A secure health app starts with a data-flow map, not a cloud choice. Identify what the app collects, why it needs it, where it goes, who can access it, how long it stays, and how it is deleted. Then apply least privilege at every boundary—from phone permissions to backend services—and assess the rules that depend on the app’s users, relationships, and functions.

This guide gives developers a U.S.-centered regulatory starting point and covers Android health-permission considerations. It is not a compliance determination, a guide to every state or country, or an Apple implementation guide. Encryption and a particular hosting provider do not, by themselves, make an app compliant with any law.

Map the data before choosing the architecture

List each health-related data item before deciding which components or vendors to use. For every item, record its source, purpose, destination, access, retention, and deletion behavior. Include less obvious paths such as analytics, crash reporting, support tools, exports, backups, and integrations—not just the main app-to-server flow.

The Federal Trade Commission (FTC) recommends minimizing collection and retention, limiting permissions, and using privacy-protective defaults. Its practical point is direct: “If you don’t collect data in the first place, you don’t have to go to the effort of securing it.” See the FTC’s Mobile Health App Developers: FTC Best Practices.

Inventory field Question to answer
Data and source What exact information is collected, and does it come from the user, the phone, a health platform, or another service?
Purpose Which user-facing feature requires it? Could the feature work with less precise, less frequent, or no retained data?
Flow and recipients Which app component, API, backend, vendor, integration, or staff role can receive or access it?
Retention and deletion How long is it kept, where are copies such as backups held, and what happens when the user deletes data or closes an account?
Disclosure and choice Where and when will users learn about collection and sharing, and what choice or consent applies?

Use this inventory to draw a flow map from collection through processing, storage, sharing, export, and deletion. The map is an engineering aid, not a regulator-prescribed architecture. Google Play expects disclosures to accurately describe actual collection, use, sharing, retention, deletion, and security practices; see Android health permissions: Guidance and FAQs.

Separate the mobile, API, and backend trust boundaries

A common design has a mobile client, platform health-data interfaces where needed, an authenticated API, backend services, storage, and deliberately approved integrations. Treat every handoff as a trust boundary. For each request, define the minimum data sent, the permitted caller, the relevant user or account context, the authorization decision, and what needs to be logged.

Keep authorization on the server

Do not rely on the mobile client alone to decide who may access health records. A modified client must not be able to bypass server-side authorization. Scope credentials, keys, and administrative functions; ensure each service receives only the access it needs. If the app operates its own authentication, plan secure credential handling, password storage, session behavior, and a way to revoke access when a device is lost. The FTC’s best-practices guidance discusses authentication, API access, password storage, and access revocation.

Make integrations explicit

For each third party, document the data it receives, the feature that depends on it, the applicable user disclosure, and the vendor’s handling and contractual terms. Avoid forwarding health information to analytics, advertising, crash-reporting, or other services by default. If a transfer has a genuine purpose, assess whether it is necessary, appropriately disclosed and consented to, and legally reviewed. Do not assume that removing a user’s name makes a data flow harmless.

Build a security baseline into every layer

Google Play’s health-permission guidance calls for encryption in transit and at rest, strong access controls, secure development practices, and vulnerability management. The FTC additionally emphasizes data minimization, authentication, review of third-party software, security ownership, and security by design. These are baseline practices, not a prescribed cloud architecture or a guarantee of compliance.

  • Verify encryption in transit and at rest, including backups and logs that may contain health information. Decide how keys are managed and limit who can use them.
  • Restrict access by role and legitimate purpose. Review administrative and customer-support access as well as ordinary user permissions.
  • Plan authentication and session handling, lost-device revocation, and account closure. Determine what data or access remains after each action.
  • Track third-party components and dependencies, monitor vulnerabilities, and establish a process for remediation.
  • Test security in realistic conditions and define how security issues can be reported, triaged, and fixed.
  • Review every recipient of health data—including telemetry and diagnostics—against the documented purpose, user-facing disclosures, and applicable legal requirements.

These controls should be checked against the actual data-flow map. For example, encryption of the primary database does not answer whether exported files, logs, backups, or an integration also retain sensitive information.

Request Android health permissions only when a feature needs them

Google describes Health Connect as a standardized way for apps to access specific health and fitness data types. A permission request should map to a concrete feature and clear user benefit. Ask only for the data types that feature needs, explain the reason for access, and make the privacy policy accessible and consistent with actual practices.

Align permissions and disclosures

Keep the permission request, in-app explanation, store listing disclosures, and privacy policy in agreement. Describe collection, use, sharing, retention, deletion, and security accurately. Google Play may review permission use and disclosures, and its policy details can change; check the current platform guidance before implementation and submission.

Keep health-permission data out of advertising

Google Play prohibits selling or transferring health-permission data to advertising platforms, and prohibits using it to serve ads. Treat that as a platform restriction when designing integrations and monetization, not merely as a disclosure issue. These are Google Play requirements; do not assume they describe iOS rules. This article does not provide an Apple HealthKit implementation walkthrough.

Determine which laws and oversight may apply

Do not start with a blanket label such as “HIPAA app.” Whether HIPAA applies depends in part on the developer’s relationship to covered entities or business associates and on how protected health information is handled. A consumer app is not automatically subject to HIPAA simply because it handles health information, and HIPAA is not the only law worth screening.

Screen the app’s relationships and data practices

HHS’s Resources for Mobile Health Apps Developers, last reviewed April 22, 2026, points developers to scenarios and an interactive tool. The tool addresses the FTC Act, the FTC Health Breach Notification Rule, HIPAA Rules, the Federal Food, Drug, and Cosmetic Act, the Children’s Online Privacy Protection Act (COPPA), and the 21st Century Cures Act and ONC Information Blocking Regulations. FTC guidance also notes that state laws may apply. Use the FTC Mobile Health App Interactive Tool to screen the facts of the product; it is not a substitute for a product-specific legal assessment.

Assess medical-device questions by intended use and function

A phone app is not automatically a regulated medical device just because it concerns health. Examine the product’s intended use and actual software functions. FDA’s September 2022 final guidance, Policy for Device Software Functions and Mobile Medical Applications, describes oversight for software functions that meet the device definition and could pose patient-safety risk if they do not work as intended.

Functions intended to diagnose disease, cure, mitigate, treat, or prevent it—or affect the body—may raise device-definition questions, subject to statutory exclusions and applicable guidance. The FTC interactive tool offers examples of functions that may be excluded. This is a screening framework, not a classification decision for an individual app.

Explain privacy choices in language users can act on

Tell users what the app collects and shares, why it does so, and how long data is retained. Put explanations near the point where users make relevant choices rather than relying only on broad policy language. The FTC recommends affirmative express consent before collecting or sharing health data, transparent practices, and privacy-protective defaults. Google Play also expects accessible privacy information and accurate in-app and store disclosures.

Make deletion behavior specific: distinguish deleting a record from disconnecting a data source, revoking a permission, or closing an account. Explain what happens to copies held by integrations or backups when applicable. Clear statements help users understand the actual data lifecycle and help teams keep disclosures aligned with implementation.

Turn the design into a review checklist

  1. Inventory. Record each data item, purpose, source, recipient, access role, retention period, and deletion behavior.
  2. Minimize. Remove fields, permissions, retention, and transfers that are not needed for a user-facing feature.
  3. Map boundaries. Trace data through the phone, APIs, backend, storage, logs, backups, support tools, and integrations.
  4. Assign controls. Specify authentication, server-side authorization, encryption, key access, monitoring, dependency review, and vulnerability response for each relevant component.
  5. Review user choices. Align permission prompts, in-app disclosures, privacy policy, and store disclosures with actual behavior.
  6. Scope obligations. Screen the product’s relationships, users, data practices, and intended functions for applicable federal, state, platform, and device rules.
  7. Recheck changes. Repeat the review when adding a data type, feature, integration, user group, or new use of existing information.
