MRI and PET are complementary tests, not competing ways to diagnose Alzheimer’s disease. MRI shows brain structure and can reveal atrophy, stroke, tumor, hydrocephalus, or other conditions that may cause cognitive symptoms. PET uses a radioactive tracer to show glucose metabolism or specific molecular changes such as amyloid plaques and tau. A clinician generally starts with a complete cognitive and medical evaluation plus structural imaging, then considers a particular PET study only when the result could resolve meaningful uncertainty or change treatment.
Neither scan is interpreted in isolation. A normal early MRI does not exclude Alzheimer’s, and a positive amyloid PET does not by itself prove that amyloid is causing a person’s symptoms. The useful question is not simply which scan is better, but which measurement answers the clinical question at hand.
What MRI and PET actually measure
MRI measures anatomy
MRI uses what RadiologyInfo describes as “a powerful magnetic field, radio frequency pulses and a computer to produce detailed pictures” of internal structures. Brain MRI can show the size and shape of regions, tissue loss (atrophy), old or recent strokes, bleeding, tumors, hydrocephalus, and other structural abnormalities. In Alzheimer’s disease, later-stage scans may show reduced volume, often most evident in temporal and parietal regions. Early in the disease, however, MRI can look normal or show changes too subtle to establish a diagnosis.

MRI does not use ionizing radiation. Its safety issue is the strong static magnetic field, changing magnetic gradients, and radiofrequency energy. Anyone with a pacemaker, implanted electronic device, aneurysm clip, cochlear implant, metal fragment, or other implant must be screened against the exact scanner and device conditions before scanning.

PET measures activity or molecular pathology
The National Institute on Aging explains that “PET uses small amounts of a radioactive substance, called a tracer, to measure specific activity — such as energy use — or a specific molecule in different brain regions.” After an intravenous tracer is given, the scanner detects where it accumulates. The tracer determines what the study can answer:
| PET type | Primary measurement | What a result can contribute |
|---|---|---|
| Amyloid PET | Beta-amyloid plaque deposition | Supports or lowers the likelihood that Alzheimer’s amyloid pathology is present. |
| FDG PET | Regional glucose metabolism | Shows patterns of reduced glucose use that can support differential diagnosis, including suspected frontotemporal dementia. |
| Tau PET | Abnormal tau accumulation | May help stage tau distribution and correlate it with clinical severity; it is not commonly used in routine practice. |
A PET image is therefore not a generic picture of “brain health.” It is a map produced by a particular tracer, and the same person could have different findings on amyloid, FDG, and tau studies.
How the two tests fit into an Alzheimer’s evaluation
A typical sequence follows the question the clinician needs answered rather than a fixed scan-for-everyone protocol.
- Clinical assessment: A clinician takes a history from the patient and an informant, performs a neurological examination, reviews medicines and sleep or mood problems, and uses cognitive testing. Laboratory tests may identify reversible contributors or competing diagnoses.
- Structural imaging: MRI is commonly used to assess anatomy and look for stroke, tumor, hydrocephalus, bleeding, or another explanation for cognitive change. CT may be used when MRI is unsafe or unavailable, but CT is not the same test as MRI.
- Targeted clarification: If a dementia specialist still cannot determine the cause, the options may include FDG PET, cerebrospinal-fluid biomarkers, or amyloid PET. The 2025 Alzheimer’s Association/SNMMI guidance says amyloid PET is appropriate when Alzheimer’s is suspected but the diagnosis remains uncertain after a thorough specialist evaluation and when the result could clarify diagnosis or influence management.
- Selected staging questions: Tau PET can be considered in specialized settings when knowing the distribution of tau would answer a specific staging or diagnostic question. The NIA notes that tau PET is not commonly used in standard practice.
This sequence prevents imaging from replacing the clinical diagnosis. PET should be ordered only when its result has a realistic chance of changing the diagnosis, counseling, treatment choice, or care plan.
What MRI can show in suspected Alzheimer’s
Atrophy and patterns of change
Alzheimer’s-related tissue loss tends to become more visible as disease progresses. Reduced volume in temporal and parietal regions can support the diagnosis when it matches the symptoms and examination. A radiologist may also comment on hippocampal or generalized volume loss, but atrophy is not unique to Alzheimer’s: aging, vascular disease, other neurodegenerative disorders, and longstanding medical conditions can produce overlapping appearances.

Alternative causes of cognitive symptoms
MRI is particularly valuable when the concern is that something other than Alzheimer’s is responsible. It can identify an infarct or extensive small-vessel disease, a mass, hydrocephalus, or other structural abnormality that may require a different treatment. Finding no alternative lesion does not prove Alzheimer’s; it simply narrows the structural differential.
Why an early normal MRI does not settle the question
Structural change may lag behind symptoms. Someone with mild cognitive impairment or early Alzheimer’s can have a report that is normal or nonspecific. The result must be read alongside the time course of symptoms, cognitive profile, examination, and laboratory findings rather than used as a stand-alone rule-out test.

Contrast is not automatic
Dementia MRI often can be performed without gadolinium contrast, depending on the clinical question and local protocol. If contrast is proposed, ask what additional finding it is intended to evaluate. Gadolinium introduces separate medication and kidney-related considerations that depend on the specific agent and the patient’s health; it is not an unavoidable part of every dementia MRI.
What PET can show in suspected Alzheimer’s
Amyloid PET: presence of plaques
Amyloid PET detects beta-amyloid plaque deposition. A scan showing few or no plaques makes Alzheimer’s less likely as the cause of the current cognitive syndrome. A positive scan supports the presence of amyloid pathology, but amyloid can occur in people who never develop Alzheimer’s symptoms. The finding is meaningful only when combined with the person’s symptoms, examination, cognitive testing, and other biomarkers.
Amyloid confirmation can affect eligibility decisions for amyloid-targeting therapies, which is why the 2025 Alzheimer’s Association/SNMMI criteria emphasize ordering the scan only when the result could influence management. A positive result does not predict an individual’s exact rate of decline and does not identify every cause of the person’s impairment.

FDG PET: glucose-use patterns
FDG PET measures regional glucose metabolism. Characteristic areas of reduced glucose use can support a suspected neurodegenerative pattern and help distinguish Alzheimer’s disease from other disorders. A specialty guideline reports published ranges of 80%–99% sensitivity and 63%–98% specificity for differentiating Alzheimer’s disease from frontotemporal lobar degeneration. For Alzheimer’s versus Lewy-body dementia, the reported ranges are 70%–92% sensitivity and 74%–100% specificity.
Those figures are ranges from published studies, not a guarantee for an individual scan. Performance depends on the population studied, the reference standard, the reader’s expertise, and the clinical context. FDG PET does not show amyloid plaques or tau directly.
Tau PET: distribution and staging
Tau PET detects abnormal tau accumulation. The 2025 Alzheimer’s Association/SNMMI criteria describe it as potentially useful for staging and for correlating tau distribution with clinical severity. Availability, regulatory status, reimbursement, and clinical experience vary, and tau PET remains less routine than structural MRI or commonly used diagnostic assessments.
What a PET appointment involves
PET requires intravenous tracer administration, a waiting period for uptake, and scanning. The exact tracer, uptake interval, radiation dose, glucose instructions, pregnancy or breast-feeding precautions, and post-scan advice are facility- and tracer-specific. Follow the imaging center’s written instructions rather than assuming that preparation for one PET type applies to another. FDG studies commonly involve glucose preparation because blood sugar can affect the measurement; the facility should provide the exact fasting and medication instructions.
MRI versus PET: practical comparison
| Question | MRI | PET |
|---|---|---|
| What it measures | Structure, tissue volume, and anatomical abnormalities. | Tracer-defined glucose metabolism or molecular targets such as amyloid and tau. |
| Main diagnostic role | Looks for atrophy and alternative structural causes; commonly used early in evaluation. | Clarifies selected uncertain cases after specialist assessment; the tracer determines the question. |
| Can it identify a tumor, hydrocephalus, or stroke? | Yes, MRI is designed to show these structural problems. | Not reliably; PET is not a substitute for structural imaging. |
| Radiation | No ionizing radiation. | Yes, from the administered radioactive tracer; dose is tracer- and facility-specific. |
| Major safety screening | Implants, metal fragments, pacemakers, claustrophobia, and motion. | Tracer allergy or medication issues, pregnancy or breast-feeding precautions, glucose preparation for FDG, and intravenous access. |
| Can a normal result exclude early Alzheimer’s? | No. Early structural scans may be normal. | No. A negative result answers only what that tracer measures and must be interpreted clinically. |
| Availability and reimbursement | Usually more broadly available, but local access and insurance rules vary. | More specialized; availability, reimbursement, and approved uses vary by region and tracer. |
| Universal price or waiting time | Not stated; depends on region, facility, protocol, and insurance. | Not stated; depends on tracer, facility, region, and insurance. |
Risks, contraindications, and comfort
MRI risks
- Implant and metal hazards: The magnetic field can move or heat unsafe metal and interfere with implanted devices. Provide the facility with the exact make and model of every implant, including devices placed years ago.
- Claustrophobia and noise: The scanner is loud and enclosed. Ear protection, communication with staff, and an agreed signal to stop can help. Sedation, if considered, requires separate planning and transportation arrangements.
- Motion: Movement degrades images and may lead to repeat sequences. Tell staff about tremor, pain, or difficulty lying still.
- Contrast-related considerations: If gadolinium is planned, discuss kidney function, prior reactions, pregnancy, and the reason contrast is needed.
The FDA says millions of MRI scans are performed in the United States each year and receives approximately 300 adverse-event reports annually from manufacturers, facilities, and patients. That is an FDA-reported count, not an individual-patient risk rate.

PET risks
- Radiation: The tracer exposes the body to ionizing radiation. The amount depends on the tracer and protocol; ask the facility for the study-specific dose if this matters to your decision.
- Injection: Intravenous placement can cause brief discomfort, bruising, or rarely a reaction. Tell staff about prior tracer or medication reactions.
- Pregnancy and breast-feeding: Notify the ordering clinician and PET center before scheduling. Precautions vary by tracer.
- Glucose preparation: FDG PET may require fasting or medication adjustments. Do not change diabetes treatment without instructions from the clinical team.
- Waiting and repeat visits: Uptake time and the need to remain still make PET a longer, more involved appointment than a simple blood test.
Which scan is better for a particular decision?
When MRI is the more useful first test
MRI is usually the better starting image when symptoms are new, the diagnosis is broad, or a structural cause must be excluded. It supplies anatomical information that PET cannot replace and does not add ionizing radiation. It is also the practical choice when PET access is limited or when the clinician needs to assess vascular injury, a mass, or hydrocephalus.
When amyloid PET may add value
Amyloid PET is most useful when a specialist has completed a thorough evaluation, Alzheimer’s remains a realistic possibility, and knowing whether amyloid is present would change counseling or treatment. It is not a general screening test for people without symptoms and should not be ordered merely because a person wants a prediction of future dementia.
When FDG PET may add value
FDG PET can be considered when the clinical pattern is atypical or when distinguishing Alzheimer’s from another neurodegenerative syndrome would affect management. Its interpretation is pattern-based and depends heavily on reader expertise and the quality of the clinical question.
When tau PET may add value
Tau PET is a specialized option when distribution or stage of tau pathology is the unresolved issue. Because it is less commonly used, confirm that the center has appropriate tracer access, qualified interpretation, and a clear plan for how the result will be used.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Do you need both MRI and PET?
Some people do, but many do not. MRI and PET answer different questions, so a PET scan does not automatically make MRI unnecessary. A clinician may recommend both when structural information is needed and the cause remains uncertain after the MRI and clinical assessment. Conversely, if MRI clearly explains the symptoms or the diagnosis is already sufficiently established, PET may add no actionable information. The decision should be tied to a specific unresolved question and a planned response to each possible result.

How to prepare and avoid common problems
- Bring a complete device and medical history: List pacemakers, stimulators, aneurysm clips, cochlear implants, metal fragments, prior surgeries, kidney disease, allergies, pregnancy possibility, and breast-feeding.
- Ask which tracer or contrast agent is planned: Preparation, radiation, and precautions are not interchangeable between amyloid, FDG, and tau PET or between MRI protocols.
- Clarify medication and food instructions: Follow the center’s written fasting, glucose, and medication directions. Never independently stop insulin or other essential medicines.
- Plan for stillness and transport: Tell staff in advance about claustrophobia, pain, tremor, or cognitive impairment. If sedation is prescribed, arrange an adult escort and follow the center’s restrictions.
- Request the clinical question on the order: Ask whether the goal is to exclude another disease, assess atrophy, confirm amyloid, evaluate metabolism, or stage tau. A precise question improves test selection and interpretation.
- Schedule a results discussion: Imaging reports can use probabilistic language. Review the images and report with the ordering clinician rather than interpreting an isolated phrase online.
How to interpret results without overreading them
An MRI showing atrophy
Atrophy is a structural observation, not a complete diagnosis. Its significance depends on the distribution, severity, age, vascular findings, cognitive profile, and progression over time.
An MRI reported as normal
A normal or nonspecific MRI lowers concern for some structural conditions but does not rule out early Alzheimer’s or another microscopic process.
A positive amyloid PET
This indicates amyloid plaque pathology is present. Because amyloid can occur without symptomatic Alzheimer’s, the result must match the clinical syndrome before it is used to explain the person’s impairment.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
A negative amyloid PET
Few or no plaques make Alzheimer’s less likely as the cause of the current syndrome, prompting consideration of other diagnoses or biomarkers rather than ending the evaluation automatically.
An abnormal FDG or tau pattern
These findings can support a pattern or stage, but the report is not a substitute for clinical judgment. Sensitivity and specificity vary with the population, reference standard, reader expertise, tracer, and disease stage.
Cost, access, and coverage
There is no single worldwide price, waiting time, insurance rule, or tracer protocol for either modality. Charges depend on the country, facility, scanner, contrast or tracer, professional fees, and whether the study is covered for the specific indication. PET is generally more specialized, and some tracers or uses may not be available locally. Before scheduling, ask the imaging center and insurer about authorization, out-of-pocket cost, preparation, cancellation rules, and when a specialist will review the result.
Questions to ask before agreeing to a scan
- What specific diagnostic or treatment decision will this scan inform?
- Why is MRI, PET, or both appropriate for that question?
- If the result is positive, negative, or indeterminate, what will we do differently?
- Which tracer or contrast agent will be used, and what preparation is required?
- What are the radiation, implant, kidney, pregnancy, or breast-feeding considerations for me?
- Who will interpret the study, and how will the result be combined with cognitive testing and other biomarkers?
- Is the scan covered for this indication, and what costs remain my responsibility?
The practical bottom line is straightforward: MRI is mainly a structural test, while PET is metabolic or molecular. MRI is often the first imaging step because it can reveal alternative structural causes without ionizing radiation. Amyloid PET, FDG PET, and tau PET are targeted tools for specific unresolved questions. The safest and most informative choice is the one linked to a clear clinical decision and interpreted by a clinician who knows the person’s symptoms and examination.
