A Node.js health endpoint is only useful when its result has a clear operational meaning. Liveness asks whether a process should be restarted; readiness asks whether it should receive traffic; an external uptime check asks whether a user-facing route works from outside the service. Record these signals alongside the deployment and rollout that produced them if they may trigger a rollback.
What does “uptime” mean for a Node.js service?
There is no single health signal that answers every availability question. A process-age value can show how long a process has been running, but it does not prove the process is responsive or that users can complete a meaningful request. A liveness probe, readiness probe, dependency check, and external synthetic check observe different failure boundaries and may lead to different actions.
| Signal | Question it answers | Typical response to failure | What it does not establish by itself |
|---|---|---|---|
| Process age or uptime | How long has this process been running? | Usually diagnostic information; policy depends on the system consuming it. | That the process is responsive, ready for traffic, or reachable by users. |
| Liveness | Can this process continue, or might restarting it help? | An orchestrator may restart the process, according to its probe configuration. | That dependencies or the complete user-facing route are healthy. |
| Readiness | Should this instance receive new traffic now? | Traffic routing can stop sending work to the instance until it becomes ready. | That the entire service is reachable from outside the cluster. |
| Dependency or component check | Is a named dependency or resource in the expected state? | Alert, report degradation, or influence readiness, depending on policy. | That every dependency failure should trigger a restart or remove the instance from service. |
| External synthetic check | Can an observer outside the process reach and use a selected route? | Alert or initiate an operational response; rollback is possible only if a deployment workflow is designed to use that evidence. | Which internal component caused a failure. |
These signals can complement one another. An application endpoint reports what the process knows; an external check can reveal failures in routing, networking, or the tested user path beyond the process boundary. Select the checks based on the question and action you need, rather than treating one endpoint as a universal uptime verdict.

Liveness: could restarting help?
Keep liveness focused on whether the process can continue operating. A failed shared dependency is generally a poor reason, by itself, to fail liveness: restarting every affected process may not repair the dependency and can add restart load while the dependency is already impaired. The Node.js Reference Architecture describes liveness and readiness as distinct probe roles and uses /livez and /readyz as examples (Node.js Reference Architecture: health checks).

Readiness: should this instance receive traffic?
Readiness informs traffic routing. It can account for whether the instance can handle new work, including dependencies that are essential for serving that work. The right policy depends on the service architecture: a degraded optional cache, for example, may warrant visibility without making the instance unready, while an unavailable dependency required for every request may justify removing it from rotation.
Path names are conventions, not behavior. Use names such as /livez and /readyz, or another unambiguous pair, and make sure the application routes and the orchestrator’s configured probes agree on their meaning. The cited Node.js guidance recommends successful 200 OK responses for successful probes, and cautions against unnecessarily aggressive intervals or startup delays that are too short. The endpoint alone does not set the operational policy; probe configuration does.
What should a health endpoint check?
Start with the service’s actual operating requirements and decide what a failure should cause. A component can be unhealthy, degraded, or unavailable without every state needing the same HTTP response or routing consequence. NestJS Terminus documents indicators for HTTP services, databases and ORMs, microservices, memory, and disk. Its documented degraded-cache example can remain HTTP 200 while the degraded condition is still shown to monitoring (NestJS Terminus documentation).
- Check critical dependencies deliberately: include a backend when its state affects the endpoint’s purpose, and decide whether that state should affect readiness, be reported as degradation, or only be logged or alerted.
- Avoid turning diagnostics into a fragile traffic gate: a check of every optional dependency can make an otherwise useful instance appear unavailable when it could still serve requests.
- Separate public response from operational detail: return only information appropriate for the endpoint’s audience; detailed component states may belong in a protected monitoring path rather than an unauthenticated public response.
- Set resource checks to a purpose: memory and disk indicators can expose risks, but their thresholds and resulting actions should reflect the service and its runtime environment.
For NestJS applications, Terminus is one documented option, with a HealthCheckService and predefined indicators. Confirm compatibility with the framework version and the application’s configuration before adopting it; the existence of an indicator does not determine the service’s readiness or restart policy.
For Kubernetes-oriented Node.js services, Lightship documents separate /health, /live, and /ready responses and graceful shutdown behavior (Lightship documentation). Treat these paths and behaviors as package-specific rather than universal Node.js conventions, and verify the current documentation for the version in use before relying on defaults.
When is an external uptime check useful?
An in-process health response cannot, by itself, prove that an outside user can reach a meaningful service path. A complementary external check can test a route from beyond the process or cluster boundary, which is useful when availability through public routing matters. This is a separate observation path, not a substitute for liveness or readiness.

Choose the observer location, tested route, authentication method, and alert threshold to match the real user journey. A simple health route may confirm that the server responds while missing a broken sign-in or other essential flow; a more demanding route may create unnecessary load or expose sensitive operations. The check should answer a defined availability question and have an owner for acting on its alerts.
What makes health evidence safe to use for rollback?
A rollback decision needs more than a failed status code. Operators should be able to reconstruct what was measured, against which threshold, during which deployment, and why the workflow acted. A practical record can include:
- Service and environment.
- Deployment or release identifier and revision.
- Rollout stage or phase.
- Check name and scope, such as liveness, readiness, a dependency, or an external route.
- Observation window and timestamp.
- Measured value, threshold or predicate, and evaluation result.
- Workflow decision and action taken, such as continue, pause, alert, or roll back.
This field set is a design pattern, not a mandated health API schema. Preserve enough information to distinguish a service regression from a noisy signal, a changed threshold, or an unrelated dependency incident. Keep the measurements and the workflow decision tied to the same rollout context.

A 2026 Meta experience report describes a deployment-time Service Health Checker that combines templated metric queries, thresholds, and workflow predicates with tiered, phased rollouts, allowing regressions to trigger automatic rollback. The report also discusses noise, alert fatigue, drift, and regressions that checks did not cover. It presents an organizational system serving “thousands of heterogeneous services,” not a universal standard or a quantified guarantee of improved reliability (Meta, “Service Health Checker: A System for Safely Deploying Changes at Scale”).
Before automating rollback, establish that the signal is relevant to the change, define an observation window and threshold, and decide how the workflow handles missing or conflicting evidence. Phased rollout can limit exposure while checks run; it cannot compensate for checks that do not cover the regression in question.
How is an application health API different from a public status API?
An application health endpoint is usually designed to inform a probe or monitoring system about a running service. A public status API can instead expose an operational record for people and integrations: summary information, component states, incidents and updates, and scheduled maintenance.
The Node.js Status API documents incident statuses such as Investigating, Identified, Monitoring, Resolved, and Postmortem, as well as maintenance statuses such as Scheduled, In Progress, Verifying, and Completed (Node.js Status API). These are labels in that status-page API, not required states for a Node.js application’s liveness or readiness endpoint. Keep public incident communication distinct from the machine-facing signals that route traffic or restart processes.
