Massachusetts announced its 13th human West Nile virus case of 2026 on October 9. The patient was a woman in her 40s who was likely exposed in Middlesex County, according to the Massachusetts Department of Public Health (DPH). The announcement’s risk classifications are dated; check the state’s daily Arbovirus Update for the current level in your community.
What the October 9 announcement says
The case count is the number Massachusetts had reported by October 9, 2026—not a prediction of future cases or an indication that exposure occurred throughout Middlesex County. DPH described the woman’s likely exposure as being in that county; the announcement did not identify a confirmed transmission site.
As of that release, DPH classified Boston, Brookline, Cambridge, Chelsea, Everett, Malden, Newton, Revere, Somerville, Watertown, and Winthrop as high risk for West Nile virus (WNV). Most of the rest of the state was classified as moderate risk. Those levels can change, and DPH refreshes its online update daily.

DPH said the first WNV-positive mosquitoes of the season were announced June 16. By October 9, 344 mosquito samples had tested positive across Barnstable, Berkshire, Bristol, Dukes, Essex, Franklin, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester counties. The release reported no animal WNV cases that year.

The same release reported 27 EEE-positive mosquito samples and no human or animal Eastern equine encephalitis (EEE) cases so far in 2026. These are season-to-date counts from October 9, not live totals.
How West Nile virus illness can present
Nationally, the Centers for Disease Control and Prevention (CDC) says about 80% of people infected with WNV develop no symptoms. About 20% develop a flu-like illness, and fewer than 1% develop severe illness affecting the central nervous system, requiring hospitalization, or resulting in death. These are general CDC estimates, not Massachusetts-specific rates.

Mild illness
Symptoms usually begin 2–6 days after an infected mosquito bite, though they can start 2–14 days later; onset may take longer for people with weakened immune systems. Mild illness can include fever, headache, body aches, joint pain, vomiting, diarrhea, or rash.
Severe illness
Severe WNV can cause encephalitis or meningitis. Warning signs include high fever, neck stiffness, disorientation or confusion, tremors, convulsions, muscle weakness, vision loss, numbness, or paralysis. CDC says about 10% of people who develop severe illness affecting the central nervous system die. Recovery can take weeks or months, and some effects may be permanent.

Who is at greater risk
Severe-illness risk rises with age. CDC identifies cancer, diabetes, hypertension, kidney disease, and a weakened immune system as risk factors. It says people aged 65 and older are three times as likely to develop neurologic illness as people younger than 65. These population-level risk factors do not determine an individual’s outcome.
When to seek medical care
Seek immediate medical attention for high fever, neck stiffness, muscle weakness, confusion, or tremors. Tell a healthcare provider about possible mosquito exposure and when symptoms began. A provider can assess symptoms and exposure history and order laboratory tests if appropriate. A mosquito bite alone does not establish that someone has WNV.

Ways to reduce mosquito bites
Use repellent as directed
- When outdoors, use a repellent with an EPA-registered active ingredient and follow its label. DPH names DEET, permethrin, picaridin, oil of lemon eucalyptus (PMD), and IR3535.
- Do not use DEET products on infants under two months. For older children, DPH advises DEET concentrations of 30% or less.
- Do not use oil of lemon eucalyptus on children under three years.
Cover up and consider timing
- Many mosquitoes bite most actively from dusk to dawn. In high-risk areas, consider rescheduling evening or early-morning outdoor activities.
- Long sleeves, long pants, and socks can reduce the amount of exposed skin.
Remove water and keep screens fitted
- Drain or discard items that hold standing water. Check gutters and drains, empty unused flowerpots and wading pools, and change birdbath water frequently.
- Install or repair tightly fitting screens on doors and windows.
Protect animals
- Remove standing water from containers and flush water troughs at least weekly during summer.
- Keep horses in indoor stalls at night. Ask a veterinarian about animal-approved repellents and WNV or EEE vaccinations.
- DPH says suspected animal WNV or EEE cases must be reported to the Massachusetts Department of Agricultural Resources, Division of Animal Health, and DPH. Use the state’s official announcement for current reporting contact details.
How long risk may continue
DPH said some WNV risk would continue until the first hard frost. That seasonal guidance does not replace checking the current community-level classification in the state’s daily Arbovirus Update.
