A signable risk acceptance record gives an authorized executive a bounded decision: which residual risk is being accepted, why it is reasonable now, what conditions apply, and when the decision must be revisited. It does not claim that the risk is eliminated. Under NIST’s definition, acceptable risk is residual risk that remains within the organization’s risk appetite and tolerance.
What the executive must be able to decide
Put the decision near the top of the record. In one paragraph, state the exposure, the affected system or business process, the likely business consequence, the current residual rating, and the decision requested. Avoid asking the signer to infer the decision from a long technical assessment.
The accountable signer is the authorized executive or authorizing official. NIST states: “The explicit acceptance of risk is the responsibility of the authorizing official and cannot be delegated to other officials within the organization.” A security analyst, project manager, or system administrator may prepare the record and recommend a response, but cannot substitute for the official who holds the delegated authority.

Build the record in a signable order
1. Decision header
Identify the record before describing the risk. Include:

- Read Before You Buy — No Video Output: These adapters support charging and USB 2.0 data transfer, but cannot transmit video signals. Except for standard USB webcams (which use USB data only), they are not compatible with HDMI/DisplayPort cables, video-capable USB-C hubs, or docking stations with video output.
- Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
- Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
- Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
- Record ID and decision date
- System, service, process, or project in scope
- Business owner and risk owner
- Information classification, if applicable
- Requested decision: accept, accept with conditions, or reject and require another response
2. Executive summary
Use plain business language. Name the exposure, the mission or revenue process affected, the current residual likelihood and impact, and the period for which approval is requested. A useful pattern is: “We request acceptance of the remaining risk that [event] could affect [asset or process], causing [business impact]. After the controls listed below, the residual rating is [rating]. Acceptance is requested until [date], subject to the conditions and monitoring in this record.”
3. Risk statement and boundaries
Write the risk as a cause-and-effect statement:
Because [condition or threat], [asset or process] may experience [event], causing [business impact].
Define what is inside and outside the decision. State the environment, data, locations, users, interfaces, and business functions covered. Record assumptions, dependencies, and uncertainty—for example, an unverified supplier control, an estimated likelihood, or a control that has not yet operated through a full cycle. A signer should be able to see where the assessment is solid and where it is conditional.

4. Assessment before and after controls
Show how controls change the exposure rather than presenting only a final color or score.
| Element | What to record |
|---|---|
| Inherent likelihood | Likelihood before controls, with the method or scale used |
| Inherent impact | Operational, financial, legal, safety, privacy, or mission effect before controls |
| Implemented controls | Preventive, detective, corrective, and compensating controls that are actually in operation |
| Control gaps | Missing, partial, overdue, or untested safeguards |
| Residual likelihood and impact | Remaining exposure after the implemented controls |
| Confidence and uncertainty | Evidence quality, assumptions, measurement limits, and dependencies |
Call the result residual risk. Acceptance is a decision about that remaining exposure, not a declaration that the underlying threat has disappeared.
Explain why acceptance is proportionate
NIST describes risk response as “Accepting, avoiding, mitigating, sharing, or transferring risk.” Show that the other responses were considered and explain the trade-off. Do not write “mitigation is too expensive” without describing what was compared.

| Response option | Decision information to show |
|---|---|
| Mitigate | Risk reduction, controls, cost and effort, deployment time, owner, and any residual risk after completion |
| Avoid | Business capability or activity that would stop, and the mission or revenue effect |
| Share or transfer | Third-party dependency, contract or insurance limits, remaining responsibility, and reversibility |
| Accept | Why the remaining exposure fits appetite and tolerance now, given business need, feasibility, cost, and timing |
Include completed mitigations, deferred actions, compensating controls, and the reason each deferred item does not prevent the requested decision. Acceptance should be selected because it is a proportionate response to the documented exposure—not because no one has assigned the work.

An executive signature is meaningful only when it has boundaries. List mandatory controls, named action owners, due dates, evidence to monitor, escalation thresholds, and reporting cadence. Examples include a monthly access review, evidence of patch deployment, a supplier assurance report, or an alert when a defined incident threshold is reached.
For every condition, specify:
- Required action or control
- Single accountable owner
- Due date and completion evidence
- Metric or artifact that will be monitored
- Threshold that requires escalation or suspension
- Recipient and frequency of reporting
Define authority, duration, and reapproval
Identify the policy, delegation, or governance instrument that gives the accepting executive authority for this system and risk level. Record the executive’s name and title, the effective date, and an expiry or formal review date. NIST authorization guidance calls for terms and conditions plus a termination date or a time-driven authorization frequency.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
Also define event-driven triggers for reapproval. Require a new decision when there is a material change in threat, scope, data, business use, control performance, system architecture, supplier, or legal obligation. A long, open-ended approval is not a substitute for monitoring; if the organization permits continuing authorization, state the review interval and the evidence required at each review.

Then provide fields for:
- Printed name of accepting executive
- Title and organizational unit
- Signature or approved electronic signature
- Date and time of approval
- Delegation or authority reference
If the executive declines, records a different condition, or limits the scope, preserve that disposition rather than silently editing the request.
Keep the decision auditable after signature
Link the signed record to the risk-register entry, assessment evidence, control test results, action plan, and every later review or reapproval. Accepted findings remain documented and must be monitored for changes in risk factors. Update the record when a condition is completed, missed, superseded, or triggered for escalation; retain the original decision so its history remains clear.
A spreadsheet template can help if it has separate areas for the acceptance decision, authority details, and expiry tracking. The format matters less than preserving the scope, residual assessment, conditions, authority, and review trail in a controlled record.
Quick Recap
Final pre-signature check
- Does the summary state exactly what exposure is being accepted?
- Are the affected assets, mission, boundaries, and assumptions explicit?
- Are inherent and residual ratings supported by identified controls and evidence?
- Were mitigation, avoidance, sharing, and transfer considered?
- Is the signer authorized for this risk and scope?
- Are owners, due dates, monitoring evidence, and escalation thresholds named?
- Is there an expiry or review date and a list of event-driven triggers?
- Are the signed record and follow-up actions linked in the risk register?
