UnitedHealth Group makes money through two connected businesses. UnitedHealthcare collects premiums for health-benefit coverage and takes responsibility for members’ medical-cost risk. Optum sells health care, pharmacy-benefit administration, data and technology, consulting, and financial services. In fiscal 2025, those operations produced $447.567 billion in consolidated revenue, although segment totals cannot be added directly because the company eliminates revenue from transactions between its businesses.
The simple explanation: an insurer plus a health-services platform
UnitedHealth Group is not only an insurance company. Its business model combines a traditional health insurer, UnitedHealthcare, with a large services platform called Optum. The four reportable segments in the 2025 Form 10-K were UnitedHealthcare, Optum Health, Optum Insight and Optum Rx.
UnitedHealthcare’s main economic activity is accepting a premium in exchange for covering specified health-care costs. The amount it ultimately earns depends on enrollment, premium rates, medical claims, government payment formulas, quality incentives and administrative expenses.

Optum earns revenue in several ways. Optum Health provides and manages medical care, often under value-based contracts. Optum Insight sells analytics, technology, consulting and managed services. Optum Rx operates pharmacy-benefit and pharmacy-care businesses. Optum Financial earns fees and investment or interest income from consumer health-financial accounts and related assets.

This combination gives UnitedHealth Group several kinds of customers and contracts. Employers, individuals, federal and state programs, health plans, providers, pharmaceutical customers, governments, life-sciences organizations and consumers can all pay different parts of the company.

The following figures are for fiscal year 2025, as reported by UnitedHealth Group. Optum’s total is shown before eliminations between UnitedHealth Group businesses, so it is not an additional amount that can be added to UnitedHealthcare revenue.

UnitedHealth Group also reports revenue by economic category: $308.810 billion from premiums, $50.226 billion from products and $36.040 billion from services. These categories describe how revenue enters the company, while the segment figures describe which operating businesses generated it. They should not be treated as separate totals to add together.

How UnitedHealthcare makes money from insurance
Premiums transfer medical-cost risk
UnitedHealthcare generally receives a fixed premium per member for a contract period. In return, it agrees to pay or arrange covered health-care services under the policy or government program. If members’ claims and administrative costs are lower than the premium and other revenue associated with coverage, the insurance operation can earn a profit. If costs are higher, the insurer bears the economic pressure.
The premium is not simply a payment for access to a network. It finances expected medical claims, risk adjustment, quality-related payments, operating expenses and the capital needed to support insurance obligations. Pricing and benefit design must account for the health status and utilization patterns of the covered population.

Employer and individual coverage
In commercial markets, employers or individuals pay for coverage, directly or through employer-sponsored plans. The insurer may provide a fully insured product, in which UnitedHealthcare retains the medical-cost risk, or administrative services for an arrangement where another party retains more of that risk. The exact economics depend on the contract.
Medicare Advantage and Medicare Part D
UnitedHealthcare participates in Medicare Advantage and Medicare Part D. Government payments are influenced by enrollment, members’ documented health conditions and program rules. Medicare Advantage revenue is affected by CMS risk adjustment and quality bonuses. A plan that accurately documents members’ conditions and performs well on applicable quality measures can receive different payments from a plan with a lower risk or quality profile.

UnitedHealthcare also serves Medicaid and other state-sponsored programs. Revenue can reflect the number of people enrolled, the acuity of the membership and negotiated or regulated rates. In 2025, UnitedHealth Group cited higher-acuity Medicaid membership as one factor contributing to growth.
Risk adjustment and medical-loss-ratio effects
Insurance revenue is influenced by mechanisms that redistribute funds according to the health risk of members. Medicare programs use CMS risk adjustment, while commercial and Medicaid products can involve Affordable Care Act risk adjustment. Commercial products can also require minimum medical-loss-ratio rebates when too little premium is spent on eligible medical care and quality improvement relative to applicable thresholds. Those rebates reduce the amount the insurer ultimately retains from premiums.
What drove UnitedHealthcare’s 2025 growth
UnitedHealthcare revenue increased 16% in 2025 to $344.903 billion. UnitedHealth Group attributed consolidated growth mainly to serving more people through Medicare Advantage, higher-acuity Medicaid membership, expansion in Optum Rx and pricing trends. The statement describes drivers of revenue growth, not a guarantee that every line of business became more profitable.
What Optum does and how each segment earns revenue
Optum Health: care delivery and value-based contracts
Optum Health combines medical-care delivery with contracts that can tie payment to the cost and quality of care. Its services include primary, specialty and surgical care; chronic, behavioral and complex-care programs; post-acute planning; telehealth; remote monitoring; and health-financial services. It served approximately 95 million people at December 31, 2025.

- Fully accountable value-based arrangements: Optum receives a monthly premium and assumes responsibility for the health-care costs of the covered population. Its results depend on managing care effectively while meeting quality and access requirements.
- Administrative-fee arrangements: Optum manages or administers products and services for a recurring monthly fee. The customer or another party may retain the underlying medical-cost risk.
- Fee-for-service arrangements: Optum delivers a medical or health-related service for a contracted fee. Revenue is linked to the services performed rather than to responsibility for all future costs of a population.
Because these arrangements differ, Optum Health is not economically identical to an insurer. A fully accountable contract resembles risk-bearing insurance economics, while an administrative or fee-for-service contract is closer to a services business.
Optum Insight: information, technology and managed services
Optum Insight sells data, analytics, research, consulting, technology and managed-services solutions. Customers include health plans, health-care providers, employers, governments and life-sciences organizations. Products can help customers process claims, understand clinical and financial data, improve operations, manage revenue cycles or make decisions using specialized health information.
Optum Insight generated $19.417 billion in 2025 revenue, up 4% from 2024. UnitedHealth Group said growth came from technology services and reduced effects from the Change Healthcare cyberattack, partly offset by lower business-services volumes. The cyberattack reference is important: it affected the comparison and does not mean that all Insight revenue is recurring software revenue.

Optum Rx: pharmacy benefits and pharmacy care
Optum Rx provides pharmacy-benefit and pharmacy-care services to health plans, employers, unions, purchasing coalitions and public-sector customers. Its network and capabilities include about 64,000 retail pharmacies, home delivery, specialty and community pharmacies, infusion services, and support for rare-disease and gene-therapy treatments.
The business can earn revenue from administering pharmacy benefits, operating or coordinating pharmacy services, and providing specialty-care capabilities. It managed $188 billion in pharmaceutical spending in 2025, including nearly $87 billion in specialty pharmaceutical spending.

Optum Rx revenue reached $154.726 billion in 2025. Adjusted scripts increased to 1,659 million from 1,623 million in 2024. UnitedHealth Group attributed revenue growth to higher prescription volume from new and existing clients and growth in pharmacy services. “Adjusted scripts” is a company operating measure, so it should not be read as the number of unique patients or prescriptions dispensed by one physical pharmacy.
Optum Financial: account fees and investment income
Optum Financial serves consumers through nearly 26 million accounts and had more than $27 billion in assets under management at December 31, 2025. It earns fees and also generates investment and interest income on managed funds and loans.

The company indicated that prior periods would be recast in 2026 filings. As a result, readers comparing future segment tables with 2025 disclosures need to check whether the figures have been recast.

Why the revenue numbers do not add up
A common mistake is to add $344.903 billion of UnitedHealthcare revenue to $270.620 billion of Optum revenue and call the result UnitedHealth Group’s sales. That would produce a number larger than the company’s $447.567 billion consolidated revenue because the segments do business with one another.
For example, an Optum unit may provide services to a UnitedHealthcare plan, or a UnitedHealthcare arrangement may fund activity delivered through an Optum operation. UnitedHealth Group records the segment activity for management reporting but removes intercompany revenue when preparing consolidated financial statements. The consolidated figure counts external revenue once.

The same caution applies when comparing segment revenue with the $308.810 billion premium, $50.226 billion product and $36.040 billion service categories. Segment reporting and revenue-category reporting answer different questions.

Diversification across payment types
Premiums provide the largest revenue category, but Optum adds businesses paid through administrative fees, professional services, pharmacy activity, technology contracts and financial accounts. This diversification means the group is exposed to more than one health-care revenue stream.
Scale can increase operating leverage
Large enrollment, prescription volume, provider relationships and data operations can support broad infrastructure. Optum Rx’s 1,659 million adjusted scripts and $188 billion of managed pharmaceutical spending illustrate transaction scale, while Optum Health’s approximately 95 million people served illustrate the reach of its care platform.
Medical-cost and utilization risk
Risk-bearing insurance and fully accountable value-based contracts can be hurt when utilization, treatment intensity or drug costs exceed expectations. Higher-acuity membership can increase premium revenue while also increasing the cost of care, so enrollment growth alone does not establish profitability.
Regulation and program rules
Medicare risk adjustment, quality bonuses, Medicaid rates, ACA risk adjustment and medical-loss-ratio requirements all affect how much revenue can be retained. Changes in government policy, coding rules, benefits or required rebates can therefore change the economics of coverage.
Operational and technology disruption
Optum Insight’s 2025 comparison reflected the effects of the Change Healthcare cyberattack. A disruption to claims, payment or health-data systems can affect customers and revenue timing even when long-term demand for those services remains.
How to analyze UnitedHealth Group’s revenue yourself
- Start with consolidated revenue. Use the $447.567 billion fiscal 2025 figure as the group-level measure.
- Separate UnitedHealthcare from Optum. Treat UnitedHealthcare’s $344.903 billion and Optum’s $270.620 billion as segment measures, not additive totals.
- Break Optum into its operating businesses. Compare Optum Health, Optum Insight and Optum Rx, and check where Optum Financial is presented for the period being reviewed.
- Identify the payment mechanism. Ask whether revenue comes from a risk-bearing premium, a value-based monthly payment, an administrative fee, a fee-for-service payment, a pharmacy service or a technology contract.
- Check the operating scale behind the number. Enrollment, people served, adjusted scripts and managed pharmaceutical spending provide context, but none is a direct profit measure.
- Read the reconciliation and accounting period. Confirm eliminations, recast segment definitions and whether a figure is for fiscal 2025 or a later reporting period.
Bottom line for investors and business readers
UnitedHealthcare is the insurance engine, collecting premiums and managing medical-cost risk across employer, individual, Medicare and Medicaid markets. Optum is a multi-line health-services platform: Health delivers and manages care, Insight sells information and technology, and Rx administers pharmacy benefits and pharmacy-care services. Together they explain how UnitedHealth Group can generate hundreds of billions of dollars from both insurance coverage and services sold throughout the health-care system.
