PCR is usually the best first test when you know which virus to look for. It targets a defined genetic sequence, delivers a focused answer quickly, and scales well for routine screening. Metagenomic next-generation sequencing (mNGS) is broader: it sequences detectable nucleic acids without requiring a prespecified viral target, so it can reveal unexpected or novel viruses, mixed infections, and genome-level characteristics.

That breadth comes with trade-offs. mNGS generally needs more laboratory steps, sequencing capacity, computational analysis, contamination controls, and money. Its sensitivity can fall when viral load is low or host nucleic acid overwhelms the specimen. The practical strategy is usually targeted PCR for screening and confirmation, with mNGS added when the cause is unexplained, an outbreak needs investigation, a new pathogen is suspected, or a virus must be characterized beyond a positive/negative result.

Metagenomic sequencing and PCR at a glance

Question PCR or RT-PCR/qPCR Metagenomic sequencing (mNGS)
What it asks Is a specific known viral sequence present? Which viral or other nucleic-acid sequences are detectable in this specimen?
Target breadth One target or a defined multiplex panel; all targets must be selected in advance Broad, target-agnostic sampling followed by comparison with reference databases
Unknown-virus discovery Cannot detect an untargeted virus unless the assay happens to cross-react Can flag unexpected, divergent, or novel viral sequences if enough of their nucleic acid is present and databases or analysis methods can recognize them
Routine turnaround Typically the faster and simpler option for a known virus Often longer because extraction, library preparation, sequencing, and bioinformatics are added
Low viral load Usually favored when the correct target and specimen are known Highly dependent on viral biomass, host background, enrichment, sequencing depth, and reporting thresholds
Genome information Limited to the amplified region, unless a separate sequencing step follows Can provide partial or near-whole-genome information, depending on coverage
Operational burden Primers, amplification instrument, controls, and result interpretation Nucleic-acid extraction, library preparation, sequencing, contamination assessment, database analysis, and specialist interpretation
Typical role Diagnosis, screening, and confirmation of a suspected virus Unexplained infections, outbreak investigation, surveillance, discovery, and genomic characterization

What PCR measures

Targeted amplification

PCR uses primers designed to bind a known genetic sequence. During repeated amplification cycles, that sequence becomes abundant enough to detect. Reverse-transcription PCR (RT-PCR) first converts RNA into a DNA form that can be amplified; quantitative PCR (qPCR) estimates signal during amplification. A multiplex assay combines several known primer sets, allowing multiple viruses to be checked in one run, but it remains limited to the targets chosen during assay design.

The method answers a narrow question such as “Is virus X present?” That narrowness is an advantage when the clinical or epidemiological suspicion is strong. A laboratory can validate the primers, define a detection threshold, process many specimens with the same workflow, and report a result without analyzing every sequence in the sample.

Why prior knowledge matters

A PCR assay can miss a virus because the organism was not included in the panel, the circulating strain has changes where a primer binds, or the specimen does not contain the expected target. Adding more targets through multiplexing improves coverage of known pathogens but does not make the method genuinely open-ended. An untargeted or unknown pathogen will not be detected simply because a multiplex panel contains many assays.

What metagenomic sequencing measures

Untargeted sampling

mNGS sequences the nucleic acids detectable in a specimen rather than amplifying only a prespecified viral region. Reads are compared with reference databases and evaluated with quality, abundance, and contamination rules. The result can include known viruses, unexpected co-infections, divergent sequences, and genomic information useful for typing or surveillance.

As Oscar Enrique Torres Montaguth and colleagues wrote in Clinical Microbiology Reviews: “Metagenomic techniques can nonspecifically sequence all detectable nucleic acids in a sample and therefore do not rely on prior knowledge of a pathogen’s genome.” In practice, “all detectable” is conditional: host nucleic acid, degradation, extraction efficiency, library bias, sequencing depth, and database coverage determine what is actually visible.

Why broad does not mean universally more sensitive

Sequencing reads are distributed across everything present. In a respiratory specimen with abundant human nucleic acid and little viral material, only a small fraction of reads may be viral. A targeted PCR reaction concentrates its chemistry on one sequence, so it can detect a known virus that produces too few reads for an untargeted workflow. Enrichment methods such as hybrid capture can increase the viral fraction, but they add cost, time, and design choices and may still trail targeted sequencing in low-biomass samples.

How the two workflows differ

A typical PCR workflow

  1. Select the assay and specimen. The laboratory chooses a validated target or multiplex panel appropriate for the suspected virus and sample type.
  2. Extract nucleic acid. Viral RNA or DNA is purified with controls for extraction failure and inhibition.
  3. Amplify the target. Primers and amplification chemistry generate a detectable signal; RT-PCR includes reverse transcription for RNA viruses.
  4. Check controls and thresholds. Positive, negative, and internal controls help distinguish a true signal from inhibition or contamination.
  5. Report the focused result. The output is generally detected, not detected, or inconclusive, with repeat or confirmatory testing when controls or signal quality require it.

A typical mNGS workflow

  1. Define the question and specimen. The laboratory decides whether broad detection, outbreak investigation, genomic characterization, or surveillance justifies sequencing.
  2. Extract nucleic acid and prepare a library. RNA and DNA may require different preparation steps; the material is converted into sequencing-ready fragments.
  3. Apply optional depletion or enrichment. Host depletion, ribosomal reduction, or targeted capture can increase the proportion of viral reads, but each intervention changes cost, bias, and turnaround.
  4. Sequence with appropriate controls. Negative extraction controls, library controls, and process controls help identify reagent or carry-over contamination.
  5. Process the reads computationally. Quality filtering, removal of host reads, taxonomic classification, alignment, assembly, and coverage assessment determine which viral assignments are credible.
  6. Interpret in clinical context. Analysts assess read counts, genome coverage, uniqueness of the match, contamination signals, and whether the finding fits the specimen and patient.
  7. Confirm or characterize. A candidate pathogen may need targeted PCR, an independent extraction, additional sequencing, or culture-independent confirmation before it is treated as definitive.

Which method is more sensitive?

There is no universal mNGS sensitivity number. Performance changes with specimen type, viral load, host background, extraction method, enrichment, sequencing depth, pathogen, and the threshold used to call a result. Published studies illustrate the range rather than establish one value for every platform.

Examples showing workflow-specific results

  • A 2026 respiratory nanopore validation reported 51% sensitivity versus PCR (95% confidence interval, 45%-57%) and 99.8% specificity (3,836 of 3,845 negative samples). Sensitivity varied from 19% to 85% by pathogen. The multiplexed sequencing workflow reported consumable costs of £112 per sample. Those figures describe that particular assay, specimen set, and analysis pipeline.
  • A 2024 Nature Communications study comparing an mNGS workflow with an RT-PCR respiratory viral panel reported 93.6% sensitivity, 93.8% specificity, and 93.7% accuracy across 191 samples. These results should not be transferred to every mNGS platform or specimen type.
  • In a 2026 wastewater comparison, targeted RT-PCR and qPCR had detection likelihoods of 68% and 65%, respectively, for SARS-CoV-2. Target-agnostic approaches detected less often in that study; hybrid capture improved mNGS detection but remained less effective than targeted sequencing.
  • A 2021 systematic review found a median sample-to-result time of 29 hours, with reported values from 4 to 144 hours, and consumable costs of $130-$685 per sample. The review emphasized substantial heterogeneity and methodological limitations.

These examples explain why “Is mNGS more sensitive than PCR?” has a conditional answer. For a known virus at low abundance, a well-designed PCR assay often has the practical sensitivity advantage. For an unexpected virus that no PCR panel targets, mNGS may be the only method capable of finding it.

Specificity, contamination, and interpretation

PCR specificity comes mainly from primer and probe design, assay validation, and controls. Non-specific amplification, cross-contamination, or a target shared by closely related viruses can still produce misleading results, so laboratories interpret controls and signal thresholds together.

mNGS has a different failure mode: it can detect sequences introduced during extraction, library preparation, sequencing, or data analysis. Low-level reads from a reagent contaminant may look plausible, especially when a specimen has little viral material. A credible report therefore considers:

  • whether the same organism appears in negative controls or neighboring batches;
  • the number and quality of reads, their genome coverage, and whether they map uniquely;
  • the specimen’s viral biomass and host-background level;
  • the possibility of colonization, residual nucleic acid, or contamination rather than active disease;
  • whether an independent PCR or repeat extraction confirms the finding.

STROBE-metagenomics guidance emphasizes quality assurance, limits of detection, confirmatory testing, cost, and reporting. Those requirements are not administrative extras: they determine whether a broad result is clinically actionable.

Turnaround time, cost, and laboratory burden

Time

Targeted PCR is generally the faster route for a known virus because amplification and analysis are narrowly defined. mNGS adds library preparation, instrument scheduling, read processing, database comparison, and human review. A systematic review’s 4-to-144-hour range shows how widely mNGS turnaround can vary; a rapid instrument run does not guarantee a rapid final report if analysis or confirmation is still pending.

Cost

PCR costs include extraction, reagents, controls, labor, and instrument operation. mNGS adds library reagents, sequencing consumables, computational infrastructure, data storage, specialist analysis, and often confirmation. The 2021 review reported mNGS consumable costs of $130-$685 per sample, while the 2026 respiratory nanopore study reported £112 per sample for its multiplexed sequencing consumables. These are not universal prices and should not be compared without matching labor, capital, controls, and reporting costs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Equipment and expertise

A PCR service needs validated thermocycling and fluorescence-detection equipment plus trained molecular staff. An mNGS service also needs sequencing hardware or access to a sequencing facility, library-preparation capability, robust quality controls, bioinformatics pipelines, reference databases, and personnel able to distinguish a true pathogen from contamination or an incidental read. Smaller laboratories may therefore send broad sequencing to a specialist center while retaining PCR in-house.

Best use cases for each method

Known suspected virus and urgent diagnosis: PCR

Use a validated PCR or RT-PCR assay when the clinical picture, exposure, or local epidemiology points to a specific virus and a prompt result will change care or isolation. A multiplex panel is useful when several known viruses produce similar syndromes, provided its targets match the question.

Unexplained illness or unexpected co-infection: mNGS

Broad sequencing is most valuable when routine targeted tests are negative or incomplete, the syndrome is severe or unusual, or more than one pathogen is plausible. It can identify an organism outside the original differential and reveal co-infections that a narrow panel would not test.

Novel or emerging pathogen investigation: mNGS

During an unusual cluster, mNGS can search without assuming the answer and can provide sequence data for comparison among cases. Discovery still requires independent confirmation and epidemiological investigation; a single read assignment is not proof of a new outbreak.

Variant identification and genome characterization: PCR followed by sequencing

PCR is efficient for screening large numbers of specimens. Sequencing a selected set of positives can then determine lineage, mutations, or other genome-level features. WHO technical guidance states: “Whole Genome Sequencing, or at least complete or partial S-gene, should be performed to confirm infection with a specific variant.” It also states: “When PCR-based assays are used, confirmatory sequencing of at least a subset of viruses should be performed to be able to use these assay results as indicators of community circulation of the variants of concern.”

Wastewater and environmental surveillance

Environmental samples often contain degraded nucleic acid, inhibitors, and a large background of non-target material. The 2026 SARS-CoV-2 comparison showed why targeted RT-PCR and qPCR can outperform target-agnostic methods for detection in wastewater. mNGS can add value when surveillance seeks unexpected viruses or broader community signals, but the detection method must be validated for the matrix and interpreted with its recovery and inhibition limits.

Can mNGS detect viruses that PCR misses?

Yes, when the virus is not represented in the PCR panel, is genetically divergent at the primer sites, or appears as an unexpected co-infection. mNGS can also provide sequence context that a short PCR target cannot. However, “PCR-negative, mNGS-positive” does not automatically mean mNGS is correct. The result may reflect contamination, non-viable viral material, a database misclassification, or a viral concentration below the PCR assay’s sampled aliquot. Laboratories should review controls, coverage, specimen quality, and an independent confirmatory test before assigning clinical significance.

A practical combined testing algorithm

  1. Start with the clinical question. If one or several known viruses are plausible and a rapid decision is needed, order the relevant PCR or multiplex panel.
  2. Check specimen adequacy. A poor collection, wrong anatomical site, delayed transport, or insufficient volume can undermine either method; sequencing does not repair a missing or degraded sample.
  3. Act on a clear PCR result. Use the validated result with the clinical context and laboratory controls rather than sequencing every routine positive.
  4. Escalate unresolved cases. If targeted tests are negative, discordant, or fail to explain a severe syndrome, consider mNGS through a laboratory with validated controls and interpretation expertise.
  5. Sequence selected positives for variants or surveillance. PCR screening provides scale; sequencing a defined subset supplies genomic characterization and community-trend evidence.
  6. Confirm unexpected findings. Repeat extraction, targeted PCR, or independent sequencing should be considered before reporting a novel or clinically consequential virus as definitive.

How to choose between PCR, mNGS, and targeted sequencing

Situation Preferred starting point Reason
One known virus, urgent result PCR/RT-PCR Focused assay with efficient processing and established interpretation
Several known respiratory viruses with overlapping symptoms Validated multiplex PCR Tests the defined differential in one workflow
Severe illness with negative routine testing mNGS, often alongside repeat targeted testing Searches beyond the original target list
Suspected novel or emerging virus mNGS followed by confirmation Does not require a prespecified viral target and can generate sequence evidence
Variant or lineage surveillance PCR screening plus sequencing of selected positives Combines throughput with genome-level characterization
Low-biomass or host-rich specimen Validated targeted assay first; interpret mNGS cautiously Targeted amplification may retain a sensitivity advantage when viral reads are scarce
Wastewater monitoring for a known virus Targeted RT-PCR or qPCR Demonstrated higher detection likelihood in the 2026 SARS-CoV-2 comparison

Reporting an mNGS result responsibly

A useful report should identify the specimen, nucleic-acid workflow, sequencing method, controls, detected organism, read and coverage evidence, database or reference context, and the assay’s validated limit of detection where established. It should distinguish a confident detection from a low-level or ambiguous assignment and state whether confirmatory testing is recommended.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Clinicians should receive the result with its limitations: mNGS may detect nucleic acid without proving viability or causation, a negative result cannot exclude infection when biomass is low, and absence from a database can prevent species-level identification. PCR remains appropriate for confirmation when a specific target is known.

Bottom-line decision

Choose PCR when the target is known, speed and throughput matter, or the specimen is expected to contain little virus. Choose mNGS when the cause is unknown, routine assays are unrevealing, an unexpected co-infection or novel virus is possible, or genome-wide information is needed. In many real programs the strongest design is not either-or: PCR provides the routine screen, while sequencing supplies discovery, variant confirmation, outbreak analysis, and broader surveillance.

Frequently Asked Questions

Is metagenomic sequencing always more comprehensive than a multiplex PCR panel?

It is broader in principle because it does not require a fixed target list, but its effective coverage depends on viral biomass, host background, sequencing depth, database quality, and analysis thresholds. A well-validated multiplex PCR panel can be more reliable for the specific viruses it targets.

What does a negative mNGS result mean?

It means the workflow did not detect reportable evidence above its analytical and interpretation thresholds. It does not rule out infection when the sample has low viral load, high host background, poor collection, degradation, or insufficient sequencing depth.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should every PCR-positive sample be sequenced?

Not usually. Routine PCR can handle diagnostic volume; sequencing is commonly applied to a defined subset for variant confirmation, surveillance, outbreak investigation, or unusual results. The subset and sampling scheme should be specified so conclusions about circulation are defensible.

Can mNGS identify a virus that has never been seen before?

It can detect sequences that do not match a known species exactly and may provide evidence of a divergent or novel virus. Confirmation requires additional testing, genome analysis, contamination review, and epidemiological assessment; an isolated read is not sufficient proof.

Why might two laboratories report different mNGS sensitivities?

They may use different specimens, extraction methods, host-depletion or capture steps, sequencing platforms, read depths, pathogen mixes, and positivity thresholds. The published 19%-to-85% pathogen-specific range in one respiratory study illustrates this dependence.

Is a high mNGS read count proof that a virus caused the illness?

No. Read abundance supports confidence in detection but does not by itself establish active replication, causation, or clinical relevance. Controls, genome coverage, specimen type, symptoms, and independent confirmation all matter.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When is targeted sequencing preferable to mNGS?

Targeted sequencing is preferable when the virus is already known and the goal is sensitive detection or characterization of a defined region, such as a variant marker. It concentrates sequencing capacity on the expected target instead of distributing reads across all nucleic acids.

What should a laboratory budget for mNGS?

Published evidence includes consumable estimates of $130-$685 per sample in a 2021 systematic review and £112 per sample in a 2026 respiratory nanopore workflow. Actual program cost also includes labor, instruments, controls, computation, storage, validation, and confirmatory testing.
