The only way to avoid ketamine’s risks is not to use it. If you or someone with you may use ketamine, risk-reduction steps can lower some foreseeable dangers but cannot make nonmedical use safe. The most urgent priorities are avoiding alcohol, opioids and benzodiazepines; preventing falls, drowning, driving and other injuries while impaired; recognizing urinary and abdominal warning signs early; and getting medical or substance-use support without delay.

What ketamine is and why harm reduction matters

Ketamine is used medically as an anaesthetic and analgesic. Esketamine, a related medicine, has licensed use for treatment-resistant depression in some jurisdictions under clinical supervision. Those medical uses do not make unsupervised or nonmedical ketamine use safe. Outside a controlled clinical setting, the substance can alter perception, coordination, judgment and decision-making. The resulting risks include falls, road traffic injuries, drowning, accidental injury, violence, exploitation and impaired ability to consent.

Ketamine can also lead to dependence and longer-term injury involving the bladder, urinary tract, kidneys, abdomen, liver and bile ducts. A “K-hole” describes intense dissociation. It is not a predictable or harmless state: a person may be unable to respond, protect their airway, recognize danger or give meaningful consent.

The Advisory Council on the Misuse of Drugs (ACMD) published a UK review on 28 January 2026. Its recommendations are UK public-health guidance, not a universal legal or clinical standard. Ketamine remains a Class B controlled substance in the UK; laws and emergency arrangements differ elsewhere.

How can I reduce the risks of ketamine?

Not using ketamine, stopping, or reducing use provides the greatest risk reduction. If stopping is difficult, a clinician or local alcohol and drug service can help you plan a safer reduction. Evidence about ketamine withdrawal and dependency treatment remains limited, so no particular withdrawal programme should be presented as guaranteed to work.

  • Do not use alone. A trusted, sober companion may notice a medical emergency and call for help. This does not make use safe or replace emergency care.
  • Choose a physically safer setting. Avoid baths, swimming, open water, heights, balconies, roads, machinery and any place where a fall or loss of awareness could be fatal.
  • Do not drive, cycle in traffic or operate machinery. Impairment may persist even when a person feels more alert.
  • Plan for consent and personal safety. Intoxication can make it impossible to communicate or consent. Everyone should be free to leave, and sexual activity should wait until all participants are sober and able to make decisions.
  • Keep amounts small and pre-measured if use occurs. The ACMD describes this as a harm-reduction principle, not a safe dose. Do not rely on a test amount to predict potency, because products and individual responses vary.
  • Space doses and use episodes. Avoid repeated dosing during an impaired period. The ACMD advises reducing the frequency of sessions and says ideally limiting sessions to at least four weeks apart; this is not a safe-use threshold or a guarantee against bladder, dependence or other harm.
  • Do not use an unknown product as if its contents were certain. Service-based drug-checking programmes may provide information where available, but no test makes ketamine use safe and consumer devices should not be treated as clinically reliable without independent evidence.

What should I avoid mixing ketamine with?

Avoid combining ketamine with depressants, particularly alcohol, opioids and benzodiazepines. The ACMD states that concurrent use substantially increases overdose risk. Health Canada warns that depressant combinations can suppress breathing or stop it. The danger is greater when a person is alone, falls asleep, vomits or cannot be roused.

Combination to avoid Why it is dangerous Safer response
Ketamine and alcohol Combined impairment can deepen unconsciousness, increase vomiting and reduce the ability to protect the airway. Do not combine them. If someone is difficult to wake or breathing abnormally, contact the local emergency service immediately.
Ketamine and opioids Both can contribute to profound sedation and respiratory depression; the combination raises overdose risk. Do not use together or use opioids to mask ketamine-related pain. Seek medical care instead.
Ketamine and benzodiazepines Additional central-nervous-system depression can make breathing and consciousness dangerously suppressed. Avoid the combination and disclose all substances to emergency clinicians.

Do not assume that a person is safe because they took substances at different times. Effects can overlap, and the amount or strength of an illicit product may be uncertain. If prescribed medicines are involved, ask the prescriber or pharmacist about interactions rather than stopping essential treatment on your own.

What should I do if someone becomes unresponsive?

Treat loss of consciousness, abnormal breathing or inability to wake as an emergency. Contact your local emergency service; the correct number depends on your country. Do not leave the person alone or assume they can “sleep it off.”

  1. Check whether the person responds and whether they are breathing normally.
  2. If they are unconscious but breathing normally, place them on their side in the recovery position and keep watching their breathing.
  3. If breathing is absent or abnormal, follow the emergency dispatcher’s instructions and begin resuscitation if trained.
  4. Keep the airway clear. Do not give food, drink or more drugs, and do not induce vomiting.
  5. Tell responders what was taken, approximately when, and whether alcohol, opioids, benzodiazepines or other medicines were involved. Honest information helps clinicians choose appropriate care.

Call urgently for severe confusion, a seizure, blue or grey lips, repeated vomiting, a serious fall, chest pain, suspected drowning, extreme agitation or any injury that may involve the head, neck or spine.

Can ketamine damage your bladder?

Yes. Repeated ketamine use is associated with inflammation and injury of the bladder and urinary tract. Damage can extend to the upper urinary tract and kidneys and may be severe. It may not fully resolve after stopping, so early assessment matters.

In the ACMD’s 2026 review, about a quarter of people who used ketamine regularly reported at least one urinary symptom, citing Winstock and colleagues (2012). That estimate applies to regular users in the cited study, not to everyone who has ever used ketamine. The report says prevalence is associated with dose and frequency.

What are the signs of ketamine bladder damage?

Seek prompt clinical assessment if you notice any of the following, particularly after repeated or escalating use:

  • Needing to urinate more often than usual or feeling an urgent need to go
  • Waking repeatedly at night to urinate
  • Leaking urine or losing bladder control
  • Pain or burning when urinating
  • Blood in the urine
  • Pain in the bladder or lower abdomen

Severe or persistent abdominal pain, flank pain, fever, vomiting or reduced urine output also warrants urgent medical attention because injury may involve structures beyond the bladder. Do not try to hide or self-treat these symptoms with more ketamine or opioids. Tell the clinician about ketamine use, how often it occurs and any other substances or medicines; that information helps distinguish ketamine-related injury from infection, stones and other conditions.

Why frequency and amount affect risk

Risk is not determined by a single number. Higher amounts, repeated doses in one session and more frequent sessions increase the opportunity for acute accidents, dependence and urinary harm. Individual vulnerability, product strength, route of administration, other substances, hydration, existing medical conditions and the setting all matter.

For that reason, the ACMD’s advice to use small, pre-measured amounts, avoid rapid redosing and reduce the frequency of episodes should be understood as risk reduction—not as a recommended recreational regimen. There is no established amount or interval that guarantees safety. The most dependable way to prevent ketamine-related harm remains not using it.

Injection-related risks and safer service support

Injecting adds risks from contaminated equipment, vein injury and blood-borne viruses. This information is not an endorsement of injection. If someone is injecting, the ACMD recommends using a needle and syringe programme or another appropriate health service for sterile supplies, using new equipment for every episode and never sharing needles, syringes, filters, water or other preparation equipment.

  • Ask a healthcare provider about testing and vaccination advice, including hepatitis B, hepatitis C and HIV testing.
  • Dispose of used sharps through an approved service; never leave them where another person could be injured.
  • Seek urgent care for spreading redness, swelling, severe pain, pus, fever, a cold or pale limb, or sudden chest or breathing symptoms after an injection.

Service-based harm-reduction programmes can also provide confidential advice, referrals and, where available, drug checking. Availability varies by location.

How ketamine use can affect dependence and daily life

Dependence may show up as craving, using more often than intended, spending increasing time obtaining or recovering from ketamine, or continuing despite urinary, relationship, financial, work or health problems. A person may also find that ordinary activities feel less rewarding without the drug. These signs are reasons to seek support, not evidence of a moral failing.

If you want to cut down, write down when and why you use, identify people and places linked with use, and tell a clinician or local substance-use service what is happening. Ask for help with sleep, anxiety, pain or depression rather than trying to manage those problems with ketamine. Because evidence on withdrawal management is still limited, a qualified professional should tailor advice and monitor physical and mental health.

How common is ketamine use?

Population estimates vary by year and geography. The ACMD reported that 160,000 people aged 16 to 59 in England and Wales (0.5%) reported past-year ketamine use in 2014–2015, compared with 299,000 (0.9%) in 2022–2023. These are period-specific estimates, not a 2026 prevalence figure and not an estimate for other countries or age groups.

Survey period Location and age range Reported past-year use
2014–2015 England and Wales, ages 16–59 160,000 people (0.5%)
2022–2023 England and Wales, ages 16–59 299,000 people (0.9%)

When should I get medical help?

Get emergency help immediately for unconsciousness, abnormal or stopped breathing, blue or grey skin, seizures, suspected drowning, severe injury, chest pain or a person who cannot be roused after mixing substances.

Arrange prompt medical care for blood in urine, painful or burning urination, new urgency or frequency, leakage, bladder pain, severe or continuing abdominal pain, flank pain, fever, vomiting or reduced urine output. Do not wait for symptoms to become unbearable. Ketamine-related damage can progress and may not completely reverse after stopping.

Tell healthcare staff what was taken and when, including alcohol, opioids, benzodiazepines and prescribed medicines. Clinicians need accurate information to assess breathing, consciousness, infection, urinary injury, liver or kidney problems and interactions.

For ongoing use, cravings or difficulty cutting down, contact a confidential local drug and alcohol service, community harm-reduction programme or treatment provider. These services can discuss withdrawal, mental health, safer-use planning, testing, housing or safeguarding concerns and referral to specialist care. The ACMD recommends integrated harm reduction that combines education, professional training, access to drug checking and safer-use services. What is available depends on your country and local health system.

If you are supporting someone, use calm, nonjudgmental language, help them avoid mixing substances and dangerous settings, and encourage medical care for urinary or abdominal symptoms. You cannot supervise away the risks, and you should call emergency services when danger is immediate.

Common mistakes that increase harm

  • Using alone: no one is present to recognize respiratory depression, injury or drowning.
  • Adding alcohol or sedatives: the combination can suppress breathing unpredictably.
  • Redosing because effects feel delayed: impairment can intensify after additional doses.
  • Using more ketamine or opioids for pain: this can worsen toxicity and delay diagnosis of bladder or abdominal injury.
  • Waiting for blood in urine: urgency, frequency, burning, leakage or bladder pain may appear earlier.
  • Sharing injecting equipment: this can transmit hepatitis B, hepatitis C and HIV.
  • Assuming a sober-looking person can consent or drive: judgment and coordination may remain impaired.

Medical ketamine is different from unsupervised use

In a medical setting, clinicians select a formulation, screen for contraindications, monitor vital signs and provide follow-up. A prescription does not eliminate adverse effects, but supervision allows complications to be recognized and treated. Never use someone else’s prescribed ketamine, change a prescribed dose without the prescriber, or combine treatment with alcohol, opioids or benzodiazepines without medical advice.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What harm reduction can and cannot do

Planning, a sober companion, sterile service supplies and early medical attention can reduce particular hazards. None prevents all overdose, accidents, dependence, urinary injury, contamination or legal consequences. Drug checking cannot guarantee a sample is safe; spacing sessions cannot create a safe threshold; and a person who appears awake may still be unable to make decisions or protect themselves. Keeping the central goal—avoiding or stopping nonmedical ketamine use—prevents harm-reduction advice from being mistaken for a safety guarantee.

Frequently Asked Questions

Can ketamine bladder damage go away if I stop?

Some symptoms may improve after stopping, but damage can be severe and may not fully resolve. Urinary urgency, frequency, burning, leakage, blood in urine or bladder pain needs prompt clinical assessment rather than watchful waiting.

Is ketamine safer if it is prescribed?

Medical treatment is supervised, with screening, monitoring and follow-up that are absent in unsupervised use. Use only as prescribed and ask the treating clinician about interactions, symptoms and any change in dose.

Should I use naloxone if ketamine is involved?

Naloxone reverses opioid effects, not ketamine itself. If opioids may also have been taken and a person is unresponsive or breathing abnormally, give naloxone if available and follow local emergency guidance while calling emergency services.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can I prevent a K-hole by taking more slowly?

There is no guaranteed way to prevent intense dissociation or dangerous impairment. Taking additional doses while waiting for effects can increase risk, so avoid rapid redosing and do not use alone.
