Neither lipid nanoparticles (LNPs) nor viral vectors is a universal winner for gene therapy. The appropriate delivery system depends on the payload, target cells, required duration of action, repeat-dosing plan, immune history, safety margin, route of administration, and manufacturing constraints. LNPs are synthetic, nonviral carriers that can deliver RNA and other nucleic acids. Viral vectors—especially adeno-associated virus (AAV)—use engineered viral particles to deliver genetic material and may provide durable expression.
That distinction matters because a delivery vector is part of a therapy’s biological design, not a generic shipping choice. A platform that is excellent for transient messenger RNA may be unsuitable for permanent gene addition, while a vector capable of long-term expression may be inappropriate when expression must be stopped or regularly adjusted.
What lipid nanoparticles and viral vectors are
Lipid nanoparticles are synthetic carriers
An LNP packages nucleic acid inside a particle made from several lipids. A common design uses an ionizable or cationic lipid, a helper lipid, cholesterol, and a PEG-lipid. The proportions and chemical identities of those components affect particle size, stability, tissue distribution, cellular uptake, and release of the payload. The formulation is therefore part of the medicine, not merely an interchangeable container. The Nature Reviews Genetics overview of RNA delivery systems explains how these components are used to protect nucleic acids and support intracellular delivery.
Many LNPs are engineered so that they are relatively less charged in the bloodstream but become protonated in the acidic environment of an endosome after cellular uptake. That change can help the particle disrupt the endosomal membrane and release RNA into the cytoplasm. The exact mechanism and efficiency vary with the lipid chemistry, payload, dose, route, and target tissue.

Viral vectors use modified viruses
Viral vectors are engineered viral particles stripped of disease-causing functions and designed to carry therapeutic genetic material. The term covers several families, including AAV, lentiviral vectors, and adenoviral vectors; their biology, payload limits, persistence, and immune effects are not interchangeable. AAV is the most relevant comparison for many in vivo gene-therapy discussions because its capsids can be selected or engineered for different tissue tropisms.
AAV generally delivers its genome to the cell nucleus, where vector-derived genetic material can support transgene expression. A 2024 review describes AAV as “non-pathogenic, rarely integrates into the host genome, and can sustain long-term transgene expression.” The same review emphasizes that this potential durability does not remove concerns about immune responses, dose-related toxicity, genotoxicity, hepatotoxicity, thrombotic microangiopathy, neurotoxicity, and manufacturing complexity. Read the full context in “Adeno-associated virus as a delivery vector for gene therapy of human diseases.”
How the delivery process differs
The LNP pathway
- Formulation: The nucleic acid is mixed with selected lipids, commonly including an ionizable lipid, helper lipid, cholesterol, and PEG-lipid.
- Administration and circulation: The particle travels through the body by the chosen route. Protein adsorption and particle properties influence where it goes.
- Cell entry: Target cells take up the particle, usually through endocytosis.
- Endosomal escape: A fraction of the payload must escape the endosome before degradation.
- Payload action: siRNA can reduce production of a target protein, while mRNA can be translated into a therapeutic protein. Other nucleic-acid payloads require their own intracellular conditions.
- Clearance and repeat dosing: The particle and payload are eventually cleared or degraded. The timing and tolerability of another dose depend on the specific formulation and clinical setting.
This mechanism makes LNPs attractive for medicines whose effect can be renewed, adjusted, or stopped. It also creates a central challenge: only a limited fraction of administered payload may reach the desired cells and escape into the correct intracellular compartment.

- Capsid selection: The vector’s capsid or serotype influences which tissues it can enter and how efficiently.
- Cell binding and entry: Surface receptors and tissue accessibility determine whether the vector reaches the intended cells.
- Intracellular trafficking: The vector must move through the cell and deliver its genome to the nucleus.
- Transgene expression: Regulatory elements in the vector control where and how strongly the therapeutic gene is expressed.
- Immune recognition: Pre-existing antibodies, T-cell responses, and responses to the capsid or expressed protein can limit efficacy or safety.
AAV’s natural and engineered tropisms can help reach some tissues, but tropism is not the same as precise targeting. Route, dose, capsid, disease state, and tissue barriers all influence biodistribution.
Side-by-side comparison
| Decision axis | Lipid nanoparticles | Viral vectors, especially AAV | What the comparison does not establish |
|---|---|---|---|
| Carrier | Synthetic lipid formulation, commonly using an ionizable or cationic lipid, helper lipid, cholesterol, and PEG-lipid. | Engineered viral particle; capsid and vector design influence tissue entry and expression. | “Viral vector” is a broad category. AAV properties cannot be assigned to every viral family. |
| Typical payloads and tasks | siRNA, mRNA, and other nucleic acids. | Genetic material for gene addition and other strategies, subject to vector-specific limits. | Versatility depends on the payload and mechanism, not on the platform label alone. |
| Payload capacity | Depends on formulation and payload chemistry; no single capacity applies to every LNP. | AAV has a limited cargo capacity, which constrains large genes and complex editing systems. | A numerical capacity comparison requires a defined formulation and vector design. |
| Expression duration | Many RNA effects are transient, although duration varies with payload and formulation. | Can support long-term transgene expression. | Long duration is beneficial only when durable expression is clinically wanted. |
| Tissue delivery | Biodistribution and cell-specific targeting remain major design problems. | Natural or engineered tropism can favor particular tissues, but precise targeting remains difficult. | Route, dose, formulation, capsid, and disease determine actual distribution. |
| Immune and safety profile | Avoids viral proteins, but lipids, payloads, dose, and biodistribution can produce immune or toxic effects. | Pre-existing or induced immunity and high-dose toxicity can restrict treatment; genotoxicity is among the concerns discussed for AAV. | Neither platform should be described as universally safe or non-immunogenic. |
| Repeat dosing | Repeat administration can be a design attraction, but performance depends on formulation and patient response. | Anti-capsid immunity can complicate repeat administration. | Some LNPs cannot be repeatedly dosed indefinitely, and some viral-vector strategies may permit limited redosing under specific conditions. |
| Manufacturing | Consistent lipid synthesis, encapsulation, scale-up, sterilization, and product characterization are critical. | Vector production, purification, potency testing, empty/full capsid control, and scale remain demanding. | There is no evidence for a universal cost or manufacturing advantage across all products. |
A recent review of emerging delivery platforms treats targeting, manufacturing, and clinical translation as linked engineering problems rather than separate checkboxes. Match the platform to the therapeutic payload
RNA medicines: a strong LNP use case
RNA does not normally need to enter the nucleus to act. siRNA can direct degradation of a chosen messenger RNA, reducing synthesis of a disease-associated protein. Patisiran, an siRNA treatment for hereditary transthyretin amyloidosis, is a clinical example of lipid-based nucleic-acid delivery. Its mechanism is gene silencing, not permanent replacement of a defective gene.

mRNA delivers a temporary instruction for producing a protein in the cytoplasm. LNP-mRNA systems therefore suit applications in which a burst or series of protein-production events is preferable to lifelong expression. The duration depends on mRNA sequence, chemical modification, dose, tissue distribution, and formulation. The Nature Reviews Materials review of LNPs for mRNA delivery covers the design and translation issues involved.
Lipid systems can also carry other nucleic acids, but adding a new payload does not automatically preserve the performance of the original formulation. Size, charge, stability, endosomal escape, innate immune sensing, and the required intracellular destination may all change.
Gene addition and genome editing
Gene addition usually seeks to provide a functional copy or therapeutic sequence for an extended period. AAV can be attractive when the target tissue is accessible to a suitable capsid and sustained expression is desirable. Its limited cargo capacity becomes a practical constraint for large genes, multiple regulatory elements, or editing systems that require more than one component.
Genome editing has a different risk-benefit problem. The editor, guide RNA, donor template, or other components must reach the right cells at useful levels, and unwanted editing must be minimized. A durable vector can maintain exposure longer than necessary, whereas a transient LNP delivery may reduce the time available for off-target activity. Conversely, transient delivery may fail if the therapeutic task requires prolonged expression or repeated editing events.
The correct question is therefore not “Which carrier is more advanced?” but “What payload must reach which cells, for how long, and with what reversibility?”

Delivery is often the limiting step in gene therapy. A treatment can have a compelling molecular target and still fail if too little payload reaches the relevant cells, if it accumulates in unwanted organs, or if it cannot cross a tissue barrier.

What controls LNP distribution
- Lipid chemistry, particle size, surface properties, and PEG density.
- Protein adsorption in blood, which can alter organ uptake.
- Administration route, dose, infusion rate, and formulation concentration.
- Endosomal escape and the fraction of payload that remains intact.
- Cell-type differences in uptake and intracellular processing.
Liver delivery has been a major practical focus for lipid-based systems, but a formulation that reaches hepatocytes efficiently may not reach muscle, lung, brain, or a specific immune-cell population. Redirecting LNPs to other tissues is an active engineering problem, not a solved property of the entire platform.
What controls viral-vector distribution
- AAV serotype or engineered capsid and its receptor interactions.
- Route of administration and physical access to the target tissue.
- Pre-existing neutralizing antibodies and other immune barriers.
- Dose, which can affect both the number of transduced cells and toxicity.
- Promoter and regulatory-element design, which influence expression after entry.
Capsid tropism can provide a useful starting point, but it does not guarantee cell-specific expression. A vector may enter several tissues while the therapeutic promoter restricts expression to only some cell types. Conversely, a capsid that performs well in an animal model may behave differently in humans because receptor abundance, immunity, and tissue architecture differ.
Duration, reversibility, and redosing
When transient action is an advantage
Transient expression is useful when the therapeutic effect should be adjustable, when a protein could become harmful at excessive levels, or when a gene-editing system should not remain active indefinitely. LNP delivery can allow a treatment schedule built around repeated administration, provided that each dose remains tolerable and effective.
Repeat dosing is not automatic. Complement activation, inflammatory responses, anti-PEG or other formulation-specific responses, organ accumulation, and declining delivery efficiency can all matter. A clinical program must evaluate the actual formulation and regimen rather than assume that every nonviral carrier is suitable for chronic use.
When durable expression is an advantage
AAV may reduce the need for frequent administration when a single treatment can establish long-lasting expression in the target tissue. That durability can be particularly valuable for diseases in which repeated dosing is difficult or where continuous protein production is needed.
The same feature can become a liability if expression is too high, occurs in the wrong cells, or produces an immune response. Anti-capsid immunity can also make repeat administration difficult, and the first dose may prevent later use of the same or a related capsid. Durability must therefore be weighed against reversibility and the patient’s future treatment options.

Safety and immune considerations
It is inaccurate to call LNPs “safe” or viral vectors “dangerous” as a class. Risk depends on the payload, carrier composition, dose, route, disease, patient, and monitoring plan.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsLNP-specific considerations
- Innate immune activation: Lipids and nucleic acids can stimulate inflammatory pathways, especially at higher exposure or with particular chemistries.
- Organ distribution: Uptake in liver and other organs can create toxicity that is unrelated to the intended target.
- Endosomal escape: Lipids effective at membrane disruption may also contribute to cellular stress.
- Formulation effects: PEG-lipids and other components can influence circulation time, tolerability, and repeat-dose behavior.
- Payload effects: RNA sequence, impurities, innate immune sensing, and protein levels can each affect safety.
The lipid-based therapeutics review in Pharmaceutics discusses efficacy, biodistribution, and toxicity as formulation-dependent issues rather than fixed properties of all LNPs.
Viral-vector considerations
- Pre-existing immunity: Antibodies against a capsid can reduce delivery before treatment begins.
- Adaptive responses: T-cell responses may eliminate transduced cells or limit persistence.
- Dose-related toxicity: High systemic exposure has been associated with serious adverse effects in some viral-therapy contexts.
- Genotoxicity: Although AAV is generally described as rarely integrating, insertional events and genomic effects remain part of safety assessment.
- Target-organ injury: Liver, nervous-system, vascular, and other toxicities must be evaluated for the specific product and route.
AAV’s potential for long-term expression is thus inseparable from its immune and safety profile. A durable benefit is not a reason to skip dose selection, biodistribution studies, or long-term follow-up.

What must be controlled for LNPs
LNP manufacturing requires reproducible lipid quality, controlled mixing or assembly, reliable nucleic-acid encapsulation, particle-size distribution, residual-solvent control, sterility, storage stability, and a potency assay that predicts biological delivery. Changing one lipid or process parameter can alter biodistribution and activity, so scale-up is not simply a matter of making a larger batch.
Manufacturers also need a clinically practical formulation: acceptable concentration, infusion or injection volume, cold-chain requirements, shelf life, and a release test that can distinguish a consistent product from one that merely has the correct nominal ingredients.
What must be controlled for AAV
AAV production involves cell-based vector manufacture, purification, genome and capsid characterization, potency testing, and control of empty or incorrectly packaged particles. Yield and consistency can be difficult at large scale. Process impurities, capsid heterogeneity, and assay comparability can affect both efficacy and safety.

The 2026 delivery-platform review and the 2024 AAV review both treat manufacturing as a core translational challenge. Neither platform should be assumed to be cheaper, simpler, or more mature in every indication. The relevant comparison is between defined products, processes, facilities, and clinical requirements.
A practical framework for choosing between them
- Define the biological task. Is the therapy silencing a gene, supplying mRNA, adding a functional gene, replacing a missing protein, or editing DNA?
- Specify the target cells. Identify the organ, cell type, accessibility, required percentage of corrected cells, and acceptable off-target exposure.
- Set the duration requirement. Decide whether expression should last minutes, days, months, or years—and whether it must be reversible.
- Assess the payload size and architecture. Account for the gene, promoter, regulatory elements, guide RNAs, donor templates, or protein-coding sequence.
- Model immune barriers. Check pre-existing anti-capsid immunity for viral vectors and evaluate innate, anti-PEG, or formulation-specific responses for LNPs.
- Plan redosing before the first dose. A therapy that may require several administrations needs evidence that efficacy and tolerability persist across the schedule.
- Compare biodistribution and pharmacology. Measure where the carrier and payload go, how much reaches the target cells, and how long the effect lasts.
- Evaluate manufacturing and delivery logistics. Include batch consistency, scale, storage, administration setting, testing, and long-term follow-up.
- Choose the platform only after these constraints are explicit. The best choice is the one that satisfies the complete therapeutic design, not the one with the most favorable reputation in isolation.
What the clinical examples do—and do not—show
Patisiran demonstrates that an LNP-enabled siRNA medicine can produce a clinically useful gene-silencing effect. LNP-mRNA systems demonstrate another use of lipid delivery: transient instructions for protein production. These examples validate particular payload-and-formulation combinations; they do not establish that all LNPs can replace viral vectors for gene addition.
AAV’s clinical prominence demonstrates that viral vectors can deliver durable genetic instructions in selected settings. It does not mean every serotype reaches every organ, that every patient is eligible, or that a durable vector is preferable when treatment must be titrated or stopped.

How do lipid nanoparticles compare with viral vectors for gene therapy?
LNPs are synthetic carriers suited to siRNA, mRNA, and other nucleic-acid payloads, often with transient or repeatable treatment designs. Viral vectors, particularly AAV, can provide longer-lasting expression but bring capsid immunity, payload-capacity limits, dose-related safety concerns, and more difficult redosing. The comparison is meaningful only for a defined payload, target tissue, route, and clinical objective.
