Yes, many people with pacemakers, defibrillators, cochlear implants, insulin pumps, and neurostimulators can travel by air and may be able to have an MRI—but compatibility is never determined by the device category alone. The exact manufacturer, model, leads, implant location, accessories, and labeled conditions control the decision. An MR Conditional device is permitted only when every condition on its labeling is met; an unknown status should be treated as unsafe until the radiology team resolves it.

Airport screening is usually a disclosure-and-screening process rather than a reason to remove an attached device. Tell the Transportation Security Administration (TSA) officer where the device is before screening begins, keep it attached unless the manufacturer and officer specifically direct otherwise, and request a private screening or pat-down accommodation if needed.

Why implantable devices change the MRI decision

MRI scanners combine a powerful static magnetic field with radiofrequency (RF) energy and rapidly switched gradient fields. Those fields can affect an implant in different ways:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Static-field force and torque: magnetic components can move or twist, creating pain, tissue stress, or device displacement.
  • RF heating: conductive parts such as leads can act like antennas. Heating at the device or lead tip can injure surrounding tissue. The U.S. Food and Drug Administration (FDA) says heating or burns account for most reported MRI patient problems.
  • Gradient-induced voltage and stimulation: changing fields can induce electrical currents, interfere with sensing, or trigger unwanted stimulation.
  • Therapy or device malfunction: an electrically active device may reset, stop therapy, deliver an inappropriate response, or require reprogramming after the scan.
  • Image artifact: metal and electronics can obscure the body region being examined, reducing diagnostic value even when the scan is physically allowed.

The FDA reports millions of MRI scans in the United States each year and around 300 adverse-event reports involving MRI scanners and coils annually (page current when accessed in 2026). That figure is a count of reports, not an injury rate; the FDA does not provide a denominator-based risk percentage in the cited passage.

FDA’s stated rule is direct: “Therefore patients with implanted medical devices should not receive an MRI exam unless the implanted medical device has been positively identified as MR Safe or MR Conditional.”

Understand the three MRI safety labels

MR Safe

MR Safe means the item poses no known hazards in any MRI environment under the definition used for medical-device labeling. The label applies to the specific item, not automatically to every product made by the same manufacturer or to an entire device family.

MR Conditional

MR Conditional means scanning is allowed only under specified conditions. Typical conditions can include a 1.5-tesla or 3-tesla field strength, a particular body region, normal or controlled operating mode, a specific absorption rate (SAR) limit, restrictions on transmit or receive coils, patient positioning, and limits on scan duration. Some active devices must be placed in an MRI mode, turned off, or monitored continuously, then checked and restored afterward.

MR Unsafe or unknown

MR Unsafe devices must not enter the MRI environment. If the status, model, leads, or accessories cannot be established, do not proceed on the assumption that a similar device is safe. The radiology and implant teams must identify the system or choose another diagnostic approach after weighing clinical benefit and risk.

Before scheduling an MRI: identify the complete system

Start with the implant card and the clinician or manufacturer that supplied the device. Ask for the current MRI conditions for the exact implanted system, not just the product name. A complete identification includes:

  • Manufacturer and exact model or model family
  • Implant location and date, when known
  • Every implanted lead, extension, electrode, generator, anchor, and accessory
  • Any abandoned, capped, fractured, or disconnected lead
  • External components, medication pumps, chargers, controllers, or wearable parts that could enter the scanner room
  • Whether the system is intact, modified, or combined with a component from another manufacturer

If the implant card is missing, ask the manufacturer for a replacement and ask the implanting clinic to confirm the device record. Do not rely on a verbal description such as “a modern pacemaker” or “a titanium implant.”

What to confirm with the imaging center

Give the imaging center the device information before the appointment. Tell the scheduler and MRI technologist about every implant, wearable, lead, pump, stimulator, cochlear implant, aneurysm clip, metallic fragment, and other device. The American College of Radiology (ACR), FDA, and RadiologyInfo guidance all emphasize complete disclosure.

  • Scanner field strength, such as 1.5 T or 3 T
  • Body region to be scanned and whether the device’s labeling permits that region
  • Operating mode and SAR or other RF exposure limits
  • Transmit and receive coil restrictions
  • Permitted patient position and any restrictions on the implant being inside or outside the bore
  • Whether the device must be programmed, disabled, or placed in a manufacturer-defined MRI mode
  • Who will monitor you during the scan and who will interrogate and restore the device afterward

Only the device labeling and the responsible clinical team can determine whether that combination of scanner, body region, settings, and device is allowed. If the center cannot verify the conditions, postpone the scan until it can.

Special case: pacemakers, defibrillators, and neurostimulators

Pacemakers and implantable cardioverter-defibrillators (ICDs) are active electrical systems. Even when an individual model is MR Conditional, the generator, leads, battery status, programming, and lead integrity all matter. The clinical team may need to change settings before scanning, monitor you throughout, and interrogate the system immediately afterward.

Neurostimulators and other electrically active implants follow the same model-specific principle. A system that is safe in one MRI mode or field strength may be restricted in another. Never infer permission from a friend’s device or from the fact that a prior scan was completed safely.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Special case: cochlear implants

Cochlear implants require particular caution because their internal magnet and implant construction differ by model. Some are MR Unsafe. Some are MR Conditional only at specified scanner strengths and may require magnet removal, a manufacturer-approved splint or bandage kit, or other positioning instructions. Use only the kit and instructions approved for that exact implant. Confirm the plan with the cochlear-implant team and MRI center before arrival.

  • 【PACKING】: Wrapped in velvet bag, Packaged in black gift box, good gift for your lover, family, friend and coworkers.

Special case: insulin pumps and other wearable devices

External insulin pumps, continuous glucose monitors, TENS units, and similar devices can have their own MRI restrictions. Some must be removed before entering the scanner room; others may remain only within a stated distance or under a stated condition. Ask the device manufacturer and imaging center rather than treating “wearable” as synonymous with “safe.” Plan how medication delivery and monitoring will continue if a pump or sensor must be disconnected.

When another imaging test is considered

If an implant is MR Unsafe or its status cannot be verified, a clinician may consider CT, ultrasound, radiography, or another method. Compare options on five practical axes:

Decision axis Questions to ask
Device safety Is the exact device system cleared for the proposed scanner and field strength?
Device management Must it be removed, reprogrammed, disabled, or monitored?
Diagnostic value Will the alternative answer the clinical question, and will device artifact obscure the target?
Team capability Does the facility have staff experienced with this implant and its manufacturer conditions?
Practical burden How do availability, delay, privacy, positioning, and accommodation needs differ?

The treating clinician should make that risk-benefit decision; do not substitute a general internet compatibility list for patient-specific review.

Airport screening with an implant or attached device

Tell the TSA officer before screening that you have an implanted or attached medical device and point to its location. TSA’s published instruction is: “If you have an insulin pump, glucose monitor or other medical device attached to your body, inform the officers where it is located before the screening process begins.” A TSA notification card or medical documentation can help communicate the condition, but it does not replace the officer’s screening procedure.

What usually happens

  • Leave an attached device in place unless its manufacturer says removal is safe and the officer instructs you to remove it.
  • The officer may use a metal detector, millimeter-wave imaging, visual inspection, additional screening, or a gentle pat-down.
  • Devices that cannot be disconnected are not required to be removed solely for screening.
  • Explain the device’s location and any area that should not be pressed or manipulated.

TSA and the FDA reported a 2016 technical evaluation of millimeter-wave advanced-imaging exposure involving more than 50 personal medical electronic devices, including sample pacemakers, neurostimulators, ICDs, insulin pumps, and TENS devices. That testing does not certify every model or every scanner; your device’s instructions and the officer’s procedure still govern.

Requesting privacy or assistance at the checkpoint

You may ask for a private screening, a pat-down by an officer of the same sex, or help from a Passenger Support Specialist. TSA Cares can provide travel assistance; check current TSA guidance and contact details shortly before departure because procedures and contacts can change.

Arrive early enough for additional screening without rushing. Keep your implant card, medication list, and essential supplies in your carry-on rather than checked baggage. Clearly identify supplies and keep medication accessible for inspection. A medical ID bracelet can make disclosure easier during travel or clinical intake, but it does not establish MRI compatibility; the implant card, manufacturer labeling, and radiology-team verification remain authoritative.

One-page preparation checklist

Before booking the MRI

  1. Locate the implant card or request a replacement from the manufacturer.
  2. Record the exact manufacturer, model, leads, implant location, and accessories.
  3. Ask the implanting clinician or manufacturer for current MRI conditions.
  4. Give the complete information to the imaging center and ask whether its scanner and staff meet those conditions.

On the appointment day

  1. Disclose every implant, abandoned lead, pump, stimulator, cochlear implant, metallic fragment, and wearable device.
  2. Confirm field strength, body region, operating mode, SAR limits, coils, positioning, programming, and monitoring.
  3. Follow instructions for MRI mode, removal, or reprogramming.
  4. Complete the required post-scan device check and restoration of settings.

Before flying

  1. Keep the implant card and medication/device list in hand luggage.
  2. Leave attached devices in place unless the manufacturer and TSA officer direct otherwise.
  3. Tell the officer where the device is before screening starts.
  4. Request private screening, a same-sex pat-down, a Passenger Support Specialist, or TSA Cares assistance if needed.

Common mistakes that create avoidable risk

  • Assuming “MR Conditional” means safe in every MRI scanner.
  • Providing only the generator model while omitting leads, abandoned leads, or accessories.
  • Failing to mention a device because it is small, external, or outside the body region being scanned.
  • Removing or disconnecting a pump or sensor without a medication and monitoring plan.
  • Using a cochlear-implant magnet kit that is not approved for the exact model.
  • Treating a notification card as proof of compatibility rather than a communication aid.
  • Assuming a previous uneventful scan or another passenger’s experience applies to your system.

If any required fact is unknown, pause the scan or screening decision and obtain model-specific instructions. A short delay is safer than entering an MRI environment without verified conditions or disconnecting a device without a safe plan.
