A digital twin finds design failures before deployment when it is built around a specific engineering decision, exercised with controlled faults, and checked against requirements and measured behavior. The practical loop is: define the failure question, identify hazards and failure modes, configure the twin, verify and validate it, inject faults, measure detection and recovery, then feed the findings back into the architecture and test plan.
A twin is not a guarantee of field safety. It is evidence within a documented operating envelope. NASA validation guidance frames the essential questions as: “Does the system work as expected? How does the system respond to failures, faults, and anomalies?”
What fault injection adds to a digital twin
A digital twin is a computational representation of a physical system, its environment, and the interactions that matter to a defined use case. NIST describes uses including monitoring, diagnosis, prediction, optimization, and decision support. Fault injection deliberately creates abnormal conditions in that representation so engineers can observe how the design detects, contains, degrades, fails over, and recovers.
The useful unit of work is not “build a perfect copy.” It is a question such as:

- Will a controller recognize a sensor dropout within its detection deadline?
- Does a redundant communication path take over before a safety limit is crossed?
- Can a thermal excursion propagate into a shutdown or unsafe state?
- What happens when corrupted time data, memory, or software state crosses an interface?
Each experiment should connect an injected condition to a requirement, hazard control, architecture decision, or unresolved test gap. A precise-looking result from an uncalibrated model is not useful evidence.
1. Define the decision and the failure boundary
Start by writing the decision the experiment must support. State the operating regime, the components and interfaces in scope, the environmental assumptions, and what counts as an acceptable degraded state. Record the relevant requirements, hazards, timing limits, resource margins, and recovery objectives before building scenarios.
Specify the question in measurable terms
- Trigger: the fault, anomaly, or environmental condition to introduce.
- Detection requirement: the maximum time or confidence level for recognizing it.
- Containment boundary: the functions, data, power domains, or networks that must remain unaffected.
- Degraded behavior: the reduced capability that is still safe and useful.
- Recovery objective: the required failover, restart, repair, or return-to-service behavior.
- Evidence: the signals, traces, and state transitions that will prove pass or fail.
For example, “test sensor failure” is too broad. “With the primary pressure sensor stuck at its last value during the high-load regime, the controller shall flag the disagreement within 200 milliseconds, reject the bad channel, and maintain pressure below the defined limit using the redundant sensor” is an actionable boundary.
Map stakeholders and operating regimes
Include operators, maintainers, safety engineers, cybersecurity staff, and anyone who supplies or consumes the affected data. A fault that appears harmless in nominal operation may be dangerous during startup, peak load, communications loss, maintenance, or an emergency transition. Define those regimes explicitly; a twin validated only at steady state cannot support claims about transients.
2. Prioritize the failures worth modeling
Do not attempt to inject every conceivable defect. Use failure-mode and effects analysis (FMEA or FMECA), fault-tree analysis, component failure-rate estimates, physics-of-failure reasoning, field history, and previous test results. NASA systems-engineering guidance calls for combining failure likelihood with consequences and modeling hazard controls and fault-management behavior.
Rank candidate scenarios by severity, likelihood, detectability, coupling to other functions, and recovery consequence. A low-probability event can still deserve early testing if it defeats a critical hazard control or creates a common-cause failure.

| Prioritization factor | Question to answer | Use in the campaign |
|---|---|---|
| Severity | What is the worst credible effect on people, mission, data, or equipment? | Put catastrophic or irreversible outcomes at the front of the queue. |
| Likelihood | How often can the initiating fault occur in the expected environment? | Use component estimates, field data, and environmental stress evidence. |
| Detectability | Can existing monitors distinguish the fault from normal variation? | Favor scenarios that expose blind spots or false confidence. |
| Coupling | Can the fault cross an interface or trigger a shared dependency? | Include bus, power, timing, data, and common-resource interactions. |
| Recovery consequence | What is lost if detection, containment, or failover is late? | Measure deadline misses and degraded-state duration, not only alarms. |
3. Choose the twin boundary and fidelity
Select the least complex model that answers the question with known uncertainty. Fidelity is valuable only when it improves the decision. A highly detailed but opaque model can be less trustworthy than a simpler model that is calibrated, observable, and tested across the relevant envelope.
| Approach | Best use | Strengths | Trade-offs |
|---|---|---|---|
| Software-only twin or emulation | Early architecture, control logic, software fault handling, large scenario sweeps | Fast, isolated, repeatable, scalable, and inexpensive to run | May omit physical timing, analog behavior, sensor noise, and hardware-specific effects |
| Physics-based model | Thermal, mechanical, electrical, fluid, or environmental failure mechanisms | Explains causal behavior and supports extrapolation when assumptions are valid | Requires parameter calibration and can be computationally expensive |
| Data-driven model | Detection, prediction, and diagnosis from observed operating data | Can capture complex patterns without explicitly modeling every mechanism | Can fail outside its training distribution and may be difficult to explain |
| Hardware-in-the-loop (HIL) | Interface timing, real controllers, buses, I/O, and hardware-specific behavior | Provides stronger timing and integration realism | Costs more, scales more slowly, and can be harder to isolate for hazardous cases |
| Hybrid twin | Questions spanning software, physics, and real hardware | Combines targeted realism with simulation scale | Requires careful synchronization, interface contracts, and provenance across models |
Set the boundary around the causal path of the failure. Include components, interfaces, timing, environmental inputs, and human or operational interactions only when they can change the outcome. If a thermal limit drives the decision, an abstract thermal load may be sufficient for a software campaign; if sensor conversion latency is the issue, the timing and I/O path must be represented accurately.
4. Build and document a trustworthy twin
NIST treats requirements formulation, data management, model development, validation, and result analysis as parts of digital-twin implementation. NASA’s proposed guidance emphasizes verification, validation, and uncertainty quantification (VVUQ), representation of the physical asset in its expected performance environment, calibration to observations, and bidirectional information exchange where the use case requires it.
Verify the implementation
- Check equations, state transitions, units, coordinate systems, limits, and conservation rules against their specifications.
- Test interfaces for message schemas, ranges, timing, initialization, reset, and error handling.
- Use boundary-value, nominal, and deliberately invalid-input tests.
- Review the fault injector itself: confirm that a commanded bias, dropout, delay, or corruption is actually applied at the intended signal and time.
Validate behavior against evidence
Compare outputs with measured or accepted reference data in every operating regime used for the decision. Record calibration data, parameter sources, version identifiers, assumptions, uncertainty, and conditions outside the model’s valid envelope. Validation is behavioral: a model can implement its equations correctly and still represent the real asset poorly.
Quantify uncertainty
Carry uncertainty into the pass/fail decision. Identify uncertain parameters, input distributions, sensor errors, timing jitter, unmodeled interactions, and numerical limitations. Use sensitivity studies or bounded cases to determine whether the conclusion changes when those values move. If a predicted recovery margin disappears under plausible uncertainty, report the scenario as unresolved rather than passing it on a point estimate.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Make provenance and synchronization explicit
Version the model, plant data, calibration set, fault scripts, software binaries, configuration, and random seeds. When the twin exchanges information with a physical asset, define update direction, latency, ownership, and access controls. A result that cannot be reproduced from its recorded inputs is not a durable engineering artifact.

5. Design a fault-injection campaign
Begin with single faults to establish causal behavior. Add combinations only after the single-fault responses are understood, then target common-cause and interaction effects. NASA’s JSTAR software digital-twin work illustrates the range of independent testing areas possible in a software-only hardware emulation: failover, guidance-navigation-control faulting, bus faults, time-synchronization corruption, SDRAM fault injection, reset behavior, watchdog logic, and null-pointer impact.
Fault classes to consider
| Class | Examples | What to observe |
|---|---|---|
| Sensor and measurement | Bias, drift, dropout, stuck value, noise increase, range violation, intermittent signal | Residual checks, voting, alarm latency, channel isolation, control stability |
| Actuator and plant | Stuck actuator, saturation, reduced authority, leakage, delayed response | Command-versus-response mismatch, safety limits, alternate control mode |
| Communication and data | Message loss, duplication, corruption, delay, reordering, bus partition | Integrity checks, timeout behavior, containment, stale-data handling |
| Timing | Clock drift, time-sync error, jitter, deadline miss, scheduler starvation | Timestamp validity, ordering, control-loop deadlines, safe transition |
| Compute and memory | CPU overload, memory corruption, software exception, null pointer, watchdog expiry, reset | Fault containment, restart state, data integrity, watchdog and recovery logic |
| Power and thermal | Voltage sag, brownout, over-temperature, cooling loss, thermal runaway | Derating, shutdown sequencing, energy isolation, restart conditions |
| Network and environment | Bandwidth reduction, packet storm, degraded connectivity, vibration or external load | Resource margins, degraded service, operator notification, recovery time |
Use realistic injection points. A fault inserted after a safety monitor may test only the monitor’s reaction; the same nominal fault inserted at the sensor boundary may reveal whether the monitor itself can be bypassed. Record the injection location, onset, duration, magnitude, distribution, and whether the fault is permanent, transient, intermittent, or progressive.
Build combinations deliberately
Pair faults when a shared cause makes that combination credible: a time-sync error with message delay, a power dip with memory corruption, or a network partition during failover. Avoid arbitrary combinatorial explosion. The purpose of combination tests is to expose interaction and common-cause weaknesses that single-fault analysis cannot reveal.
6. Define observables and pass/fail oracles first
Write the expected behavior before executing a run. Without an oracle, a visually plausible trace is inconclusive.
- Detection latency: time from fault onset to diagnostic recognition.
- False alarms and missed detections: whether normal variation triggers the same response or a real fault remains hidden.
- Containment: which functions, interfaces, data domains, or power sections remain protected.
- State transition: the exact degraded, safe, standby, reset, or shutdown state reached.
- Failover time: elapsed time to select and stabilize the alternate path.
- Data integrity: whether corrupted, stale, duplicated, or partially written data is rejected.
- Safety constraints: limit violations, unsafe commands, thermal or structural margins, and deadline misses.
- Recovery: successful restart, resynchronization, repair, or return to normal service.
- Human factors: operator workload, alarm clarity, required intervention, and risk of incorrect action.
- Resource margin: CPU, memory, bandwidth, power, thermal, and storage headroom during the response.
Define tolerances and sampling rates that can actually resolve the requirement. A one-second log cannot prove compliance with a 200-millisecond detection limit. Preserve raw traces as well as processed metrics so disputed results can be replayed.
7. Run experiments safely and repeatably
- Freeze the configuration. Record model and software versions, parameters, plant data, fault script, random seed, operating regime, and acceptance criteria.
- Establish a clean baseline. Run the same scenario without injection to separate ordinary instability from fault response.
- Inject one controlled fault. Confirm onset in the log and, where possible, in an independent monitor.
- Capture the full response. Log inputs, outputs, internal states, alarms, state transitions, timing, resource use, and operator actions.
- Repeat for stochastic cases. Use fixed seeds for replay and additional seeds or bounded variations to test sensitivity.
- Classify the result. Mark pass, fail, blocked, or inconclusive with the evidence and uncertainty that justify the label.
- Escalate selectively. Move high-value scenarios from software-only simulation to HIL or representative integration testing when physical timing, I/O, or hardware behavior can change the outcome.
- Preserve replayability. Store traces, scripts, configuration, and environment details with the finding, not in a separate informal record.
Isolate hazardous scenarios in simulation or software emulation whenever possible. Do not inject destructive faults into operational equipment merely because the twin produced a reassuring result.

8. Analyze failures and close the engineering loop
For every failed or inconclusive scenario, trace the observation to the requirement, architecture element, hazard control, or test assumption it challenges. Distinguish among four causes:

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minute- Design weakness: the implemented control or redundancy cannot meet the requirement.
- Model weakness: the twin omits a mechanism, uses incorrect parameters, or operates outside its validated envelope.
- Test weakness: the injection point, timing, oracle, or instrumentation is inadequate.
- Requirement weakness: the expected behavior is missing, contradictory, or not measurable.
Correct the design or control, update the twin and uncertainty record, add a regression scenario, and rerun both the changed case and unaffected neighboring cases. NASA describes design validation as recursive: findings change the design, which changes the validation evidence required.
Can a digital twin replace hardware-in-the-loop testing?
No. It can replace some early and high-volume experiments, but it cannot establish hardware-specific timing and interface behavior that the model does not represent. The right choice depends on the failure question, not on a universal ranking of methods.
| Decision need | Prefer software-only twin when… | Escalate to HIL or integration when… |
|---|---|---|
| Scale | You need thousands of parameter combinations or long-duration runs. | A small number of scenarios have unusually high physical consequence. |
| Timing | Logical timing is sufficient and hardware jitter is out of scope. | Bus arbitration, interrupt latency, I/O conversion, or scheduler behavior can change the result. |
| Safety | The fault is hazardous, destructive, or expensive to reproduce physically. | Physical coupling, power behavior, thermal inertia, or mechanical response is decisive. |
| Observability | Internal states are needed and can be instrumented in the model. | Only real hardware exposes the interface, electromagnetic, or sensor behavior under test. |
| Traceability | Repeatability and rapid regression are priorities. | Certification or acceptance evidence requires representative hardware behavior. |
NASA JSTAR demonstrates the value of software-only hardware emulation: a flight computer is emulated, sensors and actuators are simulated, delivered flight-software binaries execute, and ground software can be integrated as-is. That approach can find software and interface defects early; it does not remove the need to validate physical effects that were intentionally abstracted.

- the intended decision and operating envelope;
- the physical components, interfaces, and behaviors represented;
- the observations used for calibration and validation;
- verification results for equations, code, timing, and interfaces;
- uncertainty sources, sensitivity results, and confidence bounds;
- known out-of-scope conditions and unsupported failure mechanisms;
- data lineage, model version, update process, and access controls;
- criteria for when a result must be confirmed with HIL or physical testing.
Require faster-than-real-time execution only when the defined failure-prediction use case needs it. Speed does not establish validity. Likewise, bidirectional connection to a physical asset is necessary only when the decision depends on live state exchange; otherwise it adds integration and cybersecurity exposure without improving the evidence.

Security, interoperability, and operational limits
NIST identifies interoperability, VVUQ, cybersecurity, trust, and workforce readiness as continuing digital-twin challenges. A connected twin may expose sensitive state, commands, control paths, or maintenance information. Apply least-privilege access, network segmentation, authenticated updates, provenance for incoming data, continuous monitoring, and a controlled process for model and software changes.
Interoperability failures can invalidate a campaign even when each model is individually sound. Define units, schemas, timestamps, quality flags, ownership, and error semantics at every interface. Preserve the distinction between simulated data, measured data, and inferred data in logs and dashboards.
Treat predictions as evidence inside a validated envelope, not proof that every untested real-world combination is safe. A twin can produce precise but wrong answers when calibration data is poor, assumptions are hidden, the environment is misrepresented, or a fault lies outside the modeled mechanisms.
A compact example: testing redundant sensor failover
Suppose a controller regulates a process using primary and backup temperature sensors. The engineering question is whether a stuck primary value can be detected and isolated before the process exceeds its thermal limit.
- Define the regime: startup, nominal load, and peak load; record the thermal limit and maximum allowed detection and failover times.
- Build the boundary: sensor models, conversion latency, controller logic, voting and plausibility checks, actuator response, thermal plant, alarms, and operator acknowledgment.
- Validate the nominal model against measured temperature ramps and controller traces in all three regimes.
- Inject a primary-sensor stuck value at several offsets, then test dropout, intermittent recovery, and a simultaneous communication delay.
- Measure detection latency, selected sensor, actuator commands, temperature margin, alarm behavior, failover duration, and return-to-normal logic.
- Run uncertainty cases for sensor noise, thermal parameters, and communication jitter.
- If the result passes in software, repeat the highest-risk timing and I/O cases with the real controller in HIL.
- Trace any miss to the diagnostic threshold, timing budget, sensor independence assumption, or thermal-control requirement; fix it and add the case to regression.
This example illustrates the central discipline: the fault is meaningful only because the boundary, oracle, uncertainty, and escalation path were defined before the run.
Methods that complement fault-injection campaigns
Fault injection is one part of a broader reliability argument. NASA’s 2025 failure-prediction work identifies FMEA/FMECA, fault-tree analysis, physics-of-failure, remaining-useful-life analysis, reliability-centered maintenance, condition-based maintenance, and prognostics and health management as complementary approaches. Use those methods to select scenarios, interpret consequences, and decide which signals support prediction rather than merely post-failure diagnosis.
No authoritative cross-industry source establishes a universal failure-detection percentage for digital twins. Results depend on the system, model boundary, calibration, fault set, and acceptance criteria. Report those conditions with every metric.
