Menopause does not create a single “right” date for a bone-density scan. In current U.S. guidance, routine screening is recommended for all women at age 65 or older. Postmenopausal women under 65 should first have their fracture risk assessed; a central DXA scan is appropriate when that assessment shows increased risk. A prior low-trauma fracture, known osteoporosis, a bone-weakening illness, or long-term use of a bone-damaging medicine calls for clinical evaluation rather than relying on the routine screening pathway.

Why menopause changes bone health—but does not determine the scan date

Estrogen levels fall during the menopause transition, and lower estrogen is associated with faster bone loss. The size and consequences of that loss vary widely, however. Age, previous fractures, body weight, family history, smoking, alcohol use, medical conditions, medications and fall risk all affect the chance of breaking a bone. That is why screening decisions are based on overall fracture risk instead of menopause status alone.

The U.S. Preventive Services Task Force (USPSTF) final recommendation dated January 14, 2025 gives a B recommendation for two groups:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Women aged 65 years or older.
  • Postmenopausal women younger than 65 who are at increased risk after a risk-factor review and a clinical risk assessment.

The recommendation addresses adults aged 40 or older who do not already have diagnosed osteoporosis or a history of a fragility fracture. It does not cover people with secondary osteoporosis or chronic use of medicines associated with bone loss. Those situations may require diagnostic testing and treatment decisions outside a population-screening schedule.

For context, the USPSTF reports an age-adjusted osteoporosis prevalence of 12.6% among community-dwelling U.S. adults aged 50 or older. Among adults aged 65 or older, the reported prevalence is 27.1% for women and 5.7% for men. These figures describe a population, not an individual’s personal probability.

When a postmenopausal woman should discuss DXA

At age 65 or later

USPSTF says women 65 and older should be screened to help prevent osteoporotic fractures. A clinician can select the testing site and review any factors that might make the scan or its interpretation more complex.

Before age 65 when risk is elevated

For a postmenopausal woman younger than 65, the USPSTF pathway has two steps:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Review risk factors. Examples include low body weight, a parent who had a hip fracture, cigarette smoking and excess alcohol use.
  2. Use a clinical risk-assessment tool. The result helps determine whether DXA screening is warranted.

ACOG’s patient information uses the same practical framing: test all women at 65 or older, and test younger postmenopausal women when fracture risk is high. It notes that FRAX can be used from age 40. A risk factor is a reason to have an individualized discussion, not an instruction to order a scan without medical review.

Situations that need evaluation outside routine screening

Tell a clinician promptly if any of the following applies:

  • A fracture after a minor fall or other low-trauma event, especially a spine or hip fracture after age 50.
  • A previous diagnosis of osteoporosis or a known very low bone density.
  • A condition that can weaken bone, such as malabsorption or vitamin D deficiency.
  • Frequent falls or substantial balance problems.
  • Long-term treatment with a medicine linked to bone loss, such as corticosteroids (including prednisone), or other drugs your clinician identifies as a risk.
  • Insulin-treated diabetes or another illness that changes fracture risk.

The USPSTF screening recommendation specifically excludes known osteoporosis, prior fragility fracture, secondary osteoporosis and chronic use of bone-damaging medications. In these cases, a clinician may order DXA or other tests for diagnosis, assess for an underlying cause, or discuss treatment without waiting for a routine screening age.

What a central DXA scan measures

DXA (also written DEXA) is the standard central bone-density test. It usually measures bone mineral density at the hip and lumbar spine, the sites used for diagnosis and treatment decisions. You lie on a table while the scanner passes over the body. The procedure is non-invasive and painless and uses very little radiation. ACOG describes the scan itself as taking about 3 to 10 minutes.

Most people need a prescription or referral. Ask a clinician or health plan where testing is covered and whether the facility’s technologists are certified. Bring information about previous fractures, medicines, supplements, medical conditions and any prior DXA reports so the interpreting clinician has the context needed to assess the result.

Additional measurements

Some centers can add vertebral fracture assessment or a trabecular bone score. Availability varies, and these measurements answer different questions from the basic hip-and-spine scan. Ask whether an additional assessment would change management in your case.

Why a peripheral test is not interchangeable

Heel ultrasound and other peripheral tests can help identify people who may need further evaluation. Their values are not directly comparable with central hip or spine DXA values, and they cannot accurately diagnose osteoporosis or monitor a medication response. A peripheral result that raises concern generally leads to a central DXA rather than replacing it.

How to read the numbers on a DXA report

T-score categories after menopause

For postmenopausal women and men aged 50 or older, diagnosis uses the T-score. It compares measured bone density with the average peak bone density of a healthy young-adult reference population. The more negative the value, the lower the measured density.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
T-score Classification What it tells you
−1.0 or higher Normal bone density Density is within the normal range by this classification.
Below −1.0 but above −2.5 Low bone mass (often called osteopenia) Density is lower than the young-adult reference range; fracture risk still depends on age and other factors.
−2.5 or lower Osteoporosis threshold The measured density meets the DXA threshold used to classify osteoporosis.

A report may show separate scores for the left and right hip, femoral neck, total hip and lumbar spine. The lowest relevant value is not interpreted in isolation: the measured site, image quality, artifacts, previous scans and clinical history all matter. A score at or below −2.5 establishes a density category, but it does not by itself determine the best treatment for every person.

What a Z-score means

A Z-score compares bone density with what is expected for people of the same age. It is useful when a result appears unusual for age and when a clinician is looking for a secondary cause. T-scores are used for osteoporosis diagnosis in postmenopausal women and in men aged 50 or older; they are not the diagnostic standard for premenopausal women.

For a premenopausal woman, a Z-score at or below −2.0 is described as “below the expected range for age.” That finding should prompt a review of medical history and possible secondary causes rather than an osteoporosis diagnosis based on the score alone.

Why the T-score is only part of the diagnosis

Fracture history can change the interpretation

A spine or hip fracture after age 50 indicates osteoporosis according to Bone Health and Osteoporosis Foundation (BHOF) guidance, regardless of the T-score. A person can therefore have a T-score above −2.5 and still need osteoporosis treatment or additional evaluation because the fracture itself demonstrates substantial skeletal fragility.

FRAX adds estimated fracture probability

FRAX estimates the 10-year probability of a hip fracture and of a major osteoporotic fracture (typically a spine, forearm, hip or shoulder fracture). It combines age, sex, body-size information, fracture history and selected clinical risks, with or without femoral-neck bone density. Clinicians use the estimate alongside the DXA result, not as a replacement for it.

Risk estimates can be affected by details that a short questionnaire cannot fully capture, such as multiple recent fractures, dose and duration of steroid treatment, falls, or rapidly changing health. Give the clinician the complete history rather than assuming that one FRAX number settles the decision.

Look for other explanations for low density

Low bone density can result from a condition other than primary postmenopausal osteoporosis. Depending on the history, evaluation may consider nutrient malabsorption, vitamin D deficiency, endocrine disease, kidney or liver problems, or medication effects. The appropriate tests are individualized; a low DXA number is a reason to ask what caused it, not proof of one specific cause.

When should the scan be repeated?

There is no universal repeat interval. ACOG says timing depends on age, the previous scan and individual fracture risk. BHOF notes that repeating DXA is often considered one to two years after starting or changing osteoporosis medication, or in an untreated person whose result is near a treatment threshold. Later timing depends on the individual.

A repeat scan is most useful when its result could change a decision—for example, whether to start treatment, continue it, investigate an unexpected decline or reassess risk after a major health change. Scans performed too close together may not show a meaningful biological change and can add cost without improving care. Use the same facility and, when possible, the same machine for follow-up so that changes are easier to interpret. The ordering clinician should decide the interval based on the report and the person’s risk.

Questions to take to the appointment

  • Which skeletal sites were measured, and what are my individual T-scores and, if relevant, Z-scores?
  • Was the scan technically adequate, and are there artifacts or spinal changes that could distort a value?
  • How do my age, previous fractures and FRAX estimate affect the meaning of this result?
  • Could a medical condition or medication be contributing to low bone density?
  • Would vertebral fracture assessment or another evaluation change management?
  • What treatment or lifestyle change is appropriate for my level of risk?
  • When would another DXA be likely to affect a decision?

Actions that support bone health

BHOF recommends weight-bearing activity, muscle-strengthening exercise, balance and fall-prevention work, adequate calcium and vitamin D intake, avoiding smoking and avoiding excessive alcohol. These measures support overall skeletal health but do not replace risk assessment, DXA when indicated or prescribed treatment.

Do not assume that a calcium or vitamin D supplement prevents fractures for everyone. The USPSTF notes limits in the evidence for supplements used solely to prevent fractures in community-dwelling adults. Ask about dietary intake, blood tests, kidney health, interactions and the dose that is appropriate for you before starting a supplement.

A practical decision path

  1. Identify your category. If you are 65 or older, discuss routine screening. If you are younger and postmenopausal, continue to a risk review.
  2. List risk factors and events. Include body weight, parental hip fracture, smoking, alcohol, falls, prior fractures, illnesses and medications.
  3. Ask for formal risk assessment. Your clinician may use a validated tool such as FRAX; ACOG notes it can be used from age 40.
  4. Choose the appropriate test. When screening is indicated, central hip-and-spine DXA is the diagnostic test. A peripheral test is not a substitute.
  5. Interpret the report with clinical context. Review T-score category, fracture history, FRAX and possible secondary causes together.
  6. Plan only the follow-up that can change care. The interval for another scan depends on risk, treatment and the likelihood that a new result will alter management.
