A suspected hydrocephalus-shunt blockage, infection, or overdrainage is urgent. Call your local emergency number or go to an emergency department for a seizure, altered consciousness, sudden neurological change, severe or rapidly worsening headache, repeated vomiting with deterioration, marked drowsiness, breathing problems, or any rapid decline. Tell responders that the person has a hydrocephalus shunt. Do not press, pump, or adjust the valve yourself.
Why a shunt problem can become dangerous
A hydrocephalus shunt diverts cerebrospinal fluid (CSF) from the brain’s ventricles to another area of the body, commonly the abdomen, where the fluid can be absorbed. A blockage can stop that drainage and allow pressure to build again. The U.S. Food and Drug Administration (FDA) identifies blockage as the most common cause of shunt malfunction, and the NHS warns that a serious blockage can cause brain damage.
Overdrainage is the opposite problem: the system removes CSF faster than the body produces it. That can pull the ventricles inward, stretch or tear blood vessels, and cause a subdural hemorrhage or slit-ventricle syndrome. Infection can involve the wound, the tubing, the valve, or the CSF itself and may progress quickly.

Symptoms overlap. A headache, vomiting, sleepiness, visual change, or balance problem does not reliably identify the cause at home. Age, the type of shunt, the programmable-valve setting, time since surgery, and the person’s usual neurological baseline all affect the presentation. Any new or worsening symptom therefore needs prompt clinical assessment rather than a home diagnosis.

Blockage or underdrainage: when hydrocephalus symptoms return
Underdrainage means that CSF is not leaving the ventricles adequately. A kink, disconnected component, tissue obstruction, or valve problem can interrupt flow. The resulting pressure may recreate the symptoms that led to shunt placement. FDA guidance says that when symptoms of hydrocephalus return, medical attention should be sought immediately.
Common symptoms in older children and adults
- Headache that is new, persistent, or progressively worse
- Nausea or vomiting
- Blurred or double vision and other visual changes
- Worsening balance, walking difficulty, or coordination problems
- Increasing drowsiness, confusion, or a change in consciousness
- A seizure or another new neurological deficit
A return of a person’s familiar hydrocephalus pattern is particularly concerning, but a malfunction can also present with a symptom they have not had before. Do not wait for every symptom to appear.
Signs in babies
- Vomiting, irritability, unusual sleepiness, or poor responsiveness
- Abnormal or rapidly increasing head growth
- Feeding difficulty or a noticeable change from the infant’s normal behavior
Infants cannot describe a headache, so a caregiver’s observation of a rapid change matters. Emergency assessment is appropriate for a baby who is difficult to wake, repeatedly vomiting, having a seizure, or deteriorating.

Shunt infection: symptoms and timing
Possible infection signs include fever or a general feeling of being unwell, headache, vomiting, neck stiffness, or painful eyes. The skin and wound can provide important clues: look for redness, tenderness, swelling, wound breakdown, or fluid leaking along the shunt path. Clear leakage may be CSF. If the distal catheter drains into the abdomen, abdominal pain or swelling can occur.
- Systemic symptoms: fever, chills or feeling unwell, headache, vomiting, neck stiffness, and increasing sleepiness
- Local symptoms: redness, warmth, tenderness, swelling, wound separation, or drainage over the scalp, neck, chest, or abdominal incision
- Abdominal symptoms: pain or swelling when the shunt drains into the abdomen
- Infant changes: irritability, poor feeding, vomiting, or unusual sleepiness
Southampton patient guidance says infection risk is highest during the first 30 days after surgery; the NHS notes that infections are more likely during the first few months. Infection can still occur later, so timing does not rule it out. Contact the surgical or neurosurgical team immediately for fever, wound changes, leakage, new vomiting, neck stiffness, or abdominal symptoms. Use emergency services if the person is deteriorating.
Overdrainage: when the shunt drains too much
Overdrainage occurs when the shunt removes CSF faster than it is produced. Headache is common, but the pattern is not sufficiently reliable for self-diagnosis, and overdrainage can feel similar to underdrainage. In severe cases, the ventricles can collapse, blood vessels can tear, and a subdural hematoma or slit-ventricle syndrome can develop.

A magnetically adjustable (programmable) valve may allow a neurosurgeon to change the drainage pressure without replacing the hardware. That adjustment is a specialist decision made after examining the person and reviewing imaging or other tests. Never attempt to change a setting with a magnet or another device at home.

When to go to the emergency department
Call emergency services now
- Seizure
- Fainting, altered consciousness, or inability to wake the person normally
- Sudden weakness, speech difficulty, severe confusion, or another sudden neurological deficit
- Severe or rapidly worsening headache
- Repeated vomiting accompanied by worsening headache, drowsiness, confusion, or other deterioration
- Marked drowsiness, breathing problems, or a rapid overall decline
- Any strong suspicion of a blocked or infected shunt with significant deterioration
Emergency numbers and pathways differ by country. Tell the dispatcher and emergency clinicians that the person has a hydrocephalus shunt. Bring the implant or shunt-alert card, operative information, medication list, and details of the person’s usual baseline if they are readily available; do not delay transport to collect paperwork.

Contact the neurosurgical team immediately
Even without an emergency symptom, call the treating neurosurgical service promptly for a new headache, vomiting, fever, neck stiffness, redness or tenderness over the wound or shunt tract, clear leakage, abdominal pain or swelling, or a meaningful change in behavior, walking, vision, or alertness. A post-operative headache should not automatically be dismissed as normal. The team needs to know when it started, whether it is worsening, and what other symptoms accompany it.
What not to do while waiting for care
- Do not press, pump, squeeze, or massage the valve or tubing.
- Do not use a magnet or other tool to change a programmable setting.
- Do not assume that a symptom is “just” overdrainage, a routine post-operative headache, or a stomach illness.
- Do not postpone assessment because symptoms temporarily improve; intermittent obstruction and pressure changes can fluctuate.
Keep the person supervised, note the time symptoms began and any progression, and follow the emergency service’s instructions. Do not give a sedating medicine unless a clinician who knows the situation tells you to do so, because increasing sleepiness can be an important warning sign.
How clinicians diagnose a suspected malfunction
Assessment starts with the history: the person’s original hydrocephalus symptoms, current complaints, surgery date, shunt type, valve setting if known, recent procedures, and baseline neurological status. A clinician performs a neurological examination and checks the incisions and the shunt path. CT or MRI may be used to compare ventricular size or identify complications, with additional tests selected for the individual case.

Imaging does not replace clinical judgment. Ventricular size may not change dramatically even when a shunt is malfunctioning, and symptoms can be serious before a scan provides a clear answer. The treating team decides whether observation, admission, further testing, or immediate surgery is safest.
Possible treatments
Valve adjustment
For some overdrainage or underdrainage problems, a neurosurgeon can change a programmable valve’s pressure setting. The setting is verified with the device-specific method; patients should not try to adjust it themselves.
Revision, replacement, or temporary drainage
A blocked, disconnected, or damaged component may need surgical revision or replacement. A severe malfunction may require temporary external drainage while the cause is treated and a durable plan is made.

Shunt infection generally requires antibiotics. Depending on the organism, hardware involvement, and clinical condition, the team may need to remove the shunt, treat the infection, use temporary drainage, and later place a replacement system.
MRI and programmable shunts
Some programmable valves use magnetic mechanisms. An MRI can change the setting in certain devices. Before every MRI, tell the imaging staff that the person has a shunt and provide the valve model or implant card if available. Ask whether the manufacturer requires a setting check or reset after the scan, and follow the neurosurgical team’s device-specific instructions.
Information that helps the emergency and neurosurgical team
- The exact time each symptom began and whether it is getting worse
- Headache location, severity, and any relation to position, if the person can describe it
- Number and timing of vomiting episodes
- Fever readings, wound appearance, leakage, redness, or tenderness
- Changes in alertness, walking, vision, speech, behavior, or seizures
- The shunt type, programmable setting if known, surgery date, and prior malfunctions
- Recent MRI scans, infections, procedures, falls, or head injuries
These details support triage, but they are not a substitute for examination. A person with a shunt can have a life-threatening problem without displaying every classic sign.
