Researchers measure vaccine-induced T-cell immunity by exposing a participant’s immune cells to vaccine antigens and then asking a specific question: Which cells secrete cytokines, which cells carry a particular peptide-MHC receptor, which cells divide, and which subsets are responding? The main tools are ELISpot or FluoroSpot, intracellular cytokine staining (ICS) with flow cytometry, peptide-MHC multimers such as tetramers, and proliferation assays. Each produces a different readout, so a larger number in one assay is not automatically a stronger or more protective response.
These tests measure immunogenicity—the immune response generated under defined laboratory conditions. They do not, by themselves, demonstrate that vaccination prevents infection or disease. Clinical efficacy and validated immune correlates are separate questions in vaccine development.
What a T-cell assay actually measures
Most cellular assays start with peripheral blood mononuclear cells (PBMCs), a mixture that includes T cells and other blood immune cells. Investigators stimulate the cells with a vaccine antigen, peptide pool, or selected peptide-MHC reagent. The assay then records one biological feature of the responding population:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Secreted cytokine function: how many cells release a measured cytokine after stimulation.
- Intracellular function and phenotype: which cell subsets contain cytokines and what markers they express.
- Antigen-specific binding: how many T-cell receptors bind a specified peptide-MHC complex.
- Proliferation: whether stimulated lymphocytes divide.
The denominator, stimulation strength and duration, antigen composition, background correction, and response threshold all affect the reported result. A percentage of CD8 cells, spots per number of PBMCs, and multimer-positive cells are different quantities and should not be compared as if they were interchangeable.

ELISpot and FluoroSpot: counting cytokine-secreting cells
How the assay works
In an enzyme-linked immunospot (ELISpot) assay, PBMCs are placed on a plate coated with an antibody that captures a chosen cytokine. After antigen stimulation, each secreting cell leaves a localized cytokine deposit. Detection reagents convert those deposits into visible spots. The result is usually expressed as spot-forming cells per defined number of input cells, often with an unstimulated control subtracted.
FluoroSpot is a related format that uses fluorescent detection and can measure more than one cytokine in the same well. Both methods estimate the frequency of cells producing the measured cytokine under the particular stimulation and incubation conditions; they do not count every antigen-specific T cell.

- The method is sensitive and comparatively simple, qualities highlighted by the World Health Organization in its immuno-monitoring assays proposal.
- It can work with limited cell numbers and provides a direct frequency estimate for cytokine-producing cells.
- It is useful for a prespecified primary or secondary cellular-immunogenicity endpoint when the cytokine and response rules are defined in advance.
Important limitations
A single-cytokine ELISpot can miss cells that recognize antigen but produce a different cytokine, produce too little to cross the detection threshold, or respond through a mechanism not captured by the assay. Spot counts also depend on cell viability, plating consistency, stimulation conditions, washing, reader settings and background in control wells.

Intracellular cytokine staining (ICS) with flow cytometry
How it works
For ICS, investigators stimulate cells with antigen and use secretion-blocking treatment so newly produced cytokines remain inside the cells. Fluorescent antibodies then label cytokines and lineage or activation markers. A flow cytometer measures thousands of cells individually, allowing analysts to identify CD4 and CD8 populations and quantify cells positive for cytokines such as interferon-gamma (IFN-γ), interleukin-2 (IL-2) and tumor-necrosis factor alpha (TNF-α). Panels can include several cytokines and phenotypic markers, such as differentiation or activation indicators.
What ICS adds
ICS links function to phenotype. A trial can report, for example, the proportion of CD4 cells producing one, two or three cytokines, or compare cytokine production in defined memory subsets. This cell-subset resolution is difficult to obtain from a conventional single-channel ELISpot.
Trade-offs
Flow cytometry requires instrument access, trained staff, validated antibody panels, compensation or spectral-unmixing procedures, and a prespecified gating strategy. It is labor-intensive and may be less sensitive than ELISpot in some settings, a comparison noted by WHO. Small changes in gating, stimulation time, viability and instrument performance can change the final percentage.

MHC multimers are reagent complexes in which several peptide-MHC molecules are linked to increase binding avidity. A tetramer is one common format. When the peptide-MHC matches a T-cell receptor, the reagent binds and fluorescent labeling lets flow cytometry count those cells.

This is a specificity and counting readout. It answers, “How many T cells recognize this particular peptide-MHC complex?” It does not, by itself, show that those cells secrete IFN-γ, make IL-2, kill a target, or proliferate. The selected HLA allele and peptide also constrain detection: a participant whose relevant HLA type is not represented by the reagent cannot be assessed for that specificity with the same multimer.
Multimer assays therefore require careful epitope and HLA planning. They can be highly informative when a vaccine’s target epitopes are defined, but they do not provide a universal count of all vaccine-reactive T cells.
Proliferation assays: asking whether cells divide
In a proliferation assay, cells are labeled with a division-tracking dye and stimulated with antigen. As daughter cells divide, the dye is diluted. Flow cytometry then estimates the fraction of cells that underwent one or more divisions, often within identified lymphocyte subsets. CFSE-based approaches are among those described by the Clinical and Laboratory Standards Institute (CLSI).

Proliferation measures a different functional dimension from cytokine release and multimer binding. A cell can recognize antigen yet divide slowly, or divide without producing the cytokine selected for an ELISpot or ICS panel. The assay also depends on culture duration, cell health, dye loading, baseline division and the rules used to distinguish true proliferation from noise.

Why different assays can give different answers
Assay results are shaped by sensitivity, sample quality, antigen stimulation, controls and laboratory handling. Background subtraction is especially important for ICS and other stimulation assays: an unstimulated sample reveals baseline cytokine-positive cells, but subtracting that value introduces measurement error and can reduce statistical power when the stimulated and background values are close.
A clinical comparison of eight melanoma-vaccine participants found the highest counts with tetramers, followed by cytokine flow cytometry and ELISpot, with no significant concordance among the methods for peptide-specific CD8 T-cell counts. The study, published in an American Association for Cancer Research journal in 2003, was small and conducted in a cancer-vaccine setting. Its result shows that methods can disagree; it is not a universal ranking of assay sensitivity or biological importance. The article is available at AACR.
| Assay | Primary readout | Resolution | What a positive result means |
|---|---|---|---|
| ELISpot | Cytokine-secreting cells, reported as spots | Frequency of cells producing the selected cytokine | Cells secreted that cytokine under the stimulation conditions |
| FluoroSpot | Fluorescent spots for multiple cytokines | Frequency of cells producing each measured cytokine | Cells produced one or more measured cytokines under the assay conditions |
| ICS with flow cytometry | Intracellular cytokines plus surface phenotype | Defined CD4, CD8 or other gated subsets | Identified cells contained the measured cytokine(s) after stimulation |
| MHC multimer or tetramer | Peptide-MHC binding | Cells with a selected T-cell-receptor specificity | Cells bound the specified peptide-MHC reagent |
| Proliferation | Cell-dye dilution after stimulation | Dividing cells, optionally within gated subsets | Cells underwent division during the culture period |
How a clinical trial plans and runs a cellular assay
- Define the biological question. Decide whether the endpoint is cytokine function, multiparameter phenotype, selected epitope binding, proliferation, or a combination. The question determines the assay and the antigen panel.
- Choose specimens and timing. Specify blood collection time points relative to vaccination, PBMC isolation, cryopreservation or fresh processing, acceptable transport time and temperature, and the minimum viability or cell number. Poor-quality or inconsistently handled cells can obscure a real response.
- Set antigen stimulation conditions. Document the antigen or peptide pool, concentration, solvent, incubation time, cell density and culture medium. For multimer testing, document the peptide sequence and HLA restriction.
- Include controls. An unstimulated condition estimates background. Positive controls show that cells and reagents can respond, while reagent-only or staining controls help identify nonspecific signal and flow-cytometry artifacts. Control acceptance criteria should be written before database lock.
- Prespecify analysis. Record the cell denominator, gating hierarchy, cytokines and markers, background-subtraction rule, minimum event count, positivity threshold and handling of outliers or missing samples. For ELISpot, define how spots are counted and how replicate wells are combined.
- Validate and monitor the laboratory. Assess precision, linearity where relevant, repeatability, sample stability and assay failure rates. Lock instrument settings, antibody lots or bridging procedures as far as practical, and retain raw data for audit.
- Report the result with its conditions. Give the time point, antigen, controls, denominator, assay version, analysis population and confidence or uncertainty measures. A bare percentage or spot count is not reproducible evidence.
The CLSI resource ILA26, Performance of Single Cell Immune Response Assays, addresses procedure, interpretation, specimen collection and transport, sample preparation, quality control, validation and troubleshooting for methods including ICS, MHC multimers, ELISpot and proliferation. Because standards can be revised, laboratories should verify the current edition before treating an older edition as operationally current.

A trial may show that vaccination increased antigen-specific T cells, yet that finding alone cannot establish prevention of infection or disease. A flow-cytometry review describes an HIV vaccine trial with robust ICS-measured T-cell responses but no reduction in infection rates; the example demonstrates the interpretive limit rather than proving that T cells never protect. The review is available in Vaccine applications of flow cytometry.

Protection analyses require clinical endpoints and, when available, a validated immune correlate that links a defined response to outcome. Cellular immunogenicity can support dose selection, schedule decisions, mechanism-of-action work and exploratory correlate analyses, but the assay result should be labeled as an immune response rather than a surrogate for efficacy.
Making results comparable across sites and trials
Cross-site interpretation is difficult because T-cell assays are complex and costly, high-quality cells are essential, HLA distributions differ, and age and demographic composition can change the responding population. WHO also notes that procedures are not fully harmonized and that reference standards remain limited; early-phase studies often assess T cells in small donor cohorts, which constrains conclusions about cellular immunity as a biomarker or correlate of protection. WHO’s proposal states: “Most vaccines are currently approved based on data from humoral responses, as these are easier to measure and standardise.” The institutional source is the WHO immuno-monitoring assays proposal.
For a multicenter study, laboratories should use a common protocol, harmonized reagents and instrument settings where feasible, documented specimen collection and transport, identical control rules, and a shared analysis plan. External proficiency panels can reveal site-to-site drift before the trial’s primary analysis. Workshop recommendations on standardized assays, full methodology disclosure and proficiency testing are summarized by the iSBTc-SITC/FDA/NCI workshop.
Choosing an assay strategy
| Decision factor | Questions for the protocol team |
|---|---|
| Biological feature | Is the endpoint secretion, polyfunctionality and phenotype, receptor specificity, or division? |
| Cell-subset detail | Must the result distinguish CD4, CD8, memory or other phenotypic subsets? |
| Sensitivity and background | What signal level is expected, and how will unstimulated background and replicate variability be handled? |
| Sample requirements | How much blood and how many viable PBMCs are available at every time point? |
| Laboratory capability | Does the site have validated flow cytometry, ELISpot reading, trained analysts and backup capacity? |
| Antigen and HLA coverage | Will a peptide pool provide broad coverage, or does the study require selected peptide-MHC reagents? |
| Comparability | Can all sites follow the same protocol, controls, QC and proficiency plan? |
There is no universally best T-cell assay. Many trials combine a broad functional assay such as ELISpot or ICS with a specificity assay such as a multimer when the scientific question justifies the extra sample, labor and analysis. The combination is useful only when each endpoint has a defined purpose and its limitations are reported.
Minimum reporting checklist
- Participant population, HLA eligibility and vaccination time point.
- Specimen type, collection tube, processing interval, cryopreservation and viability.
- Antigen identity, peptide sequences or pools, concentration and stimulation duration.
- Assay platform, cytokines, antibodies, fluorochromes and instrument or reader.
- Positive and negative controls, replicate design and acceptance criteria.
- Cell denominator, gating strategy or spot-counting algorithm, and background subtraction.
- Definition of a positive response, handling of missing or failed wells, and statistical plan.
- Whether the endpoint is exploratory immunogenicity or linked to a separately validated clinical correlate.
Those details let readers distinguish a measured laboratory response from a claim about protection and make results interpretable across studies.
