Madhya Pradesh’s 2025–26 plan listed a Rajpur Community Health Centre (CHC) in Guna district as functional and assigned it ₹14.62 lakh for upkeep and supplies, according to Hindustan Times. Guna’s Chief Medical and Health Officer (CMHO) said no CHC by that name was operational in the district. The reported figure is an allocation—not proof that the money was transferred, spent or used to buy equipment.
What the order reportedly listed
Hindustan Times reported on October 2, 2026, that a district-wise order under the Madhya Pradesh health directorate’s 2025–26 category-C CHC-strengthening plan listed Rajpur CHC as functional. It assigned ₹14.62 lakh for annual upkeep and supplies and included an equipment list. The report did not reproduce the full order or itemise the equipment.
Guna CMHO Rajkumar Rishishwar wrote to the directorate saying that no CHC named Rajpur was operational in the district. He asked for guidance on how to proceed with the allocation and any proposed purchases. A Hindi report by Dainik Jagran on the same date also described the allocation, the non-operational-facility concern and the inquiry.
Why officials questioned the listing
An unnamed senior health officer told Hindustan Times that physical verification and a gap assessment should happen before allocation, and that funds are released based on district demand. The officer alleged that the Rajpur list was prepared without a district demand or field verification. That is an account attributed to the official; the news reports do not include the underlying written procedure or independently inspected verification records.

The discrepancy is between what the reported order listed and what the district CMHO said existed operationally: a facility recorded as functional in the plan, but no operational CHC by that name in Guna, according to the CMHO.

What officials said
Rishishwar said the district had sought guidance “regarding the allocation made for CHC Rajpur so that further action can be taken as per instructions,” as quoted by Hindustan Times. Patel said: “We came to know about it from CMHO’s letter. We are probing the matter how it happened.” The report offered no inquiry findings or further directorate response.

The episode concerns the integrity of facility records and the checks behind public allocations: whether a centre was verified, whether a district requested support, and whether the budget entry led to any transaction or purchase. It does not, by itself, show that other facilities or allocations were affected.

For context, Hindustan Times reported Madhya Pradesh counts of 348 CHCs, 1,442 primary health centres (PHCs), 52 district hospitals and 161 civil hospitals, without identifying the underlying primary dataset. It also cited a 2021–22 Jan Abhiyan Parishad study estimating rural shortages of 245 CHCs, 1,045 PHCs and 4,134 sub-health centres. Those estimates are not a current official count and do not directly audit the Rajpur case.

A clear accounting would require the order and its approval trail, the CMHO’s letter, records showing whether the allocation was released or reversed, and any procurement or expenditure documents. The probe’s written findings would help establish how Rajpur entered the plan and whether anyone was responsible. None of those records or findings was included in the news accounts cited here.
