Ask for a personalized pain plan before your IUD appointment rather than assuming an over-the-counter painkiller will prevent insertion pain. Ask which options your clinic actually provides, what each can and cannot relieve, when it must be arranged, its side effects and access limits, and how you can pause or stop the procedure. Current CDC and ACOG guidance supports shared decision-making: no single method is proven best for everyone.

Possible discussions include topical lidocaine, a lidocaine paracervical block, medication for afterward, selected use of misoprostol after a previous failed attempt, and—where clinically suitable and locally available—intravenous sedation, monitored anesthesia or general anesthesia. Your clinician can match those choices to your health history, medicines, anxiety, prior trauma or pelvic pain, and preferences.

Why pain planning belongs in the consultation

An IUD is placed through the cervix into the uterus, and people experience the examination and insertion differently. The CDC’s U.S. Selected Practice Recommendations says patients should be counseled before placement about possible pain and about the risks, benefits and alternatives of pain-management options. The plan should reflect the patient’s preference, not a one-size-fits-all protocol.

ACOG’s 2025 recommendations likewise call for individualized, trauma-informed counseling for IUD insertion and other in-office uterine or cervical procedures. As coauthor Kimberly Hoover, MD, FACOG, put it, “Comprehensive counseling is key because patients must be able to decide for themselves what interventions, if any, they would like to try.” Read the full announcement in ACOG’s May 15, 2025 release.

Bring your questions when you schedule the appointment, not only when you are already in the examination room. Some options require a clinician, extra equipment, another appointment or a separate anesthesia plan. Availability and cost vary by clinic.

Questions to ask before booking or arriving

  1. “What pain-relief options do you offer for IUD insertion, and which are available at this clinic?” Ask whether the answer differs for routine office placement versus a procedure scheduled in a surgical or hospital setting.
  2. “Could topical lidocaine or a paracervical block be appropriate for me?” Ask where the anesthetic is applied or injected, when it is given, what sensation to expect and which part of the procedure it is intended to address.
  3. “What would you suggest for cramping afterward, and is it safe with my health history or other medicines?” The clinician can check allergies, kidney or stomach problems, bleeding risks, pregnancy considerations and drug interactions before recommending a medicine.
  4. “Is there any reason to consider misoprostol in my case, such as a previous unsuccessful insertion?” Ask what benefit is expected, how far ahead it would be taken and which adverse effects could occur.
  5. “I am anxious, have pelvic pain or have a history of trauma. What can we change?” You can request a slower pace, an explanation before each step, a support person if the clinic permits one, or a plan that lets you control parts of the examination.
  6. “Can we agree on a signal to pause or stop?” Establishing this before the examination means you do not have to negotiate while uncomfortable.
  7. “Do you offer intravenous sedation, monitored anesthesia or general anesthesia, and what would arranging it involve?” Ask about referral, fasting, transportation, monitoring, recovery time and fees if any of these services are available.
  8. “Do I need to arrange an option before the appointment?” Confirm whether medication, a prescription, an anesthesiology consultation, an earlier arrival time or a different location is required.

How the main options compare

The useful comparison is not simply “strong” versus “weak.” Consider whether an option targets pain during insertion or cramps afterward, how it is delivered and timed, what discomfort or side effects it can cause, how certain the evidence is, whether your clinic provides it and whether it fits your circumstances.

Option What it may address Timing and route Evidence, trade-offs and access
Topical lidocaine Pain during parts of the cervical or uterine procedure Applied to genital or cervical mucosa before the relevant step; the clinician determines product and timing The CDC says topical lidocaine might reduce pain. ACOG’s 2025 guidance includes lidocaine spray and lidocaine-prilocaine cream among options to discuss. It may not numb every sensation, and availability differs.
Lidocaine paracervical block Pain during cervical manipulation and insertion Local-anesthetic injections around the cervix before insertion The CDC says a paracervical block might help. ACOG describes supportive but varied studies; the injections themselves can briefly hurt or feel like pressure. It requires a trained clinician and clinic supplies.
NSAID Post-procedure cramping and pain more than insertion pain An oral medicine only if your clinician says it is safe and advises a dose and timing ACOG says NSAIDs alone are usually not effective at preventing insertion pain, although they may help afterward. Safety depends on your medical history and other medicines.
Misoprostol Cervical preparation in selected situations, not routine pain prevention Prescription use according to an individualized plan CDC does not recommend routine use. It can increase pain, preplacement cramping and diarrhea; a study suggests possible benefit for a second attempt after a recent failed placement. Evidence is conflicting.
Intravenous sedation, monitored anesthesia or general anesthesia Awareness, anxiety and pain during a procedure, depending on the technique Provided only in settings with appropriate staff, monitoring and recovery arrangements ACOG says counseling should include these possibilities when and where available. Suitability, preparation, transportation and access vary; they are not offered or appropriate for every patient.
Communication and control measures Anxiety, distress and the experience of pain throughout the visit Planned before and during the examination Agree on explanations, pace, positioning, a pause signal and the right to stop. These measures do not anesthetize tissue but can make care more predictable and patient-directed.

Local anesthetics: creams, sprays and injections

Topical lidocaine

Topical lidocaine can be delivered as a spray or as lidocaine-prilocaine cream, depending on the clinic’s protocol. The clinician applies it to the relevant mucosa and allows the prescribed preparation time before proceeding. Ask exactly when it will be applied and which step it is meant to ease. Numbing the surface does not guarantee that you will feel nothing from cervical movement, uterine sounding or cramping.

The CDC describes topical lidocaine as something that might help, rather than a guaranteed solution. ACOG’s 2025 clinical consensus notes evidence for lidocaine applied to genital mucosa but also notes that studies used different techniques and measured pain at different moments. That variation is why your clinician should explain the likely benefit without promising an outcome. See ACOG’s clinical consensus.

Paracervical block

A paracervical block places lidocaine injections around the cervix. It may reduce pain from cervical manipulation and insertion, but the injections can produce a short-lived sharp sensation, pressure or additional procedural steps. Ask how many injections are planned, how long the anesthetic takes to work, what side effects the clinician watches for and whether you need to remain longer afterward.

Neither topical anesthetic nor a block should be described as universally superior. The evidence supports discussing them, while individual response and the clinician’s technique matter. If you have an allergy or a medical condition that affects local-anesthetic use, tell the clinician before choosing an option.

NSAIDs: useful for afterward, limited for insertion

Nonsteroidal anti-inflammatory drugs are often discussed because they can reduce inflammatory pain and cramping after a procedure. ACOG’s patient guidance explains that they are usually not effective alone at preventing pain during IUD insertion, although they may help with pain afterward; see ACOG’s “Pain Relief for Cervical and Uterine Procedures”.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not treat an over-the-counter label as a personal clearance. Ask your clinician whether an NSAID is appropriate with your history, allergies, kidney or gastrointestinal conditions, bleeding risks and current medicines. Ask which medicine, dose and timing they recommend, or whether a different plan is safer. The purpose of this conversation is to avoid relying on an NSAID as the sole strategy for insertion pain when another option may better match your needs.

Why misoprostol is not a routine answer

Misoprostol is a cervical-ripening medicine, but the CDC does not recommend it for routine IUD placement. The CDC evidence summary reports more pain, preplacement cramping and diarrhea in studies of routine use. A study suggests it might improve success when a second insertion is attempted after a recent failed placement, which is a different situation from using it for every patient. The 2024 CDC recommendations are available at this guidance page.

ACOG characterizes evidence for cervical-ripening agents as conflicting and also describes adverse effects. If you previously had an unsuccessful insertion, ask whether the reason for that attempt changes the balance of benefits and harms. Do not start, borrow or request misoprostol as a routine pre-appointment medicine without a clinician’s plan.

Make the appointment trauma-informed and controllable

Past trauma, depression, anxiety, pelvic pain and previous difficult examinations can influence how pain is experienced. You do not have to disclose details you do not want to share. You can say, for example, “I need you to explain each step,” “Please ask before touching me,” or “I may need a pause.” ACOG recommends individualized, trauma-informed shared decision-making; the CDC also recognizes that pain is personal and influenced by prior experiences.

Agree on a pause plan

  • Choose a clear hand signal or word that means stop immediately, and a different signal for “slow down” if that is useful.
  • Ask the clinician to explain the next step before it happens and to tell you when an instrument is being inserted or removed.
  • Ask whether you can control the pace, keep a support person present where permitted, or stop and reschedule if the examination becomes intolerable.
  • Tell the clinician about pelvic pain, vaginismus, prior traumatic care or a previous failed attempt if you want those factors considered; disclosure is your choice, but the information can change planning.

These requests are part of medical care, not an inconvenience. They give the clinician information needed to tailor positioning, communication and the choice of analgesia or anesthesia.

Sedation and anesthesia: what to clarify

ACOG says comprehensive counseling should include intravenous sedation, monitored anesthesia or general anesthesia when and where available. These are setting-dependent services, not guaranteed add-ons to an ordinary office visit. Ask whether your clinic performs IUD placement under any of them or refers patients elsewhere.

Before choosing one, clarify who administers it, what monitoring is used, whether you must fast, how long recovery takes, whether someone must drive you home and whether the IUD placement itself is otherwise unchanged. Tell the clinician about all medicines, allergies, sleep or breathing conditions and previous anesthesia problems so suitability can be assessed. A referral or separate appointment may be necessary, so ask early rather than on the day of insertion.

A practical planning timeline

When scheduling

  • State that you want to discuss pain relief and ask which options the clinic provides.
  • Ask whether a block, topical anesthetic, sedation or anesthesia requires advance booking, a prescription, a referral or a different location.
  • Request an estimate of any patient costs and ask what happens if the planned option is unavailable that day.

At the consultation

  • Review your health history, allergies, current medicines, prior insertion attempts and any pelvic pain, anxiety or trauma concerns you want considered.
  • Choose a primary plan and a backup plan, such as a local anesthetic plus agreed pause signals, or rescheduling for a setting with sedation if that is clinically appropriate.
  • Confirm what to expect during the procedure, what may help afterward and whom to contact with questions.

On the day

  • Remind the team of the agreed signal, preferred pace and explanation style before the examination begins.
  • Confirm which medicine or anesthetic, if any, is being used and when it will be given.
  • Tell the clinician promptly if the experience differs from the plan or if you want to pause.

What a patient-centered decision looks like

A reasonable plan may be no medication, a local anesthetic, an aftercare medicine, a selected cervical-preparation strategy after a failed attempt, sedation or a combination of communication measures and medication. The right choice depends on what your clinic can provide and what is medically suitable for you. The goal is not to promise a painless insertion; it is to ensure you understand the options, trade-offs, timing and control you have before consenting.
