Contain the agent first, then reconstruct exactly what it did. Pause the affected workflow, fail closed if a safety dependency is unavailable, revoke or narrow its permissions, and block risky tools or destinations. Preserve run history and system logs before anyone cleans up. Once the action is contained, correlate the request, context, model session, tool calls, approvals, effective permissions, targets, outputs, and side effects. That evidence lets you distinguish a planning mistake from poisoned context, excessive authority, a failed control, or a defective integration.

Recover from a known-good state only after you know the blast radius. Rotate exposed credentials, reconcile every external side effect, and keep autonomous execution disabled or approval-gated until a human owner verifies the fix. The controls that prevent a repeat—least privilege enforced outside the model, high-impact approvals, protected audit trails, interruption and rollback, limits, and regular exercises—must be designed into the execution path rather than left to the agent’s judgment.

1. Contain the unsafe action before investigating

Investigation is easier when the agent can no longer change the environment. Treat an unsafe action as an operational incident even when the visible result appears small; an unauthorized permission change, data transfer, or deployment can create delayed effects.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Declare the incident. Record the time, agent and workflow version, tenant or environment, operator, task identifier, and the first harmful outcome you observed. Use a single incident record so later evidence has a common reference.
  2. Pause the narrowest useful scope. Disable the specific run, workflow, queue, or integration first. If you cannot isolate it confidently, stop autonomous execution for the affected environment. Do not allow retries to continue while you are still determining the cause.
  3. Fail closed when a safety dependency is missing. If risk classification, approval validation, policy lookup, identity verification, or audit logging is unavailable or returns an indeterminate result, block the action instead of allowing a best-effort continuation.
  4. Revoke or narrow authority. Disable the agent’s tokens, service accounts, delegated sessions, and active tool grants. Remove destructive commands, external destinations, and high-risk integrations from the execution path. Preserve enough read-only access for evidence collection through a separate human-controlled channel.
  5. Preserve volatile evidence. Do not delete run history, tool traces, queues, memory stores, affected-system logs, or snapshots. Capture short-lived data such as active sessions, process state, network connections, and approval records according to your incident procedures before restarting systems.

These actions implement the interruption, rollback, least-privilege, and fail-closed practices recommended in the OWASP AI Agent Security Cheat Sheet. Containment is not the same as recovery: it stops additional change while leaving the original state available for analysis.

2. Build a defensible timeline from protected logs

A model’s final message is only a summary. The authoritative record is the sequence that crossed the tool and infrastructure boundaries. Centralize records from the agent runtime, identity provider, policy and approval services, tool gateway, target systems, network controls, and operator consoles. Protect retention and access so an account involved in the incident cannot silently alter its own evidence.

Evidence to correlate

Timeline element Capture Question it answers
Trigger and context Original request, retrieved documents, memory entries, tool output supplied as context, and the policy version in force What information shaped the plan, and could untrusted content have redirected it?
Agent identity Agent name, model and runtime version, session ID, tenant, operator, and any delegation or hand-off chain Which execution instance acted, under whose authority, and with which software build?
Tool execution Every call, exact arguments, result, retry, timeout, exception, and response transformation What did the agent actually ask a tool to do, and what did the tool return?
Authorization state Effective permissions, token scopes, policy decisions, approval records, and their timestamps Was the action authorized at the moment it executed, or did a stale or bypassed control allow it?
Targets and effects Assets touched, records read or changed, configuration diffs, messages sent, deployments, data transfers, and downstream events What changed beyond the agent’s own log, and where did the change propagate?
Response activity Alerts, human interventions, stop signals, credential revocations, rollback attempts, and final recovery state When did containment begin, which actions succeeded, and what remains unresolved?

CISA guidance calls for logging user activity, administrator actions, network traffic, application logins, and system events; centralizing those logs; alerting on high-risk events such as privilege escalation; protecting logs from unauthorized access or deletion; and assigning incident-response roles. Apply the same standard to autonomous runs. Use synchronized timestamps and immutable or append-protected storage where possible, and record failed calls as carefully as successful ones.

3. Verify the executed chain, not just the agent’s explanation

Reconstruct the chain in order: request, context retrieval, plan or decision, policy check, approval, tool invocation, tool result, follow-up call, and side effect. Include retries and parallel branches. A harmless-looking answer can conceal a successful API call, while an alarming answer can describe an action that was rejected before reaching the target.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correlate identities and sessions

Join the runtime session to the service account or short-lived token used by the tool gateway, then to the target system’s audit record. A delegated call may involve several identities; retain the complete chain rather than attributing every effect to the model name alone. Verify the model and agent version because a configuration or prompt change can alter behavior without changing the surrounding workflow.

Compare requested and effective authority

Record both the permissions the workflow was intended to have and the permissions actually evaluated at execution time. Check for inherited roles, broad organization-level tokens, emergency overrides, cached approvals, expired policy decisions, and destinations that were not on the task’s allowlist. Authorization must be evaluated at the infrastructure boundary immediately before execution; a statement in the prompt is not evidence of permission.

Account for side effects outside the primary tool

Follow notifications, webhooks, queues, ticket updates, deployment pipelines, access-control changes, and data exports triggered by the first action. Record whether a tool transformed or redacted its output before returning it to the agent. This prevents a secondary system from being mistaken for the original cause.

4. Determine the blast radius

Containment tells you when change stopped; blast-radius analysis tells you what must be repaired. Start with the earliest unauthorized or incorrect action and walk forward through every dependent system. Then look backward for earlier reads, credential access, or configuration changes that may have enabled it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assets and environments

  • List hosts, cloud projects, repositories, databases, queues, SaaS tenants, and production or non-production environments touched.
  • Separate read access from writes, deletes, permission changes, and code or configuration deployment.
  • Identify shared services or credentials that could let the same action reach additional environments.

Data and credentials

  • Identify records viewed, copied, modified, or transmitted, including secrets that appeared in prompts, tool results, logs, or memory.
  • Determine whether credentials, session cookies, signing keys, or personal data were exposed. Treat exposure as possible until logs establish otherwise.
  • Trace destinations, recipients, and retention for every outbound transfer.

Business and operational effects

  • Reconcile messages, tickets, invoices, deployments, access grants, customer-visible changes, and automated downstream jobs.
  • Check for duplicate retries or compensating actions that may have magnified the original mistake.
  • Document what was not affected as well as what was changed; that boundary supports a proportionate recovery.

5. Classify the failure without guessing from the final text

Most incidents contain more than one contributing condition. Compare the observed sequence with the intended task, policy, and authorization record, then classify each relevant failure mode.

Planning or interpretation error

The agent selected an incorrect action even though the request and available tools were clear and the action was authorized. Look for ambiguous objectives, missing success criteria, unsafe assumptions, or a plan that ignored a stated constraint. Corrective work belongs in task design, validation, and evaluation, but do not expand permissions to compensate for a reasoning error.

Context or prompt manipulation

Untrusted instructions, retrieved content, memory, or tool output altered the plan. Evidence includes injected directives in documents, conflicting system and retrieved instructions, poisoned memory, or a tool response that attempted to steer subsequent calls. Preserve the exact context presented to the model, including provenance and sanitization results, so the manipulation can be reproduced and removed.

Excessive agency

The agent had more permissions, tools, destinations, data, or autonomy than the task required. A correct plan can still be dangerous when it can reach production, send externally, alter privileges, or spend funds without a boundary. Reduce scope to task-specific credentials, narrowly defined tools, destination allowlists, and short-lived grants.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Control-plane failure

An approval, policy, identity, logging, rate-limit, or rollback control was missing, stale, misconfigured, or bypassed. Compare the decision that should have blocked the action with the decision recorded at runtime. If the audit trail is incomplete, treat that absence as a control failure and preserve the remaining infrastructure evidence.

Tool or integration defect

An API returned an unsafe result, interpreted a parameter incorrectly, applied a change more broadly than documented, or produced an unexpected side effect. Reproduce the request in a safe environment, compare the serialized arguments with the API’s accepted schema, and involve the integration owner before re-enabling the call.

6. Recover from a known-good state

Recovery should be deliberate and reversible. Keep the agent disabled or approval-gated while you repair the environment and validate the control path.

  1. Stop propagation. Isolate affected hosts, accounts, queues, repositories, or integrations. Cancel pending jobs and disable webhooks or schedules that could repeat the action.
  2. Rotate exposed credentials. Revoke tokens and invalidate sessions when secrets may have been read, copied, or logged. Replace keys in dependent services and verify that old credentials no longer work.
  3. Restore a known-good configuration or backup. Use a versioned configuration, database backup, repository commit, or infrastructure snapshot whose integrity predates the incident. Preserve the compromised state separately for analysis rather than overwriting it.
  4. Validate dependencies and data integrity. Check service health, schemas, access rules, replication, queues, and monitoring after restoration. Confirm that the restored state does not reintroduce the same unsafe policy or credential.
  5. Reconcile external side effects. Review every access change, message, ticket, deployment, financial action, and data transfer. Notify affected owners and issue corrections through the normal accountable process.
  6. Obtain human sign-off. A named owner should approve the corrected policy, permissions, monitoring, and rollback path before autonomous execution resumes. If any material uncertainty remains, keep the workflow stopped.

7. Harden the execution path before re-enabling autonomy

Prevention controls should not depend on the model remembering a rule. Put them in a gateway, proxy, identity layer, or tool-execution service that can deny an action regardless of the generated plan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Enforce least privilege externally

Issue task-scoped, short-lived credentials with only the operations and data required for the current objective. Re-check authorization immediately before each high-impact call. Use explicit destination allowlists and validate inputs, schemas, paths, and resource identifiers at the tool boundary.

Require approval for high-impact operations

Make destructive changes, financial actions, sensitive-data access or transfer, privilege changes, and production modifications approval-gated. Bind an approval to the exact target, parameters, identity, and time window; do not accept a generic “approved” flag or a stale decision from an earlier step.

Limit rate, budget, concurrency, and blast radius

Set ceilings for calls, spend, records, parallel operations, retries, and affected resources. Use sandboxes or isolated test tenants for experimentation. A limit should stop the workflow and create an alert when exceeded, not merely appear as advice in the prompt.

Make interruption and rollback real operations

Provide an operator-visible stop control that revokes execution authority and cancels queued work. Design reversible changes, transaction boundaries, checkpoints, and tested rollback procedures. Verify that a stop signal reaches every delegated worker and integration, including already-running calls where the underlying system supports cancellation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Protect auditability and detection

Retain complete, tamper-resistant action records with clear ownership and access controls. Alert on privilege escalation, unusual destinations, policy-denied attempts, repeated retries, approval bypasses, and sudden changes in volume or scope. Monitoring should cover both successful and blocked actions so attempted misuse is visible.

8. Test degraded operation and recovery

Controls that have never been exercised are assumptions. Run tabletop exercises and controlled evaluations for the exact failure paths your workflow can encounter.

  • Disable the approval service and confirm the agent fails closed rather than proceeding.
  • Remove or narrow a token while a workflow is active and verify that delegated calls stop.
  • Inject an untrusted instruction into retrieved content and confirm that provenance, filtering, and policy checks prevent unauthorized actions.
  • Trigger rate, budget, concurrency, and destination limits and verify alerts, cancellation, and operator visibility.
  • Practice restoring a known-good configuration, rotating credentials, validating dependencies, and reconciling side effects.
  • Test log unavailability or tampering detection; the workflow should stop when required audit records cannot be written.

Record exercise results as operational defects with owners and due dates. Update policies, runbooks, evaluations, and training after every incident or exercise, consistent with NIST’s requirement for continual monitoring, response, recovery, and improvement.

9. Compare autonomous-agent designs by control strength

When choosing between architectures, compare where a control is enforced and what evidence remains after a failure. A design that produces persuasive explanations but cannot stop, authorize, or reconstruct actions is not operationally safer.

Comparison axis Questions to ask Evidence of a strong design
Permission scope Are credentials task-scoped, short-lived, and limited to required resources? Effective scopes are visible at execution time and can be revoked independently.
Policy enforcement Can the model bypass the policy by choosing another tool or destination? A gateway or proxy validates every call outside the model.
Human approval Which destructive, financial, sensitive-data, privilege, and production actions require approval? Approval binds to exact parameters, target, identity, and time window.
Allowlists Are tools, resources, and outbound destinations explicitly constrained? Unknown or newly introduced destinations are denied by default.
Logging Can investigators correlate context, identity, calls, permissions, approvals, and side effects? Centralized, protected records include successes, failures, retries, and operator actions.
Interruption and rollback Can an operator stop queued and delegated work, and is rollback tested? Stop and restore procedures are exercised under realistic conditions.
Sandboxing and limits What limits constrain rate, budget, concurrency, data volume, and blast radius? Exceeded limits halt execution and raise an alert.
Recovery How quickly can owners isolate, restore, rotate credentials, and validate integrity? Known-good artifacts and named recovery responsibilities are documented.
Post-incident evidence Will logs survive account compromise or cleanup? Retention, access control, and tamper detection are independent of the agent.

10. Use the NIST AI RMF to assign ongoing responsibilities

NIST’s AI Risk Management Framework 1.0, published January 26, 2023, is voluntary, rights-preserving, non-sector-specific, and use-case agnostic. Its four functions—Govern, Map, Measure, and Manage—provide a way to assign ownership around the technical controls above rather than treating an incident as a one-time model bug.

Govern

Define who owns the agent, its tools, approvals, logs, incident response, and decommissioning. Establish escalation paths and criteria for disabling autonomous execution.

Map

Document intended use, affected people and systems, data flows, dependencies, threat assumptions, and the consequences of incorrect or unsafe actions. This map supplies the baseline for blast-radius analysis.

Measure

Evaluate task performance, policy adherence, false approvals and denials, interruption reliability, logging completeness, and recovery exercises. Measure behavior at the tool boundary, not only the quality of generated text.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Manage

NIST AI RMF Core Manage 4.1 calls for post-deployment monitoring plans with user and actor input, appeal and override, decommissioning, incident response, recovery, and change management. Manage 2.4 calls for mechanisms and assigned responsibilities to supersede, disengage, or deactivate systems whose performance or outcomes are inconsistent with intended use. Manage 4.3 requires incidents and errors to be communicated, tracked, responded to, recovered from, and documented. Map those requirements to named controls, alerts, runbooks, and review dates.

11. A concise incident runbook

Keep this checklist available to the operator who must act under pressure:

  1. Record time, workflow, agent version, tenant, operator, and first harmful outcome.
  2. Pause the workflow; fail closed if approval, policy, identity, risk, or logging services are unavailable.
  3. Revoke or narrow tokens and block destructive tools, integrations, and destinations.
  4. Preserve volatile evidence and protect centralized logs from deletion or alteration.
  5. Correlate request, context, identity, model session, delegation, calls, arguments, results, approvals, permissions, targets, and side effects.
  6. Determine affected assets, data, credentials, environments, and external actions.
  7. Classify contributing causes as planning, context manipulation, excessive agency, control failure, tool defect, or a combination.
  8. Isolate propagation, rotate credentials, restore a known-good state, validate dependencies, and reconcile side effects.
  9. Keep autonomy disabled or approval-gated until a human owner signs off.
  10. Implement the control change, test interruption and rollback, update the runbook, and document lessons learned.

12. Common symptoms and the first checks to make

Observed symptom First checks Likely control focus
An agent changed production without a recorded approval Compare the tool-gateway decision with the approval service, token scopes, and any emergency override Execution-time authorization, approval binding, and fail-closed behavior
Data was sent to an unfamiliar external destination Inspect destination resolution, allowlists, tool arguments, and retrieved or tool-supplied instructions Destination validation, context provenance, and outbound monitoring
The same unsafe action repeated after an operator stopped it Check retries, queues, delegated workers, schedules, and whether the stop signal revoked authority Interruption propagation, cancellation, and retry limits
Logs show a result but not the call that produced it Join runtime records with gateway, identity, and target-system audit logs; identify where correlation IDs were lost Centralized, tamper-resistant logging and mandatory audit writes
A tool applied a broader change than requested Compare serialized parameters, API schema interpretation, resource identifiers, and integration version Input validation, scoped tools, sandbox testing, and integration review
