Suspected infection with Naegleria fowleri requires emergency medical care. The Centers for Disease Control and Prevention (CDC) advises seeking care immediately if you suddenly develop fever, headache, vomiting, or a stiff neck—especially after warm-freshwater exposure or rinsing your nose with unsafe water. Do not wait for confusion, seizures, or coma; those are later signs of a rapidly worsening illness.
What Naegleria fowleri infection is
Naegleria fowleri is a free-living ameba found in warm freshwater, soil, and, occasionally, poorly maintained recreational water. It causes a rare disease called primary amebic meningoencephalitis (PAM) when contaminated water enters the nose. The organism can then travel along the olfactory pathway to the brain and cause severe inflammation and destruction of brain tissue.
PAM is not acquired by drinking or swallowing contaminated water, and it does not spread from person to person. The relevant exposure is water forced into the nasal passages, such as during swimming, diving, jumping, water sports, or nasal irrigation with unsafe tap water. Because the disease is uncommon and early symptoms resemble other forms of meningitis, clinicians need to know about the exposure history as soon as possible.
Symptoms and the usual timeline
Symptoms can begin quickly after the organism enters the nose. The first symptoms usually appear about five days after exposure, but the reported incubation period ranges from one to 12 days. Once illness starts, deterioration can be fast.

Early symptoms
The initial combination of headache, fever, nausea, and vomiting is the most important warning pattern. These symptoms are not unique to PAM; they can occur with influenza, foodborne illness, migraine, and bacterial or viral meningitis. The combination becomes especially concerning when it follows warm-freshwater exposure or nasal rinsing and appears within the one-to-12-day window.
Later neurologic symptoms
As inflammation progresses, a person can develop a stiff neck, confusion, reduced attention to people or surroundings, impaired balance, hallucinations, seizures, and coma. These signs indicate advanced neurologic involvement. Waiting for them before seeking care can eliminate valuable time for testing and treatment.
When to go to the emergency department
Go to an emergency department immediately if you suddenly develop fever, headache, vomiting, or a stiff neck after possible exposure. Call emergency services if the person is confused, having a seizure, losing consciousness, or cannot be safely transported. Tell the triage nurse and physician exactly when the exposure occurred, whether water went up the nose, where it happened, and whether you used tap water for a sinus rinse.

Do not wait to see whether symptoms improve overnight, and do not wait for neurologic signs. CDC’s emergency guidance specifically emphasizes acting on the early symptom cluster. A recent exposure does not prove PAM, but it changes the information clinicians need when they assess meningitis-like illness.
- Bring the name and location of the swimming area, hot spring, pool, or other water source if you know it.
- Report diving, jumping, submersion, water-sport activity, or any event that forced water into the nose.
- Report nasal irrigation, including the type of water used and whether it was distilled, sterile, or boiled and cooled.
- Do not self-drive if confusion, severe weakness, a seizure, or loss of consciousness is present.
Why PAM is difficult to diagnose
Early PAM can look like bacterial meningitis. Doctors may need to evaluate cerebrospinal fluid and other laboratory specimens while treating several possible causes at once. Laboratory confirmation is specialized: CDC reports that only a few laboratories in the United States, including CDC, can perform the required testing.
The organism is rare, and the disease can progress so rapidly that diagnosis is sometimes made only after death. A negative routine test or an initial diagnosis of another type of meningitis does not by itself settle the question. The exposure history is therefore a critical part of the first emergency evaluation, not a detail to mention later.

How PAM is treated
Treatment takes place in a hospital, usually in intensive care, and uses a combination of medications directed by infectious-disease, critical-care, neurology, and laboratory specialists. There is no validated home test, over-the-counter medicine, supplement, or home remedy that can diagnose or treat PAM safely.

Because treatment decisions depend on the patient’s condition, test results, drug availability, and specialist advice, families should not attempt to assemble a regimen themselves. The appropriate action is rapid emergency evaluation so clinicians can begin supportive care, investigate other causes of meningitis, and consult experts about combination therapy as soon as PAM is considered.
How severe is Naegleria fowleri infection?
PAM is exceptionally rare but has a very high fatality rate. CDC reported 167 U.S. cases from 1962 through 2024, with four survivors among those reported cases. A CDC Morbidity and Mortality Weekly Report analysis counted 164 U.S. infections from 1962 through 2023 and four survivors, a 2.4% survival proportion in that dataset. The different totals reflect the periods and case sets being analyzed; neither figure predicts what will happen to an individual patient.

CDC’s clinical-care information also describes five well-documented North American survivors. That number covers a broader geographic and clinical record than the U.S.-only surveillance totals above. Survivors have been reported, but the statistics should never be used to delay evaluation or to estimate a particular person’s chances.

Exposure alone does not establish infection, and there is no approved preventive drug or home screening test that can rule PAM in or out. If you remain well, write down the date, water source, and whether water entered your nose so you can provide an accurate history if symptoms occur. If any sudden fever, headache, vomiting, or stiff neck develops during the next 12 days, seek emergency care and disclose the exposure immediately.
Do not use symptoms—or the absence of symptoms during the first few days—as a reason to perform a home treatment. The incubation period is commonly about five days but can be as short as one day or as long as 12 days, so timing is not precise enough for self-clearance.

Reducing the risk of exposure
Prevention focuses on keeping potentially contaminated water out of the nose. These measures lower exposure risk; they do not diagnose or treat symptoms after illness begins.
- Hold your nose shut or wear a nose clip when jumping or diving into fresh water.
- Keep your head above the water in hot springs and other naturally warm-water settings.
- Avoid digging or stirring sediment in shallow, warm freshwater, where organisms may be concentrated.
- For sinus rinsing, use distilled or sterile water, or water that has been boiled and then cooled as directed by public-health guidance.
Swallowing water is not the mechanism that causes PAM, but avoiding forced nasal entry remains important. Nose clips and safe-water practices are prevention aids only; they cannot replace emergency assessment when symptoms begin.
Common questions answered
Can you survive a Naegleria fowleri infection?
Yes, documented survivors exist, including four among 167 reported U.S. cases through 2024 and five well-documented North American survivors cited in CDC clinical-care information. Survival is rare, and these population figures are not a personal prognosis.

No. PAM occurs when contaminated water enters the nose. Swallowing the water does not cause this infection, and PAM does not spread between people.
Should I wait for confusion or seizures before seeking help?
No. Seek emergency care for sudden fever, headache, vomiting, or stiff neck after a possible exposure. Confusion, hallucinations, seizures, and coma are later signs of progression, not symptoms to wait for.
