Indigenous health and wellness is a holistic, relational view of well-being that can include spiritual, environmental, cultural, social, physical and mental dimensions. Traditional healing and Western clinical care do not have to be kept in separate worlds. They can be coordinated when the person chooses, practitioners discuss risks and interactions honestly, confidentiality is protected, and referrals are made when urgent or specialized care is needed.
The practical rules are not universal. Indigenous Peoples, nations and communities have distinct teachings and protocols; laws governing healers, medicines, records and consent vary by country, state, province and Indigenous nation. Use this guide to frame a respectful conversation, then confirm local requirements with an Indigenous health organization or health authority.
What does Indigenous health and wellness mean?
The World Health Organization (WHO) describes Indigenous concepts of health as more than the absence of disease or a set of biomedical measurements. Well-being can be individual and collective, grounded in relationships among people, community, spirituality, culture, land, biodiversity and the wider environment. Physical and mental health remain important, but they are connected to social, cultural, environmental and spiritual conditions.
WHO summarizes this perspective by saying that “Indigenous Peoples’ concept of health and well-being is holistic and encompasses spiritual, environmental, cultural and social dimensions in addition to physical and mental health.” It also describes health as an equilibrium involving spirituality, traditional medicine, biodiversity and “the interconnectedness of all that exists.” These statements describe a framework, not a single worldwide set of ceremonies or remedies.

Why the word “Indigenous” does not mean one uniform system
More than one Indigenous nation may live within the same country, and neighboring communities can have different languages, healers, protocols, medicines and rules about who may receive particular knowledge. Some practices are public; others are restricted to specific families, ceremonial roles, seasons or places. A respectful clinician therefore asks what a practice means to the individual and community instead of assuming that a label identifies its ingredients or purpose.
Health can be collective as well as personal
An appointment usually concerns one patient’s symptoms, but housing, clean water, food systems, language, family relationships, cultural continuity and connection to land can affect the health of an entire community. A service that protects only an individual’s privacy while collecting or publishing community knowledge without permission can still cause harm. Good care addresses the person’s immediate needs while respecting collective rights and responsibilities.
What rights protect traditional medicines and health practices?
Article 24 of the United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP), adopted in 2007, recognizes that Indigenous Peoples have the right to their traditional medicines and to maintain their health practices. It also recognizes Indigenous individuals’ right to access social and health services without discrimination and to enjoy the highest attainable standard of physical and mental health.

Article 24 is a rights and governance standard, not a guarantee that every remedy is effective, risk-free or legally available in every location. A country’s implementation may appear in constitutional rules, statutes, regulations, agreements or health-service policies. For example, a Canadian statute reproduces UNDRIP, but the legal effect of a declaration or implementing law still depends on the jurisdiction and the specific issue. Check the current law where care is provided before making a claim about licensing, protected practice or access.
Rights do not remove clinical safety duties
Recognition of traditional medicine supports respectful access and nondiscrimination. It does not authorize a practitioner to conceal a product, ignore a dangerous interaction, or delay emergency treatment. Patients retain the right to understandable information, voluntary choices and appropriate clinical monitoring, while practitioners retain duties set by local law and professional standards.
Can traditional healing and Western medicine be used together?
Yes, if the person wants coordinated care and the professionals can communicate safely. Combining approaches is not automatically safe, and “traditional” and “Western” are too broad to predict an interaction. A specific plant preparation, ceremony, manual technique, diet, spiritual practice or prescribed drug must be considered on its own terms.

Questions to resolve before combining care
- What is the exact practice or product, including preparation, ingredients, route of use and intended purpose?
- Who provides it, and what training, community authorization or professional registration do they hold?
- What dose, frequency and timing are planned, and for how long?
- Could it affect a prescribed medicine, anesthesia, pregnancy, surgery, kidney or liver function, allergy, bleeding risk or another condition?
- What monitoring will show that it is helping or causing harm, and who should be contacted if symptoms change?
- When should the person be referred to emergency, specialist or diagnostic services?
Comparison without ranking one system
| Question | Traditional healing | Western clinical care | What coordination requires |
|---|---|---|---|
| Goal and concept of health | May emphasize balance, relationships, spirituality, culture, land and collective well-being; the meaning differs by community. | Usually organizes care around diagnosis, measurable symptoms, prevention and treatment based on biomedical models. | Let the person define goals and identify where approaches complement or conflict. |
| Knowledge and authority | May be held by Elders, healers, families, cultural knowledge keepers or community institutions under specific protocols. | Usually held by licensed or otherwise regulated professionals and clinical institutions. | Obtain permission before sharing knowledge and clarify each practitioner’s role. |
| Evidence and uncertainty | Knowledge may be transmitted through practice, story, observation and community experience; formal studies may be limited or not appropriate for sacred practices. | Uses clinical research, guidelines, diagnostic tests and professional standards, with uncertainty still present. | State what is known, unknown and being monitored instead of treating either system as universally superior. |
| Consent and privacy | May involve family, Elders or community protocols in addition to the individual’s wishes. | Requires the patient’s clinical informed consent and confidentiality under applicable law. | Ask whom the patient wants involved and obtain separate permission for disclosures. |
| Safety monitoring | Depends on the practice, preparation and healer’s protocol. | Uses histories, examinations, tests, medication review and adverse-event systems. | Share enough information to check interactions and agree on follow-up. |
| Regulation | Recognition and legal status vary widely by jurisdiction and by practice. | Licensing and institutional rules are generally defined in local law, though scopes still differ. | Verify credentials and referral obligations locally. |
| Access and cost | May depend on community availability, travel, cultural safety and whether payment or coverage is required. | May depend on insurance, public eligibility, waiting lists, distance and out-of-pocket fees. | Discuss practical barriers without pressuring the person to abandon a valued source of support. |
Communication is the safety intervention
Tell each practitioner about the other care, using the level of detail the patient authorizes. A clinician should listen without dismissing or romanticizing the practice, document clinically relevant information, protect confidentiality and arrange referrals when needed. A healer or cultural practitioner should be able to say when a concern is outside their role and encourage timely clinical assessment without presenting referral as a rejection of tradition.

There is no responsible universal answer that all traditional practices are safe or that all are unsafe. Risk depends on the exact intervention, preparation, dose, practitioner, condition being treated, concurrent medicines and the person’s age, pregnancy status, allergies and organ function. Product contamination, misidentification, undisclosed ingredients, excessive dosing and delayed diagnosis are possible hazards in any setting, while a respectful cultural practice may provide meaning, support and connection without being a substitute for urgent treatment.
A practical safety review
- Name the intervention precisely. Record the local and common name, ingredients if known, preparation method, route, dose and schedule. Do not pressure someone to disclose sacred or restricted knowledge; share only what is necessary for safe care.
- Identify the provider. Ask how the person learned the practice, whether the community recognizes their role, what training they have and how they handle complications or referrals.
- Check the health context. Review diagnoses, allergies, pregnancy or breastfeeding, planned surgery, kidney or liver disease, bleeding disorders and every prescription, over-the-counter medicine and supplement.
- Agree on monitoring. Set a time to reassess symptoms, define what improvement would look like and decide who will receive reports of side effects.
- Use extra caution with products of uncertain contents. Do not assume a product is tested, standardized or regulated merely because it is labeled natural or traditional.
When to obtain urgent clinical help
Seek local emergency care immediately for signs of a life-threatening reaction, such as trouble breathing, swelling of the face or throat, severe chest pain, loss of consciousness, uncontrolled bleeding, sudden severe confusion or rapidly worsening symptoms. Do not wait for a scheduled healer or clinic when delaying care could cause harm. For non-emergency concerns, contact a qualified health professional promptly and bring the product container or a written description when possible.

What is informed consent in Indigenous health care?
Clinical informed consent is an individual’s authorization for a proposed examination, medicine, procedure or other intervention after receiving information they can understand. The discussion normally covers the purpose, expected benefits, material risks, alternatives, uncertainties and the option to decline or withdraw, subject to local law and professional standards. Consent is a process, not merely a signature.
Free, prior and informed consent (FPIC) is a collective rights principle. It concerns decisions that may affect Indigenous Peoples’ rights, knowledge, lands, territories, resources or self-determination. “Free” means without coercion or manipulation; “prior” means before the activity or decision; “informed” means adequate, accessible information about the proposal, risks, alternatives and consequences; and consent must be connected to the affected Peoples’ own decision-making processes. WHO’s framework links respectful engagement, self-determination and FPIC.
| Feature | Clinical informed consent | FPIC |
|---|---|---|
| Who decides | The individual patient, or an authorized substitute under local law. | The affected Indigenous People or community through its legitimate decision-making institutions, while individuals still make their own clinical choices. |
| What it covers | A particular treatment, test, disclosure or care plan. | Activities affecting collective rights, knowledge, lands, territories, resources or governance. |
| Information needed | Benefits, risks, alternatives, uncertainties and the right to refuse or withdraw. | Accessible information about the proposal, effects, alternatives, timing and decision process. |
| Relationship | Required for the patient’s treatment even when a community has approved a broader project. | Does not replace the patient’s individual consent for an examination or treatment. |
How should a patient talk with a doctor about traditional healing?
You can disclose the practice as part of your medical history. A direct, non-confrontational script is: “I am using [practice or product] with [practitioner]. Can we review possible interactions, timing, side effects and which symptoms should prompt urgent care?” The bracketed terms are examples to replace with your own words; keep any variable text inside the conversation, not in a medical record template.

- Choose the right appointment. Ask for enough time for a medication and treatment review, especially before surgery, pregnancy care or starting a new prescription.
- Bring accurate information. Take packaging, photographs of labels, written instructions, the healer’s contact details if you consent to sharing them, and a list of all medicines and supplements.
- State your goals and boundaries. Explain what you hope the practice will help with and whether there are cultural, family or confidentiality protocols the clinician should respect.
- Ask specific safety questions. Request discussion of interactions, contraindications, timing around medicines or procedures, expected effects, warning symptoms and a follow-up plan.
- Ask for culturally safe support. You may request an interpreter, Indigenous patient navigator, family member, Elder or other support person, subject to your privacy choices.
- Confirm the plan. Use teach-back: explain in your own words what you will take or do, what you will avoid and whom you will contact if something changes.
A clinician should not stereotype, ridicule or threaten a patient for disclosing traditional care. At the same time, respectful listening does not require endorsing an intervention whose risks have not been assessed. If trust breaks down, ask for another clinician, a patient advocate or an Indigenous health service where available.
Who is allowed to provide traditional healing?
There is no single global rule. Some jurisdictions recognize particular Indigenous healers, medicines or ceremonies through Indigenous governance, agreements or professional regulations; others have limited or no formal licensing category. Rules can differ between a community-based ceremony, a product sold to the public, a person offering diagnosis for payment and a practitioner working inside a hospital. Privacy, record-keeping, advertising, scope of practice and mandatory referral duties may also differ.
How to verify a provider without disrespecting community authority
- Ask the community or Indigenous health organization which forms of recognition are meaningful for that practice.
- Check any applicable public register, facility credentialing rule or health-authority requirement in the place where care occurs.
- Ask how the provider protects confidential teachings, obtains permission for photographs or recording and handles information about other family or community members.
- Clarify fees, cancellation terms, supervision, emergency arrangements and when the provider refers to a nurse, physician, pharmacist or other specialist.
- Be cautious of anyone claiming exclusive authority over an entire Indigenous culture, promising a guaranteed cure, discouraging all medical care or demanding disclosure of restricted knowledge.
Local law should decide legal questions about who may practice. WHO’s 2026 participatory framework is identified on its page as an unedited final draft, while its Traditional Medicine Global Summit report is dated 2023; neither document creates one worldwide licensing system.

Respectful care is more than adding a traditional symbol to a clinic. It requires shared responsibility, accountability and safeguards for Indigenous knowledge and data. A service provider should:
- Ask which language, family members, Elders, healers or community supports the person wants involved, and record those preferences with permission.
- Explain options, uncertainty, costs, waiting times, alternatives and referral reasons in plain language.
- Obtain consent without pressure and make it easy to pause, decline or change the plan.
- Keep clinical records confidential and obtain specific permission before sharing information with a healer, family member, researcher or community body.
- Protect sacred, restricted or collectively held knowledge from unauthorized collection, publication or commercial use.
- Use Indigenous data-sovereignty principles, shared governance and community accountability when collecting or analyzing information.
- Build reciprocal relationships with Indigenous health organizations rather than treating consultation as a one-time formality.
How to make a personal care plan
Start with the person’s goals, cultural preferences and immediate safety needs. Write down every intervention and provider, authorize only the information sharing you want, identify interaction checks and agree on follow-up dates. Keep emergency instructions accessible and revisit the plan whenever a medicine, diagnosis, pregnancy status, surgery date or healer changes. This approach allows traditional and Western care to coexist without pretending that they have identical evidence, authority or risks.
This article provides general educational information, not individualized medical or legal advice. For a country- or nation-specific answer about practitioner recognition, product regulation, consent law or Indigenous governance, contact the relevant health authority and an Indigenous health organization in that jurisdiction.
