In Mortal Shell II, choose a Permanent Pickup for the threat or build need you have now: use an ailment counter when afflicted, a healing item when low on Health, or a utility item when its transformation or combat effect fits the situation. Nerd’s Chalk’s guide lists 14 Permanent Pickups and reports Familiarity landmarks at 1, 10, and 50 copies, but it does not provide a complete set of effect values or durations. Treat its item details as guide-reported rather than definitive: an official Steam announcement reports adjustments to Grisha Remnant and Weltcap, so behavior can change between builds.
How Permanent Pickups and Familiarity work
Nerd’s Chalk’s guide distinguishes an item’s active use from its passive effect and says a Permanent Pickup is assigned to one role at a time. Its advice is to match the item to the current problem rather than choosing solely by how many copies you own.
The guide describes 1, 10, and 50 copies as broad Familiarity landmarks. It does not establish a universal scaling formula: some proportional passive effects may continue to increase beyond 50 copies, and the guide does not give reliable exact percentages, proc chances, durations, or tier-specific values. Do not infer a precise effect from a tier landmark.

How to choose for the situation
Counter a status ailment
Match the ailment you need to address: Bloodseed for Bloodcurse, Burnt Effigy for Burn, Galvanic Shard for Lightning, Martyr’s Ash for Frost, Paradoxical Scripture for Stasis, Seedbearer’s Scripture for Cosmic Disease, or Tarspore for Poison. The guide reports that these active uses also grant temporary mitigation, but supplies no duration or magnitude.
Recover Health
Choose Mango for the guide-reported instant Health restoration. Choose Weltcap if gradual restoration suits the situation better; its passive is also described as gradually refilling Health, up to a maximum that scales with copies.
Support Resolve or defense
Common Moonshine is the listed active option for restoring Resolve and its passive is described as damage reduction. Special Moonshine is listed for a passive increase to maximum Resolve, but the guide does not state its active effect. Do not assume the two have interchangeable active uses.

Grisha Remnant is the guide’s summon option: its active summon’s damage scales with Remnants, while its passive can trigger a summon on melee hits. For transformation utility, the guide lists Sheephead Totem’s sheep form and Mango-related transformations. Trollweed provides a bush transformation and an immunity passive, but the guide does not explain the immunity’s scope.

What is known about charges and New Game Plus
Nerd’s Chalk’s guide claims the pickup itself is not consumed and that active charges replenish by resting at a Beacon. It also claims inventory and Familiarity carry into New Game Plus and that pickups respawn there. These persistence and replenishment details were not corroborated by official documentation in the sources reviewed, so check the current build before planning a duplicate-farming route around them.

Patch sensitivity matters: an official Steam community announcement reports an adjustment to Grisha Permanent Pickup and says Weltcap became stronger in both active and passive forms. The available announcement summary does not supply the full numerical effect changes, so the table above should not be read as a guarantee for every build.

The Steam listing gives August 20, 2026 as the game’s release date.
