AI can help safety teams organize near-miss reports and find recurring patterns, but a pattern is not a reliable forecast of a specific accident. The practical value is earlier attention: use a signal to investigate conditions, hear from workers, and correct hazards before someone is hurt. The evidence supports this decision-support role, not a universal claim that AI can predict workplace accidents.

What can a near-miss report reveal?

A near miss is a close call that did not result in the same harm as an injury event. Its report can capture the task, location, equipment, conditions, and controls involved. Several reports describing similar circumstances may point to a hazard that deserves investigation—even when none produced an injury.

The Occupational Safety and Health Administration (OSHA) says that incidents, including close calls and near misses, indicate where hazards exist. It recommends investigating incidents to identify underlying causes, which may be multiple, rather than stopping at individual blame. The goal is to prevent recurrence by understanding what in the work system contributed. OSHA’s hazard-identification and assessment guidance explains this approach.

That makes near-miss reporting useful even without AI. The reports are clues about hazards already present, not proof that a particular future event will occur.

How can AI help identify workplace risk patterns?

AI can help turn free-text narratives into structured information: for example, assigning standard categories, grouping reports with similar descriptions, or surfacing recurring terms, tasks, locations, or equipment. Teams may also analyze reports alongside relevant site context, such as inspection findings or sensor and image data. The output is a lead for review, not a finding that a worker caused a problem or that an accident is certain.

There is a real-world example of AI assisting with categorization. OSHA’s Injury Tracking Application (ITA) documentation says an AI-powered auto-coder generates Occupational Injury and Illness Classification System (OIICS) codes for narrative fields, making cases easier to group and analyze. Standardized categories can support analysis; they do not by themselves validate a forecast of future events. OSHA’s ITA page describes the data and its limitations.

NIOSH has discussed natural-language processing as a possible way to review safety reports, and sensor data as potentially useful for exposure estimates and predicting adverse events. These are potential analytic uses, not evidence that one model can reliably forecast a specific accident across workplaces. How should a team turn reports into prevention?

A reporting system creates value only when reports lead to investigation and verified corrective action. OSHA advises grouping similar incidents, assessing their severity and likelihood, and prioritizing corrective actions. A workable sequence is:

  1. Make reporting accessible. Define what counts as a near miss, provide simple reporting channels, and explain how information will be used. Encourage reporting without treating a report as an admission of fault.
  2. Review reports promptly. Include people familiar with the task and site, such as workers, supervisors, and safety professionals. Record what happened and the conditions surrounding it.
  3. Investigate contributing causes. Ask whether equipment, tools, time, training, maintenance, procedures, or supervision played a role. Look for more than one cause where appropriate.
  4. Standardize without losing context. Use consistent categories to group reports, but retain the original narrative and relevant site or task details so that a category does not erase important differences.
  5. Look for repeated conditions and prioritize. Treat a model flag or cluster as a reason to check the work, not as a risk score on its own. Consider plausible severity and likelihood alongside local knowledge rather than ranking hazards by raw report counts.
  6. Assign and verify controls. Give corrective actions an owner and follow-up. Communicate lessons to affected workers, then check whether the change reduced exposure or removed the hazard.
  7. Evaluate any AI assistance. Compare its categories and alerts with human review and site outcomes. Document what it can and cannot show, monitor performance over time, and keep a person accountable for decisions.

OSHA’s investigation guidance puts the purpose plainly: “The purpose of an investigation must always be to identify the root causes (and there is often more than one) of the incident or concern, in order to prevent future occurrences.” The guidance also covers incident investigation planning and follow-up.

What data problems can make an AI signal misleading?

A model can only analyze the information it receives. Incomplete narratives, inconsistent categories, missing task context, and differences in reporting practices can hide patterns or make unlike situations look comparable. A rise or fall in report volume does not automatically mean danger rose or fell; interpret changes alongside reporting practices and what workers say is happening.

Public datasets need particular care. OSHA’s ITA data covers defined groups of establishments, not every workplace. OSHA cautions that the data may not generalize to the worker population, that some submitted errors remain unresolved, and that establishment counts are not validated by the agency. It also says rates in these data should not be used alone to label establishments the “most” or “least” dangerous. Use a dataset’s covered population, year, and known limitations when interpreting it; for operational decisions, local information and people familiar with the work matter. OSHA’s ITA documentation sets out these qualifications.

Which approach fits a workplace?

Organizations can improve near-miss analysis without immediately adopting predictive software. The right level depends on whether the team can reliably collect, investigate, and act on reports.

Approach What it can do What to establish
Reporting and investigation process Capture close calls, identify causes, and assign corrective actions. Accessible reporting, consistent forms, worker participation, and follow-up to verify controls.
In-house trend analysis Group and compare reports using standardized categories and site context. Data consistency, staff able to review patterns, and a clear route from a trend to investigation.
EHS or predictive-analytics software Support categorization and analysis across larger report sets, potentially with other data sources. Understandable alerts, appropriate access controls, security, human review, and evidence that fits the actual sites and workforce.

Before adding AI, ask whether the organization has enough staff and authority to investigate alerts and close corrective actions. Also ask why a signal appeared, how a human can verify it, whether data collection is proportionate, and whether workers can add context or challenge an inference. If a system cannot support those steps, an alert may create more noise than prevention.

What do adoption figures and case studies show?

The National Safety Council’s 2024 Safety Technology Survey spotlight says 20% of surveyed employers reported currently using predictive analytics, while 45% said they were testing or considering it. The survey covered 500 employers and 1,000 employees in safety-sensitive industries; those figures describe that survey population and year, not workplace adoption everywhere. The spotlight also identifies development cost, data security, human oversight, and bias as considerations. Read the NSC 2024 spotlight.

A 2023 NSC case-study report describes a JE Dunn/Newmetrix deployment using project, weather, staffing, image, and video data from 2016 to 2021. The report says 75% of recordable incidents occurred on the seven projects ranked highest for risk each week, and reports 350 additional safety conversations. These are vendor-associated, case-specific outcomes—not independent proof of general model accuracy or results another employer should expect. The NSC case-study report also describes a separate Los Alamos National Laboratory implementation in which 75 surplus applications were decommissioned and 1–2 FTEs’ worth of work was redirected to other initiatives. That account concerns its own implementation and is not a typical savings estimate.

How should workers’ rights and privacy be protected?

Workplace AI can affect privacy, autonomy, and fairness, and a system may be difficult to interpret. Explain what data is collected and why; limit access to people who need it; secure the information; and examine whether errors or effects differ across groups, jobs, or sites. Give workers a meaningful way to provide context or dispute a misleading inference. A human—not an automated score—should remain responsible for deciding what action to take.

NIOSH’s 2024 discussion of AI-enabled workplaces addresses trustworthiness, worker rights, and risk management: Exploring Approaches to Keep an AI-Enabled Workplace Safe for Workers. Its earlier summary captures the balance: “Although research gaps exist regarding the use and impact of AI on the workforce, AI offers both the promise to improve the safety and health of workers, and the possibility of placing workers at risk in both traditional and non-traditional ways.” — Jay Vietas, PhD, CIH, CSP, NIOSH, May 24, 2021.
