Erectile dysfunction (ED) medicines can be used safely by many older men, but the safe choice depends on your heart health, other conditions, kidney and liver function, and every medicine or supplement you take. Have a clinician review those factors before your first dose or any dose change. Age itself is not an automatic reason to avoid treatment; it does make medication interactions and previously unrecognized cardiovascular or metabolic disease more likely.
The central safety rules are straightforward: never combine a PDE5 inhibitor with nitrate heart medicines unless a clinician has explicitly directed you, do not exceed the prescribed schedule, avoid unverified online pills, and get urgent help for an erection lasting more than four hours, sudden vision or hearing loss, chest symptoms, fainting or a suspected overdose.
Why a medication review matters more with age
Recurrent ED can occur alongside cardiovascular disease, high or low blood pressure, diabetes, high cholesterol, hormonal problems or side effects from another medicine. Treating the erection problem without checking those issues can miss a condition that also needs attention. Mayo Clinic patient guidance states: “Before taking any medicine for erectile dysfunction, get your healthcare professional’s OK.”
Older adults are also more likely to use several prescriptions at once. A combination that is acceptable for one person can lower blood pressure too much or change how an ED medicine is broken down in another. Bring a current list of prescriptions, over-the-counter products, vitamins and supplements to the appointment. Include the strength, how often you take each product and when you last took it.

What the review should cover
- Heart disease, chest pain, previous heart attack or stroke, blood-pressure problems and any limits your clinician has placed on sexual activity.
- Diabetes, cholesterol problems and possible hormonal causes of ED.
- Kidney and liver function, which can affect medicine selection and exposure.
- Prostate medicines, especially alpha-blockers, and every medicine for blood pressure.
- Nitrates, pulmonary-hypertension medicines, HIV medicines, antifungal drugs, antibiotics and other medicines that may interact.
- Previous reactions to ED medicines and any episode of prolonged erection, vision change, hearing change, severe dizziness or fainting.
The four main PDE5 inhibitor options
Sildenafil, tadalafil, vardenafil and avanafil are PDE5 inhibitors. They differ mainly in how quickly they may start, how long the treatment window lasts and how food or other medicines affect them. The figures below describe general guidance, not an individualized schedule.

Tadalafil’s longer possible window can provide flexibility when timing is uncertain. Sildenafil’s shorter, more planned use may suit someone who prefers an occasional dose, but its absorption can be delayed by a large or high-fat meal. Those are preference considerations, not reasons to switch without medical advice. Your prescriber should make the final choice after reviewing interactions, kidney or liver function, other conditions and your priorities.
How to take an ED medicine safely
1. Use a prescription and a clearly identified product
Take only the medicine and strength supplied or authorized by a licensed clinician. Do not use a friend’s tablets, split or combine products on your own, or substitute a “natural” erection product. Different PDE5 inhibitors are not interchangeable tablet for tablet.
2. Confirm the timing before you leave the appointment
Ask when to take the dose in relation to sex, whether food changes the timing and what to do if you miss the intended occasion. Sildenafil and vardenafil are generally taken about an hour beforehand; avanafil may work in about 30 minutes depending on dose; tadalafil can be used with or without food and may provide a window of up to 36 hours. These are general characteristics, not permission to take an extra tablet when the first dose seems slow.
3. Follow the prescribed frequency exactly
Do not repeat a dose because the first attempt was disappointing, and do not combine two PDE5 inhibitors. For tadalafil used for ED, never exceed one dose in 24 hours. If the medicine does not work as expected, record the dose, timing, meal and any alcohol or other medicines involved, then discuss the result with the prescriber rather than self-adjusting.

4. Account for food, alcohol and grapefruit
A large or high-fat meal can delay sildenafil absorption. Excess alcohol can increase the chance of dizziness or a blood-pressure drop and can make sexual function less predictable, so avoid drinking heavily around a dose. Grapefruit or grapefruit juice can alter the way some medicines are processed; follow the warning in your specific medicine guidance and ask a pharmacist if you are unsure.
5. Keep a written record
Write down the product name, strength, time taken, any other medicines used that day and any symptoms. This is especially useful when several clinicians prescribe medicines. Keep the list accessible and update it whenever a prescription changes.

Interactions that require a professional check
Nitrates: the absolute rule
Do not combine a PDE5 inhibitor with nitroglycerin (also called glyceryl trinitrate), isosorbide mononitrate or isosorbide dinitrate used for angina. The combination can cause a dangerous fall in blood pressure. Do not decide that a small dose, a long interval or a “mild” nitrate makes the combination acceptable. If you use a nitrate, tell the ED prescriber before receiving any prescription. If chest pain occurs after taking an ED medicine, seek emergency care and tell responders exactly what you took and when; do not take a nitrate unless emergency clinicians who know about the ED medicine direct you.
Alpha-blockers and prostate treatment
Alpha-blockers can lower blood pressure, and combining them with a PDE5 inhibitor may require a different plan or closer supervision. This includes medicines prescribed for urinary symptoms from an enlarged prostate. Never stop or rearrange a prostate medicine on your own. Give the clinician the exact name and dosing time so the interaction can be assessed.
Other blood-pressure medicines
Some antihypertensives may be used with a PDE5 inhibitor, while others or particular combinations may need adjustment. The safe answer depends on the complete list, your blood pressure and your kidney or liver function. A pharmacist or prescriber should check the combination before the first dose.

CYP3A4 inhibitors and specialist medicines
Medicines that inhibit the CYP3A4 enzyme can increase PDE5 inhibitor levels. A professional interaction check is particularly important with some HIV medicines, antifungal medicines and antibiotics. Pulmonary-hypertension drugs also need review because they can overlap with the same blood-pressure effects. Bring the actual containers or an up-to-date medication list rather than relying on memory.
Health conditions that change the safety decision
Cardiovascular symptoms and exercise tolerance
ED and cardiovascular disease can occur together. A clinician should decide whether sexual activity and an ED medicine are appropriate when you have chest symptoms, significant blood-pressure problems or a history of cardiovascular events. Do not use an ED tablet to test whether your heart can tolerate exertion.
Diabetes, cholesterol and hormonal problems
Diabetes and high cholesterol can contribute to ED and also affect cardiovascular risk. Hormonal problems may need separate assessment. An ED prescription should be part of an overall health review rather than a substitute for evaluating these causes.
Kidney and liver function
Reduced kidney or liver function can change how much medicine remains in the body. Tell the prescriber about diagnosed disease, abnormal test results and dialysis. Do not infer a safe dose from a relative or friend’s prescription.
Polypharmacy and changing prescriptions
An interaction check is needed again whenever a medicine is added, stopped or changed. This includes short courses of antibiotics or antifungals and medicines obtained from another clinic. Keep the ED prescriber, primary-care clinician, cardiologist and pharmacist informed of the same current list.

When oral tablets are unsuitable
If oral medicines are ineffective or inappropriate, Mayo Clinic describes other treatments, including self-injection therapy with alprostadil. This is not a do-it-yourself alternative to tablets. A clinician must teach the injection technique, choose a safe dose and explain how to handle local complications and a prolonged erection.
Ask for written instructions on storage, injection sites, disposal of needles and what to do if the erection lasts too long. Do not increase the injection dose because an earlier dose seemed weak, and do not combine injection therapy with an oral ED medicine unless the treating clinician has specifically prescribed that plan.

An erection lasting more than four hours
An erection lasting longer than four hours may be priapism and needs urgent assessment. This threshold is cited by Mayo Clinic and the NHS (2026). Do not wait for it to resolve overnight and do not take another ED dose. Contact emergency services or go to an emergency department, and tell staff the medicine name, strength, time taken and any other ED or injection treatment used.
Sudden vision or hearing loss
Sudden loss or major change in vision or hearing after an ED medicine is an emergency symptom. Stop trying additional doses and obtain urgent medical care. Bring the packet or a photograph of the label if available.

Severe dizziness, fainting or chest symptoms
Severe dizziness, fainting, chest pain or other serious chest symptoms require urgent evaluation. Sit or lie down while arranging help if you feel faint, avoid driving yourself and tell responders that you used an ED medicine. If you normally carry nitrate medicine for angina, do not take it after an ED dose unless emergency clinicians who know the timing explicitly instruct you.

Suspected overdose
Taking more than prescribed, repeating a dose too soon or mixing PDE5 inhibitors warrants immediate advice from emergency services or a poison-information service. Do not try to counteract the dose with alcohol, another medicine or exercise. Have the package and a complete medication list ready.
How to avoid counterfeit ED pills online
Unverified marketplaces and products marketed as “herbal Viagra” can contain undeclared prescription ingredients. The FDA reported on May 29, 2026, that a product called Erectus Plus contained hidden sildenafil and tadalafil. A label that says “herbal,” “all natural” or “works instantly” does not prove that the contents or strength are known.
Verify the pharmacy before paying

- A working telephone number.
- Access to a licensed pharmacist for questions.
- Appropriate licensing for the jurisdiction in which it operates.
A legitimate service should obtain a prescription or arrange a clinician evaluation rather than selling an ED tablet with no health questions. Avoid sites that hide their address, refuse pharmacist contact, sell several “sexual enhancement” products in one bundle or promise a guaranteed result. Do not assume that a familiar-looking logo, customer reviews or a low price confirms authenticity.
What to do if you already bought an unverified product
Do not take another dose until a clinician or pharmacist identifies the ingredients. Keep the packaging, lot number, receipt and website details. If you have symptoms such as chest pain, fainting, sudden vision or hearing loss, severe dizziness or an erection lasting more than four hours, seek urgent care and bring the product with you. Tell emergency clinicians that the contents may be undisclosed.
A practical appointment and use checklist
- Make a complete list of prescriptions, over-the-counter medicines, supplements and recent short-term treatments.
- Mark any nitrate, alpha-blocker, blood-pressure, pulmonary-hypertension, HIV, antifungal or antibiotic medicine for the clinician’s attention.
- Discuss heart symptoms, blood pressure, diabetes, cholesterol, hormonal concerns and kidney or liver disease.
- Ask which PDE5 inhibitor is being prescribed, when to take it, how food affects it and the maximum frequency.
- Confirm the plan if the first dose does not work; do not create your own second dose.
- Ask what symptoms require emergency care and write down the four-hour priapism threshold.
- Store the medicine in its original labeled container and keep the medication list current.
- If emergency care is ever needed, report the exact medicine, strength, time taken and any other ED treatment.
Safe use is an ongoing process: repeat the interaction review when your health or prescriptions change, not only when the ED medicine is first prescribed.
