Engineering teams should review five areas highlighted by IBM’s 2026 breach reporting: identity for AI agents and other machine accounts, data discovery and monitoring, AI application and cloud exposure, encryption and cryptographic readiness, and the speed of remediation. These are practical review areas drawn from IBM’s recommendations—not a five-part checklist formally published by IBM.

What IBM’s 2026 report covers

IBM and the Ponemon Institute’s 2026 Cost of a Data Breach Report is based on breaches experienced by 602 organizations globally between March 2025 and February 2026. A separate follow-on study, conducted in May 2026, received responses from 456 organizations drawn from that group. Figures below are IBM/Ponemon study findings, not predictions of an individual organization’s breach odds or guarantees that a particular control will produce a specific result.

IBM reports a global average breach cost of $4.99 million, up 12% year over year, and a 56% increase in AI-driven attacks. Its report page also says extensive use of AI and automation in security was associated with average cost savings of $1.93 million compared with organizations using none. That is a reported comparison, not proof that AI use alone caused the savings.

Which controls should engineering teams review?

1. Agent and machine identity

Inventory AI agents and other non-human identities, then determine who owns each identity, what it can access, and whether its actions can be traced and audited. IBM recommends dynamic identity-based access controls and tight permissions at runtime; its July 29, 2026 release also urges organizations to secure identity at runtime.

#1 Best Overall

  • Simplified Management and Deployment: Features a user-friendly management console that provides comprehensive network automation and visibility, coupled with Zero Touch Integration with Fortinet’s Security Fabric for easy deployment.
  • Check that permissions are narrowly scoped to each agent’s actual task.
  • Confirm access is enforced continuously at runtime rather than granted indefinitely based on an initial approval.
  • Make each identity attributable to a responsible human owner, with auditable records of its activity.

2. Data discovery, classification, and monitoring

Establish where sensitive data resides, how it is classified, and which services and identities can reach it. IBM identifies these as data-security fundamentals and emphasizes data integrity as AI use expands.

  • Verify that data inventories and classifications reflect current systems, not only documented or legacy stores.
  • Review access paths for services, applications, and agents that handle sensitive information.
  • Check that access and threat monitoring has a clear response path when activity is suspicious or data integrity may be at risk.

3. AI application and cloud exposure

Review the APIs, applications, plug-ins, and cloud configurations surrounding AI workloads. IBM says more than 20% of organizations reported a breach targeting AI models or applications. Among the causes IBM lists for these AI-targeting breaches, compromised APIs, applications, or plug-ins accounted for 27%, and cloud misconfigurations affecting AI workloads accounted for 27%.

FortiGate-60F Network Security Appliance Plus 1 Year FortiGuard Unified Threat Protection (UTP) and FortiCare Premium (FG-60F-BDL-950-12)
  • HARDWARE PLUS SECURITY SERVICES: FortiGate-60F Firewall Appliance bundled with 1 year of FortiCare Premium and FortiGuard Unified Threat Protection.
  • UNIFIED THREAT PROTECTION (UTP): Secures against advanced online threats with comprehensive web filtering and anti-botnet technologies.
  • OPTIMIZED FOR MEDIUM-SIZED BUSINESSES: Tailored for businesses needing robust security without the infrastructure of larger enterprises.
  • RELIABLE CUSTOMER SUPPORT: FortiCare Premium ensures high-quality support and service continuity.
  • EFFECTIVE PROTECTION: Employs advanced filtering technologies to safeguard against sophisticated threats.

These percentages describe causes reported in IBM’s study; they are not estimates that any particular API or cloud environment has a 27% chance of being breached. For an engineering review, examine the interfaces through which models and applications receive data or instructions, the permissions of connected plug-ins, and the configuration of the cloud resources supporting AI workloads.

4. Encryption and cryptographic inventory

Check whether sensitive data is encrypted both at rest and in transit, and whether the organization can identify the cryptographic assets it relies on. IBM’s report page recommends discovering cryptography, analyzing vulnerabilities, and remediating risks—including planning for post-quantum security. IBM reports that 37% of breached organizations encrypted sensitive data both at rest and in transit, while 34% had visibility into cryptographic assets.

GL.iNet GL-MT5000 Brume 3 Wired VPN Security Gateway NO Wi-Fi
  • 【Up to 1100 Mbps VPN Speed 】 Hardware-accelerated WireGuard and OpenVPN-DCO deliver up to 1100 Mbps VPN throughput, over 3× faster than Brume 2 for smooth remote access and file transfers.
  • 【Three 2.5G Ports & Multi-WAN】Tri-port 2.5GbE design with flexible WAN LAN configuration supports multi-gigabit wired setups, dual-ISP Multi-WAN and failover to keep home and SOHO networks online.
  • 【Stealth VPN Obfuscation】VPN obfuscation disguises VPN traffic as regular HTTPS, helping you evade blocking, bypass restrictive networks and maintain stable, private connections.
  • 【DPI protection】Deep Packet Inspection with visual dashboards blocks adult/gambling/malicious sites, while SQM and QoS prioritize gaming, calls, and video when bandwidth is tight
  • 【OpenWrt & USB 3.0 Expansion】OpenWrt with 1GB DDR4 and 8GB eMMC lets you install plugins and build VPN, ad-blocking or NAS, while USB 3.0 Type‑C connects high-speed storage or 4G/5G dongles

Those findings point to two separate review questions: whether data is protected in storage and transmission, and whether teams know which cryptographic systems and dependencies they will need to assess for weaknesses and migration. Treat post-quantum work as an inventory and remediation-planning task, not simply an encryption toggle.

5. Remediation in development workflows

Measure the time between discovering a vulnerability or threat and fixing it. IBM’s release calls for building remediation into development workflows and fixing risks at the speed attackers move. The practical test is whether findings reach the teams and processes that can correct them, and whether closure can be verified.

Ubiquiti Cloud Gateway Ultra (UCG-Ultra)
  • Runs UniFi Network for full-stack network management
  • Manages 30+ UniFi Network devices and 300+ clients
  • 1 Gbps routing with IDS/IPS
  • Multi-WAN load balancing
  • 0.96" LCM status display

In its reporting, more than 50% of organizations used agents for threat detection and containment, while only 18% applied agents to vulnerability management. These are adoption figures, not evidence that using agents itself improves security outcomes.

How to turn the review into engineering work

IBM’s guidance supports a review based on exposure, implementation evidence, enforcement, accountability, and remediation time. The following questions are an editorial way to apply that guidance; IBM does not publish them as a scored framework.

  • Exposure: Which identities, data stores, APIs, applications, plug-ins, cloud settings, or cryptographic dependencies are in scope?
  • Evidence: Can the team show current inventories, access policies, monitoring records, tests, and remediation records?
  • Enforcement: Are permissions and security checks applied during operation, not merely documented at design or approval time?
  • Ownership and auditability: Is there a responsible owner, and can the team reconstruct who or what accessed a resource and when?
  • Time to remediate: How long do findings remain open after discovery, and can the team confirm that the underlying exposure was fixed?

IBM’s overall findings set the stakes, but they do not establish that any one of these controls will lower a given team’s breach cost by a predictable amount. The useful engineering outcome is evidence that risky access and exposure are known, constrained, monitored, assigned to owners, and addressed promptly.
