For most people with knee osteoarthritis, non-surgical care starts with regular therapeutic exercise and movement training. A physical therapist can adapt strengthening, flexibility, balance and activity work to your symptoms and goals. A brace, walking aid or injection may be added when an assessment identifies a specific mechanical problem, persistent pain or a barrier to exercising.

These treatments manage pain and improve function; they do not restore worn cartilage or reverse osteoarthritis. The right combination depends on which part of the knee is affected, whether the joint is unstable or overloaded, your response to exercise and a clinician’s assessment.

What non-surgical treatment is trying to achieve

Knee arthritis, usually referring to osteoarthritis (OA), is managed rather than cured by the options discussed here. The practical goals are to reduce pain, preserve or increase movement, strengthen the muscles that support the knee, and make daily activities safer and easier. Treatment is usually adjusted over time rather than delivered as one procedure that solves the problem permanently.

Guidelines do not treat every option equally. The 2022 NICE osteoarthritis guideline places tailored therapeutic exercise at the center of care. The 2026 VA/DoD guideline also supports structured physical therapy but found insufficient evidence to prefer one specific program type or delivery method. Braces and injections are adjuncts for selected situations, not automatic additions for every knee.

This is general information, not a diagnosis or personal treatment plan. A clinician should confirm the cause of knee pain and check whether a device or injection is appropriate before you use one.

Exercise and physical therapy: the foundation

What a therapist can tailor

Therapeutic exercise means purposeful movement selected for your limitations and goals, rather than a generic workout. An assessment may look at knee range of motion, leg strength, balance, walking pattern, pain triggers and the activities you need to resume. The resulting plan can combine strengthening, mobility work, movement practice and a gradual return to activities such as stairs, work tasks or recreational exercise.

NICE recommends therapeutic exercise tailored to the individual and says to consider supervised sessions. Supervision can help you learn technique, adjust the load when symptoms change and progress safely. It does not mean that every person must attend the same number or type of appointments.

There is no single required therapy format

The VA/DoD guideline (2026) found insufficient evidence to identify one structured physical-therapy type or delivery mode as superior. In practice, that means the useful question is whether a program is appropriate, achievable and improving your function—not whether it carries a particular label or is delivered in one setting. An in-person therapist, a home program taught during visits or another structured format may each be reasonable when matched to your needs and access.

Expect consistency, not an instant fix

NICE advises explaining that regular, consistent exercise is beneficial and that pain may initially increase when someone starts. A temporary increase does not automatically mean the joint is being damaged, but severe, persistent or worsening symptoms warrant a review of the exercise dose and technique. A therapist can alter repetitions, resistance, range or rest periods instead of abandoning movement altogether.

Track functional changes that matter to you—such as walking farther, standing longer or using stairs more easily—alongside pain. Those measures help distinguish a useful gradual improvement from a program that needs modification.

How to make a home program workable

  • Agree on a small, repeatable routine with the therapist rather than an unrealistic list of exercises.
  • Use the prescribed technique and progression; adding load rapidly can make symptoms harder to interpret.
  • Schedule exercise regularly, because sporadic sessions do not test whether the plan works.
  • Report pain patterns, swelling, instability or loss of function so the program can be adapted.

Braces, supports and walking aids

Why recommendations are selective

A brace changes how forces are supported or how the knee is guided during movement. It is most rational when an assessment identifies instability, abnormal loading or a particular compartment that can be supported. A brace is not a routine substitute for strengthening.

Guidelines differ in breadth:

  • The VA/DoD guideline (2026) suggests bracing for selected people with knee OA.
  • The American College of Rheumatology/Arthritis Foundation guideline (2019) strongly recommends tibiofemoral bracing for tibiofemoral OA and conditionally recommends patellofemoral bracing.
  • NICE (2022) says not to routinely offer braces, insoles, tape, splints or supports unless there is joint instability or abnormal biomechanical loading, exercise is ineffective or unsuitable without a device, and the device is likely to improve movement and function.

These statements are not necessarily contradictory: they reflect different thresholds for offering a device. The common point is that the brace should solve an identified mechanical or functional problem.

Which knee area is involved?

Situation Device discussion What must be established
Tibiofemoral OA The ACR/Arthritis Foundation strongly recommends tibiofemoral bracing. The clinician should confirm that this is the affected compartment and that the brace is likely to improve function.
Patellofemoral OA The ACR/Arthritis Foundation conditionally recommends patellofemoral bracing. Expected benefit is less certain; selection and fit matter.
Instability or abnormal loading NICE allows consideration of a brace or support when these findings are present. Exercise alone must be ineffective or unsuitable without the device, and improved movement or function should be likely.

A clinician or trained fitter can identify the relevant brace type and check alignment, comfort and movement. Buying a brace without that assessment may produce a poor fit or address the wrong knee problem. A brace should complement, not replace, an exercise program when exercise is suitable.

Walking aids are a separate option

NICE says to consider a walking aid, such as a walking stick, for lower-limb osteoarthritis. The aim is to make walking safer or less painful when a temporary or ongoing aid improves function. The correct side, height and technique should be demonstrated by a clinician or therapist.

Injection options: what each one can and cannot do

Injections are clinician-delivered procedures for symptoms that remain troublesome despite, or prevent participation in, other care. They do not repair cartilage. Suitability depends on the diagnosis, previous treatments, other health conditions and the clinician’s assessment of procedural risks. Ask what benefit is expected, how long it may last and what alternatives exist.

Corticosteroid injections

NICE recommends considering an intra-articular corticosteroid injection when other pharmacological treatments are ineffective or unsuitable, or when an injection would help someone participate in therapeutic exercise. The VA/DoD guideline suggests corticosteroid injection for persistent knee OA pain that has not been adequately relieved by other interventions.

NICE describes the expected benefit as short term: 2 to 10 weeks. That is a guideline range, not a guarantee for an individual. The purpose is symptom relief and improved participation in activity, not disease reversal. If relief is brief or absent, the plan should be reassessed rather than treating repeated injections as an automatic next step.

Hyaluronic acid (viscosupplementation)

Recommendations are openly inconsistent:

  • NICE (2022) advises against intra-articular hyaluronan for osteoarthritis. Its recommendation 1.4.9 states: “Do not offer intra-articular hyaluronan injections to manage osteoarthritis.”
  • The VA/DoD guideline (2026) finds insufficient evidence to recommend for or against viscosupplementation and says the evidence does not support choosing it over a corticosteroid injection.
  • The German S3 guideline (2025) describes the evidence as contradictory and makes no recommendation.

Therefore, hyaluronic acid should not be presented as a universally accepted or clearly superior injection. If it is discussed, ask which guideline and local practice standard the clinician is following and what evidence applies to your situation.

Platelet-rich plasma (PRP)

PRP is also not established as routine treatment across guidelines:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • The VA/DoD guideline (2026) finds insufficient evidence to recommend for or against PRP for persistent knee OA pain.
  • The German S3 guideline (2025) says PRP may be considered when other pain-relieving and function-improving measures are inappropriate, contraindicated or ineffective. It notes heterogeneous study quality and says there is no basis for recommending one specific manufacturing process.
  • The ACR/Arthritis Foundation guideline (2019) strongly recommends against PRP for knee or hip OA.

The disagreement means PRP should be discussed as an uncertain, non-standard option rather than an established cartilage treatment. Ask about the rationale, alternatives, costs and how success would be judged before agreeing to a procedure.

Comparing the options at a glance

Option Type of care Who may be considered Expected role Guideline agreement
Therapeutic exercise / physical therapy Ongoing self-management with optional supervision Most people who can exercise safely, with tailoring for symptoms and goals Improve movement, strength and function; regular practice is required Broad support; no single preferred structured format identified by VA/DoD
Brace or support Device used during selected activities People with instability, abnormal loading, a relevant knee compartment or a likely functional gain Mechanical support and symptom or function assistance; not a replacement for exercise Recommendations range from selected use to strong/conditional support by compartment; NICE discourages routine use
Corticosteroid injection Clinician-delivered procedure Persistent symptoms inadequately relieved by other care, or a need to enable exercise Short-term symptom relief; NICE states 2–10 weeks Supported for selected circumstances
Hyaluronic acid injection Clinician-delivered procedure No uniform guideline-defined group Benefit remains uncertain and guidance conflicts NICE advises against; VA/DoD finds insufficient evidence; German S3 finds evidence contradictory
PRP injection Clinician-delivered procedure Only exceptional consideration in guidance that permits it Uncertain symptom and function benefit; not proven routine care VA/DoD insufficient evidence; German S3 conditional consideration; ACR/AF recommends against

A practical way to build a treatment plan

1. Confirm the problem and the functional target

Discuss where the pain is, what activities are limited, whether the knee gives way, and whether loading or alignment appears abnormal. A plan should have a concrete target, such as walking to work or managing stairs, rather than pain reduction as the only measure.

2. Start or refine therapeutic movement

Use an individually tailored program, with supervised sessions when useful. Agree how often to exercise, how to respond to an initial pain increase and when to review progress. The VA/DoD evidence does not require one delivery mode, so access and adherence can guide the format.

3. Add a device only for a defined reason

If instability, abnormal loading or a compartment-specific problem is present, ask whether a brace or walking aid is likely to improve movement and function. Have the device fitted and reassessed while you move; discontinue or adjust it if it causes problems or does not improve the intended activity.

4. Discuss an injection only when the decision is specific

For corticosteroid, clarify the short-term goal and the 2–10-week range described by NICE. For hyaluronic acid or PRP, ask why the clinician is recommending an option on which guidelines disagree, what alternatives exist and what outcome would justify not repeating it.

5. Review function and revise

Keep track of activity tolerance, walking and exercise participation. Persistent pain, new instability or failure to improve should prompt reassessment of the diagnosis, exercise dose, device fit and treatment goals rather than an automatic escalation to another procedure.

Common mistakes to avoid

  • Expecting a brace to cure arthritis: it may support mechanics or function in selected cases but does not reverse joint changes.
  • Stopping all movement after an early flare: NICE notes that pain can initially increase; ask for a graded adjustment unless symptoms are severe or worsening.
  • Choosing an injection because it sounds regenerative: corticosteroid is for short-term symptom relief, while HA and PRP have conflicting or insufficient evidence across guidelines.
  • Assuming a guideline applies everywhere: NICE, VA/DoD, German S3 and ACR/Arthritis Foundation issue recommendations for different health systems and dates. Your clinician can explain which applies locally.
  • Using a product without assessment: an ill-fitting brace or walking aid can fail to address the actual problem; fitting and functional testing matter.

Bottom line

Build knee-arthritis care around consistent, tailored therapeutic exercise, using physical therapy to teach and adjust movement. Consider a brace or walking aid only when instability, abnormal loading or a defined functional need makes a device likely to help. Corticosteroid injections can provide short-term relief—NICE cites 2 to 10 weeks—when other care is unsuitable, ineffective or insufficient to permit exercise. Hyaluronic acid and PRP remain contested or insufficiently supported, so their use requires an explicit discussion of uncertainty and alternatives.
