Immunotherapy and chemotherapy are not interchangeable treatments for bowel cancer. Chemotherapy uses anti-cancer drugs that circulate in the bloodstream and is used in several settings, including around surgery for some colon cancers and for advanced disease. Immunotherapy, usually a checkpoint inhibitor, is mainly an option for particular colorectal cancers whose tests show high microsatellite instability (MSI-H) or mismatch repair deficiency (dMMR), especially when disease is advanced or metastatic.

The choice depends on whether the cancer is in the colon or rectum, its stage and resectability, MSI/MMR and other molecular results, the aim and speed of treatment, expected benefits and uncertainties, side-effect monitoring, and practical issues such as infusions, tablets, travel and appointments. NICE recommendations discussed here describe NHS England practice and should not be applied automatically in other countries.

The short answer

For localized bowel cancer, surgery is often central and chemotherapy may be used before surgery to shrink a tumour or afterward to reduce the chance of recurrence, depending on the site and stage. For unresectable or metastatic colorectal cancer, systemic treatment is selected according to molecular tests and treatment goals. In adults in England with untreated unresectable or metastatic colorectal cancer that is MSI-H or dMMR, NICE identifies pembrolizumab and, in specified circumstances, nivolumab with ipilimumab as options; chemotherapy remains an option and may be chosen when test results are delayed or a faster response is needed.

There is no universal ranking in which one treatment is always better. Immunotherapy eligibility is strongly biomarker-dependent, while chemotherapy can be used across a broader range of bowel-cancer situations. Your oncology team must match the treatment to your tumour, tests and goals.

Question Chemotherapy Immunotherapy
How it works Anti-cancer cytotoxic drugs circulate through the bloodstream and destroy cancer cells. Checkpoint inhibitors help the immune system kill cancer cells.
Examples used in bowel cancer 5-fluorouracil (5-FU), capecitabine, irinotecan, oxaliplatin and trifluridine-tipiracil; regimens include FOLFOX, FOLFIRI, FOLFOXIRI and CAPOX. Nivolumab and pembrolizumab; nivolumab can be used alone or with ipilimumab.
Administration Injection, intravenous drip or tablets, depending on the regimen. Infusion into a vein.
Key eligibility factor Stage, tumour site, treatment setting and clinical plan. MSI-H or dMMR status is central to the pathway described by NICE, particularly in advanced or metastatic disease.

What chemotherapy does in bowel cancer

How the drugs work

Chemotherapy uses anti-cancer, or cytotoxic, drugs. They travel through the bloodstream, so treatment can reach cancer cells in the bowel and cells elsewhere in the body. The specific drug combination is selected for the cancer’s location, stage, previous treatment and the purpose of treatment.

Because chemotherapy is a category rather than one medicine, two people with bowel cancer may receive very different drugs or schedules. A plan can be adjusted to the operation being considered, whether disease is confined to the bowel, and whether the aim is long-term control or reducing the chance that cancer returns. The fact that a drug is given as a tablet rather than through a drip does not make it a different type of cancer treatment; capecitabine, for example, is an oral chemotherapy drug used in some combinations.

Common drugs and combinations

Bowel Cancer UK lists 5-fluorouracil (5-FU), capecitabine, irinotecan, oxaliplatin and trifluridine-tipiracil among commonly used drugs. These may be combined in named regimens:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • FOLFOX: a regimen containing 5-FU and oxaliplatin.
  • FOLFIRI: a regimen containing 5-FU and irinotecan.
  • FOLFOXIRI: a combination of 5-FU, oxaliplatin and irinotecan.
  • CAPOX: a regimen combining capecitabine and oxaliplatin.

Depending on the regimen, chemotherapy can be delivered by injection, through an intravenous drip or as tablets. The route does not by itself indicate how effective treatment will be; it reflects the drugs and schedule chosen by the clinical team. Ask whether tablets must be taken on particular days, whether an infusion is required, and how blood tests and appointments fit around each cycle.

When chemotherapy is used

For some colon cancers, chemotherapy is given before surgery to shrink a tumour or after surgery to reduce the chance of recurrence. The role differs between colon and rectal cancer and depends on stage. In advanced disease, chemotherapy can be used as systemic treatment to control cancer that cannot be removed surgically or has spread.

Chemotherapy may also be the practical first step while a molecular result is pending. NICE discusses FOLFOX, FOLFIRI, CAPOX and capecitabine among first-line chemotherapy options for unresectable or metastatic dMMR/MSI-H colorectal cancer when genomic information is not yet available or when the team prefers a quicker response. That decision is about timing and clinical circumstances, not proof that immunotherapy could never be used later.

What immunotherapy does

Checkpoint inhibitors

Checkpoint inhibitors are drugs that help the immune system recognise and kill cancer cells. In bowel cancer, the immunotherapies identified by Bowel Cancer UK include the checkpoint inhibitors nivolumab and pembrolizumab. They are given by infusion into a vein. Nivolumab may be used alone or combined with ipilimumab.

Unlike chemotherapy, immunotherapy is not selected simply because a tumour is present. The tumour’s molecular features are essential to deciding whether this approach is appropriate. A person can have bowel cancer and still not have the biomarker profile for which immunotherapy is recommended. The drug, combination and treatment line also depend on the clinical setting and current local guidance.

The setting covered by current NICE recommendations

NICE’s technology appraisal TA1065, published on 28 May 2025, addresses untreated unresectable or metastatic colorectal cancer in adults with high microsatellite instability or mismatch repair deficiency. In that defined population, NICE recommends nivolumab with ipilimumab as an option in England, subject to the commercial arrangement described in the recommendation. NICE also describes pembrolizumab or chemotherapy as usual treatment for untreated unresectable or metastatic colorectal cancer with MSI-H or dMMR.

The appraisal’s wording is: “Usual treatment for untreated unresectable or metastatic colorectal cancer with high microsatellite instability or mismatch repair deficiency in adults is pembrolizumab or chemotherapy.” This describes a particular population and does not mean that every person with bowel cancer should receive one of those choices.

These recommendations are specific to the population, treatment line and health system described. They do not establish that immunotherapy is suitable for every stage of bowel cancer, every molecular subtype or every country. A recommendation for an untreated metastatic cancer cannot automatically be transferred to a tumour that has already received several treatments or to disease that can be removed surgically.

Why MSI, MMR and other test results matter

MSI-H and dMMR

Microsatellite instability (MSI) describes changes detected in repetitive DNA sequences. “High” MSI is written MSI-H. Mismatch repair (MMR) is a DNA-repair system; when it is deficient, the result is called dMMR. NICE’s colorectal-cancer guidance makes MSI-H or dMMR central to the immunotherapy pathway for advanced or metastatic disease.

NICE refers to offering colorectal-cancer patients testing for dMMR or MSI at diagnosis. Testing can use immunohistochemistry (IHC), which examines MMR proteins in tumour tissue, or polymerase chain reaction (PCR), which assesses microsatellite changes. Ask whether this test was done on the biopsy or surgical specimen and whether the report says proficient or deficient MMR, or stable or high MSI. If the wording is confusing, ask the pathology or oncology team to explain whether the result supports an immunotherapy discussion.

MSI/MMR is not a prediction that treatment will work in every individual, and a test result is interpreted alongside stage, symptoms, organ function and the proposed treatment goal. If the sample is insufficient or the result is still pending, ask whether another specimen can be tested and how that delay affects the immediate plan.

RAS and BRAF V600E

NICE recommends RAS and BRAF V600E testing for people with metastatic colorectal cancer who are suitable for systemic treatment. These results may affect the wider systemic-treatment plan, so they should be discussed alongside MSI/MMR rather than treated as a replacement for it. Ask whether the report includes these results and whether they change the choice or sequence of medicines being considered.

How clinicians decide between the options

1. Confirm the cancer site and stage

Colon and rectal cancers can have different treatment sequences. The team considers whether the tumour is localized, locally advanced, unresectable or metastatic, and whether surgery is planned, possible or no longer the main approach. A treatment that is sensible before an operation is not automatically the treatment used for metastatic disease.

“Unresectable” means the team does not consider the cancer removable with surgery at that point; “metastatic” means it has spread to distant sites. Those descriptions influence whether treatment is intended to support an operation or control disease throughout the body. Confirm the exact wording in your records rather than relying on a general label such as “advanced.”

2. Define the treatment goal

The immediate goal may be to shrink a tumour before surgery, reduce the risk of recurrence after surgery, control unresectable or metastatic disease, or achieve a response quickly enough to relieve a threat from the cancer. The goal affects which treatment and schedule are considered. Do not assume that a regimen used for one goal has the same evidence or role for another.

Ask whether the proposed treatment is intended to be a bridge to surgery, an additional treatment after surgery, or ongoing disease control. Clarify how the team will judge whether it is working and what the next decision point will be. The sources available here do not provide one universal response time or a single schedule for all regimens.

3. Check the molecular results

MSI/MMR status is particularly important when immunotherapy is being considered. RAS and BRAF V600E results can further inform systemic-treatment choices in metastatic disease. If the report is not available, ask which tests were ordered, whether the sample was adequate and when the result is expected.

4. Consider timing and the need for a rapid response

NICE notes that options for unresectable or metastatic dMMR/MSI-H colorectal cancer depend partly on when genomic results become available. Chemotherapy may be used first if MMR testing is delayed or if the clinical team believes a faster response is preferred. Starting chemotherapy in that situation does not mean immunotherapy has been ruled out forever; the plan depends on the eventual results and the person’s condition.

Timing can also be affected by symptoms, the risk posed by the tumour, fitness for treatment and the logistics of arranging an infusion or obtaining tablets. Ask what cannot safely wait for a test result and whether the team would revisit the plan when the report arrives.

5. Balance benefit, uncertainty and practical burden

NICE’s appraisal describes clinical benefit for nivolumab plus ipilimumab in its defined population, but the cited recommendation does not provide a numerical outcome figure. The available material does not establish a complete side-by-side comparison of survival, response rates or toxicity across all bowel-cancer regimens. Decisions therefore require an individual discussion of expected benefit, uncertainty, likely effects, monitoring and logistics.

A practical testing and treatment pathway

  1. Ask for the pathology summary. Confirm whether the diagnosis is colon or rectal cancer and what stage and resectability have been recorded.
  2. Check MSI/MMR testing. Ask whether IHC or PCR was used and whether the result is dMMR or MSI-H, or whether the tumour is MMR-proficient or microsatellite stable.
  3. For metastatic disease, ask about RAS and BRAF V600E. NICE recommends these tests when systemic treatment is suitable.
  4. Clarify the treatment objective. Ask whether treatment is intended before surgery, after surgery, to control advanced disease or to obtain a rapid response.
  5. Review the available regimens. If chemotherapy is proposed, ask which regimen—such as FOLFOX, FOLFIRI, FOLFOXIRI or CAPOX—is planned and whether treatment will be by injection, drip or tablets.
  6. If immunotherapy is proposed, identify the drug and combination. Ask whether the plan is pembrolizumab, nivolumab alone or nivolumab with ipilimumab, and why that choice fits the test results.
  7. Make a monitoring and contact plan. Before treatment starts, ask which symptoms require an urgent call, which tests are needed during treatment and whom to contact outside appointment hours.

Take a written copy of the pathology and molecular reports to appointments if possible. Record the exact medicine names, the proposed sequence and the reason for any change. If a result is delayed, ask whether treatment is being started on the basis of symptoms or urgency and when the decision will be reviewed.

The practical burden can include infusion or clinic time, tablet schedules, blood tests, travel and the need to report symptoms between appointments. The exact schedule and monitoring requirements depend on the chosen drugs and your overall plan. Cancer Research UK advises considering these practical factors with the clinical team.

Ask how often you will attend, whether a cannula or other access is needed, how prescriptions are collected, and whether someone should accompany you. Discuss work, caring responsibilities, transport and accommodation early. These considerations do not determine eligibility, but they can affect which safe and feasible plan is most workable.

Side effects, warning signs and monitoring

Chemotherapy and immunotherapy have different mechanisms, so their potential adverse effects and monitoring are not identical. However, the sources used here do not provide a complete direct toxicity comparison for all bowel-cancer regimens. It would be unsafe to infer that one class is simply “milder” or “safer” for everyone.

Before consenting, ask for the specific side-effect information for the named drugs, how often monitoring occurs, which symptoms are urgent and what support is available. Ask whether the schedule involves a drip, tablets or both, how long appointments usually take, and how treatment interruptions or dose changes would be handled if problems occur.

Keep the treatment unit’s telephone number available and report new or worsening symptoms according to the instructions you receive. Do not wait for the next routine appointment if the team has told you a symptom needs same-day advice. Monitoring may include appointments and tests tailored to the medicines, but the exact tests and frequency are regimen-specific.

What the NICE guidance does—and does not—tell you

NICE recommendations apply to NHS England. The 2025 appraisal supports nivolumab plus ipilimumab for the specified untreated, unresectable or metastatic MSI-H/dMMR population, subject to the arrangement stated in the recommendation. It does not create a universal rule for people with earlier-stage cancer, MMR-proficient or microsatellite-stable tumours, a different treatment line or a different health system.

Questions to take to your oncology appointment

  • Is my cancer in the colon or rectum, and is it localized, locally advanced, unresectable or metastatic?
  • What is the treatment goal: shrinkage before surgery, reducing recurrence risk, disease control or a rapid response?
  • Have my tumour’s MSI/MMR results been reported? What do the terms MSI-H, dMMR, MMR-proficient or microsatellite stable mean in my case?
  • If my cancer is metastatic, were RAS and BRAF V600E tested?
  • Which exact chemotherapy regimen or immunotherapy drug are you recommending, and why?
  • Would treatment be an infusion, injection, tablets or a combination?
  • How soon is a response needed, and would waiting for a molecular result change the plan?
  • What benefits are realistic for my stage and tumour biology, and what remains uncertain?
  • Which symptoms need an urgent call, what monitoring tests are required and who should I contact between visits?
  • How might appointments, travel, work, family responsibilities and treatment support affect the plan?
  • If the first plan does not work or causes problems, what alternatives could be considered?
  • When will the decision be reviewed, and what information will trigger a change in treatment?

Bottom line for patients and families

Chemotherapy is a broad treatment category used in several bowel-cancer settings, including around surgery and in advanced disease. Immunotherapy is a more biomarker-defined option: in the NICE pathway for advanced or metastatic colorectal cancer, MSI-H or dMMR status is central. Stage, tumour site, molecular results, treatment goal, need for speed, expected benefit, monitoring and practical burden all belong in the decision. Ask your oncology team to explain the test results and the reasoning behind the specific regimen rather than choosing between the two treatment names in isolation.

Frequently Asked Questions

What is the main difference between immunotherapy and chemotherapy for bowel cancer?

Chemotherapy uses cytotoxic drugs that circulate in the bloodstream and destroy cancer cells. Immunotherapy, such as a checkpoint inhibitor, helps the immune system recognise and kill cancer cells. Chemotherapy is used in more bowel-cancer settings, while immunotherapy eligibility is strongly linked to tumour biomarkers, especially MSI-H or dMMR in advanced or metastatic colorectal cancer.

Which bowel cancers may be treated with immunotherapy?

The NICE pathway discussed here concerns adults with untreated, unresectable or metastatic colorectal cancer whose tumours are MSI-H or dMMR. Pembrolizumab and, in specified circumstances, nivolumab with ipilimumab are options in England. A person with bowel cancer who does not have that biomarker profile may not be eligible under this pathway.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What happens if my MSI or MMR result is delayed?

NICE notes that chemotherapy may be used first when MMR testing is delayed or when a faster response is preferred. Regimens discussed include FOLFOX, FOLFIRI, CAPOX and capecitabine. Ask when the result is expected and when the treatment plan will be reviewed; starting chemotherapy does not by itself prove that immunotherapy can never be considered.

What tests should I ask for?

Ask whether MSI or MMR testing was done at diagnosis, using immunohistochemistry or polymerase chain reaction, and whether the report says dMMR, MSI-H, MMR-proficient or microsatellite stable. For metastatic colorectal cancer suitable for systemic treatment, NICE recommends RAS and BRAF V600E testing as well.

Is immunotherapy safer or milder than chemotherapy?

The treatments work differently and have different monitoring needs, but the cited material does not provide a complete direct toxicity comparison across all regimens. It is not reliable to label one class safer for everyone. Ask about the specific medicines, likely effects, warning symptoms, monitoring and how problems would be managed.

How are bowel-cancer chemotherapy and immunotherapy given?

Depending on the chemotherapy regimen, treatment may be an injection, an intravenous drip or tablets. The immunotherapies identified here—nivolumab and pembrolizumab, with ipilimumab in a specified combination—are given by infusion into a vein. Your team can explain the schedule and appointment length for the named drugs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Does NICE guidance apply outside England?

NICE recommendations describe NHS England practice. Funding, availability and preferred regimens can differ in other countries or health systems. Ask your local oncology team which guidance applies where you are treated and whether the relevant medicines are funded for your stage and test results.

What should I ask before choosing a treatment?

Ask about the exact cancer site and stage, the treatment goal, MSI/MMR and metastatic RAS or BRAF V600E results, the proposed drug and route, how quickly a response is needed, expected benefits and uncertainties, monitoring, urgent symptoms, travel and appointment demands, and when the plan will be reassessed.

The Bottom Line

Chemotherapy is broadly used across bowel-cancer settings, while immunotherapy is mainly a biomarker-guided option for specific advanced or metastatic colorectal cancers. The right choice depends on stage, site, MSI/MMR and other test results, treatment goal, urgency, expected benefit, monitoring and practical circumstances. Discuss the named regimen—not just the treatment category—with your oncology team.
