An incident war room’s approval gate is only meaningful when it is clear what the gate controls. Landfall documents a gate that stages AI-responder findings before they enter shared incident context; it does not document an execution gate that pauses AI tool calls. Its documentation says agents can suggest remediation, while a human applies changes using external tools. That distinction matters: controlling what the room accepts is not the same as controlling what an AI can do.

What the incident war room is designed to do

Landfall describes a shared operational space with an event-sourced timeline for people and AI agents. Findings, charts, logs, and decisions appear in a common context so responders can coordinate around the same incident record. The room is therefore both a collaboration surface and a record of how the response developed. Landfall’s product documentation

The central safety question is not simply whether AI is present. It is whether an AI-generated claim becomes accepted incident context, whether an AI can execute a consequential action, and what evidence remains when responders decide what to trust.

What Landfall’s approval gate controls

Landfall describes responder-harness findings and hypotheses as staged rather than immediately shared with other agents. A plain observation is rechecked against its sources; higher-impact items—such as a proposed cause, correlation, or directive—require a person before admission to the room’s shared context. This is an admission gate: it controls which findings other participants and agents treat as accepted context. Landfall’s product documentation

The same documentation says that a human override takes precedence. If shared context is later found to be wrong, it is hidden from agents and the reason is recorded rather than simply deleting the item. These are documented product behaviors, not evidence that every integration or customer configuration has been independently tested against adversarial input.

Landfall’s concepts-page search listing describes three gate modes—on, shadow, and off—with on as the default, and says an organization administrator can admit a staged item directly with a recorded reason. Those details are reported in the official documentation search result; the concepts page itself was not directly available for confirmation. Landfall documentation

Why context admission is not tool approval

A finding can be prevented from entering a shared room while an agent may still have separate access to tools or integrations. Conversely, a tool call can be held for approval even if the surrounding analysis is already visible. These are different control points and should not be described as one general-purpose gate.

Control point What is held When review happens Documented example
Shared-context admission A finding or hypothesis that would become accepted room context Before other participants or agents receive it as admitted context Landfall stages responder findings for review. Landfall
Tool-execution approval A specific sensitive tool call and its arguments Before the tool call executes OpenAI’s Agents SDK documents a human-in-the-loop approval flow. OpenAI Agents SDK: Human in the loop
Remediation change The operational change itself Outside the room, when a human uses an external tool Landfall says agents suggest remediation and a human applies changes; “Landfall never approves or runs a change.” Landfall product documentation

The last row is an important boundary: the room may help responders evaluate a proposal, but the documented product is not itself the change-approval or change-execution system.

What a real execution-level approval gate needs

OpenAI’s Agents SDK documentation provides a concrete implementation pattern, not evidence that Landfall uses that SDK. A tool can require approval unconditionally or according to a per-call rule. When approval is required and no decision is stored, the agent run pauses and presents a pending approval interruption; a person approves or rejects the call before the run resumes. OpenAI Agents SDK: Human in the loop

The Python guide also says callable approval rules fail closed to manual review when arguments are absent, malformed, not a JSON object, or contain nonstandard constants. For a sensitive action, the practical design requirements are:

  • Enforce the check at the tool-call boundary, before execution—not merely in the chat or incident record.
  • Show the reviewer the specific requested action and its arguments.
  • Associate the approval or rejection with that call, and pause the run until the decision is made.
  • Route ambiguous or uninspectable arguments to manual review rather than treating them as approved.

Approval is one control, not a complete security guarantee. The surrounding system still needs defined permissions, logging, containment choices, and a clear account of who can authorize actions.

How to make the gate meaningful during an incident

Define authority before an alert arrives

OWASP’s GenAI Incident Response Guide recommends preparing secure communication channels, escalation paths, roles, notification procedures, and a physical or virtual war room for major incidents. It is guidance, not a binding standard or regulator requirement. OWASP GenAI Incident Response Guide

AWS Customer Incident Response Team guidance, in a presentation hosted by NIST, recommends setting approval authority by business-impact thresholds rather than technical severity alone. It also recommends mapping AI systems to business functions, identifying non-human identities, assigning a human owner, and describing blast radius in business terms. The presentation is AWS guidance, not a NIST standard. AWS Customer Incident Response Team presentation hosted by NIST

Keep a useful incident record

OWASP’s reporting template includes the incident ID and title, detection time, description, affected AI system or model, impact, incident type or cause, detection method, actions taken, stakeholders notified, response team, next steps, status, and timeline. For an AI-related incident, its guide also calls attention to model integrity, data trustworthiness, unusual behavior, system criticality, functional impact, and data integrity or confidentiality. OWASP GenAI Incident Response Guide

AWS’s presentation identifies AI-specific evidence responders may need, including prompt and completion logs, model-version history, training-data access, embedding queries, agent decision traces, and drift metrics. Agree in advance on what can be collected, who can access it, and how it informs containment; a record that cannot be retrieved during an incident will not help a reviewer assess a proposed action. Pre-plan containment and rollback

Before a live event, identify containment choices and their consequences: which business functions and dependent services each option affects, what fallback or rollback exists, and who has authority at each impact threshold. AWS recommends prebuilt containment choices that account for business cost and approval authority. AWS Customer Incident Response Team presentation hosted by NIST

What “can’t be faked out” can and cannot mean

A staged finding, source recheck, human admission, recorded override, and separate human-controlled change process describe useful boundaries. They do not establish that the AI cannot bypass every safeguard, that no integration can take an action independently, or that the gate has passed an adversarial security test. The available product documentation describes its workflow; no independent penetration test or adversarial evaluation of Landfall’s gate is established here.

When assessing any incident-response system, ask which boundary is enforced in software, which role can override it, whether an action can occur before review, and what record remains afterward. A confident label such as “approval gate” is less informative than answers to those operational questions.
