Next-generation sequencing (NGS) lets researchers examine large amounts of genetic sequence, making it possible to find candidate variants, study complex genetic changes, and investigate how genes are expressed. Those capabilities can inform research and clinical decisions, but detecting a variant does not establish what it means for an individual or dictate a treatment. The usefulness of a result depends on the test’s validation, the evidence behind its interpretation, and how it fits the person’s clinical context.
What is next-generation sequencing?
Next-generation sequencing is a set of technologies for reading DNA or RNA sequence at scale. Instead of examining only a small, predefined region, an NGS test can read large sections of a person’s genome. The U.S. Food and Drug Administration (FDA) describes this capacity as a way to identify genetic variants that may help diagnose, treat, or understand disease.
A sequence result is information to interpret, not a diagnosis on its own. A test may detect a difference in genetic sequence, but its significance depends on evidence about the variant and the context in which it was found. NGS can therefore expand what researchers and clinicians can examine without automatically answering what a finding means for a particular person.
How do short-read and long-read sequencing differ?
Sequencing instruments read DNA in pieces called reads. The length of those pieces affects how much surrounding sequence is available to help place and interpret a read, particularly when the genome contains repeated or structurally complex regions.

| Approach | Typical read length | What the read length can help with |
|---|---|---|
| Short-read | 50–200 base pairs | Reads shorter fragments; interpretation can be more difficult when a sequence occurs in repeated or complex regions. |
| Long-read | 10–100 kilobases and longer | Longer fragments can help resolve structural variation and characterize full-length transcripts. |
These typical lengths are described by the National Human Genome Research Institute (NHGRI) on a page updated in 2026. They are technical descriptions, not measures of diagnostic success or health benefit. The approaches offer different views of sequence; neither is a universal winner for every research or clinical question.
What can long-read sequencing reveal?
Some genetic changes involve larger stretches of DNA or rearrangements, rather than a single letter difference. NHGRI identifies improved detection of structural variation as an important long-read capability. Having longer stretches of sequence can also help researchers see how sequence pieces fit together in regions that are difficult to characterize using short fragments alone.
Long reads can also support sequencing full-length transcriptomes. A transcript is an RNA copy produced from a gene; studying full-length transcripts can give researchers a broader view of RNA molecules than examining only short pieces. This is useful for research into gene activity and transcript structure, but it does not by itself establish that a finding causes disease or changes care.

As one example of the research infrastructure enabled by multiple long-read technologies, NHGRI reported in its 2026-updated page that the Human Pangenome Reference Consortium released 47 nearly complete, haplotype-resolved human genomes from diverse backgrounds. These reference genomes are a resource for studying human genetic variation; the figure is not a measure of clinical outcomes.
How can sequencing information help researchers and clinicians?
Investigating disease
In a person being evaluated for a suspected genetic condition, sequencing may identify variants for a clinician to assess alongside symptoms, family history, and other clinical information. FDA describes NGS as a way to rapidly sequence large sections of a genome and find variants that may be useful in diagnosing, treating, or understanding disease. A finding is one contribution to decision-making, not an automatic diagnosis or treatment recommendation.
Studying treatment response
Genetic information can be part of precision medicine research, including work on pharmacogenetics and individualized therapeutics. FDA describes these as areas where genetic information may inform understanding of disease and treatment. Whether a particular result is useful for a particular treatment depends on the evidence and the clinical setting.

Connecting sequence to other evidence
Research can link sequencing data with clinical outcomes, electronic health records, imaging, and proteomic data. These connections may help researchers investigate how genetic findings relate to disease and other biological information. The value of an association still depends on the quality of the underlying data and the strength of the evidence; combining data types does not remove the need for interpretation.

What determines whether an NGS result is clinically useful?
Clinical utility is not just a question of what a sequencing technology can read. It also depends on whether the test measures the intended result reliably, whether that result has a well-supported interpretation, and whether care teams can use it appropriately. FDA’s materials distinguish analytical validity—how well a test measures what it is intended to measure—from clinical validity, the relationship between a result and a clinical condition or meaning.
NHGRI’s Clinical Sequencing Evidence-Generating Research program describes evidence for promising clinical uses as still accumulating. Integrating comprehensive sequence data into care can require interpretive tools, institutional policies, changes to clinical workflows, and plans for uncertain or incidental findings. A technically detected variant may be real without being clinically meaningful for the person tested.

How should researchers and clinicians choose an approach?
The choice should follow the question being asked, rather than an assumption that one read length is best for every purpose. Relevant considerations include:
- Required read length: Determine whether the question can be addressed with short fragments or needs longer stretches of sequence.
- Variant or transcript complexity: Consider whether structural variation, difficult genomic regions, or full-length transcripts are central to the analysis.
- Intended use and validation: Assess whether the particular test has been validated for its intended research or clinical purpose and whether its analytical performance is suitable.
- Interpretation and care context: Establish how findings will be interpreted, communicated, and used within the relevant workflow.
The cited agency and institute materials do not establish a current platform-by-platform comparison, a general cost ranking, or a universal best approach. Those questions require information about specific instruments, validated assay types, location, and use case.

What safeguards support reliable and responsible use?
Validation and reproducibility
Tests need appropriate analytical validation, and results should be reproducible across methods and testing sites. NIH and FDA highlighted reference materials and concordance—the agreement of results across approaches—as important validation issues. A result that cannot be reliably reproduced is a weak basis for interpretation or care decisions.

Reference samples and representative data
Reference materials and datasets help laboratories assess performance and compare results. In a 2021 notice, NIH and FDA described gaps at that time in diverse, well-characterized, sustainable reference materials, as well as a lack of consensus standards. This is a dated agency assessment, not a newly measured 2026 statistic. More representative and well-characterized resources can support more reliable comparison and interpretation.
Uncertain and unexpected findings
Broader sequencing can reveal results beyond the original question, including variants whose significance is uncertain or findings that are incidental to the reason for testing. NHGRI identifies ethical, legal, and psychosocial considerations around interpreting and returning such results. How those findings are handled is part of responsible implementation, not an automatic consequence of generating more sequence data.
What NGS can—and cannot—establish
NGS expands the scale and kinds of genetic information available for research and clinical evaluation. Short reads and long reads provide different views; long reads can be especially useful for structural variation and full-length transcripts. But sequencing is one part of a larger process: validation, interpretation, evidence, and clinical context determine whether a result can responsibly inform a decision.
