If you want an open-world RPG where killing a named character can permanently close quests, change faction routes, or turn an entire community against you, start with Fallout: New Vegas. For consequences that feel social and legal rather than purely narrative, choose Kingdom Come: Deliverance. Skyrim allows meaningful losses but protects many critical characters, while Red Dead Redemption demonstrates that some apparent NPC deaths can be recoverable.
What “massive consequences” means in these games
NPC death can affect a game in several distinct ways. A quest may become unavailable, a faction may become hostile, a unique item may be lost, or guards and witnesses may create an ongoing problem. Some games branch the main story; others make ordinary life in a settlement harder. A missing character is not always permanently dead, so recovery rules matter too.

Fallout: New Vegas: the broadest quest and faction fallout
Fallout: New Vegas is the strongest overall match for players who want violence to reshape an open-world story. The game uses an essential flag to stop certain important characters from being killed before their quests are complete. That safeguard does not cover everyone. Characters outside the protected set can die, and their faction relationships, quest availability and rewards can change as a result. Some NPCs become killable only after the quests that depend on them are finished.

Why the consequences feel large
Killing is evaluated through both karma and faction systems. The result depends on the victim’s faction and moral alignment, so the same act can improve one relationship while damaging another. This gives New Vegas more than a simple “quest failed” response: the political route through the game can change.
How to play without losing a route accidentally
- Make a manual save before attacking a named NPC, entering a faction base, or accepting a mission that targets a faction.
- Check whether the character is marked essential; protection means the game is preventing a progression break, not that the character is harmless to attack.
- Expect faction standing and karma to change even when no quest immediately fails.
- If a route closes, look for another faction or quest path rather than assuming the entire main story is over.
Kingdom Come: Deliverance: when a murder becomes a community problem
Kingdom Come: Deliverance is the best choice if you want the world to remember what you did. Its NPCs belong to one or more factions, and the crime system connects witnesses, reputation, relationships and law enforcement.

Witnesses and reputation
Witnesses can spread word of your behavior. Local reputation and personal relationships may change, so killing an innocent person can affect how a settlement treats you after the fight is over. The damage is social as well as mechanical: conversations, access and routine interactions can become more difficult.
Legal and practical penalties
When an offense becomes known, guards can impose penalties and confiscate loot. That means the consequences may include losing the equipment you carried away, facing enforcement and damaging your standing in the area. Killing an important person can therefore disrupt more than the associated quest.

Who should choose it
Choose Kingdom Come when you care more about believable social fallout than about a heavily branching faction campaign. It is less useful as a promise that every death creates a new story route; its strength is making crime costly in the ordinary life of the world.
Skyrim: meaningful losses behind an essential-NPC safety net
Skyrim offers a useful contrast. You can kill many characters, and some deaths make quests or unique items unobtainable. However, narrative-critical NPCs are marked essential and cannot be killed through ordinary play because their deaths would break progression.

What can still be lost
A nonessential character may be the only source of a quest, reward or unique item. If that person dies before you finish the relevant objective, the content can be gone for that playthrough. The game’s freedom is therefore real, but selective.

Essential protection keeps the main quest reliable. Skyrim is a good choice if you want the tension of accidental losses without routinely risking a completely broken campaign. It is not the best pick for a world in which every named NPC is equally vulnerable.

Red Dead Redemption: check whether the failure can be repaired
Red Dead Redemption shows why “dead” and “permanently removed” are not always the same result. In guidance for missing or dead quest givers, Rockstar Games Customer Support says: “Repeatedly save the game manually while at a house to advance the in-game time by several days until the NPC respawns.”
That advice applies to the documented recovery situation, not as a guarantee for every character or mission. If a quest giver disappears, make a manual save, advance time as instructed and check whether the NPC returns before treating the quest as permanently branched.
How to tell whether you killed the wrong NPC
- Look for an immediate quest update. A failed objective or removed marker is stronger evidence than an NPC simply vanishing.
- Check the character’s role. A named faction member, shopkeeper, companion contact or quest giver is more likely to affect future content than a generic hostile.
- Wait and revisit. Some games use protected states or respawn systems. Advance time only after creating a manual save.
- Check social symptoms. New hostility, changed reputation, guard attention or faction penalties indicate that the act had systemic consequences.
- Reload deliberately. If the lost content matters more than experimentation, return to the pre-kill save. Do not overwrite it while testing recovery.
Which game should you choose?
- Choose Fallout: New Vegas for the widest combination of faction politics, branching quests and consequences for violence.
- Choose Kingdom Come: Deliverance when witnesses, reputation and law enforcement are the consequences you want to feel.
- Choose Skyrim for meaningful but bounded losses with essential-character protection.
- Choose Red Dead Redemption if you want to verify recovery options before assuming an NPC failure is permanent.
The key comparison is not simply how many NPCs can die. It is whether the death is permanent, how widely the effects spread, whether another route exists, how society reacts and how aggressively the game protects essential characters.
