Use ChatGPT as an information and organization aid—not as a therapist, diagnostician, medication adviser, or emergency service. It can help explain terms in plain language, organize what you want to tell a clinician, and suggest questions to discuss. It can also produce confident errors, miss crucial context, and respond poorly to a crisis.

Keep prompts minimally identifying, verify medical claims with a licensed professional, and move to human help immediately when there is imminent danger, suicidal intent, self-harm or violence risk, severe confusion, psychosis, or possible mania.

What ChatGPT can and cannot do for mental-health questions

ChatGPT generates text from patterns in its training and conversation. It does not examine you, observe nonverbal cues, review a complete medical history, or carry professional legal and clinical accountability. A fluent answer is not evidence that the answer is correct or appropriate for your situation.

It may also sound empathetic without actually understanding your circumstances. It cannot reliably distinguish a harmless worry from a symptom requiring urgent assessment, and it cannot check whether your account of an event is complete. Even a carefully worded reply can omit a medical condition, medication interaction, trauma history, substance use, or environmental danger that would change a clinician’s judgment.

Reasonable, low-stakes uses

  • Ask for a plain-language explanation of a term or treatment concept, then verify it with a clinician.
  • Turn diary notes into a concise list of questions for a licensed professional.
  • Request a neutral summary of information you provide.
  • Brainstorm low-risk next steps such as improving a sleep routine, contacting someone you trust, or scheduling an appointment.
  • Ask it to identify uncertainty, offer several possibilities, and flag when professional review is needed.
  • Prepare for an appointment by organizing a timeline of symptoms, questions, and practical concerns such as sleep, work, school, or side effects.
  • Rewrite a message to a clinician in clear, neutral language without asking the model to interpret the symptoms.

These are prompts for reflection and preparation, not treatment instructions. You remain responsible for deciding whether to act, and a clinician should make decisions that require diagnosis, examination, or knowledge of your full history.

Uses to avoid

  • Do not ask ChatGPT to diagnose a disorder or declare that you are safe.
  • Do not use it to choose, start, stop, or change a medication or dose.
  • Do not treat a conversation as psychotherapy or as a replacement for a licensed professional.
  • Do not ask it to manage an immediate suicide, self-harm, violence, psychosis, mania, or severe-confusion emergency.
  • Do not use a reassuring answer to postpone an appointment, emergency evaluation, or prescribed care.

What to do in a crisis

If you might hurt yourself or someone else, have suicidal intent, are experiencing severe confusion, psychosis, or possible mania, stop using the chat as the solution. Move to real-world support now. In the United States, the American Psychological Association advises calling 911, texting or calling 988 (the Suicide and Crisis Lifeline), or going to the nearest emergency department. Elsewhere, call your country’s emergency number or crisis line, or go to an emergency department.

Immediate steps

  1. Put physical distance between yourself and weapons, medications, or other means of harm if you can do so safely.
  2. Tell a trusted person plainly that you may not be safe and ask them to stay with you or help you reach care.
  3. Call emergency services or a crisis line. If speaking is difficult, show someone this instruction or send a short message asking them to call for you.
  4. Follow the responder’s directions and remain with another person until professional help takes over.

If you are unsure whether the situation is “serious enough,” treat uncertainty as a reason to contact a live responder rather than to keep testing the chatbot. A crisis line can ask direct safety questions and help connect you with local care; a chatbot cannot reliably assess your surroundings, access to means, or ability to stay safe.

OpenAI describes de-escalation and safety-planning work, but its trusted-contact feature is explicitly not an emergency service, crisis-response system, or substitute for mental-health care. OpenAI also states that it does not share ChatGPT conversations with crisis lines and that crisis-line counselors cannot view an OpenAI account. That statement concerns access by crisis lines; it does not make a consumer chat a clinical record or create clinician-level confidentiality.

How to write a safer prompt

State the boundary you want before describing the issue. A bounded request makes it less likely that you will mistake a fluent answer for a diagnosis. Give only the context needed for the task, and ask the model to show what it does not know.

A general-information prompt

Give general information about [term or symptom]. List what is uncertain, avoid diagnosing me, and suggest questions I can ask a licensed mental-health professional.

A clinician-preparation prompt

Turn these notes into a short timeline and a list of questions for my appointment. Keep my wording neutral, mark anything that needs clarification, and do not infer a diagnosis.

A source-checking prompt

Summarize only the information in the text below. Separate what the source states from your explanation, identify missing context, and list claims I should verify with a clinician.

A coping-ideas prompt

Suggest several low-risk, non-medical coping options I could discuss with a professional. Explain possible limitations and tell me when to stop experimenting and seek care.

A safer follow-up sequence

  1. Ask for a brief, general explanation rather than a personalized conclusion.
  2. Request at least two plausible explanations and the information a clinician would need to distinguish them.
  3. Ask which parts require an examination, testing, or professional history.
  4. Turn the answer into questions for a licensed professional, not a treatment plan.
  5. Stop if the conversation becomes repetitive, frightening, or more important than contacting a person.

Ask for several possibilities rather than a single label. Request uncertainty and source links when appropriate, and verify important claims independently. If an exchange turns into repeated reassurance seeking, compulsive checking, emotional dependence, or increasing isolation, end the session and contact a person you trust.

How to protect privacy

Share the minimum information needed for the task. Remove or generalize details that could identify you or another person.

  • Names and usernames
  • Home, work, or school addresses
  • Dates of birth and exact dates linked to identity
  • Phone numbers, email addresses, and other contact details
  • Employer, school, or small-community identifiers
  • Detailed combinations of rare events that could identify someone

Replace specifics with broad descriptions, such as “a mid-sized city” or “a recent appointment.” Do not paste another person’s private messages or medical records unless you have a legitimate reason and have removed identifying information. Screenshots can contain names, notifications, file names, or location clues even when the visible text seems harmless.

Check account controls before sensitive discussions

Review the current data-controls and history settings in your ChatGPT account before entering highly sensitive material. Product settings and retention practices can change, so read the controls shown in your account rather than relying on an old guide. Do not assume a consumer chatbot has the privacy protections that apply to a clinician; the APA notes that many wellness applications are not covered by health-care privacy laws.

Privacy is not only about your own identity. Consider whether a story could identify a partner, child, patient, colleague, or family member. Generalize ages, locations, dates, and occupations when they are not necessary. If a clinician needs exact information, provide it directly through the clinician’s approved channel instead of using a chatbot as a permanent record.

How to evaluate an answer before acting

  1. Identify the claim. Is the response giving general education, or is it telling you that you have a condition or should change treatment?
  2. Check the uncertainty. A responsible answer should acknowledge missing history, alternative explanations, and the need for professional review.
  3. Look for unsafe instructions. Stop if it recommends changing medication, delaying urgent care, concealing risk, or treating a crisis as resolved.
  4. Verify externally. Compare medical claims with information from a licensed clinician or a reputable health authority.
  5. Choose the right human contact. Use a clinician for assessment and treatment, a trusted person for immediate support, and emergency or crisis services for imminent danger.
  6. Check fit and scope. Advice that might be reasonable for one adult may be inappropriate for a child, an older person, someone pregnant, or someone with psychosis, mania, an eating disorder, or medication interactions.

When ChatGPT gives bad or alarming advice

Do not debate the model until it produces a reassuring answer. Save the conversation only if doing so helps a clinician understand what happened, then stop relying on it for that decision. Tell a licensed professional exactly what advice you received and what you were considering. If the advice concerns immediate safety, contact emergency or crisis services instead of seeking a second chatbot answer.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If an answer conflicts with a prescription, discharge plan, or clinician’s instruction, follow up with that professional or a pharmacist promptly. Do not “average” conflicting answers or choose the most confident-sounding one. A correction in a later message does not prove that the earlier answer was harmless.

ChatGPT compared with human and information resources

The right option depends on urgency and on whether you need accountable clinical judgment. These distinctions are practical boundaries, not a claim that every provider or product performs identically. When comparing any mental-health app or service, ask who is licensed, what evidence supports it, how it handles crisis escalation, what data it collects, whether it can observe context and nonverbal cues, how it communicates uncertainty, and whether it is suitable for your age and condition.

Resource Licensing and accountability Crisis escalation Context and nonverbal cues Best use in this guide
ChatGPT Not a licensed clinician; consumer product with no guarantee of clinical accountability Not an emergency service; do not rely on it for imminent danger Text only; cannot observe your body language or environment Organizing information, explaining terms, and preparing questions
Licensed therapist or other qualified clinician Professional licensing, ethical duties, and clinical responsibility vary by jurisdiction Can assess risk and arrange appropriate escalation within their role Can ask follow-up questions and observe speech, behavior, and nonverbal context Assessment, diagnosis, treatment, and medication decisions within scope
Crisis line or emergency service Live responders operate under the service’s emergency or crisis protocols Designed for immediate risk assessment and connection to urgent help Can ask direct safety questions and coordinate real-world support Suicidal intent, self-harm or violence risk, and other urgent danger
Search engine or health-information site Usually provides information rather than individualized clinical accountability May list crisis resources but does not assess your immediate safety No direct interaction or observation Finding official educational material to discuss with a professional

A search result is not automatically authoritative, and a therapy directory is not itself therapy. Check the clinician’s credentials, jurisdiction, availability, and privacy terms. Do not let convenience or a low-friction chat substitute for the accountability you need.

Who needs extra safeguards

Keep a human involved when the user is a child or adolescent, socially isolated, or vulnerable to anxiety, obsessive-compulsive symptoms, disordered thinking, psychosis, mania, suicidality, or eating-disorder symptoms. The APA specifically identifies adolescents, socially isolated people, and people vulnerable to anxiety, OCD, or disordered thinking as groups that can be especially vulnerable to chatbot interactions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

These safeguards are not a judgment about the person. They recognize that a highly available, agreeable system can encourage reassurance loops, reinforce unusual beliefs, or become a substitute for relationships and care. A parent, caregiver, school counselor, clinician, or trusted adult should know when the tool is being used for a serious concern.

For parents and caregivers

  • Do not make a child’s chatbot conversation the sole basis for deciding whether they are safe.
  • Help the young person contact a qualified professional and review urgent warning signs together.
  • Keep identifying information about the child and other family members out of prompts.
  • Ask what the child was told, without shaming them for seeking information, and correct unsafe advice with a professional.

For users noticing emotional dependence

End or shorten sessions if you are checking repeatedly for reassurance, treating the model as your only confidant, withdrawing from people, or feeling distressed when you cannot chat. Message a trusted person, arrange professional support, and use emergency resources if safety is in question. Setting a time limit, writing down your question before opening the app, and stopping after one useful summary can help prevent an endless loop; these are self-management ideas, not treatment.

What current safety evidence means

Safety claims are model- and evaluation-specific. OpenAI reports that it worked with more than 170 mental-health experts in 2025. It also reports that updates reduced responses falling short of desired behavior by 65–80%, and that GPT-5 reduced undesired responses by 39% compared with GPT-4o in an evaluation of 677 prompts. These are company-reported safety evaluations; they do not show that ChatGPT delivers effective therapy or meets clinical standards in every conversation.

An American Psychological Association article published in 2026 describing a 2025 analysis of six popular chatbots reported that none met clinical standards for crisis response in simulated scenarios. That finding is a warning against relying on chatbots during crises, not proof that every response is unsafe. It also does not replace a direct assessment by a qualified professional.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Model updates can improve some measured behaviors while leaving other failure modes unresolved. Results from a particular version, test set, or simulated prompt cannot guarantee what will happen in your conversation, especially when the situation is unusual, rapidly changing, or urgent.

A practical decision rule

  • Information or preparation: ChatGPT can help, provided you remove identifiers and verify important claims.
  • Persistent symptoms or a treatment decision: Contact a licensed professional; bring a ChatGPT-generated question list if useful.
  • Uncertainty about immediate safety: Tell a trusted person and contact a crisis line or emergency service rather than continuing the chat.
  • Imminent danger: Call emergency services or go to the nearest emergency department now.

Use the tool only inside the boundary that matches your situation. Its improving safeguards may reduce some harmful responses, but they cannot supply the clinical evidence, accountability, privacy protections, or real-world observation required for mental-health care.

Frequently asked questions

Can I use ChatGPT as my therapist?

No. It can help you reflect or prepare notes, but it is not a licensed therapist, cannot establish a therapeutic relationship, and cannot take responsibility for assessment or treatment. Use a qualified professional for ongoing care.

Is it safe to tell ChatGPT that I have depression or anxiety?

You can ask for general information, but share the minimum necessary detail, remove identifiers, and verify medical claims. Do not assume the conversation has clinician-level confidentiality or that the model can determine whether symptoms are mild, severe, or urgent.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can ChatGPT diagnose me?

No. A text response cannot replace a clinical interview, examination, history, or appropriate assessment. Ask it to organize questions and list possibilities for discussion instead of assigning yourself a diagnosis.

What if the answer seems wrong?

Stop before acting, save the relevant text if a clinician needs it, and check with a licensed professional or reputable health authority. Never change medication or delay urgent care because a chatbot sounded confident.

What if I might hurt myself?

Stop the chat and contact immediate human help. In the United States call 911, call or text 988, or go to an emergency department; elsewhere use your local emergency number or crisis line. Ask someone you trust to stay with you and follow the responder’s directions.

Should children or teenagers use it for mental-health concerns?

Keep a parent, caregiver, school professional, or clinician involved. Adolescents and socially isolated users can be especially vulnerable to harmful reassurance loops or disordered thinking. A chatbot conversation should never be the sole safety assessment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is a crisis-line conversation visible to OpenAI?

OpenAI states that it does not share ChatGPT conversations with crisis lines and that crisis-line counselors cannot view an OpenAI account. This describes access by crisis lines; it does not turn ChatGPT into a clinical record or guarantee health-care privacy.

Bottom line

Use ChatGPT for bounded education, organizing information, and preparing questions. Keep identifying details out, ask it to acknowledge uncertainty, and verify anything medically important. The moment the issue involves treatment decisions, serious symptoms, emotional dependence, or possible danger, put a licensed professional, trusted person, crisis line, or emergency service—not the chatbot—in charge.

Frequently Asked Questions

Can I use ChatGPT as my therapist?

No. It can support reflection and appointment preparation, but it is not a licensed therapist and cannot provide accountable assessment or treatment.

Can ChatGPT diagnose depression, anxiety, or another disorder?

No. Ask it to explain terms and organize questions for a licensed professional rather than assigning yourself a diagnosis.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should I do if ChatGPT gives unsafe advice?

Do not act on it. Stop the exchange, verify the claim with a licensed professional, and contact emergency or crisis services if immediate safety is involved.

What if I might hurt myself or someone else?

Stop using the chat as the solution. Call emergency services or a crisis line, go to an emergency department, and ask a trusted person to stay with you.

Is my ChatGPT mental-health conversation confidential like therapy?

Do not assume that. Remove identifying details and review current account controls; consumer chat services may not have clinician-level privacy protections.

Should a child or teenager use ChatGPT for mental-health questions alone?

No. Keep a trusted adult or qualified professional involved, and never use the chatbot as the sole safety assessment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The Bottom Line

ChatGPT is a useful organizer, not a mental-health professional. Use it for general explanations and question preparation, protect privacy, verify claims, and escalate any treatment need or safety concern to qualified human help.
