Peripheral nerve stimulation (PNS) and spinal cord stimulation (SCS) are different treatments. PNS delivers electrical pulses to a selected nerve outside the brain and spinal cord. SCS places leads near the spinal cord and sends pulses through those leads. Neither category is universally “better”: the appropriate choice depends on the nerve or spinal target, your diagnosis, the exact device’s cleared indication, trial results, surgical suitability, and the practical burden of living with the equipment.

“Nerve stimulator implant” is also an imprecise phrase. Some PNS systems use a percutaneous electrode and an external stimulator for a limited treatment course; others are implantable. SCS systems generally use implanted leads and an implanted pulse generator, but their waveforms, controls, charging requirements, and approved uses differ. This guide explains the distinctions you should discuss with a pain specialist.

What is the essential difference?

The anatomical target is the clearest distinction:

  • SCS: Leads are positioned in the epidural space near the spinal cord. An implanted pulse generator sends electrical pulses through the leads. An FDA description of an Abbott system places the generator near the hip and connects it to leads near the spinal cord.
  • PNS: An electrode is positioned next to a named peripheral nerve, such as a nerve supplying the painful area. “Peripheral” means outside the brain and spinal cord. The electrode may be temporary and connected to equipment outside the body, or part of an implanted system, depending on the product.

Because the targets differ, the two treatments are not interchangeable versions of the same operation. A person with pain in a particular nerve distribution may be evaluated for PNS, while pain involving several regions of the trunk or limbs may be evaluated for SCS. Those are general patterns, not automatic rules; the diagnosis, imaging, neurological examination, prior treatment, and device labeling determine whether either option is reasonable.

How spinal cord stimulation works

Hardware and stimulation

SCS uses one or more leads placed close to the spinal cord and a pulse generator, usually implanted under the skin. A patient controller and a clinician programmer allow stimulation settings to be adjusted. Systems can offer different waveforms and programming approaches. FDA device descriptions include conventional tonic stimulation and burst modes. Another FDA-cleared system, Evoke, uses closed-loop control: it measures spinal-cord activation after each pulse and adjusts the stimulation current in that mode.

Conditions and labeled uses

FDA descriptions characterize selected SCS systems as aids in managing chronic, intractable pain of the trunk or limbs. Examples in device supplements include failed-back-surgery syndrome, intractable low-back and leg pain, and particular presentations of diabetic peripheral neuropathy. A January 18, 2022 FDA supplement for the Senza system describes an implanted rechargeable system for chronic intractable trunk or limb pain and a specific 10-kHz indication for certain nonsurgical back-pain patients. An Abbott supplement dated May 30, 2024 lists selected failed-back-surgery, nonsurgical back-pain, and diabetic-neuropathy uses with qualifications tied to stimulation mode and patient population.

These examples do not mean that every SCS system treats every type of back, leg, or nerve pain. The exact model, lead configuration, waveform, age group, diagnosis, and prior-treatment requirements in current labeling matter.

How peripheral nerve stimulation works

Temporary and external configurations

FDA’s classification for temporary PNS describes a prescription system with one or more external electrical stimulators and percutaneous electrodes placed at peripheral nerves. The treatment is intended to provide sustained pain relief after the stimulation period ends. In this configuration, the electrode passes through the skin for the prescribed course and the stimulator remains outside the body.

Implantable PNS configurations

Professional guidance, including a 2024 American Society of Interventional Pain Physicians (ASIPP) guideline article, also describes implantable PNS systems. An implantable system can involve a lead, receiver, or generator that remains in the body, with device-specific controls or external components. Therefore, ask whether the proposed PNS is temporary or intended to remain implanted, what equipment stays outside the body, and how removal or revision would work.

Indications depend on the product

PNS indication is not defined by the word “PNS” alone. The cleared use of the specific system determines which nerves, diagnoses, body regions, treatment duration, and patient characteristics are covered. The FDA 510(k) record for the SPRINT PNS device identifies that device and its substantial-equivalence decision; that record by itself does not establish that the system is suitable for an individual patient or for every PNS use.

PNS and SCS compared

Decision point Peripheral nerve stimulation Spinal cord stimulation
Anatomical target A selected peripheral nerve outside the brain and spinal cord. Leads positioned near the spinal cord in the epidural space.
Typical hardware pattern May be a percutaneous electrode with an external stimulator, or an implantable system. Design varies substantially by product. Implanted leads connected to an implanted pulse generator, plus a patient controller and clinician programmer.
What determines eligibility The named nerve, diagnosis, pain distribution, and the exact device’s cleared indication. The pain diagnosis and distribution, prior treatment, surgical suitability, trial response, and the exact system’s labeling.
Trial approach Depends on the device and treatment plan; some temporary systems are themselves a limited-course treatment. Many treatment pathways use a temporary stimulation trial before permanent implantation, with success criteria agreed in advance.
Stimulation options Product-specific; do not assume that one PNS system’s settings or duration apply to another. Systems may provide tonic, burst, high-frequency, or closed-loop modes, depending on the model.
Equipment outside the body External stimulator or controller may be present during treatment; requirements differ by system. Usually a handheld patient controller; charging and programming needs depend on the implanted generator.
Duration Some treatments are temporary; an implantable system may remain until removal, revision, or battery replacement. Implanted hardware is intended for ongoing therapy, with follow-up, reprogramming, and eventual generator management as applicable.
Evidence comparison The available evidence summarized here does not provide a direct head-to-head trial establishing that one category is more effective, safer, less invasive, or longer-lasting overall.

Who might be considered for a stimulator?

These procedures are generally considered after a clinician has identified a plausible pain generator and conservative measures have not provided adequate relief or have caused unacceptable effects. Selection is individualized rather than based on pain intensity alone.

Questions clinicians typically assess

  • Is the pain neuropathic or otherwise consistent with the proposed nerve or spinal target?
  • Is the painful area stable enough to target, and does the distribution match the lead location?
  • What diagnoses, operations, injections, medicines, rehabilitation, and behavioral treatments have already been tried?
  • Can you undergo the procedure safely, considering infection risk, bleeding risk, medical comorbidities, and ability to heal?
  • Can you understand, operate, charge, or otherwise manage the controller and follow-up plan?
  • Do your goals involve pain reduction, improved sleep or function, reduced medication use, or another measurable outcome?

FDA labeling for particular SCS systems can state that a patient should not proceed when the system cannot be operated, when effective relief was not achieved during a trial, or when the person is a poor surgical candidate. Those are device-specific restrictions, not a universal checklist for every PNS or SCS product.

What happens during an SCS trial?

A trial is a temporary test of stimulation before a permanent generator is implanted. Exact steps vary by clinician and system, but the decision process usually follows this sequence:

  1. Define the target and goals. Your clinician maps the pain distribution, reviews the diagnosis and prior treatment, and records baseline pain and functional measures. Agree on what improvement would justify permanent implantation, such as a specified reduction in pain or a concrete ability to walk, sleep, or work.
  2. Review safety and device restrictions. Discuss medications that affect bleeding, infection prevention, anesthesia or sedation, positioning, and any condition that could make implantation unsafe. Confirm that you can use the controller and attend follow-up.
  3. Place temporary leads. Leads are inserted through the skin and positioned near the spinal cord under procedural guidance. The generator used for the trial is external, so the system can be removed if the trial is unsuccessful.
  4. Use the system during the trial period. You may be asked to record pain, activity, sleep, medication use, and stimulation settings. Follow instructions about dressing care, bathing, bending, lifting, and contacting the clinic for fever, drainage, neurological symptoms, or lead movement.
  5. Evaluate meaningful benefit. The team compares your agreed measures with baseline. A favorable trial supports discussion of permanent implantation; it does not guarantee durable benefit. ASRA consensus guidance addresses patient selection and trial stimulation, while FDA labeling for specific systems makes trial response a condition of use.
  6. Remove temporary equipment or plan implantation. If benefit is inadequate or complications occur, the temporary leads are removed. If the result is meaningful and you remain a suitable candidate, the clinician discusses permanent hardware, expected follow-up, and alternatives.

A trial can reveal whether stimulation reaches the painful area and whether you can manage the device. It cannot establish that pain relief will never fade or that revision will not be needed.

What happens with PNS?

The procedure depends on whether the proposed system is temporary or implantable.

Temporary percutaneous PNS

A clinician places an electrode through the skin next to the selected peripheral nerve. The electrode connects to an external stimulator for the prescribed treatment period. You receive instructions for dressing care, activity, bathing, controller use, and symptoms that require urgent contact. At the planned endpoint, the electrode is removed if the system is designed for temporary use. The FDA classification describes the goal as sustained pain relief after treatment ends, but the degree and duration of benefit are not guaranteed for every patient.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Implantable PNS

An implantable system may require a procedure to position a lead or receiver and place a generator or other implanted component. Some systems use external equipment to communicate with the implant. Ask which parts remain in the body, whether charging is required, how programming is performed, and under what circumstances removal or revision is recommended.

Benefits, limitations, and risks to discuss

Potential benefit is condition-specific. A stimulator may reduce pain and support sleep, movement, rehabilitation, or medication reduction, but relief is not assured and may change over time. Do not transfer an outcome from one diagnosis or device to another.

Procedure and hardware burdens

  • Temporary or permanent infection, bleeding, pain at the implant site, and wound-healing problems.
  • Lead migration, breakage, loss of stimulation coverage, uncomfortable stimulation, or hardware malfunction.
  • Need for reprogramming, revision, removal, or generator management.
  • Restrictions on bathing, lifting, bending, driving, or vigorous activity during healing, according to the treating team.
  • Time and travel for programming visits and follow-up.
  • Imaging, surgery, and other procedures may require device-specific precautions. Confirm MRI eligibility and emergency-procedure instructions for the exact model rather than assuming all stimulators have the same rules.

Why a trial does not guarantee success

ASRA guidance recognizes that some patients report inadequate long-term pain relief with SCS. A good trial can support a decision, but it is not a promise of permanent benefit. Pain may change location, the therapeutic effect may diminish, or mechanical and medical complications may arise. The same principle applies to PNS: short-term response, temporary relief, and durable relief are not interchangeable outcomes.

What does the evidence say about durability?

The evidence summarized here supports the anatomical distinction and device-specific indications but does not establish a universal winner. Studies of unlike diagnoses, waveforms, lead designs, and follow-up periods should not be ranked as if they were a direct comparison.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

One specific figure illustrates why context matters: a 2023 U.S. Food and Drug Administration supplement reports 53% average pain relief maintained for more than five years at clinically meaningful levels, defined there as at least 50% relief, in evidence concerning Abbott Prodigy, Proclaim, and Proclaim XR systems for lower-extremity diabetic peripheral neuropathy. This is an indication-specific, system-specific summary. It is not a general SCS average, not a PNS result, and not a head-to-head comparison.

Commercial availability and regulatory status can change. The 2024 ASIPP article’s list of commercially available FDA-cleared PNS systems reflects information at publication and is not a universal endorsement. Before consenting, ask the clinician to verify the current labeling for the exact model being proposed.

How to choose between PNS and SCS with your specialist

  1. Name the pain precisely. Bring a diagram showing where pain, numbness, tingling, and weakness occur. Note whether symptoms follow one nerve, a dermatome, several limbs, or a broader trunk pattern.
  2. Identify the proposed target. Ask which peripheral nerve or spinal level the clinician believes is driving symptoms and what examination, imaging, nerve study, or diagnostic block supports that conclusion.
  3. Verify the exact device. Record the manufacturer, model, stimulation mode, labeled diagnosis, body region, and whether the system is temporary or intended to remain implanted.
  4. Define success before the procedure. Choose measurable goals and a time frame. “Some relief” is too vague to guide a permanent-implant decision.
  5. Compare the complete burden. Include anesthesia, recovery, external equipment, charging, programming, activity restrictions, follow-up, MRI rules, revision, removal, and possible battery management.
  6. Review alternatives. Ask what remains if a trial fails, benefit fades, or the target proves incorrect. Alternatives may include further diagnostic work, rehabilitation, medication changes, injections, surgery when appropriate, or no implant.

Questions to take to a pain appointment

  • Which nerve or spinal target do you believe is driving my pain, and what evidence supports it?
  • What is the exact device and its current labeled indication for my diagnosis?
  • Is the proposed PNS system temporary or intended to remain implanted? What equipment stays outside my body?
  • Will I have a trial? What specific improvement would count as enough benefit to proceed?
  • What are the device-specific risks and the likelihood of revision or removal?
  • How often will I need programming or other follow-up?
  • How will the system affect MRI scans, daily activities, charging, controller use, and future procedures?
  • What happens if the trial does not help or if benefit fades?

Bottom line for chronic pain patients

PNS is targeted stimulation of a selected peripheral nerve; SCS is stimulation delivered by leads near the spinal cord. Some PNS treatment is temporary and externally powered, while SCS usually involves implanted leads and a generator; both categories contain substantial device-to-device variation. Match the anatomical target and diagnosis to current labeling, use a clearly defined trial or success plan when appropriate, and weigh expected benefit against surgery, hardware, maintenance, and follow-up. The available evidence does not justify choosing one category as the best option for everyone.
