Hydrocephalus is a buildup of cerebrospinal fluid (CSF) in and around the brain that can increase pressure and harm a developing brain. In babies, rapidly increasing head size, a tense or bulging soft spot, downward-looking eyes, feeding problems, vomiting, unusual irritability or sleepiness need prompt medical assessment. Older children may have morning headaches, vomiting, vision changes, drowsiness, confusion or trouble walking.

Only a clinician can diagnose hydrocephalus. Evaluation usually combines a physical and neurological examination with ultrasound, CT or MRI. Treatment is generally surgical: a shunt drains CSF to another part of the body, while endoscopic third ventriculostomy (ETV) creates a bypass inside the brain for selected cases.

What hydrocephalus means in a child

CSF is a clear fluid that cushions the brain and spinal cord, carries nutrients and removes waste. It is continuously produced, circulates through cavities called ventricles, and is then absorbed into the bloodstream. Hydrocephalus develops when CSF accumulates faster than it can circulate or be absorbed. The ventricles enlarge and pressure may rise.

In an infant, the skull bones are not yet fused, so the head can enlarge as the ventricles expand. In an older child, the skull is less flexible; pressure may therefore produce headache, vomiting, visual symptoms or changes in alertness before any obvious change in head size. Untreated pressure can injure brain tissue, which is why suspected symptoms require prompt clinical advice rather than watchful waiting.

A large head by itself does not prove hydrocephalus. Some children have familial or otherwise harmless large head size, while others have enlarged ventricles without dangerous pressure. Doctors interpret head-growth measurements, development, examination findings and imaging together.

What causes hydrocephalus?

Hydrocephalus can be present before birth (congenital) or develop later (acquired). Sometimes testing does not identify a single cause. The underlying problem may be an obstruction to CSF flow, impaired absorption, or bleeding and inflammation that disrupt circulation.

Cause or association How it can lead to hydrocephalus When it may appear
Spina bifida and related structural differences Altered brain and spinal-fluid pathways can obstruct or disturb CSF circulation. Often recognized during pregnancy or shortly after birth.
Dandy-Walker malformation A congenital difference in the structures around the fourth ventricle can impair drainage. Prenatal imaging, infancy or childhood, depending on severity.
Arachnoid cysts A fluid-filled cyst can press on or block normal CSF pathways. May be found on fetal, infant or later imaging.
X-linked hydrocephalus A genetic condition affecting brain development and CSF pathways. Usually congenital; genetic counseling may be offered.
Prematurity-related brain bleeding Bleeding can block CSF flow or interfere with absorption. During the newborn period or weeks after a premature birth.
Meningitis or another infection Inflammation and scarring can obstruct flow or reduce absorption. During or after the infection.
Brain tumor or other mass A mass can compress a ventricle or narrow a CSF channel. Any age; symptoms depend on location and growth.
Bleeding, venous thrombosis, head injury or stroke Blood, swelling or damaged absorption pathways can cause acquired hydrocephalus. After the event, sometimes immediately and sometimes later.
Unknown cause Despite appropriate evaluation, no single explanation is found in some children. Any age.

A cause can influence the safest procedure, the likelihood of recurrence and the child’s developmental outlook. It is reasonable to ask the treating team whether imaging suggests an obstruction, impaired absorption, bleeding, infection, a mass or a congenital condition.

Warning signs in babies

Infants cannot describe headache or visual changes, so changes in head growth, behavior and feeding are especially important. Congenital hydrocephalus may be detected on a pregnancy ultrasound or soon after delivery; other cases develop over days or weeks.

  • Head circumference increasing faster than expected, or a head that appears unusually large.
  • A tense or bulging fontanelle (soft spot), especially when the baby is calm and upright.
  • Thin, shiny scalp with unusually visible veins.
  • Eyes that consistently look downward, sometimes called a “setting-sun” appearance.
  • Poor feeding, repeated vomiting or difficulty coordinating sucking and swallowing.
  • Unusual irritability, high-pitched crying, reduced interaction or excessive sleepiness.
  • Stiffness, spasms or abnormal movements in the legs.

One sign can have many causes. A soft spot can look fuller while a baby is crying, and vomiting can result from common illnesses. The combination of progressive changes, abnormal head growth or reduced alertness is more concerning and should be discussed urgently with a clinician.

Warning signs in older children

Acquired hydrocephalus in a child with a closed skull may develop gradually or suddenly. Symptoms can fluctuate and may be mistaken for migraine, a stomach illness, vision problems or fatigue.

  • Headache that is worse on waking or aggravated by coughing, straining or lying down.
  • Nausea or vomiting without a clear gastrointestinal explanation.
  • Blurred or double vision, difficulty focusing or other visual changes.
  • Sleepiness, slowed thinking, confusion, personality change or declining school performance.
  • Unsteady walking, poor coordination or new difficulty with balance.
  • New bladder-control problems.

Seek urgent medical advice for a severe or rapidly worsening headache, repeated vomiting, marked drowsiness, confusion, a seizure, new weakness, or a child who is difficult to wake. These symptoms do not prove hydrocephalus, but raised pressure and other serious conditions must be excluded quickly.

When to seek help

The NHS advises: “Call a GP or use NHS 111 if you think you or your child may have symptoms of hydrocephalus.” Use your local emergency number instead when the child is unconscious, having a seizure, struggling to breathe, repeatedly vomiting with worsening alertness, or deteriorating rapidly. Do not wait for a routine appointment if a child with a known shunt develops possible malfunction symptoms.

Write down when symptoms began, how often vomiting or headaches occur, recent infections or injuries, medications, and any change in walking, vision, feeding or behavior. Bring the child’s head-growth record and previous scan reports if available.

How clinicians diagnose hydrocephalus

Diagnosis is based on the pattern of symptoms and examination, not head size alone. A clinician may assess alertness, eye movements, muscle tone, strength, reflexes, coordination, walking, feeding and head circumference. Babies may have their fontanelle and scalp veins examined, and clinicians compare measurements with earlier visits.

Prenatal and newborn imaging

Routine pregnancy ultrasound can show enlarged ventricles or associated structural differences. When more detail is needed, fetal MRI can provide a clearer view of the developing brain and help identify a possible cause. After birth, ultrasound through an open fontanelle may be useful because it avoids radiation and can be performed at the bedside.

CT and MRI

CT scans are fast and can show enlarged ventricles, bleeding or urgent pressure-related changes, but they use radiation. MRI provides detailed images of brain structures, CSF pathways and possible tumors or malformations without ionizing radiation; a young child may need preparation or sedation to remain still. The NHS notes that CT and MRI can demonstrate fluid buildup, increased pressure and structural defects.

Questions to ask after imaging

  • Are the ventricles enlarged, and is there evidence of raised pressure?
  • Is CSF flow blocked, or is absorption impaired?
  • Is there bleeding, infection, a tumor, cyst or congenital malformation?
  • Which findings require immediate treatment, and which can be monitored?
  • How will head growth, development, vision and symptoms be followed?

Treatment: why surgery is usually needed

The NHS states: “Hydrocephalus (excess fluid in the brain) is treated with surgery.” The objective is to lower harmful pressure and restore a safer CSF pathway before brain injury occurs. Medicines do not provide a dependable long-term solution for established childhood hydrocephalus. The operation selected depends on the cause, the child’s age, anatomy, previous procedures and the neurosurgical team’s assessment.

Shunt surgery

A shunt is a system of tubing and a valve that drains CSF from a brain ventricle to another body area, most commonly the abdomen (a ventriculoperitoneal or VP shunt), where the fluid can be absorbed. Other drainage destinations may be used when the abdomen is unsuitable. The device remains under the skin and regulates flow according to its valve design.

Shunts can be lifesaving, but they can block, disconnect, drain too much or too little, or become infected. A child may need additional operations during growth. Ask the team which valve is implanted, whether it is programmable, what activities or magnets matter for that device, and who to call day or night.

Endoscopic third ventriculostomy (ETV)

During ETV, a neurosurgeon uses an endoscope to make a small opening in the floor of the third ventricle. CSF then bypasses an obstruction and reaches spaces where it can circulate and be absorbed. ETV is mainly considered for obstructive hydrocephalus and is not suitable for every child; age, anatomy, cause and prior infection or bleeding affect success.

ETV avoids a permanently implanted shunt, and the NHS notes a lower infection risk than shunt surgery. However, the opening can close (sometimes months or years later), causing recurrent hydrocephalus. Repeat ETV or later shunt placement may then be required. Families should know which symptoms indicate recurrence and how quickly to seek help.

Shunt versus ETV: the practical differences

Decision factor Shunt ETV
Best fit Can treat several patterns of excess CSF, including impaired absorption, when a drainage route is needed. Primarily considered when an obstruction can be bypassed and anatomy is favorable.
Implanted device Yes: catheter, valve and drainage tubing remain in the body. No permanent shunt hardware is required for the procedure itself.
Main failure modes Blockage, infection, disconnection or drainage problems. Closure of the created opening and return of hydrocephalus.
Further surgery Revision may be needed if the system fails or as the child grows. Repeat ETV or a later shunt may be needed if the opening closes or does not control pressure.
Infection risk Present because foreign material is implanted. Generally lower than with shunt surgery, according to NHS guidance, but not zero.
What determines choice Cause, age, anatomy, prior infection or bleeding and the surgeon’s assessment. The same factors, with particular attention to whether CSF can circulate after the bypass.

There is no universally “better” operation. A procedure that works well for one child may be inappropriate for another, so the decision should be individualized rather than based on age alone or on the presence of a large head.

Recognizing shunt blockage or infection

A child with a shunt can develop recurrent hydrocephalus if the system blocks or fails. Infection may occur soon after surgery or later. Contact the child’s neurosurgical team immediately for a combination of neurological or wound symptoms.

  • Headache, repeated vomiting, increasing sleepiness, irritability or reduced responsiveness.
  • Fever, neck stiffness or a child who appears acutely unwell.
  • Redness, swelling, warmth, tenderness or drainage along the shunt track or incision.
  • Abdominal pain, swelling or persistent tenderness in a child with a VP shunt.
  • New visual changes, imbalance, weakness or a return of the symptoms that led to surgery.

Do not press, pump or adjust the shunt yourself. Emergency clinicians may need the shunt model, operation date and neurosurgical contact details. A shunt alert card can help identify the device during an emergency; it is an identification aid, not a treatment and does not prevent blockage or infection.

Follow-up, development and prognosis

Hydrocephalus is managed over time, even after a successful operation. Follow-up may include head measurements in infants, neurological and developmental checks, vision assessment, monitoring of school or behavior changes, and imaging when symptoms or growth patterns warrant it. Some children need rehabilitation, speech and language support, physical or occupational therapy, or educational accommodations because the underlying cause—not only the fluid buildup—can affect development.

Outcome varies with the cause, how early pressure is controlled, prematurity or bleeding, associated brain differences, infections or tumors, seizures, and the child’s response to treatment. No single scan or operation predicts an individual child’s future. Ask for a written plan covering warning symptoms, routine appointments, activity restrictions if any, and emergency contacts.

Practical questions for the neurosurgical team

  • What is the suspected cause and is CSF flow obstructed?
  • Why is a shunt or ETV recommended for this anatomy and age?
  • What are the expected benefits, important risks and alternatives?
  • Which symptoms mean “call today” and which mean “go to emergency care”?
  • How will you monitor vision, head growth, learning and development?
  • If a shunt is placed, what valve type and alert information should we carry?
