the same as replacement

NICE advises against arthroscopic lavage or debridement for OA. AAOS also advises against lavage and debridement for primary knee OA. However, AAOS allows arthroscopic partial meniscectomy for selected people with mild to moderate knee OA and a meniscal tear after physical therapy or other non-surgical care has failed. That specific indication should not be broadened into “all arthroscopy works” or “no arthroscopy is ever useful.”

For AAOS’s knee-surgery recommendations, see the Surgical Management of Osteoarthritis of the Knee overview. The broader knee guideline is summarized at AAOS Osteoarthritis of the Knee.

A practical decision path

  1. Define the problem: identify the joint, the activities affected, symptom pattern and personal goals. A scan result without meaningful symptoms is not, by itself, a reason for surgery.
  2. Build the foundation: receive education and start a tailored strengthening and aerobic programme. Consider supervised sessions if technique, confidence or progression is difficult.
  3. Add weight management when relevant: any loss may help; a 10% reduction is likely to help more than 5% according to NICE, while keeping access to care separate from body weight.
  4. Use a mobility aid selectively: ask for fitting advice if a cane or other aid could improve safety or walking.
  5. Choose symptom relief to enable activity: for knee OA, discuss a topical NSAID first when suitable. Consider an oral NSAID only after reviewing gastrointestinal, kidney, liver, cardiovascular, pregnancy and interaction risks, with gastroprotection as advised.
  6. Reserve short-term options for a clear purpose: a corticosteroid injection may create a 2-to-10-week window for exercise or flare control when appropriate. Do not use hyaluronan routinely.
  7. Reassess function and quality of life: if symptoms remain substantially limiting despite suitable non-surgical management, discuss referral for replacement of the affected hip, knee or shoulder.

Questions to take to a clinician or physiotherapist

  • Which joint and activities should the programme target first?
  • How should exercise be modified if pain increases at the start?
  • Do my stomach, kidney, liver, heart, pregnancy or other medicine factors change NSAID choices?
  • Would a walking aid or brace address instability or abnormal loading in my case?
  • If considering an injection, what short-term goal will it support and what alternatives are available?
  • How is my quality of life and function being weighed if replacement referral is discussed?

Guideline wording and availability differ by country. NICE’s rationale and implementation context are explained in its NG226 rationale and impact, while AAOS notes that final knee-procedure decisions must account for the patient’s concerns and local resources in its non-arthroplasty clinical practice guideline.
