Treat every AI-generated vulnerability report as a hypothesis, not proof. Before changing code, notifying customers, or assigning a severe incident, confirm authorization, reproduce the claimed effect independently when safe, validate the evidence against the named vulnerability class, and have a human resolve material uncertainty.
What “verified” should mean
A report is verified only when the observed behavior matches the claim under documented conditions and the result can be checked independently of the AI system that produced it. A fluent explanation, a convincing proof-of-concept, or tool output generated inside the same agent session is not independent evidence. The agent may have inferred an effect, replayed stale output, or produced artifacts that do not come from the target.
Use three separate questions:
- Does the behavior occur? Can an authorized tester reproduce the predicted result against the specified target and version?
- Is it a vulnerability? Does the behavior violate a security property rather than represent documented or intentional functionality?
- How serious is it? What access, data exposure, or control does the demonstrated behavior actually provide, under which preconditions?
A “yes” to the first question does not automatically establish the second or third. Keep those decisions separate in your record.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteStop first: authorization, scope, and safety
Do not send the proof-of-concept until you know that the target, account, environment, and proposed test are authorized. A report can identify a real weakness while still directing you toward an unlawful or unsafe test.

Confirm the target and permitted actions
- Match the hostname, application, API, package, device, cloud account, and software version in the report to an asset your organization controls or is explicitly permitted to test.
- Check the vulnerability disclosure policy, penetration-testing rules of engagement, contract, bug-bounty terms, or written owner approval.
- Verify whether the policy allows the proposed technique, such as authentication testing, automated requests, uploaded payloads, callbacks, denial-of-service testing, or access to test data.
- Identify rate limits, maintenance windows, test accounts, data-handling requirements, and a contact who can stop the test.
CISA’s July 15, 2026 joint-guidance announcement describes a vulnerability disclosure policy as defining which systems researchers may search, which test types are allowed, and safe-harbor language for legal protection. Use that as a practical checklist for policy completeness; it is not permission to test a system that your organization does not own or otherwise have authority to assess.
Define a safe stopping point
Write down the least-invasive observation that would confirm or disprove the claim. Avoid destructive actions, persistence, privilege escalation beyond what is necessary to demonstrate the issue, and access to real personal or confidential data. Do not repeat a one-time action merely to make a screenshot cleaner. If the only available test could disrupt service or expose data, stop and escalate to the asset owner or an authorized human tester.
Preserve the original report before interpreting it
Save the report exactly as received, including its timestamp and conversation or run identifier. Preserve the model name and version if available, prompts or tool instructions that materially affected the result, attached files, commands, HTTP requests, responses, screenshots, logs, and hashes of downloaded artifacts. Record the target, component, version, claimed preconditions, account roles, and the reporter’s proposed impact.
Do not edit the original into a cleaner narrative. Create a separate analyst record for corrections and tests. This prevents later conclusions from being mistaken for what the AI actually asserted.
Minimum intake fields
- Target identifier, environment, region, and software or firmware version
- Claimed vulnerability class and affected endpoint, function, or component
- Required privileges, inputs, configuration, and network position
- Exact reproduction steps and predicted observable effect
- Raw request, response, trace, log, or file artifact supplied by the report
- Claimed confidentiality, integrity, availability, and business impact
- Authorization reference and restrictions on testing
- Initial uncertainty, missing evidence, and assigned human owner
Turn the narrative into a falsifiable claim
Separate what the agent observed from what it inferred. Rewrite the report as a short statement that specifies an input, a condition, and an observable result. For example: “With a standard user session, submitting this value to endpoint X causes the server to execute an unintended database expression, observable in the response or a controlled test record.” That is testable. “This endpoint is critically vulnerable to SQL injection” is a conclusion that still needs evidence.
Ask what would disprove it
Before testing, list a result that would falsify the claim. A response that remains identical to a safe control, a callback that never arrives, or a server-side log showing the value was treated as data rather than code may disprove the predicted effect. This discipline prevents confirmation bias and makes a failed replay useful rather than inconclusive.

Separate weakness, impact, and severity
Record three fields independently:
- Weakness: the suspected technical condition, such as an authorization check missing on one object.
- Demonstrated impact: the access or state change actually observed, such as reading another test user’s record.
- Severity assessment: the risk after considering privileges, exposure, affected assets, exploit reliability, and business consequences.
An AI-generated severity label is only an assertion. Do not let it determine the test plan.
Reproduce independently when it is safe
OWASP APTS advisory practice APTS-RP-A01 recommends independent replay for reproducible effects, with an out-of-band signal that the discovering agent does not control. The appendix describes this as advisory or candidate material, not a final mandatory standard, but the principle is strong: use a separate tester or harness and evidence from outside the generating agent’s own output.
Build an independent harness
- Start a fresh test process or use a separate tool and operator. Do not rely on the agent’s conversational state, hidden variables, cached responses, or automatically supplied credentials.
- Reconstruct only the inputs and preconditions necessary to test the claim. Use a disposable environment, synthetic records, and a test account where possible.
- Establish a safe control case, such as a benign input or an account that should not have access, so you can distinguish the alleged effect from normal behavior.
- Capture raw requests and responses, timestamps, response codes, server-side logs, database changes, audit events, and network traces that the agent cannot rewrite.
- Repeat enough to establish reliability, but stop if the effect is destructive, irreversible, rate-limited, or likely to expose protected information.
Prefer an out-of-band confirmation
Use a signal generated by the target or an independently controlled system: a controlled callback to a monitored endpoint, a test-side state change, a server audit event, or a record created in a disposable database. A screenshot produced by the same agent, a message it generated, or a value copied from its own context is not out-of-band confirmation.
Out-of-band evidence must still be interpreted carefully. Verify that the callback came from the authorized target and test action, not from a background job, a browser extension, a scanner, or another environment. Preserve timestamps, request identifiers, and network metadata sufficient for a human to correlate cause and effect.
When replay is unsafe or impossible
Static examination is a fallback, not an equal substitute for replay. OWASP’s APTS guidance describes artifact analysis as weaker evidence when a live, safe reproduction cannot be performed.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Inspect the artifacts for internal consistency
- Check whether the supplied request actually reaches the claimed target and code path.
- Look for hard-coded “success” strings, fabricated response bodies, timestamps that predate the test, or output that appears without a corresponding network or process trace.
- Compare stack traces, line numbers, package versions, and configuration values with the deployed artifact.
- Check whether the claimed result is compatible with the method. For example, a remote exploit claim that has no target interaction needs an exceptional explanation.
- Ask a maintainer or system owner to verify behavior in a controlled environment when your team cannot access one safely.
Label the outcome as unverified when the available artifacts cannot establish the effect. “Not reproduced” and “not testable under current authorization” are different dispositions from “false.”

Match the evidence to the vulnerability class
The raw evidence must demonstrate the named weakness, not merely an unusual response.
Injection claims
For an SQL injection claim, look for behavior showing that input changes database interpretation, such as a controlled condition that alters the result or a test-only database effect. A generic error message, reflected input, or a response containing the submitted characters does not by itself prove SQL injection.
Cross-site scripting claims
For an XSS claim, demonstrate script execution or the claimed DOM manipulation in the affected context. A string echoed in an HTML response without execution may be output encoding behavior, a safe text node, or a non-exploitable reflection.

Use separate identities and records. Show that an account can read or change an object outside its permitted scope, then verify the server-side authorization decision and the exact data or state exposed. A client-side control being hidden is not proof that the server accepts unauthorized access.
Cryptography and secret-exposure claims
Identify the secret, its intended audience, and the trust boundary. A public API key deliberately embedded in a client application may be documented and restricted by design. Demonstrate what an unauthorized party can actually do with it rather than treating every key-shaped string as a credential compromise.
Rule out intended or documented behavior
An observed behavior can be surprising without being a vulnerability. OWASP’s examples include public API keys intended for client-side use, intentionally broad CORS headers, and documented public endpoints. Check authoritative product documentation, configuration references, deployment settings, and owner explanations before assigning a flaw.

Intent does not automatically make dangerous behavior acceptable. If a public endpoint exposes more data than its documentation promises, or a supposedly restricted key has excessive permissions, document the mismatch and assess the resulting impact. The question is whether the implementation violates a security expectation or control, not whether the behavior looks unusual.
Reassess impact instead of accepting the severity label
Describe the demonstrated consequence in operational terms:
- What authentication, privilege, network position, or user interaction is required?
- Which asset, tenant, account, record, or process is affected?
- Can the tester read, alter, delete, execute, impersonate, or persist?
- Is the result reliable, repeatable, and scalable?
- Does exploitation cross a trust boundary or remain within the attacker’s existing permissions?
- What evidence supports confidentiality, integrity, and availability impact?
OWASP advises that a Critical classification requires commensurate evidence, with remote code execution, authentication bypass, and mass data exposure as examples of effects that would need direct support. A mismatch between the reproduced effect and the proposed severity is a reason to reclassify or send the case to human review; it is not proof that the underlying behavior is harmless.
Use a severity-evidence table
| Question | Evidence to record | Why it matters |
|---|---|---|
| Reachability | Network position, endpoint, account, and preconditions | Determines who can attempt the action |
| Reliability | Successful runs, failed runs, rate limits, and environmental dependencies | Separates a repeatable exploit from a fragile anomaly |
| Access gained | Exact records, functions, privileges, or process control obtained | Anchors impact to an observed result |
| Scale | Whether one object, one tenant, or many assets are affected | Prevents a local finding from being overstated as mass compromise |
| Recovery | Reversibility, cleanup, and residual state | Guides safe testing and remediation urgency |
Compare findings by evidence and exposure
When several AI-generated reports compete for attention, rank them using comparable facts rather than prose quality.
| Decision axis | Stronger position | Weaker position |
|---|---|---|
| Evidence strength | Independent replay plus a target-side or other out-of-band signal | Agent narrative, screenshot, or static artifact only |
| Authorization | Target and test method explicitly permitted | Permission unclear, expired, or outside policy |
| Exposure | Internet-facing or otherwise reachable by the relevant threat | Isolated, disabled, or reachable only under narrow conditions |
| Demonstrated impact | Observed unauthorized access, execution, or state change | Severity inferred from a label or generic pattern |
| Threat context | Listed as a known exploited vulnerability (KEV) or readily automatable | No evidence of exploitation or automation potential |
| Uncertainty | Safe to repeat and independently recheck | Irreversible, unsafe, or dependent on missing artifacts |
CISA’s August 26, 2026 vulnerability-review summary identifies exposure, KEV status, potential exploit automation, and technical impact as prioritization factors, and calls for prioritizing KEVs and exposed assets. These factors help order response after technical assessment; they do not replace verification.

Verified
Use this when the authorized test reproduced the claimed behavior, the evidence matches the vulnerability class, and the impact is supported by observed results. Record scope, preconditions, reliability, affected versions, and containment or remediation actions.
Flagged for human review
Use this when the claim is plausible but replay is unsafe, authorization is incomplete, artifacts conflict, intent is unclear, or severity and impact do not align. Name the decision-maker, the missing evidence, and the deadline or trigger for review. OWASP recommends erring toward human review for uncertain cases.
Rejected
Use this when evidence contradicts the claim, the behavior is documented and expected, or the alleged vulnerability cannot be reproduced under an authorized, adequately controlled test. Preserve the failed test and rationale so the same report is not reopened without new evidence.

Communicate through a defined disclosure process
Keep an auditable chain from intake to remediation: original report, authorization check, test plan, raw observations, analyst interpretation, disposition, owner, notification, and closure evidence. Restrict sensitive artifacts to people who need them, and avoid sending exploit details to parties who are not authorized to receive them.
NIST Special Publication 800-216, Recommendations for Federal Vulnerability Disclosure Guidelines, published in May 2023, recommends a federal framework for receiving, assessing, managing, and communicating suspected vulnerability disclosures. Its scope covers software, hardware, and digital services under federal control. It is useful process guidance for other organizations, but it is not a universal rule for every private system.
NIST’s Generative AI Profile supports reviewing generated code for validity and safety and evaluating AI-system security and resilience. It does not prescribe the specific replay-and-out-of-band workflow for vulnerability reports, so use it as complementary AI-governance guidance rather than as a substitute for technical verification.
A practical decision record
Use a concise record that another engineer can audit without reopening the AI conversation:
Claim: [one-sentence observable prediction]
Target and version: [authorized asset and build]
Authorization: [policy, contract, or owner approval]
Safe test boundary: [allowed actions and stop conditions]
Independent method: [harness, tester, account, and control case]
Observed evidence: [raw response, log, callback, or state change]
Vulnerability-class match: [what proves the named class]
Demonstrated impact: [access or consequence actually observed]
Disposition: [verified, human review, or rejected]
Priority factors: [exposure, KEV status, automation potential, impact]
Owner and next action: [person, remediation or communication step]
Do not fill a field with an assumption. Mark it as unknown and route the report for review when the missing fact affects authorization, safety, validity, or priority.
Fast checklist before anyone acts
- Is the target within our authority, and is this exact test allowed?
- Have we preserved the original report and raw artifacts?
- Can we state the predicted observable effect in one falsifiable sentence?
- What result would disprove the claim?
- Can a separate tester reproduce it safely with a control case?
- Is there evidence outside the AI agent’s control?
- Does the evidence demonstrate the named vulnerability class?
- Could the behavior be documented or intentionally public?
- What access or consequence was actually demonstrated?
- Does the severity match that demonstrated impact?
- How do exposure, KEV status, exploit automation, and technical impact affect priority?
- Is the disposition recorded with an owner and next action?
Bottom line for security teams
Act on the evidence, not the confidence of the prose. An AI report can accelerate triage by proposing a precise hypothesis, but authorization, independent replay, out-of-band confirmation, vulnerability-class matching, impact assessment, and human review determine whether the hypothesis becomes a verified finding. When safe reproduction is impossible, preserve the uncertainty explicitly and use the defined disclosure process rather than treating an untested claim as either a confirmed breach or a harmless false positive.
